Personal invitations were sent to individuals representing various community, business, and educational groups throughout Harrison County. Individuals and organizations that participated include officials from the Harrison County Health Department, Family Children First Council, Harrison County General Health District, Harrison Metropolitan Housing, Ohio Department of Youth Services, Harrison County Office of Job Family Services, Harrison Hills School District and the Hospital board of directors. Group Harrison Community Hospital Line Part V, Section B, Line 11 The advisory committee members agreed on a set of criteria to use to evaluate the list of potential needs identified through the fact finding process. The criteria included 1 Potential to impact community health 2 Cost to the community 3 Community urgency. The advisory committee members discussed each of the identified health issues. Each member of the advisory committee had the opportunity to vote on which issues they felt met the criteria listed above. Some needs were eliminated from the list while others were prioritized. This process involved group discussion and voting amongst the advisory committee members. The advisory committee decided to adopt the top 3 health care related issues, and created an organization specific implementation strategy. Those 3 health care issues were 1 Chronic Disease Management 2 Unhealthy Lifestyles 3 Drug and Alcohol Abuse. Harrison Community Hospital has been limited in recent years in its ability to dedicate financial and personnel resources to the implementation plan items identified in prior years CHNAs. Progress on the identified health priorities in FY 09/30/21 was severely limited due to the continuation of the COVID-19 pandemic. In addition, the hospital has experienced several consecutive years of operating losses. As a result, very limited resources were available to allocate to the identified community health needs. Group Harrison Community Hospital Line Part V, Section B, Line 13b Harrison Community Hospital offers discounted care only to individuals who qualify for free care. Harrison Community Hospital offers a Prompt-Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either prompt pay discount, the patient/guarantor must meet certain requirements. Group Harrison Community Hospital Line Part V, Section B, Line 16j Harrison Community Hospital became a member of the West Virginia United Health System effective April 1, 2021. On March 1, 2022, Harrison Community Hospital adopted the Financial Assistance Policy of West Virginia United Health System. Prior to March 1, 2022, Harrison did maintain a Financial Assistance Policy that was publicized, it was available on their website along with the FAP application, and a plain language summary of the FAP. While the FAP that was in place during the tax year-end is no longer online it is available upon request. The current, as of the date of filing, policies can be found here https//wvumedicine.org/bill-pay/financial-assistance/ Part I Line 3c Eligibility for financial assistance is determined by referencing the financial assistance matrix, which is based on the federal poverty guidelines for the most current year available as issued by the department of health and human services. Applicants are required to provide proof of income, such as unemployment, workers compensation, social security, and alimony/child support. They are also asked if they Medicaid or any other third party insurance. Applications claiming that they have no income are required to provide a letter of support from the individual providing the support for the patient. Additionally, applicants may provide letters of denial from insurance providers as support. Assets are also considered when deciding whether to grant financial assistance as the patient is required to provide copies of bank statements, certificates of deposit, money market accounts statements, etc. Part I Line 6a HCH community benefit numbers are reported in total with other hospitals within the West Virginia United Health System. Part I Line 7 Total community benefit expense for FY 09/30/2021 is 4,158,160 and is 19.55 of total net expenses. To calculate net expense, bad debt of 861,934 was deducted from total expenses of 22,128,058 as shown in Part IX line 25 of the core Form 990, for a net expense of 21,266,124. Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. Part I Line 7 Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-Charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. Part II No community building activities to report for the FY 9/30/2021. Part III Line 2 Bad Debt Expense at cost was calculated by multiplying bad debt expense of 861,934 by our cost to charge ratio of 55.19 for a total of 475,701. Part III Line 3 The estimated amount of bad debt expense that could reasoably be attributable to patient who likely would qualify for financial assistance under the hospitals charity care policy was calculated by applying the percentage of charity care to gross patient revenue to the total bad debt expense at cost. Part III Line 4 Harrison Community Hospital does not have separate audited financial statements. However, consolidated financial