<ExplanationTxt>THE SYSTEM IS A NOT-FOR-PROFIT CORPORATION THAT HAS BEEN RECOGNIZED AS TAX EXEMPT PURSUANT TO SECTION 501(C)(3) OF THE INTERNAL REVENUE CODE. THE SYSTEM APPLIES ACCOUNTING POLICIES THAT PRESCRIBE WHEN TO RECOGNIZE AND HOW TO MEASURE THE FINANCIAL STATEMENT EFFECTS OF INCOME TAX POSITIONS TAKEN OR EXPECTED TO BE TAKEN ON ITS INCOME TAX RETURNS. THESE RULES REQUIRE MANAGEMENT TO EVALUATE THE LIKELIHOOD THAT, UPON EXAMINATION BY THE RELEVANT TAXING JURISDICTIONS, THOSE INCOME TAX POSITIONS WOULD BE SUSTAINED. BASED ON THAT EVALUATION, THE SYSTEM ONLY RECOGNIZES THE MAXIMUM BENEFIT OF EACH INCOME TAX POSITION THAT IS MORE THAN 50% LIKELY OF BEING SUSTAINED. TO THE EXTENT THAT ALL OR A PORTION OF THE BENEFITS OF AN INCOME TAX POSITION ARE NOT RECOGNIZED, A LIABILITY WOULD BE RECOGNIZED FOR THE UNRECOGNIZED BENEFITS, ALONG WITH ANY INTEREST AND PENALTIES THAT WOULD RESULT FROM DISALLOWANCE OF THE POSITION. SHOULD ANY SUCH PENALTIES AND INTEREST BE INCURRED, THEY WOULD BE RECOGNIZED AS OPERATING EXPENSES. BASED ON THE RESULTS OF MANAGEMENT'S EVALUATION, NO LIABILITY IS RECOGNIZED IN THE ACCOMPANYING BALANCE SHEET FOR UNRECOGNIZED INCOME TAX POSITIONS. FURTHER, NO INTEREST OR PENALTIES HAVE BEEN ACCRUED OR CHARGED TO EXPENSE AS OF DECEMBER 31, 2013 AND 2012 OR FOR THE YEARS THEN ENDED. THE SYSTEM'S TAX RETURNS ARE SUBJECT TO POSSIBLE EXAMINATION BY THE TAXING AUTHORITIES. FOR FEDERAL INCOME TAX PURPOSES, THE TAX RETURNS ESSENTIALLY REMAIN OPEN FOR POSSIBLE EXAMINATION FOR A PERIOD OF THREE YEARS AFTER THE RESPECTIVE FILING DEADLINES OF THOSE RETURNS.</ExplanationTxt>

<ExplanationTxt>FINANCIAL COUNSELOR PROCEDURE: IF A PATIENT IS UNINSURED, THE HOSPITAL FINANCIAL COUNSELOR WILL ATTEMPT TO IDENTIFY IF THE PATIENT IS INDIGENT AT THE TIME OF ADMISSION OR WHILE THE PATIENT IS IN-HOUSE. ONCE IDENTIFIED, THE FINANCIAL COUNSELOR SHOULD REFER THE PATIENT FOR ASSISTANCE IN DETERMINING WHETHER THEY ARE ELIGIBLE FOR MEDICAL ASSISTANCE. THE MEDICAID ELIGIBILITY VENDOR WILL WORK WITH THE PATIENT TO DETERMINE IF THEY ARE ELIGIBLE FOR THE MEDICAID PROGRAM AND ASSIST THEM IN FILING THE NECESSARY DOCUMENTS. IF THE PATIENT IS FOUND TO BE INELIGIBLE FOR MEDICAID, PATIENTS WHO HAVE NO THIRD-PARTY COVERAGE AND/OR BENEFITS AVAILABLE WILL BE OFFERED THE FACILITY FLAT RATE. IF THE PATIENT STATES THAT THEY ARE UNABLE TO AFFORD THE FACILITY ESTABLISHED SELF-PAY RATE, THEY WILL BE ASSESSED FOR FINANCIAL ASSISTANCE UNDER THE GUIDELINES SET FORTH BY THE FINANCIAL ASSISTANCE POLICY. MEDICAID