Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND ARE IDENTIFIED THROUGH CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - NEW LONDON HOSPITAL ASSOCIATION. Recruitment Messaging: Nothing about us without us. To emphasize the importance of local voices, the CHNA 2024 recruitment theme is the well-known advocacy phrase hing about us without us". The theme is a celebration of New London Hospital's commitment to "providing high-quality healthcare to our communities" that is responsive to the uniqueness of our rural catchment area. Sample Size To ensure a representative survey sample capture the "minimum sample size" is calculated. The minimum sample size reflects the minimum number of respondents needed to produce a desired result. Specifically, if the study were to be reproduced 100 times, over that 95% of the time the result would be the same with a 5% margin of error. For our minimum sample size, the total population size is our entire catchment area (n=33123) and the cohort for sampling is the town level (see Table 3). Using the proportion of the catchment population, the minimum sample size calculator estimates the sample size that will produce a meaningful result per cohort (town). While the town cohorts have a range of demographics (i.e., gender, age, etc) we will not stratify by demographics due to the small sample size. In order to achieve the minimum survey sample size, multiply the Minimum Survey Sample Number by the expected response rate of 33%. The previous leader response rate is 59% yet the resident response rate is unknown, so using the average survey response rate of 33% seemed a reasonable approach. This provides us the benchmark for recruitment. To achieve the overall NLHA Catchment goal of 1391 respondents, we will need to recruit 4,212 individuals to participate. The overall NLHA catchment goal is supported by individual recruitment numbers by town (see Table 3). Implementation Strategy Both targeted respondent pools in the CHNA (leader and resident) will have the option of filling out the 10 minute survey either by paper or via Qualtrics (a digital link). The 2024 CHNA deployed a series of techniques to ensure broad community input. This included: 1. Direct engagement in the community (physical as well as digital) sorted/identified by town of residence - four in person community events including in Newport (2), Bradford, and Franklin which serves Danbury and Wilmot to provide people an opportunity to fill in the hard copy survey. We also sent over 400 paper copies to the meals on wheels program through Newport, Bradford, and Franklin serving Danbury and Wilmot. 2. Community Health Partner Email Distribution Lists, restricted to town of residence 3. Social Media (New London Hospital as well as partner organizations with highlights by town) 4. Hard copies of the survey were shared with the 14 libraries in our service area with survey drop boxes for people to drop off the survey in their community. To enhance town participation, town assets will be included in the recruitment strategy using 'snowball recruitment' which asks participants to share the survey link with others in their community. Specifically, the local public assets are: 1. Town Library: libraries are being contacted within each town to serve as a point of distribution for both the digital link and the paper surveys. For each library, a "ballot box" will be placed to maintain confidentiality of the respondents. 2. Town Halls: administrators at each town hall are being recruited to share the digital links and provide the paper surveys to the public. 3. Local Leaders: local community leaders such as school administrators, fire and police, as well as elected officials are being recruited to solicit resident input. 4. Business: the largest employer(s), and/or local small business, are being identified and will be recruited to engage employees. The Program Manager for Community Health engaged in outreach with each of these domains at the town level to promote equity in recruitment success.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - NEW LONDON HOSPITAL ASSOCIATION. The 2024 Community Health Needs Assessment Partnership includes New London Hospital Association, Dartmouth Health, Alice Peck day memorial Hospital, Valley regional Healthcare, Mt. Ascutney Hospital and Health Center, Cheshire Medical Center, Visiting Nurse and Hospice for Vermont and New Hampshire with technical support from the New Hampshire Community Health Institute/JSI.
Schedule H, Part V, Section B, Line 6b Facility , 1Facility , 1 - NEW LONDON HOSPITAL ASSOCIATION. In addition to working with the entities listed on line 6A, NLHA worked closely with the member organizations of the Greater Sullivan County Public Health Network (GSCPHN) to disseminate surveys virtually. NLHA also worked closely with Newport Senior Center, Bradford Senior Center, Franklin Senior Center and 14 libraries within the NLH service area to provide paper ballots for community members.
