UTILIZING A MULTIDISCIPLINARY, HOLISTIC APPROACH, OUR PHYSICIAN AND CARE MANAGER TEAMS PROVIDE DIAGNOSTIC WORK-UP, DISEASE MANAGEMENT PLANNING, TRAINING AND MEDICAL ASSISTANCE TO HANDLE CHALLENGING BEHAVIORS, CAREGIVER TRAINING, COUNSELING AND SUPPORT. WE HELP COORDINATE AVAILABLE COMMUNITY RESOURCES, ASSIST LOW INCOME ELDERLY IN APPLICATIONS FOR MEDICAL ASSISTANCE, AND COUNSEL INDIVIDUAL/FAMILIES ABOUT END OF LIFE DECISION MAKING.
