Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationSchedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. Their related income is exempt from federal income tax under Section 501(A). Some of the subsidiaries are taxable entities, and some of the income of the tax-exempt entities is subject to taxation as unrelated business taxable income. The Company and its subsidiaries file U.S. federal income tax returns, and they also file in various state and foreign jurisdictions. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist as of December 31, 2023. Accounting for uncertainty in income taxes, ASC Topic 740-10 prescribes a comprehensive model for how an organization should measure, recognize, present and disclose in its financial statements uncertain tax positions that an organization has taken or expects to take on a tax return. The Company is subject to routine audits by taxing jurisdictions. There are no current audits in progress. As of December 31, 2023, the Company has no uncertain tax positions. As of December 31, 2023 and 2022, the Company's taxable subsidiaries had net operating loss carryforwards for Federal income tax purposes of $182,444 and $169,279, respectively, which are available to offset future Federal taxable income subject to various limitations. The Company had net operating loss carryforwards for state income tax purposes of $119,717 and $113,528 as of December 31, 2023 and 2022, respectively, which are available to offset future state taxable income subject to various limitations. The Company accounts for income taxes under the asset and liability method. Under this method, deferred tax assets and liabilities are recognized for the estimated future tax consequences attributable to differences between the financial statement carrying amounts of existing assets and liabilities and their respective tax bases. Deferred tax assets and liabilities are measured using enacted tax rates expected to be in effect during the year in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. Interest and penalties related to income taxes are accounted for as income tax expense. As of December 31, 2023, the Company has placed a partial valuation allowance of $83,168 and $18,945 on the gross deferred Federal and state tax assets, respectively. As of December 31, 2022, the Company has placed a partial valuation allowance of $156,538 and $103,396 on the gross Federal and state deferred tax assets, respectively. When determining the valuation allowance for deferred tax assets, the Company annually considers whether it is more likely than not that these amounts will not be utilized in future periods. There was a decrease in the valuation allowance for gross deferred Federal and state tax assets of $73,370 and $84,451, respectively, as of December 31, 2023. There was an increase in the valuation allowance for gross deferred federal tax assets of $1,354 and a decrease in the valuation allowance for gross deferred state tax assets of $10,532 as of December 31, 2022.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - Bon Secours - St. Mary's Hospital of Richmond LLC, Bon Secours - Memorial Regional Medical Center LLC, Bon Secours - St. Francis Medical Center LLC, Bon Secours - Richmond Community Hospital, LLC, Rappahannock General Hospital. The Bon Secours Richmond Health System includes seven acute hospital facilities serving the entire Bon Secours Richmond market. The Bon Secours Richmond Health System hospitals are: Memorial Regional Medical Center, Rappahannock General Hospital, Richmond Community Hospital, Southern Virginia Medical Center, Southside Medical Center, St. Mary's Hospital, and St. Francis Medical Center. Collectively, these hospitals prepared a joint CHNA and are referred to as "Bon Secours Richmond." A survey to assess community health needs was conducted as part of the CHNA process during a six-month period between October 2021 and April 2022. The survey was offered in Spanish and English. Individuals were asked to "Please choose the TOP 5 health issues you think should be addressed in your community:" from a list of 13 health issues. Respondents were also asked "On a scale of 1 - 6 (1 having little impact, and 6 having significant impact), how do each of the underlying causes below impact the health needs of the community. To assist Bon Secours Richmond Community Health leaders in interpreting this data from the survey, a Community Health Advisory Council was convened. This Community Health Advisory Council was a diverse and representative mix of nonprofit and other agency leaders from across the Bon Secours Richmond Health System market. The Community Health Advisory Council was reflective of the CHNA service area in gender, race, and geography and provided an inclusive