UTILIZING A MULTIDISCIPLINARY HOLISTIC MODEL OF CARE, OUR PHYSICIAN AND CARE MANAGER TEAMS PROVIDE DIAGNOSTIC WORK-UP, DISEASE MANAGEMENT PLANNING, TRAINING AND MEDICAL ASSISTANCE TO MANAGE DIFFICULT BEHAVIORS, CAREGIVER TRAINING, COUNSELING AND SUPPORT. WE ALSO HELP COORDINATE AVAILABLE COMMUNITY RESOURCES, HELP LOW INCOME ELDERLY APPLY FOR MEDICAL ASSISTANCE, AND COUNSEL FAMILIES REGARDING END OF LIFE DECISION MAKING.
