This program focuses on improved health outcomes for moms and babies by coordinating care across providers and supporting medical visits, providing health education with a focus on the prenatal and early childhood period, providing extensive resources to participants including free diapers and formula, operating both support groups and one-on-one support to help families navigate pregnancy and after. Children born in Mississippi have the lowest life expectancy in the country and the state has one of the highest rates of maternal mortality.