perspective on the community served by Bon Secours hospitals. The Community Health Advisory Council met three times, focusing on interpreting survey results, identifying significant and prioritized needs, and emphasizing the overarching values that will enable successful implementation of programs and collaborations to address prioritized needs. Additionally, Five Community Conversations occurred in March of 2022 as part of the CHNA process. Three community conversations were geographically specific, with in-person community conversation opportunities offered in the East End of Richmond, in Petersburg City, and in Kilmarnock in the Northern Neck. Two of the community conversations were virtual and offered via Zoom, drawing attendees from across the Bon Secours Richmond service area and providing an opportunity to offer qualitative feedback in a COVID-safe environment. Overall, the racial and geographical diversity of the participants represented a healthy mix of the region's residents. To dive more deeply into root causes and to increase qualitative data collection, the Bon Secours Richmond Community Health team completed over ten key informant interviews. These key informant interviews were with a wide variety of community, nonprofit, and government leaders with diverse areas of expertise, including transportation, maternal and child health, housing, the Latinx community, and other areas that were viewed as top health issues by survey respondents. These conversations with key leaders helped interpret and clarify the results of the survey and the Community Conversations into categories and themes that could be discussed with the Community Health Advisory Council. Bon Secours Richmond Health System (BSR) BSR is comprised of Bon Secours - St. Mary's Hospital of Richmond LLC, Bon Secours - Memorial Regional Medical Center LLC, Bon Secours - St. Francis Medical Center, Bon Secours - Richmond Community Hospital, LLC, Rappahannock General Hospital, Bon Secours - Southern Virginia Medical Center, and Bon Secours - Southside Medical Center. BSR will address each need identified in its CHNA with regional strategies. Unless noted below, all BSR hospitals participated in the regional strategies. Social and Economic Disparity Implementation Activities: Goal 1: Invest resources to expand the capacity of organizations who aim to reduce social and economic disparity through the development of additional affordable housing units, programing along the educational achievement continuum, workforce development initiatives, and entrepreneurship. Goal 2: Prioritize staffing models in which community health workers and other allied health workers utilize community-based care and address social and economic barriers to health. 2023 Strategies and Outcomes: * BSR invested in community-based partnerships that address social determinants of health such as projects and programs that seek to reduce social and economic disparity, including affordable housing, education, workforce development, and the built environment. During 2023, BSR had the goal to invest $1 million and this was surpassed with a total investment of $2.25 million to approximately 30 regional nonprofits engaged in work addressing Social and Economic Disparity throughout the year. * Another priority for BSR was to explore opportunities to build community health worker (CHW) models of care, with the intent of growing the regional community health worker and other allied health worker network. BSR introduced seven new CHWs in 2023 compared to a goal of one new CHW. Of the seven new CHWs added during 2023, two CHWs were added to a Violence Response Team focused on disrupting community violence in the Petersburg area; one CHW was added to a Neighborhood Engagement team as a community liaison; and four CHWs were added as part of an AmeriCorps grant - with two supporting Richmond Community Hospital in the ED and two supporting a hypertension management program. * Utilization of evidence-based community-focused models to screen for and address social determinants of health (SDoH) continued to be a priority for BSR during 2023. The goal was to increase SDoH screening, documentation, and referral completion rates of all patients by at least 20% from the previous year's total of 28.2%. BSR exceeded this goal by having the new SDoH screening, documentation, and referral completion rate of 74% of all patients. * BSR continued to deploy technology solutions to connect individuals to the resources they need, with the aim of training and deploying at least 30 associates to utilize Unite VA social needs platform for patient referrals and tracking. In 2023, 30 associates were trained to be users for SDoH referrals. The increased training and use of the Unite VA Social Needs Platform resulted