<ExplanationTxt>THE HOSPITAL IS A NOT-FOR-PROFIT CORPORATION THAT HAS BEEN RECOGNIZED AS TAX-EXEMPT PURSUANT TO SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. THE SEGREGATED PORTFOLIO CONDUCTS ITS AFFAIRS IN A MANNER IN WHICH IT WILL NOT BE SUBJECT TO U.S. FEDERAL INCOME TAX OR GEORGIA INCOME TAX. THE HOSPITAL APPLIES ACCOUNTING POLICIES THAT PRESCRIBE WHEN TO RECOGNIZE AND HOW TO MEASURE THE FINANCIAL STATEMENT EFFECTS OF INCOME TAX POSITIONS TAKEN OR EXPECTED TO BE TAKEN ON ITS INCOME TAX RETURNS. THESE RULES REQUIRE MANAGEMENT TO EVALUATE THE LIKELIHOOD THAT, UPON EXAMINATION BY THE RELEVANT TAXING JURISDICTIONS, THOSE INCOME TAX POSITIONS WOULD BE SUSTAINED. BASED ON THAT EVALUATION, THE HOSPITAL ONLY RECOGNIZES THE MAXIMUM BENEFIT OF EACH INCOME TAX POSITION THAT IS MORE THAN 50% LIKELY OF BEING SUSTAINED. TO THE EXTENT THAT ALL OR A PORTION OF THE BENEFITS OF AN INCOME TAX POSITION ARE NOT RECOGNIZED, A LIABILITY WOULD BE RECOGNIZED FOR THE UNRECOGNIZED BENEFITS, ALONG WITH ANY INTEREST AND PENALTIES THAT WOULD RESULT FROM DISALLOWANCE OF THE POSITION. SHOULD ANY SUCH PENALTIES AND INTEREST BE INCURRED, THEY WOULD BE RECOGNIZED AS OPERATING EXPENSES. BASED ON THE RESULTS OF MANAGEMENTS EVALUATION, NO LIABILITY IS RECOGNIZED IN THEACCOMPANYING CONSOLIDATED BALANCE SHEET FOR UNRECOGNIZED INCOME TAX POSITIONS. FURTHER, NO INTEREST OR PENALTIES HAVE BEEN ACCRUED OR CHARGED TO EXPENSE AS OF JUNE 30, 2015 AND 2014 OR FOR THE YEARS THEN ENDED. THE HOSPITALS TAX RETURNS ARE SUBJECT TO POSSIBLE EXAMINATION BY THE TAXING AUTHORITIES. FOR FEDERAL INCOME TAX PURPOSES, THE TAX RETURNS ESSENTIALLY REMAIN OPEN FOR POSSIBLE EXAMINATION FOR A PERIOD OF THREE YEARS AFTER THE RESPECTIVE FILING DEADLINES OF THOSE RETURNS.</ExplanationTxt>

<ExplanationTxt>THE COMMUNITY ADVISORY COMMITTEE (CAC) WAS A KEY COMPONENT OF COMMUNITY ENGAGEMENT IN THE PROCESS AS REQUIRED BY THE IRS MANDATE. THE CAC WAS COMPOSED OF 15-25 MEMBERS REPRESENTING A CROSS-SECTION OF THE DEFINED COMMUNITY (TARGET AREA). TO FORMALIZE THE PROCESS, THE COLLEGE OF PUBLIC HEALTH, GEORGIA STATE UNIVERSITY WHICH CONDUCTED THE CHNA, PROVIDED THE ORGANIZATION WITH A LETTER TO RECRUIT CAC MEMBERS AND A LIST OF POTENTIAL CAC MEMBERS. HOSPITALS WERE SPECIFICALLY INSTRUCTED TO RECRUIT PEOPLE, OR AGENCIES, REPRESENTING TRADITIONALLY UNDERSERVED AND MINORITY POPULATIONS WITHIN THE TARGET AREA. IN ADDITION, HOSPITALS WERE ENCOURAGED TO SEEK DIVERSITY WITH RESPECT TO RACE, ETHNICITY, SOCIAL, ECONOMIC, AND EDUCATION BACKGROUNDS. FOR BACON COUNTY HOSPITAL, CAC MEMBERS WERE RECRUITED FROM THE HOSPITAL AND LOCAL HEALTH DEPARTMENT. THE BACON COUNTY HOSPITAL CEO, CINDY TURNER, WORKED WITH THE HOSPITAL BOARD OF DIRECTORS TO DEVELOP A LIST OF POTENTIAL CAC MEMBERS. THIS LIST BECAME THE INVITATION LIST FOR THE CAC. DATA COLLECTION EFFORTS WERE FIRST INITIATED BY SURVEYING CAC MEMBERS USING THE COMMUNITY-BASED SURVEY. IN GENERAL, THIS TOOK APPROXIMATELY 10 TO 15 MINUTES. CAC MEMBERS WERE ALSO GIVEN