Schedule H, Part V, Section B, Line 7 Facility , 1Facility , 1 - NEW LONDON HOSPITAL ASSOCIATION. The CHNA has been made available both digitally and hard copy to 14 libraries in our 15-town service area (each town in our service area except Grantham and Croydon share a library). Additionally, 14 community conversations were held in the 14 libraries to provide an opportunity for the community to review results in person in their town and provide input into the direction of the CHIP. We have hard copies in the waiting areas of Newport Health Center (NHC) and in the Lobby, Emergency Department, and Express Care at New London Hospital location. The CHNA has also been provided to community partners and presentations have been given to requesting organizations on the results of the CHNA. It is also available on request.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - NEW LONDON HOSPITAL ASSOCIATION. Given the cyclical nature of the Community Health Needs Assessment (CHNA), the Community Health Improvement Plan (CHIP) will focus on strengthening interventions on persistent themes (e.g., healthcare affordability and availability) across cycles that remain a priority area and evaluating additional programming for new and emerging themes (e.g., provider cultural awareness). To achieve these goals, the CHIP will utilize a combination of these four strategies: 1. Strengthening existing NLH efforts that seek to improve healthcare availability and affordability. This includes access to affordable medications as well as mental and dental health care. 2. Evaluate and engage partnerships for impact with a specific focus on special populations identified in the 2024 CHNA: seniors, LGBTQIA+, veterans, youth, and indigenous populations. 3. Engage in community health programming that seeks to address social determinants of health (SDoH) factors influencing our service area. 4. Develop a health literacy plan that will expand on the communities understanding of health resources in our service area. The CHIP categorizes issue areas by AIM including 1) access to healthcare, 2) connect public health programs in the community, and 3) address social determinants of health (SDoH). A full list of NLH programs is listed in our CHIP online at https://www.newlondonhospital.org/about/annual-and-community-reports

Schedule H, Part V, Section B, Line 16 Facility , 1Facility , 1 - New London Hospital. THE FINANCIAL ASSISTANCE POLICY IS POSTED ON NLHA'S WEBSITE, INCLUDING THE VERBATIM POLICY AND A SHORTER, MORE PATIENT-FRIENDLY PLAIN-LANGUAGE SUMMARY. NLHA PROVIDES THE PLAIN-LANGUAGE SUMMARY BROCHURE TO ALL INPATIENTS, EMERGENCY DEPARTMENT INTAKES, UNINSURED, AND SELF-PAY INDIVIDUALS. NLHA CONTINUES TO NOTIFY PATIENTS ON THE BACK OF THE BILLING STATEMENT ABOUT FINANCIAL ASSISTANCE AVAILABLE TO THEM. ADDITIONALLY, NLHA POSTS INFORMATION ABOUT THE POLICY IN PUBLIC AREAS THROUGHOUT THE FACILITIES INCLUDING ADMISSION OFFICES, PUBLIC AREA BOARDS THROUGHOUT THE FACILITIES, THE EMERGENCY ROOMS, AND FINANCIAL ASSISTANCE POLICY BROCHURES IN PATIENT AREAS. NLHA SCREENS 100% OF UNINSURED INPATIENT AND SAME-DAY PATIENTS PRIOR TO ADMISSION. AS PART OF THIS PROCESS, NLHA CHECKS ALL STATE AND FEDERAL PROGRAMS TO SEE IF INDIVIDUALS ARE ELIGIBLE FOR ASSISTANCE. PATIENTS ARE ALSO SCREENED TO DETERMINE QUALIFICATION FOR FINANCIAL ASSISTANCE AND THE APPLICATION IS PROVIDED AND/OR COMPLETED AT THIS TIME.