ELIGIBILITY VENDOR PROCEDURE PATIENTS WITHOUT INSURANCE COVERAGE WILL BE SCREENED FOR POTENTIAL LINKAGE TO GOVERNMENT PROGRAMS BEFORE ANY FINANCIAL ASSISTANCE APPLICATION IS PROCESSED. DURING THE SCREENING PROCESS, THE REPRESENTATIVE SHOULD SECURE A CONFIDENTIAL FINANCIAL APPLICATION. THE APPLICATION IS TO BE USED FOR POTENTIAL FINANCIAL ASSISTANCE DETERMINATION ONLY IN THE EVENT THE REPRESENTATIVE IS UNABLE TO OBTAIN ELIGIBILITY FOR THE PATIENT FOR GOVERNMENT PROGRAMS REIMBURSEMENT. IF, DURING THE INITIAL INTERVIEW WITH THE PATIENT, IT IS REVEALED THAT THERE IS NO VIABLE SOURCE OF PAYMENT AND THE PATIENT WILL NOT QUALIFY FOR ANY GOVERNMENTAL PROGRAMS, THE MEDICAID ELIGIBILITY SPECIALIST REPRESENTATIVE WILL: 1.OFFER THE PATIENT A CONFIDENTIAL FINANCIAL APPLICATION FORM. 2.ASSIST THE PATIENT IN COMPLETING A CONFIDENTIAL FINANCIAL APPLICATION, WHICH WILL DOCUMENT THE PATIENT'S FINANCIAL NEED. 3.OBTAIN THE PATIENT'S SIGNATURE ON THE CONFIDENTIAL FINANCIAL APPLICATION AND FORWARD THE APPLICATION TO THE FINANCIAL COUNSELOR, AS DEEMED APPROPRIATE. IN THE EVENT THAT IT IS DETERMINED THAT THE PATIENT HAS SOME ABILITY TO PAY, PAYMENT ARRANGEMENTS WILL BE MADE ACCORDING TO THE PATIENT'S MEANS. ANY DISCOUNT GIVEN TO THE PATIENT WILL BE PRESENTED TO THE PFS DIRECTOR. FINAL DETERMINATION WILL BE BASED NOT ONLY ON INCOME, BUT A REVIEW OF THE APPLICANT'S PERSONAL AND BUSINESS ASSETS AND LIABILITIES. FOR THOSE ACCOUNTS THAT REMAIN IN THE DETERMINATION PHASE FOR MEDICAID ELIGIBILITY PAST 30 DAYS FROM ASSIGNMENT WITH NO GOVERNMENT PROGRAM LINKAGE AND THAT MEET THE FINANCIAL CRITERIA FOR FINANCIAL ASSISTANCE, MEDICAID ELIGIBILITY SPECIALIST SHOULD HAVE GATHERED ALL SUBSTANTIAL INFORMATION TO ENABLE THE HOSPITAL TO UTILIZE THE FINANCIAL ASSISTANCE POLICY. INCLUDED IN THE FINANCIAL ASSISTANCE PACKET IS A CONFIDENTIAL FINANCIAL APPLICATION. IF THE MEDICAID ELIGIBILITY SPECIALIST REPRESENTATIVE HAS EXHAUSTED ALL EFFORTS TO SECURE ALL NECESSARY VERIFICATIONS, THE APPLICATION FOR FINANCIAL ASSISTANCE SHOULD BE SUBMITTED TO PFS FOR REVIEW AND FINALIZATION WITHOUT THE VERIFICATIONS. EARLY OUT PROCEDURE: THERE WILL BE CASES WHERE A PATIENT'S ABILITY TO PAY MAY CHANGE AFTER THEY LEAVE THE HOSPITAL. IN ORDER TO RECOGNIZE THIS, THE EARLY OUT AGENCY MUST FOLLOW THESE STEPS TO DO AN INITIAL SCREENING FOR FINANCIAL ASSISTANCE. IF IT IS DETERMINED BY THE ANSWERS OF THE PATIENT THAT THEY MAY QUALIFY FOR FINANCIAL ASSISTANCE, THE ACCOUNT IS THEN RETURNED BACK TO THE FACILITY BY CHANGING THE PAYOR