Form 990, Part VI, Section B, line 12c All board members receive a copy of the Organization's Conflicts of Interest and Documentation Policy. All board members sign a conflict of interest form annually. Form 990, Part VI, Section B, line 15 There is currently only one SOCI employee, the Executive Director. Performance Metrics: In the first three years, the executive director will be the only staff member and is responsible for planning, program administration/management, community partner management, and oversight of hte accountability process. The Executive Director will be assessed based on program results generated, the financial sustainability of the organization and strategic planning designed to take the organization from one region to multiple regions over time. Annual Performance Assessment: The board President will conduct a performance review of the Executive Director on an annual basis. In establishing compensation of the Executive Director the Board will follow the conflict of interest policy, approve compensation in advance of payment and document in writing the date and terms of approved compensation. Form 990, Part VI, Section C, line 19 The Organization makes its Form 990 available for public inspection if anyone requests to see the Form. Financial statements are not available to the general public, but are available to all members of the Board of Directors.
