As of December 31, 2013, CCDA had no uncertain tax positions that qualify for recognition or disclosure in the financial statements. CCDA files information tax returns in the U.S. and various states. CCDA is generally no longer subject to U.S. federal and state income tax examinations by tax authorities for years before 2009. Form 990, Part VI, Section A, line 2 Chris & Tammi Jehle - Family Relationship Form 990, Part VI, Section B, line 11 The Form 990 is prepared and reviewed by an independent CPA firm. The form is then reviewed by management and presented to the Finance Committee before filing. The Finance Committee, led by the Board Treasurer, is responsible for financial oversight of the organization. The board also receives a copy of the Form 990 before filing with the IRS. Form 990, Part VI, Section B, line 12c The conflict of interest policy is provided to each Board Member at the annual meeting. Members must sign a statement acknowledging they've received, read, understand, and agree to comply with the policy. The statements are reviewed by the CEO and COO. If new conflicts arise, members must promptly disclose. If a true conflict exists (majority vote by board), that member would be precluded from discussion and voting on the matter. Violations of the policy result in appropriate disciplinary or corrective action from the governing board. Form 990, Part VI, Section B, line 15a Line 15a - Compensation for the CEO is decided by the Board of Directors. The board uses comparable market data in the determination process and records its deliberation and decision in the board minutes. This process last took place during the tax year ending 12/31/13. Form 990, Part VI, Section C, line 19 The organization's governing documents, conflict of interest policy, and financial statements are available upon request. Form 990, Part XII, Line 2c: The organization's Board assumes responsibility for oversight of the audit of its financial statements and selection of its independent accountant.
