He has a week to return any questions or comments on the return. Upon agreement of the Finance Committee Chair, the Form 990 is approved for final submission. Pt VI, Line 12c: The Foundation requires the Board and all staff to annually read and sign the conflict of interest policy. During each board meeting when a vote is on the floor for approval, the chair will ask if anyone has a conflict and if so, that board member abstain from voting. Pt VI, Line 15a: The Executive Director is evaluated annually by the Board of Directors. Comparability data and a written report are placed in personnel file. Pt VI, Line 15b: The Executive Director performs evaluations of other managers. Copies of the evaluations are placed in the manager's personnel file. Pt VI, Line 19: The Foundation's governing documents, all policy documents, and financial statements are available upon request from an individual or organization to the Foundation.
