Nutrition education information on the back o false false 4398655 Home-Delivered Meals:The Home-Delivered Meals program (HDM) serves frail and homebound seniors not only with nutritious meals but also provides social contact, ongoing welfare checks, nutritional information, and referrals to community-based services. Contact with a home-delivered meal driver also reduces the isolation experienced by many seniors who live alone. As the number of frail, homebound seniors increases, combined with their growing desire to maintain independence in their own homes, the need for home-delivered meals becomes greater each year.Congregate Meals (or All Seasons Cafes):The Congregate Meals program serves mobile seniors with noon meals.This program reduces food insecurity, nutrition risk, isolation and loneliness by encouraging seniors to get out of their homes to visit friends and make new friends. During daily meals, program participants are encouraged to take part in activities offered at All Season Caf locations (e.g. bingo, board games, trivia, holiday parties, and more).The following was accomplished during the 2018-19 fiscal year:a. MoW served 5,100 unduplicated seniors and delivered over 510,000 meals to congregate and home-delivered meal participants.b. 500 volunteers contributed over 52,000 hours of volunteer service to the home-delivered and congregate meal programs.c. Ninety-four percent of seniors who ate at the All Seasons Caf said that the program helped them make more friends.d. Forty-one percent of home-delivered meal participants served were aged 80 or older.e. The home-delivered meal program operated 59 delivery routes throughout Sacramento County.f. The congregate meal program operated at 20 All Seasons Caf locations.g. MoW secured a grant to provide culturally responsive meal offerings to Japanese/ Asian Pacific Islander (API) older adults at two Japanese churches in Sacramento to improve meal/program satisfaction as well as to attract younger older adults of Japanese/API ancestry.h. MoW partnered with Sacramento Municipal Utility District (SMUD) to deliver approximately 420 meals to homebound seniors on Thanksgiving Day.i. MoW established a partnership with Rebuilding Together to make minor modifications and repairs that improve accessibility and safety within the homes of program participants aged 60 and older.j.