ID TO FINANCIAL ASSISTANCE. A CONFIDENTIAL FINANCIAL APPLICATION WILL BE SENT TO THE PATIENT AND DETERMINATION WILL BE MADE BY NMC UNDER THE NON-DISCRETIONARY SEGMENT OF THIS POLICY. THE PATIENT WILL BE REQUIRED TO COMPLETE THE APPLICATION AND PROVIDE THE NECESSARY DOCUMENTATION TO PROCESS THE APPLICATION. IF THE APPLICATION IS NOT RETURNED OR THE REQUIRED DOCUMENTATION IS INCOMPLETE TO THE BUSINESS OFFICE WITHIN 30 DAYS, THE ACCOUNT WILL BE ASSIGNED BACK TO EARLY OUT TO RESUME COLLECTION EFFORTS. ACCOUNTS GREATER THAN 6 MONTHS FROM THE SERVICE DATE MAY NOT BE ELIGIBLE FOR THE FINANCIAL ASSISTANCE PROGRAM. IF THE ACCOUNT EXCEEDS 6 MONTHS, IT WILL BE DETERMINED BY THE PFS DIRECTOR IF THE PATIENT WILL BE ALLOWED TO APPLY. AT THE TIME OF BAD DEBT REVIEW, E/0 WILL MAKE A DETERMINATION FOR A FINANCIAL ASSISTANCE RECOMMENDATION UNDER THE DISCRETIONARY PORTION OF THIS POLICY. THE ACCOUNTS, ONCE DETERMINED FOR RECOMMENDATION, WILL THEN BE RETURNED TO THE FACILITY FOR REVIEW AND APPROVAL BY THE PFS DIRECTOR, CONTROLLER AND/OR ASSISTANT ADMINISTRATOR FOR FISCAL SERVICES. IF THE ACCOUNT IS APPROVED BY TWO OF THE THREE ABOVE STATED MANAGERS FOR NONDISCRETIONARY FINANCIAL ASSISTANCE, AN ADJUSTMENT WILL BE MADE TO THE ACCOUNT WITH THE DESIGNATED WRITE-OFF CODE. ALL OTHER ACCOUNTS WILL BE RETURNED TO E/O TO RESUME COLLECTION EFFORTS. DOCUMENT REQUIREMENTS WHEN A PATIENT IS IDENTIFIED AS UNINSURED, OR REQUESTS A CONFIDENTIAL FINANCIAL APPLICATION, NMC PERSONNEL OR A REPRESENTATIVE OF NMC SHALL PROVIDE THE PATIENT WITH THE CONFIDENTIAL FINANCIAL APPLICATION. THE PATIENT MUST COMPLETE AND SIGN THE CONFIDENTIAL FINANCIAL APPLICATION. SOME OR ALL OF THE FOLLOWING DOCUMENTATION WILL BE REQUIRED AT THE TIME OF THE APPLICATION: 1.MEDICAL ASSISTANCE ELIGIBILITY SCREENING/DENIAL NOTICE THE MEDICAID ELIGIBILITY VENDOR MUST HAVE SCREENED PATIENT PRIOR TO APPLICATION. SCREENING RESULTS SHOULD BE SHOWN BY DOCUMENTATION IN THE MEDITECH SYSTEM. 2.INCOME TAX RETURNS FOR THE MOST RECENTLY FILED YEAR. 3.PROOF OF INCOME AND ADJUSTED GROSS INCOME SUCH AS THE PATIENT'S YEAR END PAY STUBS, W-2, OR THEIR ANNUAL MOST RECENT TAX RETURN. 