The survey, coordinated with the Maine CDC, engaged public health expertise throughout the process. The survey was administered using a snowball approach where stakeholder agencies agreed to send the surveys to their members and stakeholders for participation. 1,639 people, representing more than 80 organizations and businesses in Maine, completed the survey. Respondents represented health care agencies, public health agencies, law enforcement, municipalities, schools, local businesses, social service agencies, and nongovernmental organizations. In addition to the stakeholder survey, each county conducted a number of community engagement opportunities (forums) to ensure broad interests of the local community were represented; Obtain stakeholder input on identifying significant health needs based on review of shared CHNA data; Solicit stakeholder feedback on prioritizing significant health needs, and; Identify local assets and resources that potentially may address local health priorities. The local forums were conducted in collaboration with the Maine CDC to solicit input from individuals representing populations with health disparities. The following organizations were in attendance at the January 11, 2016 Cumberland County Shared CHNA Community Engagement Forum and provided valuable feedback on the Shared CHNA.Details about organizations: Sample of organizations United Way, ME Health Access Foundation, Southern ME Area Agency on Aging, Preble St. Resource Center, legislator, Somali community members, USM/UNE/SMCC/Tufts medical School, ME Health Mgt Coalition, MaineMedical Center (hospital facility), Spring Harbor Hospital, Mercy Hospital, Bridgton Hospital, HMP, & Portland Public Health. Hospitals and health care providers serve low income patients through Medicare/MaineCare and provide low-cost/no-cost services to medically underserved residents. Many public health programs and services, social service agencies, funding agencies, and non-profit agencies specifically serve racial/ethnic minorities, low-income, and medically underserved residents. Somali community members attended the forum. Part V, Line 6a - List Other Hospital Facilities that Jointly Conducted Needs Assessment The Shared CHNA was conducted through the Maine Shared Health Needs Assessment Planning Process (SHNAPP) a collaborative effort among Maines four largest health-care systems Central Maine Healthcare, Eastern Maine Healthcare Systems (EMHS), MaineGeneral Health, MaineHealth and the Maine Center for Disease Control and Prevention, an office of the Maine Department of Health and Human Services (DHHS). Part III, Line 2 - Methodology Used To Estimate Bad Debt Expense The costing methodology used to determine the amount is cost to charge ratio. Part III, Line 4 - Bad Debt Expense Patient and trade accounts receivable are stated at the amount management expects to collect from outstanding balances. Management provides for probable uncollectible amounts through a charge to earnings and a valuation allowance based on its assessment of the current status of individual accounts. Balances that are still outstanding after management has used reasonable collection efforts are written off through a charge to the valuation allowance and the applicable patient accounts receivable. Credit is extended without collateral. Part III, Line 8 - Explanation Of Shortfall As Community Benefit Medicare losses should be treated as a community benefit because the losses are incurred in performing an important public service and Maine hospitals experience one of the lowest Medicare reimbursement rates in the country. Part III, Line 9b - Provisions On Collection Practices For Qualified Patients All account guarantors who express an inability to pay inpatient and outpatient services will be screened for eligibility for charity care using an application and guidelines established by Mercy Hospital. An account may be reconsidered for charity care at any time when new information is available about a patients inability to pay. Part VI, Line 3 - Patient Education of Eligibility for Assistance Mercys Financial Assistance policy describes how the organization informs and educates patients on Financial Assistance. Public Notice of the availability of assistance through this policy is made through each of the following means: 1. Posting notices in a visible manner in locations where there is a high volume of inpatient or outpatient admitting/registration, such as emergency departments, billing offices, admitting offices, and hospital outpatient service settings. 2. Posting notice of the availability of assistance and contact names and phone numbers on Mercy Hospitals website. 3. Providing uninsured patients a matrix outlining the types of financial assistance available. Upon request, a full text copy of the Financial Assistance policy is made available. Mercy Hospital has developed a professional model for financial counseling for all patients without insurance and has staff onsite to meet individually from 8am to 5pm on Monday through Friday. This includes comprehensive screens for potential resources such as governmental insurance and disability, enrollment on the Health Insurance Marketplace, and commercial insurance. Financial counselors have daily huddles, assigned work lists, and accountabilities to engage and develop an ongoing rapport with uninsured and vulnerable populations. They are available Monday through Friday from 8am-5pm and routinely meet with patients in the community to complete applications. With the advent of the ACA, the financial counselors have all been trained to act as Certified Application Counselors and helped to enroll both Mercy patients as well as to provide staffing and services for local community and municipal organizations. Part VI, Line 4 - Community Information Mercy Hospital is a community hospital comprised of two acute inpatient locations, six primary care practices, five Express Care facilities, and 13 specialty practices. Mercys service area is the greater Portland metropolitan area, roughly comprised of Maines Cumberland County. The Portland area is experiencing an increase in diversity, with at least six percent of area residents born outside of the United States. Many of these new Mainers are refugees and asylum-seekers, who bring a unique set of circumstances and health needs to the community. Approximately 11 percent of Cumberland county households fall below the federal poverty level.Mercy is a Catholic hospital founded by the Sisters of Mercy in 1918. Its mission is to carry out the healing work of Christ by providing clinically excellent, compassionate health care for all, with special concern for the poor and disadvantaged. It has fulfilled that mission for 100 years, both in the hospital and in the community. Mercys mission-based programs such as McAuley Residence, a comprehensive two generational program with housing for families affected by substance use disorder and