INSTRUCTIONS FOR DISTRIBUTING THE SURVEY TO THE COMMUNITY. IN ADDITION TO SURVEY COMPLETION AND INSTRUCTIONS FOR DISTRIBUTION, CAC MEMBERS WERE ASKED TO VOLUNTEER TO PARTICIPATE IN ONE OF THREE FOCUS GROUPS TO BE CONDUCTED IN THE COMMUNITY. THESE MEMBERS WERE ALSO ASKED TO ASSIST THE HOSPITAL IN RECRUITING POTENTIAL COMMUNITY MEMBERS TO PARTICIPATE IN THE REMAINING TWO FOCUS GROUPS. INITIALLY, THE COMMUNITY ADVISORY COMMITTEE WAS SELECTED, ITS 8 MEMBERS REPRESENTING A CROSS-SECTION OF THE TARGET AREA, INCLUDING HEALTHCARE PROFESSIONALS, COMMUNITY ACTIVISTS, AND OTHER COMMUNITY MEMBERS. COMMUNITY-BASED SURVEYS AND FOCUS GROUPS PROVIDED A WIDER ARRAY OF COMMUNITY INPUT.</ExplanationTxt>

<ExplanationTxt>FY 2015 CHNA IMPLEMENTATION PLAN PROGRESS: THE HOSPITAL PUT CONSIDERABLE EFFORT TOWARD ADDRESSING THE NEEDS IDENTIFIED IN THE CHNA. EDUCATIONAL INFORMATION WAS BROADCAST VIA FACEBOOK AND OTHER SOCIAL MEDIA SOURCES. OUR BREAST CANCER INITIATIVE IN OCTOBER HAS CONTINUED TO GROW GARNERING MORE AND MORE PUBLIC PARTICIPATION. THIS ALLOWS US TO EDUCATE AND PROMOTE DISCOUNTED SCREENING FOR THE UNINSURED AND UNDERINSURED WOMEN IN OUR COMMUNITY. THE NUMBER OF FREE HEALTH FAIRS WAS ALSO INCREASED INCLUDING THE NUMBER SERVICES OFFERED AT THE HEALTH FAIRS. A PEDIATRICIAN HAS BEEN RECRUITED AND IS IN PRACTICE. HE IS WORKING WITH OUR DAYCARE, SCHOOLS AND AT COMMUNITY EVENTS TO EDUCATE THE PARENTS OF THOSE CHILDREN. THIS HAS THE POTENTIAL OF REACHING OUT TO HUNDREDS OF FAMILIES WITH CHILDREN, INCLUDING THE EARLY HEAD START FAMILIES THAT SHARE THE BUILDING SPACE AT THE HOSPITAL'S DAYCARE. HE ALSO SEES MANY HISPANIC CHILDREN AND THEIR PARENTS, WHO USUALLY DON'T HAVE EDUCATIONAL OPPORTUNITIES INCLUDING GENERAL WELLNESS, NUTRITION, AND APPROPRIATE EXERCISE TO HELP COMBAT JUVENILE DIABETES. WE HAVE IMPLEMENTED A TOBACCO FREE POLICY FOR OUR ENTIRE ORGANIZATION. THIS POLICY IS APPLICABLE TO EMPLOYEES AND VISITORS. WE HAVE INCREASED OUR PRESENCE AT THE TEEN MAZE EVENT. THIS EVENT IS HELD AT THE LOCAL HIGH SCHOOL AND COMBINES COMMUNITY RESOURCES TO RAISE AWARENESS FOR TEENAGERS. WE SPONSOR MULTIPLE BOOTHS WHICH RAISES AWARENESS FOR TEEN PREGNANCY, STD'S, ETC. WE SHOWCASE CHILDCARE ITEMS ALONG WITH THE COST OF THESE ITEMS, WITH THE INTENTION OF DISCOURAGING TEENAGERS TO GET PREGNANT BECAUSE OF THE RELATED COST. IN ADDITION, PICTURES OF STD'S ARE DISPLAYED AND EDUCATION IS PROVIDED TO THE TEENS ABOUT THESE DISEASES, AS WELL AS, THE IMPORTANCE OF SAFE SEX AND/OR ABSTINENCE.</ExplanationTxt>

<FormAndLineReferenceDesc>BAD DEBT EXPENSE FOOTNOTE TO FINANCIAL STATEMENTS</FormAndLineReferenceDesc>
