<ExplanationTxt>PART V, SECTION B, LINE 5: JOHN MUIR BEHAVIORAL HEALTH COLLECTED COMMUNITY INPUT IN THREE WAYS. FIRST, COMMUNITY INPUT WAS PROVIDED BY A BROAD RANGE OF COMMUNITY MEMBERS THROUGH THE USE OF KEY INFORMANT INTERVIEWS AND FOCUS GROUPS. REPRESENTATIVES FROM STATE, LOCAL, TRIBAL OR OTHER REGIONAL GOVERNMENT PUBLIC HEALTH DEPARTMENTS WITH KNOWLEDGE, INFORMATION AND EXPERTISE RELEVANT TO THE HEALTH NEEDS OF THE COMMUNITY WERE CONSULTED. OTHER INDIVIDUALS WITH EXPERTISE OF LOCAL HEALTH NEEDS WERE ALSO CONSULTED. TEN COMMUNITY REPRESENTATIVES OF VARIOUS ORGANIZATIONS AND SECTORS WHO EITHER WORK IN THE HEALTH FIELD OR IMPROVE HEALTH CONDITIONS BY SERVING THOSE FROM THE TARGET POPULATIONS WERE CONSULTED. SECOND, TEN CONTRA COSTA COUNTY EXPERTS FROM VARIOUS ORGANIZATIONS INCLUDING PUBLIC HEALTH OFFICERS, COMMUNITY CLINIC MANAGERS AND CLINICIANS WITH COUNTRYWIDE EXPERIENCE AND EXPERTISE WERE INTERVIEWED. THEY WERE ASKED TO IDENTIFY THE TOP NEEDS OF THEIR CONSTITUENTS, INCLUDING SPECIFIC GROUPS OR AREAS WITH GREATER OR SPECIAL NEEDS. THIRD, DISCUSSIONS WITH FOCUS GROUPS THAT REPRESENTED UNDERSERVED, IN POVERTY, AND/OR SOCIALLY OR LINGUISTICALLY ISOLATED WERE HELD. THE DISCUSSION INCLUDED QUESTIONS ABOUT THE COMMUNITY'S TOP HEALTH NEEDS, THE DRIVERS OF THOSE NEEDS, THE COMMUNITY'S EXPERIENCE OF HEALTHCARE ACCESS AND BARRIERS TO ACCESS AND ASSETS AND RESOURCES THAT EXIST OR NEEDED TO ADDRESS THE COMMUNITY'S TOP HEALTH NEEDS. THE FOUR FOCUS GROUPS CONSIST OF:1. PITTSBURGH HIGH SCHOOL AFRICAN AMERICAN PARENT GROUP: 7 PARTICIPANTS REPRESENTING AFRICAN AMERICAN, LOW INCOME MINORITY.2. FIRST 5 CONTRA COSTA: 11 PARTICIPANTS REPRESENTING LATINO, SPANISH SPEAKING LOW INCOME MINORITY.3. MONUMENT CRISIS CENTER: 11 PARTICIPANTS REPRESENTING MEDICALLY UNDERSERVED, LOW INCOME.4. OBESITY, DIABETES, HEALTHY EATING AND ACTIVE LIVING2. ECONOMIC SECURITY3. HEALTHCARE ACCESS AND DELIVERY INCLUDING PRIMARY AND SPECIALTY CARE4. ORAL/DENTAL HEALTH5. MENTAL HEALTH6. SUBSTANCE ABUSE7. UNINTENTIONAL INJURIES8. VIOLENCE/INJURY PREVENTIONHOSPITAL REPRESENTATIVES PRIORITIZED HEALTH NEEDS WITH LOCAL COMMUNITY AND PUBLIC HEALTH REPRESENTATIVES. TO FURTHER REFINE AND SELECT THE COMMUNITY HEALTH NEEDS WE PLAN TO ADDRESS, FOUR CRITERIA WERE UTILIZED.SELECTION PROCESS: THE COMMUNITY BENEFIT OVERSIGHT COMMITTEE IS CHARGED WITH OVERSEEING THE STRATEGIC DIRECTION OF COMMUNITY BENEFIT PROGRAM AND ACTIVITIES. THE COMMITTEE IS COMPOSED OF JOHN MUIR HEALTH EXECUTIVES AND COMMUNITY BOARD OF DIRECTORS MEMBERS. THE COMMITTEE MET ON JULY 6, 2016 TO REVIEW THE LIST OF COMMUNITY HEALTH NEEDS IDENTIFIED AND PRIORITIZED BY THE CHNA REPORT AND TO SELECT THE COMMUNITY HEALTH NEEDS THAT JMH WILL FOCUS COMMUNITY BENEFIT