THROUGH 1I The Organization is a member of the Kaiser Permanente Medical Care Program and participated in and benefited from lobbying activities conducted at the regional and national levels for the benefit of its enrolled members and for the health care industry as a whole. As an organization exempt from income tax under Internal Revenue Code Section 501(c)(3), Health Plan has a policy prohibiting internal involvement in any political campaigns. This policy is closely monitored for compliance. During the year this Organization may have made comments or statements concerning legislation which may affect the health care industry. Health Plan may have engaged in conversations with and/or written communications to various Federal, state, and local officials regarding matters which affected the healthcare industry as a whole. The amount of time and money involved in the activities is detailed on lines a through i. Health Plan employs individuals, including one or more registered lobbyists and/or may retain one or more professional consultants to represent Health Plan's interests in various legislative and regulatory bodies and from time-to-time to keep informed about Federal and state legislation having an impact on Health Plan's charitable activities as an exempt Health Maintenance Organization. These individuals attempt to ensure that proposed legislation and enacted laws are compatible with the interests of Health Plan and its members by performing the following activities: - Collecting, analyzing and distributing within the Organization, public and private policy recommendations regarding proposed legislation that affect the operation of Health Plan and its ability to provide quality health and medical care services to its members in a cost effective environment. - Providing appropriate informational materials to legislators and to their staffs that pertain to matters of common interest in the health care community and in the not-for-profit community. - Also by preparing written and oral testimony, these individuals appear at legislative hearings, monitor legislative proceedings and meet with legislators and/or their staffs regarding issues pertinent to the mission of Health Plan. CARE AND COVERAGE FOR LOW-INCOME PEOPLE IN 2012, KAISER FOUNDATION HEALTH PLAN, INC. EXPENDED APPROXIMATELY $756 MILLION (AT COST, NET OF $466 MILLION OF RELATED REVENUES) TO ADDRESS THE FINANCING AND DELIVERY OF HEALTH CARE FOR POPULATIONS VULNERABLE DUE TO SOCIO-ECONOMIC