Form 990, Part VI, Section B, line 15a A committee made up of Horizons Board Members(Chariman, Medical Director and Hospital CEO) confer with the hospital's VP of Human Resources to determine a salary range upon hire. This committee had for its use the biannual salary survey conducted by the Association of Community Mental Health Centers of Kansas (ACMHCK). At each anniversary date an evaluation of the CEO's performance is solicited from each Board member and reviewed by the Committee. A percentage wage increase is suggested and voted upon by the full Board. Prior to hiring an officer and/or key employee, the CEO confers with the VP of Human Resources at Promise Regional to review the job description and place the position on the appropriate salary scale.
