Albany Medical Center provides a self pay/customer service unit to support the processing of Financial Aid/Charity Care applications and Presumptive Eligibility for all patients who may be determined eligible based on income and family size.Albany Medical Center also offers tailored payment plans upon request.We continue to build community partnerships that support the enrollment of patients in programs that will provide the most complete coverage for the care they need. Part VI - Needs Assessment Coordinated through the Healthy Capital District Initiative (HCDI), the counties of Albany, Rensselaer, and Schenectady implemented a joint project to engage health providers and community members in a regional health assessment and prioritization process. Between 2012 and 2013, Albany Med participated in a two year community health planning process during which a comprehensive assessment of public health needs was completed for the counties of Albany, Rensselaer, and Schenectady. The assessment was followed by the prioritization of the findings and the development of regional Community Health Improvement Plans. The Capital District Public Health Prioritization Task Force was formed to review data analyses prepared by HCDI and to select the top two priorities for the region. Data presentations were given at the meetings to provide summarize available data on the leading problems in the capital district. Health indicators from the 2013 Community Health Needs Assessment were included in the presentation if: At least two of the three Albany, Rensselaer, and Schenectady county rates were significantly higher than the New York State, excluding New York City data; or At least two of the three county rates are in the highest risk quartile in the state; or Rates for the health condition worsened over the past decade in two of the three counties; or The health condition was a leading cause of death in two of the three counties; or Disparity between rates was clearly evident in sub-populations; or There were a high absolute number of cases in the region. Health indicators that met the criteria were included from all five of the Prevention Agenda areas: Promote a Healthy and Safe Environment, Prevent Chronic Diseases, Prevent HIV/STDs, Vaccine Preventable Diseases and Healthcare-Associated Infections, Promote Healthy Women, Infants, and Children, and Promote Mental Health and Prevent Substance Abuse. More than a dozen health indicators were selected to be presented. Available data on prevalence, emergency department visits, hospitalizations, mortality and trends were included for each indicator. Equity data for gender, age, race/ethnicity, and neighborhood groupings were presented as available. After each health indicator was presented, a discussion was held to answer any questions or for individuals to share their experiences with the health condition in the population. Participants did a preliminary vote on the importance of the condition in the community based on the impact of the condition on quality of life and cost of health care, if there was community awareness and concern about the condition, and the opportunity to prevent or reduce the burden of this health issue on the community. They were provided with a Prioritization Tracking Tool to record their own comments and measure their thoughts on the severity, community values, and opportunity regarding each health indicator. Task force participants shared their views for each indicator on three qualitative dimensions: the impact of the condition on quality of life and cost of health care, community awareness and concern about the condition, and the opportunity to prevent or reduce the burden of this health issue on the community. A basic representation of the groups view on each dimension was recorded. Task force participants were provided with a Prioritization Tracking Tool to record their own comments about the importance of each indicator as a priority and record their thoughts on the severity, community values, and opportunity for improvement.The result was a report of findings, the 2013 Community Health Needs Assessment, which includes county and zip code group level analyses of causes of death, hospitalizations and emergency room use. Twenty year trends, and gender, ethnicity and age patterns were also examined when available.These findings were reviewed with representatives from hospitals (including Albany Med), local health departments, community health centers, community organizations, members of the public and business representatives to prioritize the most pressing regional health issues considering: the number of people impacted, a disproportionate burden in New Yorks Capital Region, a disproportionate burden for a particular subpopulation, impact on quality of life, cost of health care, community concern and the opportunity to prevent or reduce its prevalence.Community health improvement task forces were formed around the top priorities of chronic diseases diabetes and asthma, and behavioral health (opiate abuse and smoking among the mentally ill). Albany Med is an active participant on each of the task forces, which have developed community health improvement plans that build upon community resources with evidence-based strategies for improving health. The plans set the agenda for a 3 year community-wide effort, from 2014-2017, to make significant improvements in each of these health areas.Albany Meds Community Service Plan is built around these health