SDIM collects client input by holding Community Advisory Board meetings, which improve the meaningfulness, user-friendliness, linguistic comprehensiveness, cultural sensitivity, and age appropriateness of programming.Taking Back our Power: Taking Back our Power (TBOP) is a transgender self-empowerment group meeting intervention that includes the following: 1) reach out to, recruit, and enroll incarcerated transgender individuals to participate in TBOP groups after release from correctional custody; 2) provide complete group classes to incarcerated transgender individuals on the underlying causes of discrimination and related health risks they face while incarcerated and post-release; 3) address stigma in healthcare settings and the broader society, and challenge discrimination that drives them away from HIV prevention, treatment and care; 4) engage 5 post-incarcerated transgender individuals that completed TBOP's educational component in activities that enable them to make positive changes in their lives, form healthy friendships and bonds with their peers, and become leaders and activists; and 5) educate and inform public health professionals and criminal justice experts about incarcerated transgender people's needs; and how to incorporate similar programs within their own organizations and institutions.Transitional Case Management: The Transitional Case Manager (TCM) Program provides individual level counseling, mentorship, and linkage to care services for individuals who are being released from LAC Jails. CHJ's TCM assesses clients' physical, psychosocial, environmental and financial needs to facilitate their access to, maintenance of, and adherence to health care, risk education activities, mental health care, substance abuse treatment, partner notification, and other supportive services. Inside the LAC Jail System, LAC's Department of Health Services coordinates the provision of TCM services for individuals who are eligible for the Program.