STATUS, ILLNESS, ETHNICITY, AGE, OR OTHER FACTORS. PROGRAM BENEFICIARIES (UNDER- AND UNINSURED) RECEIVED FREE OR DISCOUNTED CARE. 1. CHARITABLE CARE (MEDICAL FINANCIAL ASSISTANCE AND CHARITABLE HEALTH COVERAGE PROGRAMS) KFHP PROVIDES CHARITY CARE TO LOW-INCOME VULNERABLE POPULATIONS THROUGH THE MEDICAL FINANCIAL ASSISTANCE (MFA) AND CHARITABLE HEALTH COVERAGE (CHC) PROGRAMS. IN 2012, KFHP SPENT APPROXIMATELY $213 MILLION (AT COST, NET OF $24 MILLION OF RELATED REVENUES) ON UNDER- AND UNINSURED PATIENTS. 1) MEDICAL FINANCIAL ASSISTANCE (MFA) PROGRAM - IN CALIFORNIA, THE MFA PROGRAM'S ELIGIBILITY CRITERIA ALLOWS MOST PATIENTS BELOW 350% OF THE FEDERAL POVERTY GUIDELINES (FPG) TO RECEIVE FULL WRITE OFF OF MEDICAL CHARGES. UNINSURED PATIENTS WHO DO NOT QUALIFY FOR MFA BUT MAKE LESS THAN 400% OF FPG WILL RECEIVE UP TO A 70% DISCOUNT ON CHARGES. ADDITIONALLY, ANY PATIENT EXPERIENCING FINANCIAL HARDSHIP DUE TO HIGH MEDICAL COSTS RELATIVE TO THEIR INCOME MAY QUALIFY FOR THE PROGRAM UNDER "SPECIAL CIRCUMSTANCES". IN 2012, THE PROGRAM ASSISTED MORE THAN 146,000 QUALIFYING APPLICANTS IN CALIFORNIA. - IN HAWAII, THE MFA PROGRAM HELPS PEOPLE WITH VERY LIMITED OR NO FINANCIAL RESOURCES TO OBTAIN MEDICALLY NECESSARY CARE. THE PROGRAM IS OPEN TO MEMBERS AND NONMEMBERS WHO CANNOT PAY FOR ALL OR PART OF THE COST OF HEALTH CARE SERVICES AND SUPPLIES. INDIVIDUALS APPLYING FOR THE PROGRAM HAVE TO EXHAUST ANY PRIVATE OR PUBLIC SOURCES OF SUPPORT AND MUST MEET THE FINANCIAL ELIGIBILITY CRITERIA. IN 2012, THE MFA PROGRAM IN HAWAII ASSISTED MORE THAN 2,300 QUALIFYING APPLICANTS. 2) CHARITABLE HEALTH COVERAGE PROGRAM CHARITABLE HEALTH COVERAGE (CHC) IS A UNIQUE APPROACH TO CARING FOR LOW-INCOME UNINSURED PERSONS IN THE COMMUNITY. ELIGIBLE PARTICIPANTS RECEIVE A REGULAR KAISER PERMANENTE MEMBERSHIP CARD AND ACCESS TO THE FULL RANGE OF SERVICES AND PROVIDERS - A MUCH BETTER ALTERNATIVE TO BRIEF AND COSTLY EMERGENCY ROOM VISITS OR HOSPITALIZATION. THIS ALLOWS US TO INVEST IN THE LONGER TERM HEALTH OF PATIENTS AND THE COMMUNITIES. SINCE INCEPTION IN THE EARLY 1980S, CHC PROGRAMS HAVE MADE A REAL DIFFERENCE IN THE LIVES OF LOW INCOME PEOPLE WHO MIGHT OTHERWISE HAVE NO OTHER SOURCE OF CARE. - THE FOLLOWING ARE THE CHC PROGRAMS OFFERED WITHIN THE STATE OF CALIFORNIA: KAISER PERMANENTE CHILD HEALTH PLAN (KPCHP) THE CHILD HEALTH PLAN PROVIDES MEDICAL AND DENTAL COVERAGE TO ELIGIBLE CHILDREN (BIRTH THROUGH AGE 18) IN CALIFORNIA FAMILIES WITH INCOME UP TO 300% OF THE FEDERAL POVERTY GUIDELINES WHO DO NOT HAVE