Funds are targeted for community programs such as scholarships food programs that serve the elderly and other underserved individuals and other programs that benefit members living in our community. We support other non-profit organizations by way of donations and small grants to help them carry out their community service programs. For grants awarded to other organizations we ask for written updates on their projects or documentation via pictures or invites to their program when it occurs. Carolyn Edwards is the mother of Kendra Edwards-Waters who is a recently added board member. Part VI, Line 11b The form is completed by the treasurer and a copy is emailed to the board members for review and comments for suggested changes to the responses in the 990 form. Part VI, Line 12c The organization has a disclosure form that each board member must fill out. Each board member must review the Conflict of Interest policy and complete the disclosure form for potential conflicts of interest. The disclosure forms are kept on file with the Secretary and reviewed and updated annually for any potential conflicts of interest related to the operations of the organization. Part VI, Line 19 The organization makes it governing documents conflict of interest policy and financial statements available upon request. The documents may be emailed to the requestor uploaded to their website or sent via regular US mail.