improvement goals and evidence-based strategies. Most facilities in New York State define their health planning service areas by zip code, not county. Because of our role in the region, we define our health planning service area by county. About 2/3 of our patients are from the four counties in our immediate Primary Service Area Albany, Rensselaer, Saratoga and Schenectady counties. Here, we function to a larger degree as a primary hospital, particularly for Albany, Rensselaer and Schenectady Counties. Outside the Capital Region we complement existing acute services. Our 24/7 access to specialists unavailable elsewhere in the region continues to drive an increase in patient transfers. In 2013, for example, more than 9,800 patients were transferred from other hospitals and health facilities throughout the region.For Community Health Needs Assessment and Improvement PlanThe 830,000 residents of the Capital District depend on Albany Medical Center for a vast range of preventive services and advanced care. Analysis of Saratoga County utilization, however, shows that residents particularly from the southern region depend on Albany Med for high-end care. As a result, the percentage of Albany Med patients diagnosed with hypertension who lowered their blood pressure through treatment rose to 90 percent in 2014 from 66 percent in 2012.) - Prevent HIV/STDs NYS Department of Health designated Albany Medical Center as a regional AIDS Treatment Center, charged with the provision of a wide range of services to people with human immunodeficiency virus (HIV) infection and AIDS. - We provide a wide range of services on a 24-hour basis for people with HIV infection and AIDS and our case managers will help patients identify resources available to help pay bills if needed. Albany Med strives to provide the most comprehensive services, including medical, social, nutritional, psychological, educational and clinical research services to both hospitalized patients and outpatients regardless of their location or ability to pay. - Our goal is to offer accessible, quality health care to those who become HIV-infected in the service area of Albany Medical Center. We help coordinate support services for patients seeking care by working with community organizations. Our expert staff, health care providers, and community members receive continued education about HIV infection and provide compassionate and appropriate care to patients. Some of the services we provide include patient education, a 24-hour provider hotline, nutrition program, substance abuse programOther examples of ways that Albany Med furthers its exempt purpose include: Albany Medical Center accepts all patients, regardless of ability to pay. We have never and will never allow financial status to impact the level of care we provide despite the fact that the financial impact is significant. We perform this financial mission for our community in an effort to keep those most in need as healthy as we can.We are the dominant provider of services for the Medicaid and uninsured populations, and as such, have been designated as a Safety Net Provider. Albany Med has been designated as a lead hospital for one of upstate New Yorks DSRIP networks. With more than 170 partner organizations, we are beginning to develop implementation plans to address health challenges and barriers across 11 projects for Medicaid beneficiaries within a 5-county region. Because of the vast range of programs and services available at academic medical centers, Albany Med retains a wide array of physician specialists not found elsewhere in our region, including critical care medicine and surgery; HIV medicine; neuro-interventional care; pediatric subspecialties; trauma; plastic reconstructive surgery; surgical oncology; endovascular surgery; transplant surgery; and various other critical services.We have many enrollment assistance programs in place and continue to expand those. The fee is solely covered by Albany Med on behalf of our patients.As the regions Level I Trauma Center and one of the busiest in New York State, we provide comprehensive care to the critically ill and injured that is not available at other hospitals. As a result we receive thousands of patient transfers from other hospitals and health facilities due to the absence of a qualified specialist on staff or on-call at the time of need or because the patient required a higher level of care that the hospital could not provide. As a result, we receive a significant number of transfers from other hospitals and health facilities. Over the past 5 years, transfers increased 54%, to just under 10,000 patients in 2013.Our medical staff includes over 600 community-based physicians.We provide Continuing Medical Education to over 4,000 medical professionals annually.Albany Med invests in meeting community health needs at all levels, from community-based programs such as diabetes and other chronic disease management to highly specialized inpatient programs like or end-stage Congestive Heart Failure Clinic and Chronic Kidney Disease Clinic.Additional disease-based primary care outpatient programs assist patients with HIV/AIDS, cystic fibrosis, hemophilia, Gauchers Disease, and provide services through our psychiatry clinic.Albany Med runs the regions sexual assault examiner program.In addition to serving as a resource for specialty services and transfer of patients, we work with other health systems and service providers to ensure that community needs are met.