ACCESS TO EMPLOYER-SUBSIDIZED COVERAGE AND DO NOT QUALIFY FOR PUBLIC PROGRAMS BECAUSE OF FAMILY INCOME OR IMMIGRATION STATUS. THE CHILD HEALTH PLAN PROVIDES COMPREHENSIVE BENEFITS INCLUDING PREVENTIVE CARE, INPATIENT AND OUTPATIENT SERVICES, PRESCRIPTION DRUGS, AND VISION AND DENTAL CARE. PREMIUMS ARE $8 OR $15 PER CHILD PER MONTH, DEPENDING ON FAMILY INCOME, FOR A MAXIMUM OF THREE CHILDREN (ADDITIONAL CHILDREN ARE COVERED FREE OF CHARGE). AT THE END OF 2012, APPROXIMATELY 81,000 CHILDREN WERE RECEIVING CARE AND COVERAGE THROUGH THIS PROGRAM. KAISER PERMANENTE STEPS PLAN THE STEPS PLAN PROVIDED A SMALL NUMBER OF CALIFORNIA MEMBERS THE OPPORTUNITY TO CONTINUE THEIR HEALTH CARE COVERAGE AT REDUCED COST WHEN EXPERIENCING FINANCIAL DIFFICULTY DUE TO JOB LOSS, INVOLUNTARY REDUCTION IN WORK HOURS, LEGAL SEPARATION, DIVORCE, OR DEATH OF A SPOUSE. TYPICALLY, PARTICIPANTS ARE NOT ELIGIBLE FOR ANY PUBLIC OR PRIVATE GROUP HEALTH INSURANCE PLAN, AND HAVE FAMILY INCOME BETWEEN 100% AND 300% OF THE FEDERAL INCOME GUIDELINES. THE PLAN IS AVAILABLE TO PARENTS OF CHILDREN ENROLLED IN ACCESS FOR INFANTS AND MOTHERS (AIM), HEALTHY FAMILIES, OR KAISER PERMANENTE CHILD HEALTH PLAN AS WELL AS TO INDIVIDUALS PARTICIPATING IN VOCATIONAL TRAINING PROGRAMS OFFERED THROUGH GOVERNMENT, PRIVATE INDUSTRY COUNCILS, AND SOCIAL AGENCIES. THE STEPS PLAN PREMIUM IS SUBSIDIZED AT FOUR LEVELS OR STEPS: 20%, 40%, 60%, AND 80%. PARTICIPANTS ARE PLACED IN AN INITIAL PREMIUM STEP BASED ON THEIR CURRENT FAMILY INCOME. THEY REMAIN AT THE INITIAL STEP FOR ONE YEAR AND ARE THEN MOVED TO THE NEXT HIGHER STEP IN SUBSEQUENT YEARS. - THE FOLLOWING DESCRIBES THE CHC PROGRAM OFFERED WITHIN THE STATE OF HAWAII: BRIDGE PROGRAM THE BRIDGE PROGRAM PROVIDED SUBSIDIZED COVERAGE TO 157 HAWAIIANS AT THE END OF 2012. THE PROGRAM OFFERS FREE CHARITABLE COVERAGE TO YOUNG ADULTS 19-26 YEARS OLD WHO ARE AT OR BELOW 250% OF THE FEDERAL POVERTY GUIDELINES. COVERAGE PREMIUMS ARE 100% SUBSIDIZED, BUT THERE ARE LOW LEVELS OF PATIENT COST SHARING SUCH AS COPAYS FOR SOME HEALTH CARE SERVICES. THE PROGRAM MEETS COMMUNITY NEEDS BY PROVIDING COVERAGE FOR THE VULNERABLE POPULATION WHO WOULD OTHERWISE LOSE COVERAGE DUE TO AGING OUT OF QUEST, HAWAII'S MEDICAID PROGRAM. 