In 2015, 906 older adults or their family caregivers (unduplicated count) received services. -Care Management: Our goal is to provide older adults the opportunity to pursue a life of optimum independence and satisfaction, while living in their desired environment, which is usually their own home. Care management is a process that assesses; plans care; implements, coordinates, monitors, and evaluates the services required to meet the client's health and human service needs. -CareLinkTM: provides Aging Life Care services to assist older adult to maintain their independence, health, safety and plan for their future. -Center for Holocaust Survivors: Holocaust survivors receive care management; restitution assistance; therapeutic socialization activities; audiology, vision, medical and dental services; and home care services. -Counseling: Clients receive therapy under a treatment plan to meet their goals. -Independent Living Assistance: Clients receive assistance with banking, bill paying, insurance claims, housing searches and assistance in obtaining benefits. 718883 64674 VITAL SERVICES offers safety net services for people facing challenges. -Emergency Intervention and Case Management: To prevent homelessness, hunger, and deprivation, Jewish Family Service distributes third party payments for rent, mortgage, utilities, medical care, transportation, and other necessities. JFS case managers work closely with clients to ensure they receive all public benefits and assistance for which they qualify.-Heldman Family Food Pantry: A five-day emergency supply of food supplements limited budgets. This includes meat, fresh vegetables, pantry staples, household cleaning, and personal care items. Our pantry promotes Client Choice, which allows the selection of items to fit individual needs. It is the only food pantry in the region that includes a full array of kosher foods and meat.-Shelter Diversion: Jewish Family Service helps clients prevent homelessness by offering them case management and emergency financial assistance toward obtaining and maintaining stable housing. Jewish Family Service is one of only a few emergency assistance providers selected for this federally funded homelessness prevention program.-Mental Health Case Management: Individuals with chronic mental illness are engaged in ongoing case management services with the goal of helping this highly vulnerable population maintain stability and security. -Peer-based Socialization Activities: Recognizing that socialization and spiritual needs are important to achieving wellness, Jewish Family Service offers this best-practice program to address the unique needs of people experiencing hunger, poverty and mental illness who desire to be served within a Jewish cultural setting. -Information and Referral: Jewish Family Service Case Managers refer clients to community resources and provide them with up-to-date information to meet their needs. 410890 6422 43550 EDUCATION & MENTORING (formerly Family Life Education) addresses issues and problems people confront at various stages of life, combining both direct services and learning opportunities that result in stronger individuals and families. 798 people (unduplicated count) benefited. -Bigs and Littles promotes the development of disadvantaged youth ages 7-18 by matching them with caring, adult role models who guide them in developing the social- emotional skills necessary to succeed in school and reach their full potential. JFS offers Bigs and Littles through a community based model and a group-based model, both of which create and support one-to-one matches that are positive, safe, lasting, and tailored to each child's needs. -Community Education Programs are offered through a three tiered engagement strategy:1) Large group events featuring national speakers raise awareness of key issues in the community, including mental health, domestic violence, parenting, and aging;2) Mid- sized group events featuring local speakers also address issues of interest and attract participants interested in increased engagement through more regular learning opportunities;3) Small discussion groups allow individuals to forge meaningful relationships with others in the community who are facing similar challenges, including parenting, interfaith relationships, mental health, and more.