<ExplanationTxt>ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE HOSPITAL ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYOR SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE HOSPITAL ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY (FOR EXAMPLE, FOR EXPECTED UNCOLLECTIBLE DEDUCTIBLES AND COPAYMENTS ON ACCOUNTS FOR WHICH THE THIRD- PARTY PAYOR HAS NOT YET PAID, OR FOR PAYORS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY). FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS (WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLE AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL), THE HOSPITAL RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE STANDARD RATES (OR THE DISCOUNTED RATES IF NEGOTIATED) AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED, IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE HOSPITALS ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS INCREASED FROM 87% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2014, TO 92% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2015. THE HOSPITALS SELF-PAY WRITE-OFFS DECREASED APPROXIMATELY 45,000 FROM APPROXIMATELY 4,530,000 FOR FISCAL YEAR 2014 TO APPROXIMATELY 4,485,000 FOR FISCAL YEAR 2015. THE INCREASE IN SELF-PAY ALLOWANCES IS THE RESULT OF NEGATIVE TRENDS EXPERIENCED IN THE COLLECTION OF AMOUNTS FROM SELF-PAY PATIENTS IN FISCAL YEAR 2015. THE HOSPITAL DID NOT CHANGE ITS CHARITY CARE OR UNINSURED DISCOUNT POLICIES DURING FISCAL YEAR 2014 OR 2015. THE HOSPITAL DOES NOT MAINTAIN A MATERIAL ALLOWANCE FOR DOUBTFUL ACCOUNTS FROM THIRD-PARTY PAYORS, NOR DID IT HAVE SIGNIFICANT WRITE-OFFS FROM THIRD-PARTY PAYORS.</ExplanationTxt>

        <FormAndLineReferenceDesc>PART VI, LINE 5 - PROMOTION OF COMMUNITY HEALTH</FormAndLineReferenceDesc>
<ExplanationTxt>THE ORGANIZATION AND ALL ITS VOLUNTEER BOARD ARE COMPOSED OF COMMUNITY MEMBERS WITH DIVERSE PROFESSIONAL AND COMMUNITY SERVICE BACKGROUNDS. OUR EMERGENCY DEPARTMENT HAS 24/7 PHYSICIAN COVERAGE AND IS OPEN TO ALL PERSONS, REGARDLESS OF ABILITY TO PAY. THE ORGANIZATION SEEKS OUT DELIVERY OF CARE MODELS THAT BEST MEET THE NEEDS OF THE COMMUNITY. THE HOSPITAL'S RURAL HEALTH CLINIC (RHC) SERVES MANY LOW-INCOME AND MEDICAID ELIGIBLE PATIENTS BY APPOINTMENT AND ON A WALK-IN BASIS. THE RHC'S PATIENTS TYPICALLY NEED BASIC HEALTH ATTENTION AND THE RHC STAFF PROMOTES WELLNESS TO ALL OF ITS PATIENTS ON AN AS-NEEDED BASIS DURING THEIR VISIT. THIS WELLNESS MAY BE IN THE FORM OF EDUCATION MATERIALS AND/OR SUGGESTIONS FOR IMPROVING THE GENERAL HEALTH OF THE PATIENT. THE ORGANIZATION HAS A MULTI-PRONGED APPROACH TO IMPROVING THE HEALTH OF THE COMMUNITIES IT SERVICES: INCREASING ACCESS, BUILDING CAPACITY, INVESTING IN "UPSTREAM" PROGRAMS THAT GET AT THE CAUSE OF DISEASE AND ILLNESS, BUILDING COMMUNITY PARTNERSHIPS, ADVOCATING CHANGE, AND DEVELOPING LEADERSHIP. THE ORGANIZATION HOSTS HEALTH FAIRS TO PROMOTE WELLNESS AND ENCOURAGE PREVENTATIVE MAINTENANCE BY OFFERING FREE GLUCOSE AND CHOLESTEROL TESTS, BLOOD PRESSURE CHECKS, BONE DENSITY TESTS, MEDICAL BROCHURES WITH MEDICAL INFORMATION SUCH AS STROKE, HEART ATTACK, DIABETES, BLOOD PRESSURE, BREAST CANCER AND MORE. PHYSICIANS, DENTISTS, OPTOMETRISTS, MID-LEVEL PRACTITIONERS, REGISTERED NURSES AND OTHER LICENSED PROFESSIONALS ATTEND THESE FUNCTIONS. THE PRIMARY PURPOSE OF INCORPORATING MULTIPLE