Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts On an annual basis the System sends a Conflict of Interest statement to all Board Members, Officers, Directors, Key Employees, and management staff asking them to disclose any potential conflict of interest that could compromise their position within the Corporation. These statements are reviewed by the CFO of the Corporation to determine if there are any issues that require further follow-up. All disclosures are presented to the Audit Committee and the CFO points out any disclosures that he feels are significant. If there is a disclosure that is of concern, a determination is made for further follow-up.
