<Desc>AGING AND CAREGIVER SERVICES offers a wide range of programs and services to improve the quality of life of individuals age 60+. In 2013, 1044 seniors (unduplicated count) received services. -Case Management: Our goal is to provide older adults the opportunity to pursue a life of optimum independence and satisfaction, while living in their desired environment, which is usually their own home. Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the services required to meet the client's health and human service needs. -The Center for Holocaust Survivors: provides case management; home visits; restitution assistance; counseling and referral; social therapeutic activities; free hearing tests; and safety/adaptive equipment. -Ways to Wellness: The Council on Aging (COA) contracted JFS to provide wellness programming. Wellness is an active process of becoming aware of and making choices toward a more successful existence. JFS is the only non-senior center chosen by COA because JFS specializes in a "senior center without walls" approach to meeting needs in the community.-Independent Living Assistance (ILA): JFS is one of only two agencies contracted by the Council on Aging of Southwestern Ohio to provide assistance with banking, bill paying, insurance claims, assistance in obtaining benefits, and the like. -Options Cincinnati: is a program to meet the needs of aging residents of two naturally occurring retirement communities providing case management, wellness programs, and social events to people in their own home environments. -Immigration Services: Our Russian-speaking specialists helped refugees from the former Soviet Union learn English, successfully integrate into their new community, and gain American citizenship. Interpretation and translation services help in day-to-day situations such as doctor visits.</Desc>

Payments were made for rent, mortgage, utilities, medical care, transportation, and other necessities. In most cases, recipients were assisted by case managers to be sure that each was receiving all benefits and assistance for which they qualified. -Homelessness Prevention and Rapid Rehousing: Jewish Family Service is one of only six area social service agencies selected by the Continuum of Care for the Homeless and the United Way of Greater Cincinnati to provide case management for this special HUD program to avert eviction of renters. -Mental Health Case Management: individuals with severe mental illness received specific and ongoing case management services with the goal of helping this highly vulnerable population maintain stability and security. -Information and Referral: individuals received accurate information or referral to community resources that assisted them to solve their problem or meet their needs. -Counseling: JFS offered services by licensed social workers, on a sliding-fee scale, for people in crisis or needing to make changes to improve and strengthen the quality of their lives.</Desc>
