The shared CHNA includes a large set of statistics on health status and risk factors from existing surveillance and health data sets. This CHNA includes input from a broad set of stakeholders from across the state as facilitated by the 2015 Shared Health Needs Assessment & Planning Process (SHNAPP) Stakeholders' Survey with input from 1,639 people representing more than 80 organizations and businesses in Maine. In Washington County, a total of 133 stakeholders responded to the survey. Also, on November 20, 2015 a Washington County Community Engagement Forum was held to solicit input from various community stakeholders and multi-sector groups on the health indicators from Maine CDC statistics and stakeholder survey results for Washington County, Maine. Down East Community Hospital Part V, Section B, Line 11: DECH addressed the needs in its most recent CHNA by preparing and adopting an implementation strategy. DECH plans to address all priorities identified through the management and budget of community benefit activities. In addition, DECH also takes advantage of partnering with other community plans by participating, supporting or promoting programs which support community health needs. No needs are not addressed. DECH's detailed implementation strategy can be found on the hospital website, www.dech.org. Part I, Line 6a: The senior leadership team, with approval by the board of trustees, manages the community benefit activities of the Hospital. An expense budget is identified annually for community benefit activities. Community benefit activities are reported each year in the Hospital's annual report. Part III, Line 4: See Pages 7 & 8 of audited financial statements for footnote disclosure.Bad debt expense represents actual and estimated write-offs as identified by the Hospital. Actual write-offs represent accounts that have exceeded 90 days in Accounts Receivable with no payment plans established. Estimated write-offs represent bad debts that are expected based on payment percentage assumptions as determined by the age and payor type of the receivable. Part VI, Line 2: See Schedule H Section C Part VI, Line 3: DECH has a financial assistance policy that outlines all the discounted cost opportunities that the Hospital has to offer. The complete financial assistance policy, plain language summary, and free care application is located on the Hospital's website at www.dech.org. Copies of the plain language summary is also posted in all main waiting areas of the facility and satellite locations and full copies of the policy can be obtained per request and without charge. Annually, DECH also advertises its plain language summary in local media outlets. Each patient is given the Hospital's financial assistance policy plain summary prior to the commencement of each date of service. The summary and contact information to obtain a free care application is also listed on the patient's bill. At every opportunity, the Hospital's billing specialists and financial counselors will communicate our policies to patients after billing and encourage patients to apply for financial assistance. During discussions, if patients are identified to be eligible for other governmental insurances, DECH will assist the patient with the application process if requested by the patient. Part VI, Line 4: Dempographic and Socioeconomic Factors:Washington County was home to 32,190 people in 2013. It is considered a rural county,according to the urban and rural classifications defined by the New England Rural Health RoundTable. It is worse off than the state as a whole in many demographic and socioeconomic characteristics, including income, employment and poverty rates. Key demographic featuresinclude:- Median household income of $37,236 (2009-2013)- 26.2 percent of children and 19.5 percent of all individuals live in poverty (2009-2013)- The unemployment rate was 8.4 percent compared to the state at 5.7 percent (2014)Access to Health Care/Quality:Access to care in Washington County is worse than the state; specifically, a significantly lower percentage of Washington County residents have health insurance, although a lower proportionreported a lack of care due to cost. A higher proportion of adults and children have MaineCare as their medical insurance. The ambulatory care sensitive-conditions hospital admission rate in Washington County was also significantly above the state. Key features for Washington Countyinclude:- 13.7 percent of residents did not have health insurance (2009-2013); 9.6 percent experienced cost-related barriers to getting healthcare in the last year (2011-2013)- 83.6 percent of adults reported having a personal doctor or other health care provider (2011-2013)- 40.3 percent of adults were enrolled in MaineCare, 61.5 percent of children were enrolled (2015)- The hospitalization rate for ambulatory care-sensitive conditions was 1,809 per 100,000 population (2011) Part VI, Line 5: The Hospital furthers its exempt purpose by promoting the health of the community. The board consists of volunteer community members that are elected by the Corporators of the Hospital.The Hospital has an open medical staff where all applicants have the opportunity to obtain membership as long as the guidelines as determined by the medical staff bylaws are met.