Schedule H, Part VI, Line 2 Needs assessmentNLH assesses the needs of the community primarily through a triennial Community Health Needs Assessment (CHNA) which most recently occurred in 2024. Since the ACA was enacted, NLHA has conducted four CHNA (2015, 2018, 2021 and now 2024) using a combination of surveys (leader and resident) as well as a small number of focus groups and a few key informant interviews. The surveys are provided as both a digital option and paper. Each cycle solicited feedback using 'a convenience sampling' approach for the entire NLHA catchment area. Convenience sampling is a low-cost approach which is dependent on the availability and willingness of a survey participant. While the convenience sampling approach is an easily understood, as well as arguably the least costly method for soliciting input on surveys, NLH engaged in an revised recruitment strategy to strengthen the sample proportion of the population within each town for our 2024 CHNA. Historically, the CHNA convenience sample has produced a very low response rate outside of New London. For example, in the 2021 CHNA, the response rate in New London was 31% , however, New London's population represents 13% population within our catchment area . Conversely, Newport's population represents roughly 21% of the total population within our catchment yet the response rate was less than 7%. The rates are even more disproportionate with the smaller communities within our catchment area. In response to this differential, in 2024 NLHA was committed to revisiting the recruitment strategy to increase the likelihood that the response rate will proportional to the town population. The overall goal of the recruitment strategy was to reach a minimum number of samples to reach a 95% confidence interval with a 5% margin of error. For the 2024 NLH CHNA recruitment strategy please see section Schedule H, Part V, Section B, Line 5 - Input from persons who represent broad interest of community. Based on this revised recruitment strategy, NLH CHNA convenience sample includes 1,352 survey respondents with the distribution of these responses better aligned with the population distribution. For example, in 2024 the response rate in New London was 17% instead of 31% (2021) because New London's population represents 13% population within our catchment area. Additionally, Newport's response rate for 2024 was 13% instead of 7% (2021) which is closer to the proportion of the population for Newport (represents roughly 21% of the total population within our catchment area). For some of the most rural towns, the recruitment strategy made a dramatic difference in having a better distribution in our survey responses. For example, the Town of Washington saw a +455% increase in their response rate from 2021 to 2024 (11 in 2021 versus 61 in 2024). To maintain awareness of the communities needs, NLH has continued to engage with the infrastructure set up in the recruitment strategy bringing both the board approved CHNA and subsequent Community Health Improvement Plan (CHIP) results to the towns through the library infrastructure hosting 14 CHNA community conversations and 11 CHIP community conversations. - Community Partnerships - NLH Pharmacy - Newport Police Department - Sullivan County Sheriffs Department - Colby Sawyer College - Granite Hope Veterans Coalition (co-chair) - Spring Ledge Farm - Morgan Hill Bookstore - NH Falls Task Committee - Sullivan County Community Partners - Sullivan County Public Health Network - Sullivan County Transportation Network - Kearsarge Neighborhood Partners (KNP) - Kearsarge Community Network (KCN) - Climate and Health Initiative for Children in Kearsarge & Sunapee (CHICKS) Committee - NH Hunger Solutions - TLC Rainbow Roundtable - NH Queer Consortium - Foundation for Health Communities (Health Equity Peer Group, Population Health Peer Group, Community Benefit - co-chair) - Dartmouth Health and New London Hospital (NLH) Committees - DH Population Health Group - DH Community Health Group - NLH Falls Committee
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceNLHA provides the Financial Assistance Policy (FAP) for patients both in person and online depending on the patients preferences. The policy is available on NLH website, upon admission, upon request and throughout the billing process.