FACETS OF CARE IN THE FAIRS IS TO GIVE THE COMMUNITY A WIDE SPECTRUM OF KNOWLEDGE TO CONSULT WITH, FREE OF CHARGE, TO HOPEFULLY ENCOURAGE WELLNESS AND PREVENT FUTURE ILLNESS. DURING BREAST CANCER AWARENESS MONTH, WE FOCUS ON WOMENS HEALTH BY OFFERING AN ART EXHIBIT FOLLOWED BY A RECEPTION WITH A KEYNOTE SPEAKER. THE HOSPITAL ALSO OFFERS MAMMOGRAMS TO UNINSURED PATIENTS AT DISCOUNTED RATES. THIS GIVES ADDITIONAL WOMEN THE ACCESS TO MAMMOGRAMS, WHO MIGHT NOT GET SCREENED OTHERWISE. WE ASSIST WITH THE ANNUAL MULE ROUND-UP EVENT FOCUSING ON PROVIDING MEDICAL INFORMATION ON BREAST CANCER TO THE COMMUNITY. THESE BREAST CANCER AWARENESS PROGRAMS ARE AN ONGOING COLLABORATIVE FACILITATED BY THE ORGANIZATION TO BUILD AWARENESS OF BREAST CANCER IN THE COMMUNITY. THESE EVENTS ALSO BRING IN RESIDENTS FROM ALL OF THE SURROUNDING COUNTIES. THE ORGANIZATION, IN AN EFFORT TO INCREASE HEALTH AWARENESS IN OUR SERVICE AREA, ALSO PARTICIPATES IN THE ANNUAL BLUEBERRY FESTIVAL BY SETTING UP A BOOTH WITH MEDICAL INFORMATION ON DIABETES, HTN, VARIOUS TYPES OF CANCER, HEART DISEASE AND OTHER INFORMATION. PRIMARY CARE PHYSICIANS AS WELL AS MEMBERS OF THE HOSPITAL CLINICAL STAFF ARE ON HAND AT THIS EVENT, SIMILAR TO THE HEALTH FAIRS, TO ANSWER QUESTIONS FROM THE PUBLIC FREE OF CHARGE. THE ORGANIZATION HOSTS AN ANNUAL 5K RUN RUN/WALK DURING THIS EVENT WHERE THERE IS AN AVERAGE OF 200 PARTICIPANTS WITH THE TOTALS RISING EACH YEAR. THE PURPOSE IS TO TRY AND MOTIVATE THE PUBLIC TO BECOME INVOLVED AND LEAD AN ACTIVE AND HEALTHY LIFESTYLE. DURING THE DAY OF THE CHILD, A COMMUNITY EVENT GEARED TOWARDS THE YOUNGER POPULATION, THE ORGANIZATION HOLDS THEIR ANNUAL TEDDY BEAR CLINIC. THE TEDDY BEAR CLINIC IS A PROGRAM THAT BEGAN IN 1995, DESIGNED TO HELP PUT CHILDREN AT EASE WHEN THEY VISIT THEIR HEALTHCARE PROVIDER. THE WHOLE IDEA IS FOR THEM TO BE COMFORTABLE WITH THE HEALTHCARE ENVIRONMENT AND MAKE THE EXPERIENCE FUN FOR THEM. BECAUSE JUVENILE DIABETES IS A PREVALENT ILLNESS IN THIS REGION, THE ORGANIZATION INCORPORATES AWARENESS IN THIS EVENT THROUGH PROVIDING EDUCATIONAL BROCHURES AND PAMPHLETS TO THE GUARDIANS SO THAT THEY BECOME AWARE OF SYMPTOMS AND TREATMENTS. BECAUSE THE ORGANIZATION SERVES A POPULATION THAT HAS A HIGH PROPORTION OF LOW-INCOME INDIVIDUALS, WE PARTICIPATE IN THE COMMUNITY FALL FESTIVAL HOSTED BY THE ALMA BACON COUNTY CHAMBER OF COMMERCE. WE USE THE OPPORTUNITY TO REACH OUT TO AN AUDIENCE THAT IS TYPICALLY UNEDUCATED IN THE AREA OF HEALTHCARE. OUR FOCUS IS TYPICALLY PROVIDING GUIDANCE AND EDUCATION ON THE IMPLICATIONS OF NOT HAVING REGULAR CHECK-UPS WITH PRIMARY PHYSICIANS, SYMPTOMS OF ILLNESSES THAT ARE PREVALENT IN THIS REGION AND VARIOUS OTHER TOPICS THAT COULD AFFECT THE WELL-BEING OF THE INDIVIDUAL. AN ANNUAL SANTA'S WORKSHOP IS HOSTED FOR CHILDREN AS A FUN DAY DURING THE CHRISTMAS HOLIDAYS. IN ESSENCE, THE ORGANIZATION USES THE CHILDREN'S WORKSHOP TO REACH OUT TO GUARDIANS THAT WOULD NOT BE REACHABLE UNDER NORMAL CIRCUMSTANCES.</ExplanationTxt>
