The facilities are adequate for treatment or care of individuals suffering from mental or physical illness, disease, injury or deformity in an acute care, skilled nursing care, outpatient or emergency room setting. LCH cares for residents of the Smoky Valley with the operation of a 24/7 emergency room. 1,741 emergency room patients were seen in the 2025 fiscal year. 3,935 urgent care visits and 291 cardiac rehab visits improved the health of community members. 9,896 visits were made in the Family Health Care Clinic to provide acute treatment, manage chronic conditions, or provide preventive care. 4,968 meals on wheels and 3,753 frozen meals were supplied to community members. Education was provided to the Smoky Valley Community through print, digital forums and on-site in FY2025. Employees of LCH partner with community groups such as Lindsborg Ad-Hoc and Strategic Alliance to enhance the Lindsborg experience. Staff also provide leadership with the Sunflower Health Network, KHA Advisory Councils, the Riverview Nursing Home Board of Directors, the McPherson County Coalition, and the Sunflower (Kansas) HFMA Chapter.Desc

Lindsborg Community HospitalPart V, Section B, Line 5: In November 2024, the facilities involved identified key stakeholders in all of the communities. An online survey was sent to all stakeholders on November 15, 2024. The purpose of the survey was to identify McPherson County Health Needs. On February 21, 2025 a Town Hall was held to review and discuss the basic health data and rank the health needs. Representatives from the Hospitals, McPherson County Health Department, and United Way represented the underserved population at the Town Hall.

Lindsborg Community HospitalPart V, Section B, Line 7d: www.lindsborghospital.org/community-health-needs-assessment-chna/
Lindsborg Community HospitalPart V, Section B, Line 11: In the most recent CHNA conducted the following needs were identified: mental health, affordable insurance coverage, substance abuse, affordable and accessible childcare, workforce staffing, obesity, and accessible, affordable and safe housing. The organization will not address the need of affordable health insurance coverage due to this being a social determinant of health. We will partner with others as appropriate and continue to support Medicaid enrollment and enhancement of the CMS Transparency website reporting. The organization will not address the need of affordable and accessible childcare due to this being a social determinant of health but we will continue to encourage local businesses to offer child care benefit options. The organization will not address accessible, affordable, and safe housing as this is a social determinant of health but we will continue to work with and communicate the results of the McPherson Countywide Housing Development Plan. ObesityThe facility has taken the following actions to address obesity. Obesity is a by-product of two other issues, poor eating habits and lack of exercise. Poor eating habits often occur from a basic lack of knowledge of what foods and food preparation techniques are better than others. The Dietician and Food Service Manager have put together recipes and information on how to utilize food received from the local food bank. They have also communicated how to call the LCH Food Service Department for assistance with questions regarding food preparation. The goal is to improve knowledge and ease of use of more nutritious foods. LCH Food Service and Marketing developed a pamphlet with information on where to receive food assistance in Lindsborg, Salina, McPherson, and Marquette. To add to the exercise and wellness opportunities, LCH resumed pre-pandemic hours in the Wellness Center. It is available to the public at no cost. LCH also sponsored the Bike Share program with the City of Lindsborg. Bicycles can be rented in three different locations in Lindsborg. In addition, the Family Health Care Clinic program, Lifestyle Health has a goal of weight management and disease control.Mental Health and Substance AbuseThe facility has taken the following actions to address mental health and substance abuse. LCH works with Prairie View mental health services to screen and assist in appropriate placement or follow-up in emergent cases. Referrals are also made to Veridian, CKF Addiction Treatment and other mental health and/or substance abuse treatment centers as needed. In FY2025, LCH convened a meeting with representatives Valley Hope Addiction Treatment & Recovery Program and nursing staff. The purpose was to discuss improved ways to manage people in a mental crisis. Quite often, acute or chronic pain is the cause for substance use and abuse. In May of 2023, LCH began a partnership with Holistic Pain Management to provide clinical exams and procedures for pain management. The demand for this service continues to grow throughout FY2025. The number of days the service is offered continues to increase, depending upon availability.Workforce StaffingTo address workforce staffing, the Lindsborg Community Hospital made a donation to the Lindsborg Community Healthcare Foundation, Inc. to support the Workforce Scholarship. The first Workforce Scholarship was awarded in 2024 to a Radiology Tech who started full-time employment with Lindsborg Community Hospital in the summer of FY2025. LCH continues to work with Salina Regional Health Center Talent Acquisition to identify individuals for our open positions. LCH was able to secure a new-graduate physician in 2025 who will begin providing services in September 2026.AgingAging was also identified as a need in the prior year CHNA with falls and dementia being major issues to the aging population. In the current year the facility has taken the following actions to address falls. LCH has offered three sessions of Stepping On classes. Stepping On provides education in fall prevention strategies and teaches exercises. The second leading reason for an ER visit over a year of time was due to a fall. The facility has addressed dementia through the Family Health Care Clinic's participation in the Cognitive Care Network with the University of Kansas Health System. The Network has provided education to LCH providers and staff on the diagnosis and management of dementia. It has also provided resources, including social services, to affected patients and families. High Blood PressureHigh blood pressure was also identified as a need in the prior year CHNA. The facility has taken the following actions to address high blood pressure. Through the EMR, provider and staff receive lists of patients with blood pressure measurements outside of acceptable limits. Medication management and other interventions will be attempted to reduce the number of patients with high blood pressure. The Chronic Care Management program in the FHCC is also providing home blood pressure monitoring devices.
Lindsborg Community HospitalPart V, Section B, Line 13h: The methodology used to determine eligibility for financial assistance takes into consideration income, net assets, family size and resources available to pay for care. In addition, presumptive charity care may be applied in situations where all other avenues have been exhausted.
Lindsborg Community HospitalPart V, Section B, Line 24: The hospital financial assistance policy does not cover elective procedures. The hospital may have charged FAP eligible patients gross charges for services that are not covered under the financial assistance policy.