2. PARTICIPATION IN MEDICAID AND OTHER GOVERNMENT-SPONSORED PROGRAMS KAISER FOUNDATION HEALTH PLAN PROVIDED COVERAGE AND SERVICES VALUED AT $543 MILLION (AT COST, NET OF $442 MILLION OF RELATED REVENUES) FOR MEMBERS AND NONMEMEMBERS IN GOVERNMENT-SPONSORED PROGRAMS. CALIFORNIA KAISER PERMANENTE MEDI-CAL MANAGED CARE, MEDI-CAL FEE-FOR-SERVICE, HEALTHY FAMILIES, AND HEALTHY SAN FRANCISCO ARE SPECIFIC GOVERNMENT-SPONSORED HEALTH CARE COVERAGE PROGRAMS IN CALIFORNIA. - MEDICAID/MEDI-CAL - KFHP'S MEMBERSHIP INCLUDED APPROXIMATELY 200,000 MEDI-CAL (MEDICAID) MANAGED CARE MEMBERS IN CALIFORNIA. KFHP CONTRACTS WITH VARIOUS LOCAL AND STATE GOVERNMENT ENTITIES TO PARTICIPATE IN THIS PROGRAM. - MEDI-CAL FEE-FOR-SERVICE - KFHP EXPENDED APPROXIMATELY $116 MILLION TO SUBSIDIZE CARE FOR APPROXIMATELY 68,800 MEDI-CAL FEE-FOR-SERVICE PATIENTS. - HEALTHY FAMILIES - KFHP ENROLLED AND PROVIDED ACCESS TO HEALTH CARE SERVICES FOR MORE THAN 196,000 CALIFORNIA CHILDREN IN THIS FEDERAL-AND STATE-FUNDED INSURANCE PROGRAM THAT PROVIDES LOW- AND MODERATE-INCOME FAMILIES WITH HEALTH INSURANCE FOR THEIR CHILDREN UNDER 19 YEARS OF AGE. THE PROGRAM PROVIDES COMPREHENSIVE HEALTH BENEFITS, INCLUDING DENTAL AND VISION CARE. TO QUALIFY, FAMILIES MUST HAVE A TOTAL INCOME BETWEEN 100% AND 250% OF THE FEDERAL POVERTY GUIDELINES AND THE CHILDREN MUST BE INELIGIBLE FOR MEDI-CAL COVERAGE. - HEALTHY SAN FRANCISCO - OVER 2,800 MEMBERS WERE PARTICIPATING IN THE HEALTHY SAN FRANCISCO PROGRAM AT THE END OF 2012. HEALTHY SAN FRANCISCO IS A PROGRAM CREATED BY THE CITY AND COUNTY OF SAN FRANCISCO TO MAKE HEALTH CARE SERVICES ACCESSIBLE AND AFFORDABLE FOR UNINSURED RESIDENTS. WHILE THIS PROGRAM IS NOT AN INSURANCE PLAN, IT DOES PROVIDE ACCESS TO PRIMARY CARE, PREVENTIVE SERVICES, AND HOSPITALIZATION WITHIN THE CITY AND COUNTY OF SAN FRANCISCO. HAWAII IN HAWAII, KFHP SERVED BOTH MEDICAID MEMBERS AND MEDICAID FEE-FOR-SERVICE BENEFICIARIES AND PARTICIPATED IN THE CHILDREN'S HEALTH INSURANCE PROGRAM. - QUEST AND MEDICAID FEE-FOR-SERVICE - QUEST IS A MEDICAID MANAGED CARE PROGRAM RUN BY HAWAII'S DEPARTMENT OF HUMAN SERVICES. THE STATE ADMINISTERS THE QUEST PROGRAM AND PAYS HEALTH PLAN TO PROVIDE COVERAGE OF MEDICAL AND MENTAL HEALTH SERVICES. KFHP PARTICIPATES IN THE QUEST PROGRAMS ON THE ISLANDS OF OAHU AND MAUI. KFHP AND KFH PROVIDED CARE TO OVER 20,000 INDIVIDUALS ENROLLED IN QUEST AND EXPENDED APPROXIMATELY $21 MILLION (AT COST, NET OF RELATED REVENUE) ON SUBSIDIZED MEDICAL CARE SERVICES. THE HAWAII REGION ALSO CONTRIBUTED AN ADDITIONAL $198 THOUSAND (AT COST, NET OF RELATED REVENUES) ON SUBSIDIZED CARE FOR MEDICAID FEE-FOR-SERVICE PATIENTS. - CHILDREN'S HEALTH INSURANCE PROGRAM (CHIP) - THIS GOVERNMENT PROGRAM PROVIDES HEALTH INSURANCE COVERAGE TO CHILDREN WITH FAMILY INCOME UP TO