in 55 referrals primarily for individuals experiencing food insecurity or requesting wrap-around senior support services. * BSR started a new program, Impact Workforce Commitment to hire 10% of Workers from Economically Disadvantaged Areas. During the year, BSR held a two-day neighborhood employment recruitment, education, health fair event with on-the-spot applications. This resulted in 6 individuals being hired by BSR. * BSR partnered with the Supporting East End Entrepreneurship (SEED) program by investing $140k into the program and provided area small businesses with coaching and planning support. * During 2023, BSR provided Richmond and Petersburg Financial Opportunities Centers with $200k in funding to help them continue to provide clients in underserved areas with HUD approved rental counseling, housing navigation services, peer support services, employment coaching, digital navigation training, and assistance to obtain and maintain permanent housing. Engagement and Inclusion Implementation Activities: Goal 1: Increase frequency and depth of authentic community level engagement that solicits ongoing community member insight and perspective into the evolving health needs of the community. Goal 2: Identify opportunities to foster stakeholder and community leader engagement that facilitates a more inclusive community and advances an environment of justice. 2023 Strategies and Outcomes: * Throughout the year BSR partnered with trusted organizations to continue to identify real-time community needs and build social cohesion and hosted community engagement events that are centered around neighborhood level engagement, social connections, and diversity, equity, and inclusion. During 2023, BSR hosted 16 community events focused on neighborhood or regional level engagement, social connections, and diversity, equity, and inclusion. * To help inform service delivery, Bon Secours - Richmond Community Hospital will create opportunities in the form of town halls and other engagement meetings at least on a quarterly basis for patients and community members. In 2023, Bon Secours - Richmond Community Hospital held two community town hall meetings to provide updates on an initiative called "Community Today, Community Tomorrow" which lays out a plan for Bon Secours services and investment in the East End Community. * BSR implemented strategies to enhance and support community advisory councils that reflect the voice of the community BSR serves. During the year, BSR developed a matrix to ensure that Advisory Group invitations were sent to individuals that represent the diversity of the community. These invitations will be sent out in 2024 to prospective members. * BSR continued to serve as an anchor institution for collaboration across the region to address existing and emerging needs. With a goal of achieving five collaborations in 2023, BSR participated in nine collaboratives and steering committees focusing on violence response, anti-human trafficking, heart disease and stroke, health equity, and school readiness. * BSR provided a total of $305,000 via Community Benefit Investment funding to four regional nonprofits engaged in work promoting Engagement and Inclusion. * Patient Family Advisory Councils were established or maintained, and Community Advisory Councils were hosted across the BSR service area. * As a means to deepen partnerships with local faith communities in the service area, BSR developed 25 relationships with faith partners during 2023. This resulted in 12 vaccine flu clinics, one class on strangulation, and four educational workshops. * BSR participated in a community engagement event in the East End led by AmeriCorps CHWs by providing food, basic needs gift bags, and resource lists to homeless individuals. * BSR supported and sponsored the 2023 State of Housing in the Richmond Region Event, which was a partnership for housing affordability. Violence and Trauma Implementation Activities: Goal 1: Promote safer communities by working with and investing in community partners who utilized trauma informed approaches to community health and wholeness. Goal 2: Increase and deepen geographic footprint and provision of services to best care for victims of violence, including additional support services for families experiencing community violence, gun violence, and interpersonal violence. 