Schedule H, Part VI, Line 4 Community informationNLHA includes fifteen towns within its primary catchment: Andover, Bradford, Croydon, Danbury, Goshen, Grantham, Lempster, New London, Newbury, Newport, Springfield, Sunapee, Sutton, Washington, Wilmot. The primary catchment area includes two physical facilities, a critical access hospital headquartered in New London as well as a satellite health center in Newport which includes an ancillary support site for pediatrics through the Newport Middle/High School called Tiger Treatment Center. The total population of the New London Hospital primary service area in 2022 was 33,192 according to the United States Census Bureau (American Community Survey) equivalent to about 2.4% of the total population of New Hampshire. The estimated service area population has increased by approximately 0.4% or about 120 people over the last 3 years. New Hampshire's population grew by about 2% over the same time frame. Compared to New Hampshire (NH) overall, the service area population has proportionally more seniors - about 24% are 65+ compared to about 19% in NH overall - and the median age of service area residents is 6 years older than the state overall. A substantial range is observed for this statistic within the region from 18% of residents in both Newport and Springfield aged 65+ to 36% of New London residents. A similar range is observed for the percentage of residents who are under 18 years of age from 11% in Washington to 24% in Lempster. The region overall has similar demographics compared to New Hampshire with regard to median household income, the percent of people living in poverty, the percent of household with children headed by a single parent, and the percent of people living with a disability. However, there is a substantial range within the region on these measures. For example, the town with the highest median household income community (Sutton, $130,568) has median household income over twice as high as the lowest income community (Goshen, $59,896). Similarly, a substantial range is observed for the percent of people living below the federal poverty level (FPL) with an estimate of 0% in Grantham compared to about 13% of residents in Danbury and 14% of residents in Newport and Sunapee. Overall, the percent of residents within the service area living below the federal poverty level mirrors the statewide percentage at about 7% of households. 96% of the population of the New London Hospital service area identifies as 'White about 2% identify as Hispanic ethnicity. In general, the service area is somewhat less diverse with regard to race and ethnicity compared to New Hampshire overall.
Schedule H, Part VI, Line 5 Promotion of community healthThe New London Hospital Association plays an active role in promoting community health by supporting community based educational programs such as the Tai Ji Quan program which is an evidence based program for falls prevention, Drug Take Back programming in the spring and fall to reduce substance misuse in the community, as well as a wide variety of educational materials provided throughout the clinical areas of both NLH and NHC on topics related to healthy lifestyle, smoking cessation, cancer screenings, programs to support new parents. NLHA partners with libraries within our fifteen town service area to provide educational materials for patrons directly in the community. Additionally, cash contributions, contracted services, and in-kind contributions of staff time and expertise are used to support these strategies to improve the health of the community.
Schedule H, Part VI, Line 6 Affiliated health care systemNEW LONDON HOSPITAL ASSOCIATION IS AN AFFILIATE OF DARTMOUTH-HITCHCOCK HEALTH. THIS RELATIONSHIP ALLOWS THE STREAMLINING OF PATIENT CARE BETWEEN MEMBERS OF THE TWO HEALTHCARE SYSTEM AND STRENGTHENS THE CENTER AS A REGIONAL RESOURCE FOR THE KEARSARGE/LAKE SUNAPEE REGION IN CENTRAL NEW HAMPSHIRE. THE AFFILIATION ALLOWS MORE PATIENTS TO RECEIVE CARE CLOSER TO HOME, FREES UP BED CAPACITY FOR TERTIARY CARE PATIENTS AT DARTMOUTH-HITCHCOCK MEDICAL CENTER IN LEBANON, AND ALLOWS THE NEW LONDON HOSPITAL ASSOCIATION TO CONTINUE ITS ROLE IN PROVIDING PRIMARY CARE AND SPECIALIZED CLINICAL SERVICES IN A PATIENT AND FAMILY-FRIENDLY ENVIRONMENT. IT ENABLES NLHA TO MORE EFFECTIVELY PURSUE A COMMON VISION WITH OTHER D-HH ENTITIES OF THE FUTURE OF HEALTH CARE THAT IS BASED ON CREATING A SUSTAINABLE HEALTH SYSTEM, FOCUSING ON POPULATION HEALTH, DELIVERING VALUE-BASED CARE, AND EMBRACING THE OPPORTUNITIES PROVIDED BY NEW PAYMENT MODELS.
