Under the Family Based Services umbrella, St. Anne's has entered into a partnership with other youth-serving agencies to develop an evidence informed workforce development service model for transition-age youth, focusing on the particularly high-risk individuals with connections to the child welfare system that our agencies serve. 1781713 Our Transitional Housing Program (operating around the clock) offers up to 40 young women ages 18 to 24 aging out of the foster care or juvenile probation systems and their children two-year subsidized housing and a continuum of services to foster their stability and independence. Our Transitional Housing Placement Plus Foster Care Program provides similar services to young women who have chosen to remain wards of the court past their 18th (up to their 21st) birthdays.