2023 Strategies and Outcomes: * During 2023, BSR worked to identify and participate in interventions focused on keeping neighborhoods safer by increasing partnerships with schools, housing entities, and other community-based organizations who are working to address increases in community violence. BSR invested $260k into 4 regional nonprofits that address violence and trauma. The investment also supported the launch of Tri-Cities Community Violence Collaborative which includes over 15 Petersburg area community organizations. The collaborative hosted 2 meetings during its launch year. * BSR started to explore additional trauma-informed care training opportunities to be made available to all associates across the market. BSR launched the trauma informed care collaborative in May 2023, conducted a needs assessment generating over 70 responses. BSR plans to rollout training in 2024. Additionally, during the year BSR developed and sent out trauma informed resources and prepared various trauma trainings. * To expand capacity to meet the growing demand around community violence, particularly in the market south, BSR deployed the Bon Secours Violence Response Team to support victims of violence. BSR served 3410 patients with the expansion of services in Market South. Of these patients served by the Violence Response Team, 1037 enrolled to receive additional victim advocate support. * BSR continues to promote the safety of all associates and patients who work in or receive care from its facilities or partner organizations by reviewing workplace violence protocols and implementing security measures that promote safety through a trauma informed lens. BSR implemented more than six initiatives and security measures. These initiatives include the establishment of a Workplace Safety (WPS) team with subcommittees addressing workplace violence, workplace injury, trauma-informed care, safety, and security; establishment of CODE Behavioral Emergency Response Team (BERT); creating a workplace safety newsletter with the opportunity for associate feedback; and other initiatives in progress. * With a commitment to combatting human trafficking, BSR served 35 patients with confirmed or suspected ties to human trafficking during the year. * BSR hosted nine community education events led by the Violence Response Team, to prevent strangulation injury and death. These events were offered in a variety of community settings across the BSR service area, including train the trainer events. * BSR participated in a task force to provide services for Survivors of Sexual Assault, which was created to develop the transfer and treatment documents, along with education for the hospitals self-designated as transfer or treatment facilities. * During 2023, BSR participated in the Gun Lock Distribution Program, where 27 free gun locks were handed out to community members at a GospelFest event. Chronic Disease Implementation Activities: Goal 1: Ensure that all patients, regardless of insurance status, have access to high quality chronic disease management services, same day acute care, annual wellness visits, wellness education, health screenings, and vaccinations. Goal 2: Strengthen and sustain the health safety net continuum through partnerships and investments in primary care and specialty care, food access, medications, and other prevention services. 2023 Strategies and Outcomes: * To expand access to high quality and disease-specific prevention and wellness services, BSR opened and operated a new prevention suite inside Richmond Community Hospital's Medical Office Building. The suite saw its first patients in February. * BSR expanded access to primary and specialty care services, with an emphasis on uninsured or underinsured patients, by opening and operating the new Manchester Community Health Clinic and redeploying the Care-A-Van mobile health team. During 2023, the Care-A-Van mobile health team provided 13,873 patient visits for 5,869 individuals across 24 counties in the services area, and the Manchester Community Health Clinic served 3,287 patients from the time it opened in April. * BSR worked on increasing access to nutrition education, diabetes prevention and management execution, and health cooking and eating programs through the Sarah Garland Jones Center and the addition of community dietitians to the community health team. * To meet the needs of patients more adequately in underserved communities, BSR will explore the reorganization of existing clinical services and the development of new clinical services using a team-based care model to support primary and specialty practices. BSR surpassed its goal for 2023 with four expansions of Community Health services in the Market South and East End areas. The areas of expansion included: prioritization of East End service area residents' access to medical services, reestablishment of OB/GYN outpatient services, expansion of geriatric services, and establishing a multispecialty clinic with cardiology, surgery, oncology, and podiatry services. * During 2023, BSR invested $667,000 to nine regional healthcare nonprofits addressing Chronic Diseases across the services era in an effort to help the most vulnerable, uninsured, and