Conflict of interest policy compliance Part VI line 12c The Organization annually distributes the code of ethics and disclosure statement forms to the Board of Directors and each member reviews and signs as documented in the minutes of the Board of Director meetings. This form documents that each board member is independent and does not have any conflict of interest.