statements are prepared for the West Virginia United Health System, which Harrison Community Hospital is now a member of. The patient accounts receivable footnote from the consolidated audited financial statements dated 12/31/2020 is as follows Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. The System grants credit without collateral to its patients, most of whom are local residents and are insured under third-party payor agreements, primarily with Medicare, Medicaid and various commercial insurance companies. The System records accounts receivable net of estimated price concessions and such amounts have historically been within managements expectations. The mix of accounts receivable at December 31, 2020 and 2019 from patients and third-party payors is as follows. Medicare 29 and 27, Medicaid 17 and 17, Blue Cross 21 and 24, Commercial, managed care and other 28 and 26 and Patients 5 and 6, respectively. Part III Line 4 For receivables associated with self-pay patients, which includes both patients without insurance and insured patients with deductible and copayment balances, the System records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. Part III Line 8 The amount reported in Part III Line 6 2,335,328 was calculated using the total Medicare allowable cost from the Medicare cost report, less Medicare reimbursement of direct GME. Part III Line 8 Harrison Community Hospitals shortfall, 261,865 of Medicare program reimbursement should be considered a community benefit because we are relieving a government burden by providing care in excess of our costs to these patients. Part III Line 9b Harrison Community Hospital does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy. Part VI Line 2 Harrison Community Hospital performed a formal needs assessment to assess the health care needs for the community that it serves. This assessment was completed in November 2016. An implementation plan was established by Harrison Community Hospital in response to the 2016 CHNA, and updated for 2017 and 2018 efforts. An updated CHNA and implementation plan same document was performed in September 2019. This is currently the CHNA and implementation plan available on Harrison Community Hospitals website. Part VI Line 2 Ongoing community health fairs, CPR classes, diabetes education and healthy lifestyle seminars are the primary method of gauging community health needs throughout the year. Many of these programs are coordinated with Wheeling Hospital, Inc. The communities usually contact the hospital facility and ask for assistance. The breakfast for kids program was started by the Hospital knowing that there are kids needing a Christmas. Use of Hospital cardio exercise equipment by the community, as a fitness resource, was also enacted due to requests/inquiries from the community. Part VI Line 3 Harrison Community Hospital HCH employs financial counselors to meet with patients and discuss eligibility to qualify for charity care. HCH provides brochures discussing charity care and the qualifications for receiving charity care. These brochures are available at each registration area in the facility. HCH provides financial assistance to patients who do not qualify for any state or federal programs. Part VI Line 3 Our charity care eligibility guidelines are also listed on our website at www.wvumedicine.org under the Billing and Insurance - Online Bill Pay section of the Patient and Visitor page. HCH also has contracted a third party organization to be on-site to help patients, that qualify for charity care, Medicaid, or other types of financial assistance, complete the required applications and provide the correct documentation to receive these benefits. Part VI Line 4 Harrison Community Hospital predominantly services rural Harrison County, Ohio with an approximate population of 14,477. The racial makeup of Harrison County, OH according to the estimated July 1, 2021 census data was 95.8 white, 2.1 African American, 0.2 Asian, 0.2 Native American, with the remainder from other races or a combination of two or more races. The median household income for 2021 was approximated at 49,454 while the median US household income was 64,994. The percentage of households below the poverty level in July 2021 was approximately 13.7 compared to 11.4 nationally. Part VI Line 5 We continue to provide quality services to the rural and needy community regardless of major reductions in reimbursement. The organization remains committed to the community even when resources are limited and/or less than cost to provide coverage. We continue to make efforts to attract physicians and medical services for those that cannot connect to programs in other service areas. Part VI Line 6 Harrison Community Hospital is a part of the WV United Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery system. WVUHS serves as the parent corporation to an affiliated group of healthcare providing entities. The strategic plan of the System states intent to build a regional health care delivery system in its service areas, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves. Part VI Line 6 System management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems service areas.
