THROUGH FINANCIAL AND RESOURCE DONATIONS OF MANY INDIVIDUALS AND ORGANIZATIONS, THESE SERVICES HAVE BEEN POSSIBLE. IN ADDITION, THE COMMUNITY HEALTHCARE SYSTEM HOSPITALS PROVIDE FINANCIAL, IN-KIND AND PERSONNEL SUPPORT FOR MANY OF THE ACTIVITIES OF THE FOUNDATION. COMMUNITY SUPPORT: We are thankful for the many community groups and businesses who continue to support our organization and account for a large part of our income. We have had numerous individuals and organizations hold fundraisers to benefit the Cancer Resource Center and we truly appreciate their selfless efforts. In addition, the Community Foundation of Northwest Indiana and a number of its entities contribute not only monetarily, but also through the time and talents of many employees. FOUNDATION ACTIVITIES The foundation continuously seeks opportunities to educate the public and professionals about the latest in cancer care from prevention and screening through survivorship. It also supports clinical trials in the community so individuals diagnosed with cancer do not always have to travel to distant hospitals to receive this type of treatment. The Administrator participated in Cancer Committee for Community Hospital, St. Catherine Hospital and St. Mary Medical Center. The administrator also participates in the Breast Leadership Committee both at Community Hospital and St, Mary Medical Center, Chemotherapy Committee, Research Committee at Community Hospital, Oncology Steering Committee at St. Mary's, Cancer Quality Committee and Cancer Care Leadership Committee. The Cancer Committees plan activities of the cancer program and help maintain and improve practice standards to meet the criteria outlined by the American College of Surgeons Commission on Cancer. The goal is to have continuity of care throughout the healthcare for the cancer patient and for public education and awareness. Cancer Research Department Activities/Accomplishments - Established a Cancer Services Leadership Committee with participants from executive administration, Chief Executive Officer of St. Catherine's Hospital, Chief Medical Officer, Cancer Care Services Medical Director, Cancer Research Medical Director, Vice President of Marketing, Administrator, three surgeons, three medical oncologists, one radiation oncologist, and two pathologists. This committee oversees all cancer care services throughout the healthcare system. - Developed and established Medical Director of Cancer Care Services. - Established a Cancer Quality Committee, led by one Pathologists and includes medical oncology, radiation oncology, surgical oncology as well as multiple nursing, therapy, and registry staff members. - Navigation Services were restructured to better serve patients from screening & diagnosis to survivorship. - Navigation Manager named to assist in developing workflows, implement new navigation technology, and support nursing staff. - Strategic focus on breast and lung cancer. - Expanded navigation from survivorship to the entire continuum between screening/diagnoses to survivorship; two nurse navigators were hired, one for breast and one for lung navigation. - Changed navigation workflow to increase in-person support of patients, notification of diagnosis in breast cancers and close follow up through treatment. - The COVID-19 EPIC brought a halt to in-person activities at the Cancer Resource Center March 13, 2020 through August 2021. The Cancer Resource Center opened its doors to visitors in August 2021. - Due to Covid-19, established tumor boards on web-ex, hosting more than 50 multidisciplinary specialties of physicians, midlevel practitioners, rehabilitation therapists, social workers and nursing navigation staff. - Due to Covid-19, established multiple support groups on web-ex, supporting patients and care givers through their journey. - Received GO 2 Foundation's award for Care Continuum Center of Excellence for our Lung Programs - Completed and passed NRG's Audit in March 2021. - Commission on Cancer completed successful virtual site visits at both St. Mary (April 2021) and Community Hospital/St. Catherine Hospitals (June 03, 2021). - National Accreditation Program for Breast Centers completed their tri-annual site visits on April 7, 2021 for St. Mary Hospital. - A successful drive -through Cancer Survivors Day was held on June 06, 2020 for 29 participants. - A successful Lights of Life presentation and virtual tree lighting ceremony was held in December 2020. Growth Initiatives Cancer Research studies annual accrual 2020 calendar year - Patients screened - 850 - Patients enrolled - 10 - Patients followed - 122 - Open Protocols - 13 - March 2020 to May 2021 Staff was reduced due to Covid-19 Pandemic. Research limited to follow up tracking and reporting during Covid 19 months. No in-person consenting taking place during Covid 19 months New Programs/Projects Navigation System: - Adopted new navigation system (EON) to identify, track and follow incidental lung and breast nodules. Supportive Care Model - Hired Licensed Clinical Social Worker for Cancer Care Services to support patients with case management/social deterrents in barriers to care as well as psychological support. - Enhanced Genetic Program with a newly hired Genetic Counselor - Adopting patient survey for investigative assessment of eligibility for genetic testing. Exploring two technological workflows to increase patient awareness and referrals for genetic counseling. - Expanded on Palliative Care Services for the system. Education on the benefits of palliative care given to providers and staff. Identification of resources, i.e. oncology rehabilitation, hospice, home palliative care, in-house palliative care, psychological support - Oncology Rehabilitation. It is offered by the rehabilitation specialists at all three hospitals; cancer patients are being referred in the early stages of diagnosis in preparation for symptom management, physical limitations and progress during the course of treatment with surgery, radiation and chemotherapy treatments. Data Reporting - Adopted new cancer registry system for chart abstraction and reporting of analytical cases. - Creating informational dashboard to track quality and growth with Data Analytics New Research Opportunity - NCI Affiliated Cancer Site - Contracted with Carle Cancer Clinic to become a National Cancer Institute (NCI) affiliated site. This will allow CHS access to more than 100 NCI clinical trials. - Once contract was signed in January 2021, all NCI clinical trials were placed on hold as we transitioned into the new affiliation. Existing follow up patients were uploaded into the Carle Cancer Registry while FWA certificates, CITI training for providers, staff and nurses, COIs were signed, and the NCI credentialed and loaded all providers into their system under the new affiliation. - New research manager - 12/13/21. FUNDRAISING EVENTS Unite & Fight 2020: Major fundraiser for the Cancer Resource Centre. This event was canceled due to Covid-19. Tree Lighting Ceremony: This year's event raised over $12,000 for cancer research. Our honored guests to light the holiday trees were Leo Correa, CEO of St. Catherine's Hospital and Executive Sponsor of Cancer Care Services, Jon Lee, MD, Chairman of Radiology and Medical Director of Cancer Care Services and Marie Forszt, Vice President of Marketing Services for the health system and advocate for Cancer Care Services. Fundraising Activities - Lights of Life--2020 $12,978 - Research Income FYE 2020-21 $ 5,380 TOTAL $18,358 During FYE 2021, we have brought new programs to the Cancer Resource Centre to meet our visitor's needs.