underinsured patients receive the needed care. * Continued participation in collaboratives, such as the Greater Richmond Healthcare Safety Net Collaborative and the Healthy East End Steering Committee, to drive scale towards addressing the prevalence of chronic diseases and root cause health needs, remains a priority for BSR. BSR regularly engaged with the Healthy East End Steering Committee during the year. * Through the Every Woman's Life program, BSR served 1363 women from 30 counties, offering access to mammograms and PAP smears. In addition, 37 women received treatment or resource connection for abnormal or cancerous diagnoses as a result of their connection to the EWL program. * BSR participated in the Health Equity Intensive program through the Virginia Hospital and Healthcare Association to pursue projects to improve chronic disease outcomes by addressing social drivers of health. Schedule H, Part I, Line 6a Community benefit report prepared by related organizationBon Secours Mercy Health, Inc.
Schedule H, Part I, Line 7g Subsidized Health ServicesBon Secours Richmond Health System, Inc. OPERATES THE RICHMOND HOPE THERAPY CENTER CLINIC. THE AMOUNT OF SUBSIDIZED HEALTH SERVICES ATTRIBUTABLE TO THE PHYSICIAN CLINIC IS $640,546.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation113870480
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCost of financial assistance at cost was calculated with a cost to charge ratio using worksheet 2. The cost related to Medicaid patients was determined using Bon Secours Mercy Health's cost accounting system and included both inpatients and outpatients for traditional Medicaid and Medicaid managed care plans. For subsidized services Bon Secours Mercy Health's cost accounting system used to determine cost related to the specific service excluding traditional Medicaid and Medicaid managed care patients. Costs for charity and bad debt accounts are deducted using a ration of cost to charge specific to that subsidized service. Costs for other programs reflect the direct and indirect costs of providing those programs.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. NET PATIENT ACCOUNTS ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL RECEIVABLES BASED UPON BON SECOURS MERCY HEALTH'S (BSMH) HISTORICAL COLLECTION EXPERIENCE ADJUSTED FOR CURRENT ENVIRONMENTAL RISKS AND TRENDS FOR EACH MAJOR PAYOR SOURCE. SIGNIFICANT PROVISION IS MADE FOR SELF-PAY PATIENT ACCOUNTS IN THE PERIOD OF SERVICE BASED ON PAST COLLECTION EXPERIENCE. BSMH'S CONCENTRATION OF CREDIT RISK RELATED TO NET PATIENT ACCOUNTS IS LIMITED DUE TO THE DIVERSITY OF PATIENTS AND PAYORS. NET PATIENT ACCOUNTS CONSIST OF AMOUNTS DUE FROM GOVERNMENTAL PROGRAMS (PRIMARILY MEDICARE AND MEDICAID), PRIVATE INSURANCE COMPANIES, MANAGED CARE PROGRAMS AND PATIENTS THEMSELVES. NET PATIENT SERVICE REVENUE FOR SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY PAYOR COVERAGE IS RECOGNIZED BASED ON CONTRACTUAL RATES FOR SERVICES RENDERED. BSMH RECOGNIZES A SIGNIFICANT AMOUNT OF PATIENT SERVICE REVENUE AT THE TIME SERVICES ARE RENDERED EVEN THOUGH IT DOES NOT ASSESS THE PATIENT'S ABILITY TO PAY. AS A RESULT, THE PROVISION FOR BAD DEBTS IS PRESENTED AS A DEDUCTION FROM PATIENT SERVICE REVENUE (NET OF CONTRACTUAL PROVISIONS AND DISCOUNTS). AMOUNTS RECOGNIZED ARE SUBJECT TO ADJUSTMENT UPON REVIEW BY THIRD-PARTY PAYORS. FOR UNINSURED PATIENTS THAT DO NOT QUALIFY FOR CHARITY CARE, BSMH RECOGNIZES REVENUE WHEN SERVICES ARE PROVIDED. BASED ON HISTORICAL EXPERIENCE, A SIGNIFICANT PORTION OF BSMH'S ININSURED PATIENTS WILL BE UNABLE OR UNWILLING TO PAY FOR SERVICES PROVIDED. THUS, BSMH RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS RELATED TO UNINSURED PATIENTS IN THE PERIOD THE SERVICES ARE PROVIDED. ANY DISCOUNTS APPLIED TO SELF-PAY PATIENTS WOULD BE DEEMED EITHER CHARITY OR A CONTRACTUAL ADJUSTMENT. BAD DEBT WOULD BE BASED ON THE BALANCE AFTER THE CHARITY OR CONTRACTUAL ADJUSTMENT THAT IS DEEMED UNCOLLECTABLE FOLLOWING A REASONABLE COLLECTION EFFORT.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBON SECOURS MERCY HEALTH'S (BSMH) FINANCIAL ASSISTANCE POLICY DOES NOT PERMIT THE COST OF PATIENTS WHO ARE UNCOOPERATIVE OR UNABLE TO BE LOCATED TO BE RECLASSIFIED FROM FINANCIAL ASSISTANCE TO BAD DEBT. BSMH'S FINANCIAL ASSISTANCE POLICY REQUIRES AN APPLICATION AND SUPPORTING DOCUMENTATION. THEREFORE, ZERO DOLLARS ARE BEING REPORTED ON PART III, LINE 3 AS AMOUNTS INCLUDED IN BAD DEBT THAT COULD BE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER BSMH'S FINANCIAL ASSISTANCE POLICY. THE HOSPITALS FOLLOW THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE BAD DEBT EXPENSE AS COMMUNITY BENEFIT.