Pt VI, Line 7a The members elect the Board of Directors. Pt VI, Line 11b The form 990 will be completed annually and copies will be provided to the entire governing board as well as the President/CEO of the organization. At that time the President/CEO will review the Form 990 with the Executive committee and fiscal officer. Any necessary changes will then be updated on the form. Once all necessary changes are made and the President/CEO is in agreement with the Executive Committee and Fiscal Officer on the finished form 990, it will be signed by the Board President, dated and submitted by the filing deadline. A copy of the approved form 990 will be provided to all of the officers, directors and trustees before the return is filed. Pt VI, Line 12c The Chamber Board of Directors must fill out a conflict of interest questionnaire annually and disclose any interests that could have a potential conflict. Pt VI, Line 18 The form 990 and form 990-T are available upon request. The form 990 is available for public inspection at www.guidestar.org. Pt VI, Line 19 All financial information, governing documents, and all policies of the Chamber of Commerce are available upon request.
