The Hospital delivered state-of-the-art primary inpatient services to approx. 25,840 acute care days, 3,637 critical care days, 4,866 observation days during the period. Since 1903, the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of the persons ability to pay. The Hospital has earned several recognitions from HealthGrades, the nations leading independent healthcare ratings organization. The Hospital draws the majority of its patients from Fayette County. Although the population of the County has been decreasing, the demand for the Hospitals services remain due to the downsizing and service cuts at nearby facilities along with the growth in both the regions uninsured and elderly populations. The Uniontown Hospital offers lines of services that are unavailable elsewhere in Fayette County. These services include a state-of-the-art Outpatient Diagnostic Facility that opened in 2006. The West Virginia United Health System is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery team. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities. The strategic plan of the System states intent to build a regional health care delivery system in its service areas, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves. Barnesville maintained their previously approved non-profit status with the IRS after joining the System. Group Uniontown Hospital Line Part V, Section B, Line 5 The 2022 Community Health Needs Assessment CHNA was prepared by the West Virginia University Health Sciences Center - Health Affairs Institute. The CHNA process began with a thorough review of the previous cycles needs assessment report, and included review of publicly available secondary data related to counties within Uniontowns service area. These data included the above census data and County Health Rankings Data Appendix B. Using these reports as a springboard, the leadership team started the initial discussion around critical health needs, changes in the community demographics over the previous few years, and lessons learned from the last CHNA cycles health topic prioritization and development of implementation activities. Primary data collection was comprised of surveying community members to gather perceptions of health issues, quality of life, and other related topics. Survey data was analyzed and presented back to the Uniontown leadership team, including community partners. This meeting was focused on reviewing the survey data, discussing community assets that impact population health in Uniontowns service area, as well as discussing the needs of those not well-represented in the survey data. At this meeting, the group worked to discuss possible strategies for each area of concern, as well as existing resources to address these needs. Year three progress - Uniontown Hospital has partnered with agencies including the Fayette Chamber of Commerce, the City of Uniontown, Fayette County Social Services, other area health organizations and programs, WVU Medicine Home Care, the Fayette Living Well Coalition, area school districts, the Food Bank, St. Vincent DePaul and other to support healthy living activities. B Continue to partner with community agencies regarding cardiac rehab/maintenance and other related programs. Year three progress - Uniontown Hospital no longer partners with the YMCA around cardiac rehab/maintenance program as COVID-19 shuttered many such engagements. However, Cardiac Rehab is offered at the hospital. C Refine the end of life/palliative care program. Year three progress - The hospital has instituted an Ethics Committee, with a detailed process for entering an Ethics Consult, Patient Assessment and Management screening for Ethics, Ethics Charter, and Flowchart to seamlessly obtain a consult and have it in front of the Primary Ethics physician within 24 hours or less. D Through the Uniontown Hospital Special Events Engagement Team and the Community Relations Department participate in community events that promote healthy behaviors including area health fairs targeting seniors, students, and veterans. Year three progress - Uniontown Hospital sponsored weekly events through the Storey Summer Concert Series and educated residents about healthy living and promoting better health. Hospital staff also participated. Year three progress - Uniontown Hospital discontinued the Stroke Support Group around COVID-19 concerns but did engage in two education opportunities in the community to continue to educate around stroke. Goal Work to improve health outcomes related to substance abuse and mental health. Strategies to be implemented FY 2020-2022 A Participate in the Opiod Use Disorder quality initiative. Year three progress - Our case management department consults with social workers to initiate care coordination for the patients who are identified as having addictions to complete an intake on the patients to connect them to community resources and support centers. B Develop better processes to link inpatient and outpatients to providers. Year three progress - Uniontown Hospital has developed a collaboration with The American Addiction Center to assist in bridging the gap between the acute care setting with inpatient and outpatient services prior to the patient being discharged from the acute care setting. This gives the Addiction Specialist the time to build a rapport with the patient and their loved ones and the opportunity to ask questions related to their journey to recovery. C Train social workers in American Society of Addiction Medicine. Year three progress - Uniontown Hospital continues to review opportunities for the social workers to attend ASAM online training and has plans to have a social worker become accredited. D Develop a tele-psychiatry program. Year three progress - Uniontown Hospital is continuing to explore having a tele-psych embedded within our ED and on the nursing units to assist our physicians with providing holistic care. This strategy has been paused for system integration with WVU Medicine and around considerations regarding multi-state physician credentialing. E Foster stronger relationships with local inpatient psychiatric hospital. Year three progress - Uniontown Hospital was unable to conduct a week long Symposium on site as planned due to COVID-19 however, the Department of Mental Health, Adult Protective Services and Area Agency on Aging did conduct staff training and education. Group Uniontown Hospital Line Part V, Section B, Line 11 Priority 3 Health Education. Goal Improve community knowledge related to health care availability. Strategies to be implemented FY 2020-2022 A Revamp the Community Physician Referral Line and advertise the Line providing a Hotline for patients to refer to when looking for a new physician. Year three progress - Uniontown Hospital became a part of WVU Medicine Health System and provided a multitude of resources for patients to connect with primary care physicians and specialists. B Develop communication/marketing tools for patients directing them to alternative care providers related to services that were scaled back at the Uniontown Hospital such as Obstetrics. Year three progress - With the move to WVU Medicine, Uniontown Hospital provided all the resources available through the health systems vast network of specialists and locations for area patients to obtain the care they require. Additionally, in year three, service lines were established at the hospital in many specialties including, Pain, Pulmonary, Orthopedics and more. C Develop communication related to available transportation to alternative care providers. Year three