TWICE THE FEDERAL POVERTY GUIDELINE FOR HAWAII UNDER TITLE XXI OF THE SOCIAL SECURITY ACT. CHIP IS ONE OF SEVERAL AID CATEGORIES UNDER THE QUEST PROGRAM. MORE THAN 4,100 CHILDREN ENROLLED IN THIS PROGRAM WERE CARED FOR BY KFHP AND KFH IN HAWAII AT A TOTAL EXPENSE OF $2.7 MILLION (AT COST, NET OF RELATED REVENUE) IN 2012. B. COMMUNITY HEALTH INITIATIVES KAISER FOUNDATION HEALTH PLAN DESIGNS, DELIVERS, AND SUSTAINS LONG-TERM PROGRAMS THAT ENGAGE COMMUNITIES IN WORK TO IMPROVE CONDITIONS IN THEIR NEIGHBORHOODS, WORKPLACES, AND SCHOOLS. THE COMMUNITY HEALTH INITIATIVES (CHI) STREAM OF WORK STARTED WITH A FOCUS ON NUTRITION AND PHYSICAL ACTIVITY. THE FOCUS HAS SINCE EXPANDED TO ADDRESS COMMUNITY ECONOMIC DEVELOPMENT, ENVIRONMENTAL SUSTAINABILITY, AND NEIGHBORHOOD SAFETY - ADDITIONAL FACTORS THAT ARE KEYS TO PROMOTING HEALTH COMMUNITIES. 1. GRANTS & DONATIONS THE MAJORITY OF THE $10.8 MILLION SPENT ON THIS AREA IN 2012 WAS COMPRISED OF A SINGLE CONTRIBUTION TO THE EAST BAY COMMUNITY FOUNDATION FOR THE PROGRAM OFFICES FUND FOR COMMUNITY BENEFIT. THE MISSION OF THIS FUND IS TO: 1) ADDRESS THE NEEDS OF SAFETY NET PROVIDERS AND OTHER COMMUNITY BASED OR PUBLIC INSTITUTIONS THAT SERVE GROUPS FACING CULTURAL, LINGUISTIC OR SOCIO-ECONOMIC BARRIERS TO CARE; AND 2) SUPPORT THE DEVELOPMENT AND IMPLEMENTATION OF COMMUNITY-BASED HEALTH INITIATIVES THAT PROMOTE HEALTHY EATING, ACTIVE LIVING AND OTHER HEALTHY LIFESTYLE CHOICES. 2. HEALTHY EATING ACTIVE LIVING (HEAL) PROGRAMS - THE HEAL INITIATIVE COMBATS OBESITY BY PROMOTING PLACE-BASED HEALTHY EATING AND ACTIVE LIVING PROGRAMS AND INTERVENTIONS IN THE COMMUNITY. THE INITIATIVE SUPPORTS COMMUNITY HEALTH COALITIONS EFFECTING MEDICAL, ENVIRONMENTAL, AND COMMUNITY-LEVEL CHANGES SUCH AS EMPOWERING RESIDENTS TO EAT HEALTHY FOODS, CHANGING PHYSICAL AND SOCIAL ENVIRONMENTS TO PROMOTE PHYSICAL ACTIVITY, AND SUPPORTING POLICY CHANGES TO REDUCE RACIAL AND ETHNIC HEALTH DISPARITIES, PARTICULARLY THOSE RELATED TO POOR NUTRITION AND INACTIVITY. - IN 2012, KAISER PERMANENTE SUPPORTED MORE THAN 40 COMMUNITY-BASED COLLABORATIVES FOCUSING ON HEALTHY EATING AND ACTIVE LIVING, AND DEMONSTRATED IMPACTS ON HEALTH BEHAVIORS AND POPULATION-LEVEL HEALTH OUTCOMES THROUGH EVALUATON STUDIES REPORTED IN PEER-REVIEWED JOURNALS. IN ADDITION, WORKING WITH OTHER ORGANIZATIONS IN THE CONVERGENCE PARTNERSHIP WHICH INLCUDES MAJOR NATIONAL FUNDERS AND THE CENTERS FOR DISEASE CONTROL AND PREVENTION, WE SUPPORTED A NUMBER OF NATIONAL INITIATIVES FOCUSSING ON HEALTHY EATING