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteBON SECOURS MERCY HEALTH'S (BSMH) AUDITED FINANCIAL STATEMENTS DO NOT CONTAIN A FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE. BSMH ELECTED TO EARLY ADOPT ASU 2011-07. ACCORDINGLY, BAD DEBT EXPENSE IS REFLECTED AS A DEDUCTION FROM REVENUE RATHER THAN AS AN OPERATING EXPENSE. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS, 2. SIGNIFICANT ACCOUNTING POLICIES, (d) NET PATIENT ACCOUNTS AND NET PATIENT SERVICE REVENUE (PAGES 10 & 11) STATES Patient receivables are recorded at net realizable value based on certain assumptions determined by payor class. For third party payors including Medicare, Medicaid, and commercial insurance, the net realizable value is based on the estimated contractual reimbursement percentage, which is based on current contract prices or historical paid claims data by payor. For self-pay receivables, which includes patients who are uninsured and the patient responsibility portion for patients with insurance, the net realizable value is determined using estimates of historical collection experience. These estimates are adjusted for estimated conversions of patient responsibility portions, expected recoveries and any anticipated changes in trends.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsBON SECOURS MERCY HEALTH (BSMH) FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE MEDICARE LOSSES AS COMMUNITY BENEFIT. BSMH'S COST ACCOUNTING SYSTEM WAS USED TO DETERMINE THE MEDICARE AMOUNTS IN PART III.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS KNOWN TO QUALIFY FOR CHARITY CARE OR FINANCIAL ASSISTANCE ARE NOT SENT TO A COLLECTION AGENCY. THE ORGANIZATION REPEATEDLY OFFERS PATIENTS ACCESS TO FINANCIAL HELP DURING THEIR HOSPITAL STAYS AND AFTER, AS WELL AS WITH EACH BILLING NOTICE. BILLS ARE SENT TO A COLLECTION AGENCY AS A LAST RESORT AND ONLY: WHEN PATIENTS HAVE THE ABILITY TO PAY SOME PORTION OF THEIR HEALTHCARE EXPENSES BUT REFUSE TO DO SO; WHEN PATIENTS REFUSE TO WORK WITH THE ORGANIZATION TO DETERMINE IF THEY QUALIFY FOR FREE OR DISCOUNTED CARE VIA FEDERAL, STATE, LOCAL OR HOSPITAL ASSISTANCE PROGRAMS; WHEN THE ORGANIZATION IS UNABLE TO LOCATE THE PATIENT OR PERSON RESPONSIBLE FOR THE BILL. Patients that are presumed to be eligible for financial assistance based on individual life circumstances will be provided 100% financial assistance. Patients determined to have presumptive financial assistance eligibility will not be required to meet income criteria, asset eligibility criteria, or fill out a financial assistance application. BSMH utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain information such as credit scores to assist in determining a patient's presumed eligibility.

Schedule H, Part VI, Line 4 Community informationTHE COMMUNITY FOR EACH HOSPITAL IN THE BON SECOURS RICHMOND HEALTH SYSTEM (BSR) IS DEFINED BOTH BY MISSION AND GEOGRAPHY. THE GEOGRAPHIC COMMUNITY IS DEFINED BY EACH HOSPITAL'S IMMEDIATELY CONTIGUOUS AREAS AS WELL AS BY THE BROADER SURROUNDING COUNTIES/REGIONS WHERE THE MAJORITY OF DISCHARGED PATIENTS RESIDE. ADDITIONALLY, THE COMMUNITY INCLUDES PATIENTS WHO REQUIRE THE EXPERTISE AND SPECIALIZED SERVICES OF A BSR HOSPITAL. Bon Secours Richmond Health System includes seven acute hospital facilities serving the entire Bon Secours Richmond market area which includes Amelia County, Brunswick County, Chesterfield County, Colonial Heights City, Dinwiddie County, Emporia City, Goochland County, Greensville County, Hanover County, Henrico County, Hopewell City, King and Queen County, King William County, Lancaster County, Middlesex County, New Kent County, Northumberland County, Petersburg City, Powhatan County, Prince George County, Richmond City, Southampton County, Surry County, Sussex County. Combined, the 24-locality service area consists of nearly 1.4 million people, and is comprised of 57% Non-Hispanic White, 29% Non-Hispanic Black, 6% Hispanic or Latino, 4% Asian, 4% Two or more races, and <1% Native American. The population is 51% Female, 49% Male. Additionally, 62% of residents are between the ages of 18-64, 22% are between the ages of 0-17, and 16% are 65 years and older. While the largest population in the CHNA Service Area is 18-64 years old, there are a significant number of 45-64-year old's who will be moving into the 65+ category over the next 10 years. Communities with a large aging population will have significantly different needs than communities with a large youth population. Median household incomes in the United States are less than those found in Virginia overall. The data shows that households living in the cities represented in the Bon Secours Richmond market area on average have a lower median household income when compared to the counties in the same area. The percentage of individuals living below the federal poverty level is slightly above the state