Action plans and activities for each of the strategies are summarized below. Based on Shannon's evaluation for the fiscal year (FY) ending September 30, 2023, Shannon has either met their goals or is still in the process of meeting their goals for each strategy listed. Shannon continues to track activities and evaluate progress to reach implementation strategy goals through the end of FY 2025. The below CHNA action plan includes details on how Shannon Medical Center is addressing significant needs identified in the CHNA conducted in 2022. The needs are categorized in the following priority areas: Improve Access to Care, Health Education/Knowledge, Healthy Living and Adult Obesity. Priority 1: Improve Access to Care Shannon Medical Center connects patients with appropriate health services to improve the availability and delivery of care. Shannon Medical Center partnered with Shannon Clinic to increase access to care. The Shannon Transitional Care Program served 903 patients and the Chronic Care Program served 25 patients. The programs provided medication management, addressed social barriers, and helped patients manage their chronic condition at home. Shannon utilized Service Line Patient Navigators and Community Health Workers to coordinate access to care and ensure the continuum of care throughout the health system. The event drew more than 800 attendees and provided summer camp and physical activity information from numerous community organizations. Needs Not Addressed Some issues identified through the Community Health Needs Assessment have not been addressed in the Implementation Strategy. During initial discussions and subsequent prioritization, Shannon considered the levels to which some needs were already being addressed in the service area. Additionally, some community needs fall out of the scope of expertise and resources of Shannon. Shannon will continue to explore potential partnerships and internal strategies to find a way to provide these essential health services to our patients and community.

SCHEDULE H, PART VI, LINE 2THE CHNA PROCESS INVOLVED: A community health needs assessment was conducted from January 2022 to August 2022. Community input was provided through key stakeholder interviews and a community survey. Population demographics and socioeconomic characteristics of the community were gathered and reported utilizing various third parties. The health status of the community was then reviewed. Information on the leading causes of death and morbidity information was analyzed in conjunction with health outcomes and factors reported for the community by CountyHealthRankings.org and other third parties. This data was analyzed and reviewed to identify health issues of vulnerable populations and the community as a whole. As a result of the analysis described above, the following health needs were identified for the community: Adult Obesity Adult Smoking Cancer Children in Single-Parent Households Excessive Drinking/Alcohol-Impaired Driving Deaths Healthy Behaviors/Lifestyle Heart Health High Cost of Health Care Lack of Health Knowledge/Education Lack of Mental Health Providers Language/Cultural Mindset Physical Inactivity Poverty/Financial Resources/Children in Poverty Preventable Hospital Stays Sexually Transmitted Infections Shortage of Dentists Shortage of Primary Care Physicians Stroke Substance Abuse Teen Birth Rate Violent Crime Rate The identified health needs were reviewed by Shannon's Executive Leadership Team, the community health needs assessment committee, and BKD. To facilitate prioritization of identified health needs, a ranking process was used. Each factor received a score between 0 and 5, with a total maximum score of 20. Then priority areas were determined based on their assessment of the qualitative and quantitative data. Identified needs were prioritized based on the following criteria: How many people are affected by the issue or size of the issue? What are the consequences of not addressing this problem? Prevalence of common themes. Organizational capacity and existing infrastructure to address the health need. Based on the criteria outlined above, the health needs that scored a 15 or more (out of a possible 20) were identified as a priority area. Shannon has identified the following priority areas based on the above criteria: Improve Access to Care Health Education/Knowledge Healthy Living and Adult Obesity As a result of the needs assessment, Shannon developed and adopted an Implementation Plan to be executed during FY 2023 through the end of FY 2025. Shannon has worked with community partners and health experts on the following for each of the approaches to addressing the identified health needs: Develop work plans to support effective implementation Create mechanisms to monitor and measure outcomes Provide ongoing status and results of these efforts to improve community health Shannon Medical Center is committed to conducting another community health needs assessment in FY 2025.

SCHEDULE H, PART VI, LINE 5Promotion of Community Health: In a county with no hospital taxing authority, a hospital whose mission reflects commitment to treating patients without regard to ability to pay is a highly desirable and pivotal point of community health care. For more than 90 years, Shannon has embraced such a mission and follows through on the wishes of our benefactor, Margaret Shannon, in providing care for the people of West Texas. Our charity care figure of more than $72,170,085.05 would be a burden to the taxpayers of Tom Green County, and many other counties whose citizens directly benefit from her generosity. Shannon partners with the American Heart Association to educate, raise awareness and prevent heart disease and stroke. Shannon understands that continued education and awareness is crucial for women and men to fight this deadly disease. This sponsorship is provided at a cost of $10,750 to Shannon. Shannon provides a host of community education events related to topics such as diabetes management and prevention, summer safety for children, fitness and nutrition, childbirth and childcare, stress management, cancer prevention and management, and Alzheimer's. Through educational events and various community events, health professionals relay current health information to the public. Representatives from different departments provide support and participate in local health fairs and health-related community events where they provide educational material. In addition to public outreach events, Shannon publishes the Healthbeat newsletter magazine which is delivered to 30,000 households. Shannon contributed $62,976 for these programs. Shannon supports health and fitness activities, as well. To address the growing concern over childhood obesity, the Kids' Marathon event provides an opportunity for students, ranging from kindergarten through sixth grade, to participate in a program that encourages healthy habit formation early in life. A full marathon is considered 26.2 miles. Students accumulate miles and run the last 0.2 lap during a celebratory event at the San Angelo Stadium. The event hosts organizations that offer local summer programs and camps that encourage children to remain active through the summer months. This event is provided at a cost of $2,239 to Shannon. Shannon has recognized a community need to offer supportive services for community members suffering from Parkinson's Disease and to parents and families suffering from early pregnancy loss, stillbirth, or newborn death. These events and programs are provided at a cost of $19,816 to Shannon. The Shannon Care Coordination program is designed to assist in the healthcare of chronically ill patients with multiple disease processes. The program manages high-risk patients with multiple disease processes, addresses social and healthcare barriers, and supports patients' goals of independence in their health care management. The Shannon Care Coordination program was developed as a patient-centric strategy to impact patient care beyond the four walls of the hospital. The Shannon Care Coordination Program is designed to improve patient outcomes by utilizing a team to focus on population health efforts as it relates to chronic disease. The program's structure and ability to utilize a team to focus on population health efforts as it relates to chronic disease has provided Shannon Health the opportunity to identify possible areas for patient care. The program serves patients discharging from the hospital with Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, as well as patients at high-risk for readmission, which includes COVID-19 patients. Key areas of the program include: medication management, addressing social barriers, and helping the patient manage their chronic condition at home. Shannon provided $872,639 for the operation of this program. Shannon provided access and support services for indigent patients, including $10,778 in transportation vouchers and $19,351 for durable medical equipment, outpatient oxygen, outpatient prescription assistance, and lodging expenses for patients discharged from the hospital. Shannon offered Pastoral Care services that were outside the scope of the normal routine spiritual and pastoral care provided in day-to-day patient care practices. Shannon provided the No One Dies Alone (NODA) and No One Stays Alone (NOSA) program, presided over funeral/memorial services, organized vigils, offered Advanced Care Planning services at community events, and held Critical Incident Stress Management community support interventions.