Garys House, a hospitality house are tremendous community resources. The Medical Neighborhood project, a formal health collaborative created by Mercy, proactively seeks uninsured and under-insured persons in Greater Portland to complete a comprehensive search of all coverage options and facilitate access to concrete supports such as housing, food, and employment services. Mercy Hospital served as the convener and facilitates the ongoing work of the Greater Portland Addiction Collaborative, a formal collaborative charged with optimizing the care delivery system and creating safe, sober housing for uninsured persons with opioid use disorder. Mercys financial counselors routinely meet patients at the local shelters or other social service agencies. Part VI, Line 4 - Community Building Activities Mercy Hospital's community building activities include participating in community events with Portlands Public Health and Health Equity Departments, non-profit community-based organizations, social service providers, and faith communities to deliver health education sessions for the general public and vulnerable populations several times throughout the year. Garys House, a department of Mercy Hospital, is a safe, comfortable, and affordable home-away-from-home for families and individuals with loved ones receiving medical treatment in any greater Portland area hospital. Nine individual apartments and beautiful dining area and kitchen facilities provide respite for families. A donation of $15 per night is accepted for those able to pay for lodging. Several local organizations such as the United Way and UNUM volunteer regularly at Garys House. Mercy Hospital also provides $25,000 to the City of Portlands Health Equity Department and $25,000 to Amistads Peer Support and Recovery Coach Program each year in support of the Community Health Outreach Workers, whose role is to provide health education, outreach, and patient navigation for vulnerable populations. Part VI, Line 5 - Promotion of Community Health The majority of Mercy Hospitals Board of Directors resides in the primary service area and is not employees or independent contractors. Mercy also has an open medical staff. Mercy Hospital donates food weekly as part of a food rescue program to local food pantries and soup kitchens. Part VI, Line 6 - Affilated Health Care System The 2016 Shared Community Health Needs Assessment was conducted by EMHS (Eastern Maine Healthcare Systems) in collaboration with several member/affiliated hospitals and the many public health and community organizations across the state. The Shared Community Health Needs Assessment informs initiatives to promote community health across the system as well as within each member hospitals local service area. Each member hospital adopted a local implementation strategy and community health improvement plan, tailored to meet local needs. Part VI, Line 7 - States Filing of Community Benefit Report N/A Part VI - Additional Information Disclosure in accordance with Revenue Procedure 2015-21:Part V Facility Information, Section B Facility Policies and PracticesIn accordance with Revenue Procedure 2015-21, Mercy Hospital is disclosing the following information with respect to Section 501(r) issues that were discovered during the 2018 tax year and how these have been corrected to satisfy 501(r) requirements.For all items:A good-faith effort was made to implement the requirements of the Section 501(r) regulations. Mercy Hospital obtained information about the requirements and sought to implement them accordingly, but later identified some compliance gaps.The issues were discovered in fiscal year 2018 during an internal audit of Mercy Hospitals compliance with Sections 501(r). To ensure full compliance with Section 501(r) regulations, Northern Light Healths Community Health and Revenue Cycle Departments perform routine reviews to assure Mercy Hospital is in compliance with Section 501(r). In addition, Northern Light Health continues to engage Northern Light Health Internal Audit to review Mercy Hospitals compliance with Section 501(r).Specific issues and corrections:1. Incorrect calculation of Amounts Generally Billed (AGB) a. Requirement: A hospital facility may determine AGB for any emergency or other medically necessary care it provides to a FAP-eligible individual by multiplying the hospital facilitys gross charges for the care by one or more percentages of gross charges (AGB percentage(s)). A hospital facility using this method must calculate its AGB percentage(s) at least annually by dividing the sum of the amounts of all of its claims for emergency and other medically necessary care that have been allowed by health insurers during a prior 12-month period by the sum of the associated gross charges for those claims.b. Condition: Mercy Hospital incorrectly calculated the AGB because it did not use actual amounts allowed by health insurers or associated gross charges in its calculation of AGB. Instead, Mercy Hospital used adjusted net patient revenue divided by adjusted gross patient revenue in calculating the AGB.c. Corrective Action: In response, Mercy Hospital recalculated the AGB using the required data and retroactively adjusted its AGB for FY 2017 and 2018 to the correct percentage. Mercy Hospital has identified 1246 patient accounts affected by the correction and to date has corrected 972 accounts and has adjusted $67,979 in order to restore each affected individual to the position in which he or she would have been had the failure not have occurred. Of this total adjustment amount, $12,409 was actually required to be refunded to the individual; the remainder was in the form of accounting adjustments only. At this time Mercy Hospital has 274 accounts remaining to be corrected with an anticipated total adjustment of $39,956. d. She finds a shady spot on a sunny summer day to feed afternoon snacks to her donkey, Clementine, and her mule, Mr. Tibbs. She cracks a smile and laughs as Clementine quickly scoffs down a fresh carrot. Im the beast of burden. They dont do any work at all, she laughs, I do all the work and thats my therapy which has been awesome.To look at Lori, with her thick wavy brown hair and healthy complexion, its hard to imagine that just months ago she was battling cancer and undergoing chemotherapy and radiation treatments.Its such a shocking thing when you hear you have cancer, and I just thought its going to be stage one, itll be no problem; it was actually stage three! It makes you think much differently about your time here, she says.Lori was diagnosed with stage III colon cancer in October 2017 and would undergo treatments through May 2018. She had to go to Northern Light Cancer Care for radiation treatments but was able to take oral chemotherapy medication at home. Having already started on chemotherapy treatments at the hospital, Lori knew how it made her feel.Youre kind of in the cloud. Its kind of foggy for lack of a better word.