4.BANK STATEMENTS - CHECKING AND SAVINGS PAST 60 DAYS 5.ANY OTHER DOCUMENTATION THAT MAY SERVE AS PROOF OF ELIGIBILITY FOR FINANCIAL ASSISTANCE 6.A CREDIT BUREAU REPORT MAY BE NEEDED IN DETERMINING ELIGIBILITY THE FINANCIAL RESOURCES OF A PARENT OR GUARDIAN MAY BE CONSIDERED IN DETERMINING THE ELIGIBILITY OF A PATIENT WHO IS DEPENDENT ON THE PARENT OR GUARDIAN FOR FINANCIAL SUPPORT. THE FINANCIAL COUNSELOR WILL APPLY FPG GUIDELINES BY USING THE FPG TABLE, WHICH IS UPDATED ANNUALLY. <ExplanationTxt>AMOUNTS INCLUDED ON PART III LINE 2 REPRESENT THE AMOUNT OF CHARGES CONSIDERED UNCOLLECTIBLE AFTER REASONABLE ATTEMPTS TO COLLECT, AND WRITTEN OFF TO BAD DEBT EXPENSE. NEWTON MEDICAL CENTER IS THE SOLE PROVIDER OF 24 HOUR, 7 DAY A WEEK EMERGENT CARE IN NEWTON COUNTY, GEORGIA. IN ADDITION, MANY CITIZENS OF NEWTON COUNTY HAVE NO ACCESS TO PRIMARY CARE AS A RESULT OF THEIR INABILITY OR UNWILLINGNESS TO PAY FOR SERVICES. OTHERS HAVE NO ACCESS TO PRIMARY CARE PHYSICIANS DUE TO THEIR PARTICULAR INSURANCE PLAN. THIS IS ESPECIALLY TRUE OF MEDICAID RECIPIENTS. MANY OF THESE COMMUNITY MEMBERS SEEK CARE IN THE EMERGENCY ROOM AT NEWTON MEDICAL CENTER FOR BOTH EMERGENT AND NON-EMERGENT HEALTH CARE NEEDS. SOME OF THESE PATIENTS REQUIRE EXTENDED CARE AS INPATIENTS OR EXTENSIVE OBSERVATION. OUR EMERGENCY ROOM ALSO SERVES AS A HOLDING AREA FOR THE MENTALLY ILL, OFTEN KEEPING THESE CITIZENS FOR SEVERAL DAYS BEFORE THEY ARE PLACED IN A MORE APPROPRIATE SETTING. MANY OF THESE PATIENTS DO NOT MEET CRITERIA OR ARE NON-COMPLIANT IN COMPLETING THE PROCESS NEEDED TO SHOW ELIGIBILITY FOR CHARITY CARE. OTHERS ARE UNWILLING TO PAY, EVEN THOUGH THEY MAY HAVE THE MEANS TO DO SO. MANY INSURED PATIENTS HAVE HIGH DEDUCTIBLES AND ALSO FAIL TO PAY FOR THEIR CARE. IN ADDITION, NEWTON MEDICAL CENTER PROVIDES SERVICES TO PATIENTS ON AN ELECTIVE BASIS THAT ARE UNABLE TO MEET THEIR OUT OF POCKET EXPENSES UNDER THEIR INSURANCE PROGRAM, MEDICARE, OR OTHER THIRD PARTY PROVIDER. IN THESE CASES, THE HOSPITAL DOES WORK WITH THE PATIENTS TO MAKE PAYMENT ARRANGEMENTS, HOWEVER, MANY PATIENTS FAIL TO FOLLOW THROUGH OR ARE FINANCIALLY UNABLE TO COMPLETE THOSE ARRANGEMENTS. CARE IS PROVIDED DESPITE THE RISK THAT MANY PATIENTS WILL NOT MAKE AGREED UPON PAYMENTS. IN ALL OF THE CASES DESCRIBED ABOVE, THE RESULT IS THE HOSPITAL INCURRING BAD DEBT EXPENSE BY PROVIDING HEALTH CARE SERVICES THAT ARE NEEDED BY COMMUNITY MEMBERS. ACCORDINGLY, IT IS CLEAR THAT THE COMMUNITY BENEFITS FROM THE HOSPITAL INCURRING BAD DEBT EXPENSE FOR CARE PROVIDED.</ExplanationTxt>