progress - Uniontown Hospital provided printed materials and verbal assistance to patients in need of transportation help to obtain services not available locally. Strategies to be implemented FY 2020-2022 A Develop strategies to help skilled facilities reduce readmissions to the hospital. Year three progress - Uniontown Hospital is continuing to develop a program with Skilled Nursing Facilities with the greatest readmissions rates to determine issues that led to readmission. Group Uniontown Hospital Line Part V, Section B, Line 11 Some issues identified through the Community Health Needs Assessment have not been addressed in this plan. In initial discussion and subsequent prioritization, Uniontown Hospital considered the levels to which some needs were already being addressed in the service area. However, Uniontown does not offer discounted care to individuals who fail the 200 FPG test. To calculate net expense, bad debt of 5,186,818 was deducted from total expenses of 160,607,463 as shown in Part IX line 25 of the core Form 990, for a net expense of 155,420,645. Part I Line 7 Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. Part II No community building activites to report for 2022. Part III Line 2 Bad Debt Expense at cost was calculated by multiplying bad debt expense of 5,186,818 by our cost to charge ratio of 37.95 for a total of 1,968,397. Part III Line 3 Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 25,000. The total of that report was 404,896 which we then multiplied by our cost to charge ratio of 37.95 for a total of 153,685. Part III Line 3 The estimated bad debt attributable to patients eligible for charity care of 153,685 should be considered community benefit due to the fact that anyone with outstanding balances of 25,000 or greater usually qualifies as catastrophic if the patient completes the application process. Part III Line 3 In our charity care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year. Regardless, that bad debt should be considered community benefit as we provide services to those in need regardless of their ability to pay. Part III Line 4 Uniontown Hospitals financial statements are prepared on a consolidated basis as a member of the WV United Health System. The footnote for Accounts Receivable is as follows Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. Part III Line 4 The System grants credit without collateral to its patients, most of whom are local residents and are insured under third-party payer agreements, primarily with Medicare, Medicaid and various commercial insurance companies. The System records accounts receivable net of estimated price concessions and such amounts have historically been without managements expectations. Part III Line 4 The mix of accounts receivable at December 31, 2022 and 2021, from patients and third-party payors is as follows For the year-end December 31, 2022 Medicare - 27, Medicaid - 16, Blue Cross - 19, Commercial, managed care and other - 35, Patients - 3, totaling 100. For the year-end December 31, 2021, Medicare - 31, Medicaid - 16, Blue Cross - 21, Commercial, managed care and other - 28, Patients - 4, totaling 100. Part III Line 8 The amount reported in Part III Line 6 5,912,080 was calculated using the total Medicare allowable cost from the Medicare cost report, less Medicare reimbursement of direct GME. This calculation also excpdues Medicare costs associated with providing subsidized healcare services as reported on Part i line 15g. Had that cost been inlcuded in this calculation the amount on line 6 would have been 20,628,989 and would reflect a truer surplus of 1,099,545. Part III Line 9b The Uniontown Hospital does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy. Part VI Line 2 The West Virginia United Health System leadership team develops an operating plan every year. This operating plan establishes several priorities for the Uniontown Hospital, all of which are focused on meeting the ever changing healthcare needs of the community. The leadership team develops this operating plan with knowledge gathered from direct contact from patients patient satisfaction surveys outside consultants professional health care groups such as the Hospital Association of Pennsylvania and the Healthcare Financial Management Association and research obtained from various sources including the federal and state government and various other outreach programs and sources. Part VI Line 3 As part of the Revenue Cycle policy entitled Service Assess it states under Patient Advocacy that the health care system will actively assist all individuals in seeking resolution to their financial obligations. This includes helping them understand what services are covered by their health insurance, providing assistance in obtaining health insurance and helping seek eligibility for financial assistance. Currently this program is called the Uniontown Hospital Community Charity Care UHC3. UHC3 is available to all residents of Fayette County and patients will be assisted by Financial Care Consultants that are part of the Patient Financial Services department. The Hospital has brochures located in its waiting rooms referencing all financial services and patient obligations. In addition, the Patient Access staff makes the brochures readily available to the population at the time of registration. Part VI Line 4 The Uniontown Hospital is a 130 bed community hospital which is located 50 miles southeast of Pittsburgh in the City of Uniontown, Fayette County. Since 1904 the Hospital has been serving the greater Uniontown and Fayette County area, providing quality healthcare regardless of a persons ability to pay. Census.gov estimates the population of Fayette County to be 125,755 as of July 1, 2022. The current unemployment rate is 4.8. Approximately 14.7 of the population is over 65 with 22.8 of the population below the poverty level and 25.8 of the population eligible for Medicaid. Part VI Line 5 The general health and well being of the community residents is central to the mission of Uniontown Hospital. The Hospital responds to the healthcare needs of the community with a number of community education and outreach programs. Additionally, numerous educational seminars, classes, and health fairs are offered free of charge to the public. The Hospital sponsors Blood Drives with the American Red Cross and outreach activities with organizations such as the American Heart Association and March of Dimes. Also, in June 2004 the Hospital joined forces with health organizations and other community groups to organize the Fayette County Community Health Improvement Partnership CHIP. CHIP has initiated numerous programs to reduce teen pregnancy and smoking, and several health issue related task forces. Under the auspice of CHIP is Fayette County STEPS initiative which is a national pilot program funded by the Centers for Disease Control which targets chronic disease prevention. The STEPS program builds partnerships among area school districts, workplaces, doctor offices, and neighborhood coalitions to educate and promote healthier lifestyles. Part VI Line 6 On January 1, 2021, Uniontown Hospital became a member of the West Virginia United Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery team. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities. The strategic plan of the System states intent to build a regional health care delivery system in its service areas, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves. Part VI Line 6 System management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems service areas.