average but falls below the national average. Median household income is an important determinant of health. Communities with higher poverty rates often experience poorer health. Higher median household incomes are often associated with higher educated residents and lower unemployment rates. High School graduation is a critical measure of overall health. Obtaining a high school diploma leads to increased employment opportunities, higher wages, and economic stability. Although the BSR service area rate is nearly the same as the state average, there is a significant disparity among the Hispanic or Latino population when compared to other races and ethnicities. As a state, Virginia has a slightly higher high school graduation rate at 90.34% than the national average of 88.53%. The overall uninsured rate in the CHNA Service area is 14.09% of adults. The uninsured rate is significantly higher in the urban and rural localities than in the suburban localities. ADDITIONAL DETAIL REGARDING THE COMMUNITY FOR EACH HOSPITAL IN THE BSR SYSTEM CAN BE FOUND IN EACH HOSPITAL'S CHNA AT THE FOLLOWING LINK: https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment

The System was organized in June 1983 to fulfill the healthcare mission of the United States Province of the Congregation of the Sisters of Bon Secours of Paris, a congregation of religious women of the Roman Catholic Church founded in France in 1824. The System's activities are in the states of Ohio, New York, Pennsylvania, Maryland, Virginia, Kentucky, South Carolina, and Florida, each referred to as a local system. The Ministry of BSMH aids those in need, particularly those who are sick and dying, by offering services that include but are not limited to acute inpatient, outpatient, pastoral, palliative, home health, nursing home, rehabilitative, primary and secondary care and assisted living without regard to race, religion, color, gender, age, marital status, national origin, sexual orientation, or disability. As a member of the Catholic health ministry and a member of BSMH, this organization and its related entities are called to continue the healing ministry of Jesus. We exist to benefit the people living in the communities it serves. Through all of the services offered to the community, the mission is "to bring compassion to health care and to be good help to those in need, especially those who are poor and dying. As a System of caregivers, we commit ourselves to help bring people and communities to health and wholeness as part of the healing ministry of Jesus Christ and the Catholic Church." This organization and related organizations share the BSMH Vision. We at BSMH are committed to improve the health of the communities by seeing health broadly though a lens of equity. This allows the Ministry to be recognized as a national leader in addressing the root causes of health disparity through intentional engagement, comprehensive investment and impactful program delivery. To achieve this goal of improving the health of our communities, our work is guided by the BSMH Community Health Strategic Plan. This strategic plan is broken down into four pillars, each with a corresponding focus area. We believe these pillars will allow us to expand upon our already existing commitment to the communities we serve. Pillar 1: Improve Health and Well-Being - Targeted interventions on focused populations that reduced utilization, enhanced well-being, and increased access to care. Pillar 2: Expand Community Development Principles - Clarity and focus on system wide fundraising strategy for community health priorities. Pillar 3: Partner to Build Scale - Work with partners of local, regional, and national prominence and excellence in addressing root causes of health and well-being. Pillar 4: Cultivate Investment and Business Development in the Social Determinants of Health - Test and develop new ventures in the community health worker arena. Please see Schedule R for listings of the related organizations. Each of the reported entities play a role in achieving the vision of BSMH and the SQP (Strategic Quality Plan). IT UTILIZES A MULTI-PERSPECTIVE APPROACH AND PERFORMANCE MEASURES WHICH ARE LINKED TO THE ORGANIZATION'S LONG-TERM STRATEGIC PLAN, ACHIEVEMENT OF ANNUAL SYSTEM OBJECTIVES, AND PERSONAL OBJECTIVES. THE CEO IS NOT PRESENT WHEN THE COMMITTEE DISCUSSES AND ESTABLISHES HIS COMPENSATION. IN ADDITION, THE COMMITTEE DETERMINES IF THE THRESHOLD REQUIREMENTS FOR INCENTIVE AWARDS ARE MET, CONSISTING OF THE ORGANIZATION'S PERFORMANCE RESULTS FOR COMMUNITY BENEFIT, QUALITY, AND FINANCIAL PERFORMANCE, THE COMMITTEE RECOMMENDS TO THE FULL BOARD THE CEO'S SALARY ADJUSTMENT AND INCENTIVE AWARD AS WELL AS THE INCENTIVE AWARD LEVELS FOR WHICH OTHER LISTED INDIVIDUALS MAY BE ELIGIBLE.