<ExplanationTxt>ACCOUNTS RECEIVABLE ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF ACCOUNTS RECEIVABLE, THE SYSTEM ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. MANAGEMENT REGULARLY REVIEWS DATA ABOUT THESE MAJOR PAYOR SOURCES OF REVENUE IN EVALUATING THE SUFFICIENCY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE SYSTEM ANALYZES CONTRACTUALLY DUE AMOUNTS AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY (FOR EXAMPLE, FOR EXPECTED UNCOLLECTIBLES, DEDUCTIBLES AND COPAYMENTS ON ACCOUNTS FOR WHICH THE THIRD-PARTY PAYOR HAS NOT YET PAID, OR FOR PAYORS WHO ARE KNOWN TO BE HAVING FINANCIAL DIFFICULTIES THAT MAKE THE REALIZATION OF AMOUNTS DUE UNLIKELY). FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS (WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND PATIENTS WITH DEDUCTIBLES AND COPAYMENT BALANCES DUE FOR WHICH THIRD-PARTY COVERAGE EXISTS FOR PART OF THE BILL), THE SYSTEM RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE STANDARD RATES (OR THE DISCOUNTED RATES IF NEGOTIATED) AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE SYSTEM'S ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS INCREASED FROM 71% OF SELF-PAY ACCOUNTS RECEIVABLE AT DECEMBER 31, 2012, TO 81% OF SELF-PAY ACCOUNTS RECEIVABLE AT DECEMBER 31, 2013. IN ADDITION, THE SYSTEM'S SELF-PAY ALLOWANCES INCREASED APPROXIMATELY 2,100,000 FROM APPROXIMATELY 6,800,000 FOR 2012 TO 8,900,000 FOR 2013. AN EXAMPLE OF A CONFLICT OF INTEREST OCCURRING AND HOW IT WOULD BE DEALT WITH IS DESCRIBED HERE. CAPITAL BUDGETS AND CAPITAL CONSTRUCTION ARE APPROVED BY THE ENTIRE BOARD OF DIRECTORS. IF A CONFLICT OF INTEREST WAS PRESENT, IT WOULD BE IDENTIFIED AT THAT TIME OR SHORTLY THEREAFTER AND CERTAINLY BEFORE ANY AGREEMENTS WERE SIGNED. IF A CONFLICT OF INTEREST WAS IDENTIFIED IT WOULD BE BROUGHT TO THE APPROPRIATE PERSON'S ATTENTION. THIS PERSON WOULD REMOVE HIMSELF FROM ANY POSITION OF KNOWLEDGE OR INFLUENCE ON THE ISSUE AT QUESTION. COMPETITIVE QUOTES WOULD BE SOLICITED FROM SIMILAR TYPES OF VENDORS. THEN THE BEST AND LOW COST VENDOR WOULD BE SELECTED. EACH YEAR BOARD MEMBERS AND MEMBERS OF MANAGEMENT REVIEW THE CONFLICT OF INTEREST POLICY. THEY THEN COMPLETE A FORM WHICH IDENTIFIES WHETHER OR NOT THEY HAVE OR HAVE HAD A CONFLICT OF INTEREST IN THEIR DEALINGS WITH THE HOSPITAL. THIS FORM IS SIGNED AND RETAINED IN THE ADMINISTRATOR'S OFFICE. AN EXISTING CONFLICT IS FIRST REVIEWED BY ONE OR MORE EXECUTIVE MANAGEMENT MEMBERS. IF THE MATTER CANNOT BE RESOLVED AT THAT LEVEL IT WOULD BE REFERRED TO THE CORPORATE COMPLIANCE COMMITTEE OR TO THE BOARD OF DIRECTORS FOR CLARIFICATION AND RESOLUTION. RESTRICTIONS PLACED ON THE APPROPRIATE PERSON WOULD RANGE FROM NOT HAVING ACCESS TO COST INFORMATION OF OTHER VENDORS, NOT BEING INVOLVED IN NEGOTIATIONS, NOT BEING INVOLVED IN SELECTING VENDORS TO SELL TO THE HOSPITAL.