Part I, Line 3c:The methodology used to determine eligibility for financial assistance takes into consideration income, net assets, family size, medical indigency, insurance, underinsurance and resources available to pay for care. In addition, presumptive charity care may be applied in situations where all other avenues have been exhausted.
Part I, Line 7:Charity care expense was converted to cost on line 7a based on an overall cost-to-charge ratio addressing all patient segments. Amounts on lines 7e, 7f, and 7i are from the actual general ledger. Lines 7b and 7g were calculated utilizing the cost reports.
Part III, Line 2:The amount on line 2 represents implicit price concessions. The Organization determines its estimate of implicit price concession based on its historical collection experience with this class of patients.
Part III, Line 4:The footnote to the Organization's financial statements that describes implicit price concession is located in the audited financial statement report on page 16.
Part III, Line 8:The payments are compared to the actual cost of providing the service as arrived at through the Medicare cost reports. Medical services are provided to patients with Medicare coverage regardless of whether or not a surplus or deficit is realized. Providing Medicare services promotes access to healthcare services which are vitally needed by our community.Hospital services reimbursed on a fee schedule are not included in the Medicare calculation per the 990 instructions. Had this been reported the Hospital would have had a greater surplus from Medicare services of $72,017.Medicare fee schedule revenue $423,926Medicare estiamted costs of relating to payments ($351,909)Net Surplus (shortage) $72,017
Part III, Line 9b:LCH will not engage in extraordinary collection activities prior to making reasonable efforts to determine if a patient is eligible for financial assistance. The notification period for reasonable efforts starts with the date care is provided and ends 120 days after the date of the first statement. Statements and a final notice during the 120 days meet the reasonable efforts standard. If, during the collection process, the patient is deemed to qualify for financial assistance the account will be returned to the Hospital for resolution.Extraordinary collections activities may include referral to a collection agency and/or legal action. Collection/legal action may be used to collect amounts due if the responsible party refuses to cooperate in the financial assistance determination process and make and follow suitable payment arrangements. Failure to cooperate includes failure to submit required data and/or failure to submit required data within the specified time period. In addition, collection/legal action may be used to collect amounts due that remain after financial assistance determinations have been made and the responsible person fails to make and follow suitable payment arrangements. Legal action may be taken to attach wages when it is believed that there is sufficient income to pay the amount due. The Administrator or Chief Financial Officer must give authority for such legal action.
Part VI, Line 2:In addition to the CHNA process, LCHA participates in Strategic Alliance meetings which are held every 1-2 months. The Alliance consists of businesses and organizations in Lindsborg with the purpose of discussing strategic needs of the community and identifying where synergies can be developed. LCHA also participates in weekly meetings called the Ad Hoc RoundTable. This group meets to discuss activities and needs in the community. LCHA participates in a county Behavioral Health Task Force that meets every 2-3 months. The task force members include McPherson hospital administration and physicians, Prairie View Behavioral Health staff, law enforcement, the McPherson mayor and McPherson County Attorney, the Lindsborg Chief of Police, and LCHA's administrator.Medical Staff meets monthly to discuss clinical management of LCHA. Clinic providers and management meet monthly to discuss the operations of the Family Health Care Clinic. In both meetings, issues concerning needs of the community are discussed and addressed or at least brought to the attention of administration for development of an action plan.LCHA has a Patient and Family Advisory Council (PFAC) which is made up primarily of community members who are patients or family members of patients. The group meets monthly to discuss quality and other projects that LCHA is performing. As a part of the regular agenda, community members are given an opportunity to voice comments and concerns about needs of the community.
Part VI, Line 3:The Hospital's financial assistance policy is posted in admission areas, on the website, and with billing statements. Hospital employees have been instructed to direct anyone with questions regarding assistance to the Hospital's business office. Patients are educated by the financial services staff and social services staff who are able to provide assistance upon request and through policies posted to our website.
Part VI, Line 4:The Organization serves the Smoky Valley Area including Lindsborg, Marquette, Falun, Roxbury, Gypsum, Assaria and other areas including McPherson, Salina, Smolan, Canton, and Galva. Approximately 6% of the Hospital's patients are uninsured or Medicaid patients. The community served by the Hospital is also served by two other hospitals, plus a Federally Qualified Health Center in Salina.
Part VI, Line 5:The organization's governing body is comprised primarily of persons who reside in the organization's primary service area who are neither employees nor contractors of the organization nor family members thereof. The organization extends medical staff privileges to qualified physicians in its community for some or all of its departments. Surplus funds of the organization are returned to the organization to improve patient care through updated equipment and technology and care team member education. The organization operates an emergency room that is available 24/7 regardless of ability to pay. Through the assistance of a donor, the organization has trained 1,066 CNA students over the last 22 years through a partnership with Hutchinson Community College, Bethany Home, and The Cedars. The students receive the training at no cost.
Part VI, Line 6:LCH is an affiliate of Salina Regional Health Center (SRHC) through a management and operating agreement. SRHC provides Accounting, Marketing, Administrative, IT, Payroll, HR, Purchasing and Revenue Cycle services. SRHC also helps support our community by employing Specialty physicians that travel to our location to provide healthcare.