AND ACTIVE LIVING. THESE INITIATIVES INCLUDE THE ADOPTION AND IMPLEMENATION OF THE FEDERAL HEALTHY FOOD FINANCING INITIATIVE TO BRING HEALTHY FOOD TO UNDER SERVED COMMUNITIES, AND SUPPORT FOR 14 REGIONAL CONVERGENCE INITIATIVES INLCUDING STATE AND LOCAL FUNDERS AND PUBLIC HEALTH AGENCIES WHICH ARE COLLABORATING TO ADDRESSING OBESITY THROUGH MULTI-FIELD, POLICY AND ENVIRONMENTAL CHANGE APPROACHES. C. SAFETY NET PARTNERSHIPS IN 2012, KFHP EXPENDED APPROXIMATELY $741 THOUSAND TO SUPPORT SAFETY NET PARTNERSHIPS. THE FOLLOWING ARE EXAMPLES OF GRANTS AWARDED IN ACCORDANCE WITH THE OBJECTIVES OF THE SAFETY NET PARTNERSHIPS STREAM OF WORK DURING 2012: 1. THE ACCOUNTABLE HEALTHCARE ALLIANCE FOR RURAL OAHU (AHARO) WAS AWARDED A GRANT IN THE AMOUNT OF $22,412 TO SUPPORT AN EFFORT TO IMPLEMENT THE FIRST COLLABORATIVE PEDIATRIC PROJECT IN HAWAII BY OPTIMIZING THE QUALITY OF SERVICES RELATED TO CHILDHOOD OBESITY. AN INTER-AGENCY COLLABORATION OF 3 SAFETY NET PARTNERS (KO'OLAULOA COMMUNITY HEALTH AND WELLNESS CENTER, WAIANAE COAST COMPREHENSIVE HEALTH CENTER, AND WAIMANALO HEALTH CENTER) COLLABORATED ON AN IMPLEMENTATION OF ELECTRONIC MEDICAL RECORD-BASED CLINICAL TOOLS TO ASSESS AND MANAGE OBESITY FOR CHILDREN AGES 2-18. THE OBJECTIVE IS TO ENABLE PEDIATRICIANS AND CARE-TEAMS IN THESE SAFETY NET ENVIRONMENTS TO PROVIDE OBESITY-BASED EDUCATION/REINFORCEMENT AS PART OF THE CARE-EXPERIENCE. 2. KFHP OF HAWAII SUPPORTED THE UNIVERSITY OF HAWAII, MAUI COLLEGE AND THEIR PARTNERS I.E. PEDIATRICIANS, SAFETY NET CLINICS AND COMMUNITY ORGANIZATIONS, TO PROMOTE ORAL HEALTH OF CHILDREN IN MAUI. THE EFFORT ENTAILS DISTRIBUTION OF BABY DENTAL KITS AND IN-HOME VISITS TO REINFORCE ORAL HEALTH OF CHILDREN. A SMALL GRANT INVESTMENT OF APPROXIMATELY $13,000 WILL IMPACT OVER 4,000 CHILDREN AND THEIR FAMILIES. ORAL HEALTH OF HAWAII'S CHILDREN IS AMONG THE WORST IN THE NATION. D. DEVELOPING AND DISSEMINATING KNOWLEDGE KAISER FOUNDATION HEALTH PLAN SPENT APPROXIMATELY $4.8 MILLION IN 2012 TO SUPPORT PROGRAMS AND SERVICES FOR THE DEVELOPMENT AND DISSEMINATION OF KNOWLEDGE WHILE ALSO PROVIDING GRANTS AND DONATIONS TO A NUMBER OF OTHER NONPROFIT ORGANIZATIONS. 1. MEDICAL RESEARCH PROGRAMS FOR MORE THAN 60 YEARS, KAISER PERMANENTE RESEARCHERS AND THEIR WORK HAVE PROVEN PIVOTAL IN IMPROVING HEALTH CARE. IT'S NOT JUST OUR MEMBERS WHO BENEFIT, BUT THE BROADER COMMUNITY. OUR RESEARCHERS MAKE IMPORTANT CONTRIBUTIONS TO BOTH CLINICAL MEDICINE AND TO THE UNDERSTANDING OF THE CAUSE AND PREVENTION OF DISEASE.