ON FROM 2016 TO 2018.THE FOLLOWING SELECTION CRITERIA WERE USED AS A METHOD TO PRIORITIZE AND SELECT THE COMMUNITY BENEFIT PRIORITIES. THE SELECTION CRITERIA BUILT ON THE CHNA PRIORITIZATION CRITERIA, WHICH INCLUDED (1) SEVERITY OF NEED; (2) MAGNITUDE/SCALE OF NEED; (3) CLEAR DISPARITIES OR INEQUITIES AND (4) COMMUNITY PRIORITY.THE COMMITTEE MEMBERS FIRST AGREED UPON THE SELECTION CRITERIA. THE COMMITTEE THEN SCORED EACH COMMUNITY HEALTH NEEDS BETWEEN 1 (DOES NOT MEET CRITERIA) AND 3 (STRONGLY MEET CRITERIA). THE SCORES WERE COMPILED TO IDENTIFY THE TOP SCORING HEALTH NEEDS. AFTER DISCUSSION BY ALL MEMBERS REGARDING RATIONALE FOR SELECTION, THE COMMITTEE CAME TO A CONSENSUS ON THE FOLLOWING NEEDS THAT JMH PLANS TO ADDRESS:1. HEALTH CARE ACCESS AND DELIVERY, INCLUDING PRIMARY AND SPECIALTY CARE - JOHN MUIR HEALTH NEW FAMILY RESIDENCY PROGRAM ALLOWS NEW RESIDENTS TO SEE PATIENTS WHO OFTEN EXPERIENCE BARRIERS TO ACCESSING CARE, INCLUDING OUR MOST VULNERABLE RESIDENTS, AT OUR FAMILY RESIDENCY CLINIC AT THE WALNUT CREEK OUTPATIENT CENTER AND THROUGH OUR FREE MOBILE HEALTH CLINIC THAT SERVES EAST CONTRA COSTA COUNTY.2. MENTAL HEALTH - JOHN MUIR HEALTH STARTED THE MENTES POSITIVAS PROGRAM BY LAUNCHING AN EIGHT-WEEK ROBUST TRAINING FOR PROMOTORES. THE PROGRAM, FACILIATED ENTIRELY IN SPANISH, REACHES THOSE WHO SELF-IDENTIFY AS LIVING WITH STRESS AND DEPRESSION. COGNITIVE BEHAVIORAL STRESS MANAGEMENT TOOLS ARE USED TO SUPPORTIVELY TEACH PARTICIPANTS TO LOWER STRESS AND DEPRESSION LEVELS AND TO CONTROL REOCCURRENCE.3. OBESITY, DIABETES, HEALTHY EATING AND ACTIVE LIVING - JOHN MUIR HEALTH CONTINUED MONETARY SUPPORT TO THE FOOD BANK OF CONTRA COSTA AND SOLANO COUNTY MAKES IT POSSIBLE FOR THE COMMUNITY TO GET FRESH FRUIT AND VEGETABLES INTO THEIR DIET. WITHOUT THIS SUPPORT, PEOPLE IN CERTAIN NEIGHBORHOODS WOULD HAVE A HARD TIME GETTING THE FOODS THAT HELP THEM EAT HEALTHY.JOHN MUIR HEALTH WILL NOT ADDRESS THE FOLLOWING HEALTH NEEDS AS PART OF THIS COMMUNITY HEALTH IMPROVEMENT PLAN DUE TO THE NEED TO ALLOCATE SIGNIFICANT, FOCUSED RESOURCES TO THE THREE PRIORITY HEALTH NEEDS IDENTIFIED ABOVE:1. ECONOMIC SECURITY - ADDRESSING ECONOMIC SECURITY IS A LARGE SCOPE INITIATIVE THAT JOHN MUIR HEALTH CANNOT ADDRESS ALONE, DUE TO LIMITED INFRASTRUCTURE AND RESOURCES. OTHER ORGANIZATIONS ARE CURRENTLY WORKING TO ENHANCE THE SAFETY NET IN CONTRA COSTA COUNTY BY ADDRESSING ECONOMIC SECURITY. JOHN MUIR HEALTH WILL CONTINUE TO BE A PART OF THE DIALOGUE ON THIS IMPORTANT ISSUE BUT WILL NOT CONTRIBUTE RESOURCES GIVEN ITS CONTRIBUTION IN OTHER AREAS.2. ORAL/DENTAL HEALTH3. UNINTENTIONAL INJURIES4. VIOLENCE/INJURY PREVENTION5. SUBTANCE ABUSE, INCLUDING ALCOHOL, TOBACCO, AND OTHER DRUGS</ExplanationTxt>
