Our services are open to all. 0 0 13437629 0 14237030 Developmental Services: LFSVA offers a wide continuum of care for individuals with intellectual and developmental disabilities so that they may live in their preferred community setting. Our services, which include group homes, day support, in-home care, sponsored residential services and in-home support, use a person-centered approach that helps individuals meet their medical, dietary, behavioral, personal care and daily living needs, as well as achieve the maximum degree of independence. All services are for adults 18 years of age or older, except for in-home support which is provided to all ages. Services are available in Central, Northern and Western Virginia, the northern Shenandoah Valley, Tidewater and the areas surrounding Roanoke, Richmond and Fredericksburg. During the year ended June 30, 2014, we helped 308 individuals. 5312648 0 7032307 Minnick School Programs - LFSVA Minnick Schools, located in Roanoke, Harrisonburg, Wytheville and Wise, work with public schools in 27 localities, and during the fiscal year ended June 30, 2014 helped more than 200 students reach their academic and behavioral goals. And, 25 children were able to transition back into their local public schools. All Minnick Schools offer individualized instruction and a treatment team that works closely with the public schools, students and their families. A rounded curriculum is aligned with the state Standards of Learning and includes art, music, physical education and driver's education. Technology-rich classrooms and online curriculum programs such as NovaNet help students keep up with their public school peers. To prevent more restrictive out-of-school placements, we offer educational aides to assist students in the program. We also offer vocational and community-based instruction. Our autism program offers individualized instruction geared to the unique needs of each student and their families. Our classrooms are based on the principles of Applied Behavioral Analysis, successful research-based strategies that systematically teach behavior in a highly structured environment. All staff are trained in Applied Behavior Analysis strategies. With the goal that students will transfer their new behavioral and life skills from the classroom to home and from home to the community, we give children many opportunities to learn and practice. 3071717 0 3122778 Treatment Foster Care & Adoption - LFSVA recruits and trains therapeutic foster parents so that children in foster care will have a stable, loving, home environment while they are healing from the situation that caused the family disruption. We provide in-home services and emergency access to an agency social worker 24/7. Our goal is to help children move toward permanency. In fact, many of our families adopt the children they foster. We also offer families the option to be certified as both foster and adoptive parents. During FYE 6/30/2014, we helped 93 children in foster care find homes in which to heal and grow. LFSVA unites children waiting for adoption with "forever families". We offer adoption services to individuals and couples interested in special needs adoption and foster-to-adopt. Our in-depth training on parenting and support throughout the adoption process is considered one of the best in the state. We assist families in adopting from foster care in Virginia and throughout the U.S. and, through a state grant, accelerate adoptions by providing an array of printed materials and vigorously pursuing electronic strategies to bring eligible foster children to the attention of families wishing to adopt. In the year ended 6/30/2014, 15 children found permanent, loving families through LFSVA. 1138504 0 1171656 Community-Based Services - (Expenses $1,127,805; Revenues $1,171,656) LFSVA is committed to helping families deal constructively with problems and crises so that they can remain together as healthy families. Community-based services include behavioral treatment services, in-home counseling, intensive in-home services, therapeutic support services, social groups and "Essential Pieces", which provides vital education and supports for families with children on the autism spectrum. By providing research-based, community-centered services, we work to eliminate the need for out-of-home, out-of-community or out-of-state placements. During the year ended June 30, 2014, we provided services to 235 individuals, supporting and strengthening families. Community-based services also include Therapeutic Day Treatment (TDT), which is a school-based program that provides services to help children overcome the mental health, emotional, behavioral and substance abuse issues that create barriers to learning. LFSVA staff members develop interventions that help children improve behavior, which allows them to participate more fully in the classroom. During the year ended June 30, 2014, we provided TDT services to 79 children. 71893 648 Community Engagement and Grief Services - (Expenses $71,893; Revenues $648) LFSVA continues to grow its community engagement program to strengthen connections among LFSVA and church, community and civic groups. Our goal is to build a network of caring individuals and strong civic and business involvement that will enable the individuals we serve live fuller and more complete lives in their communities. We continue to offer grief support groups in the Tidewater and Waynesboro areas that provide a safe environment for individuals to share and validate their feelings of grief and loss. We also offer free training and support for Caregiver U, a curriculum that provides education, community resources and support to caregivers and their families.
