Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - BAYCARE ALLIANT HOSPITAL, INC.. In 2022 BayCare Alliant Hospital SOLICITed INPUT FROM KEY INFORMANTS, THOSE INDIVIDUALS WHO HAVE A BROAD INTEREST IN THE HEALTH OF THE COMMUNITY, via AN ONLINE KEY INFORMANT SURVey. A LIST OF RECOMMENDED PARTICIPANTS WAS PROVIDED BY BayCare Alliant HOSPITAL; THIS LIST INCLUDED NAMES AND CONTACT INFORMATION FOR PHYSICIANS, PUBLIC HEALTH REPRESENTATIVES, OTHER HEALTH PROFESSIONALS, SOCIAL SERVICE PROVIDERS, AND A VARIETY OF OTHER COMMUNITY LEADERS. POTENTIAL PARTICIPANTS WERE CHOSEN BECAUSE OF THEIR ABILITY TO IDENTIFY PRIMARY CONCERNS OF THE POPULATIONS WITH WHOM THEY WORK, AS WELL AS OF THE COMMUNITY OVERALL. KEY INFORMANTS WERE CONTACTED BY EMAIL INTRODUCING THE PURPOSE OF THE SURVEY AND PROVIDING A LINK TO TAKE THE SURVEY ONLINE; REMINDER EMAILS WERE SENT AS NEEDED TO INCREASE PARTICIPATION. IN ALL, 119 COMMUNITY STAKEHOLDERS IN THE BayCare Alliant HOSPITAL SERVICE AREA TOOK PART IN THE ONLINE KEY INFORMANT SURVEY, AS OUTLINED ON APPENDIX E OF THE CHNA. SEVERAL OF THE PARTICIPANTS RESPONDING TO THE SURVEY REPRESENT ORGANIZATIONS WHICH WORK WITH LOW-INCOME, MINORITY OR OTHER MEDICALLY UNDERSERVED POPULATIONS. Additionally, in an effort to ensure the voices of those who are historically underrepresented in our communities were heard, specific focus groups were conducted by engaging individuals from diverse backgrounds including children, older adult populations, Hispanic/LatinX, LGBTQIA+, and Black/African American communities.

Although the majority of the overarching goals, objectives and efforts focus on these top three focus areas, BayCare Alliant Hospital remained committed to supporting positive advancements in addressing diabetes, heart disease and stroke, while addressing the focus area of exercise, nutrition and weight. There were five additional health focus areas identified through the CHNA. These were: * Cancer * Immunization and infectious disease * Maternal, fetal and infant health * Oral health * Respiratory disease These remaining health needs are significant, but they're not directly addressed in 2023-2025 health improvement plan strategies. However, they continued to be impacted through existing hospital strategies for clinical excellence and BayCare Alliant Hospital partnerships within the community, including the Florida Department of Health and community organizations who may be in a better position to address these health issues. THESE PATIENTS ARE SCREENED BY DESIGNATED TEAM MEMBERS IN OUR FINANCIAL ASSISTANCE DEPARTMENT. THE AGENCY FOR HEALTH CARE ADMINISTRATION (AHCA) DEFINES CHARITY ELIGIBILITY AT 200 PERCENT OF THE FEDERAL POVERTY GUIDELINES, UNLESS THE TOTAL HOSPITAL BILL IS MORE THAN 25 PERCENT OF THE PATIENT'S ANNUAL INCOME. Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceFINANCIAL ASSISTANCE AND MEANS-TESTED GOVERNMENT PROGRAMS COSTS (LINES A THROUGH D) ARE DETERMINED USING OUR COST ACCOUNTING SYSTEM, WHICH CAPTURES ALL INPATIENTS AND OUTPATIENTS, INCLUDING EMERGENCY ROOM PATIENTS. THE SYSTEM ALSO CAPTURES ALL PATIENT PAY TYPES - PRIVATE INSURANCE, MEDICARE, MEDICAID, UNINSURED AND SELF-PAY. THE COSTS HAVE BEEN OFFSET BY ANY PAYMENTS RECEIVED FROM MEDICAID OR ANY OTHER UNCOMPENSATED CARE PROGRAM. OTHER BENEFITS AT COST (LINES E THROUGH J, AS WELL AS AMOUNTS REPORTED IN PART II) WERE COMPILED BY THE COMMUNITY BENEFIT DEPARTMENT USING THE CATHOLIC HEALTH ASSOCIATION GUIDE FOR PLANNING AND REPORTING COMMUNITY BENEFITS.
Schedule H, Part II Community Building ActivitiesBAYCARE ALLIANT HOSPITAL SUPPORTS ACTIVITIES THAT PROMOTE THE HEALTH AND WELLBEING OF COMMUNITY MEMBERS THROUGH COMMUNITY SUPPORT.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBAD DEBTS reported in Part III (if any) represent charges written off due to demise of the individual prior to the balance being paid in full or financial obligations discharged in bankruptcy. THIS ADDITIONAL EXPENSE INCLUDES COSTS OF PHYSICIAN SERVICES (EMERGENCY ON-CALL FEES, HOSPITALIST PROGRAM, RECRUITMENT, ETC.), ADVERTISING COSTS, CAFETERIA COSTS FOR MEALS SOLD TO VISITORS, ETC. THE HOSPITAL ATTEMPTS TO COLLECT COINSURANCE AND DEDUCTIBLES FROM MEDICARE BENEFICIARIES. TO THE EXTENT COLLECTION EFFORTS ARE UNSUCCESSFUL, MEDICARE REIMBURSES THE HOSPITAL AT 65% OF UNPAID AMOUNTS. ANY SHORTFALL AMOUNTS HAVE NOT BEEN TREATED AS COMMUNITY BENEFIT.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS WHO ARE UNABLE TO PAY ARE ENCOURAGED BY BAYCARE ALLIANT HOSPITAL REPRESENTATIVES, VIA PERSONAL INTERVIEWS, SIGNAGE ON PATIENT BILLING STATEMENTS, BROCHURES OR CUSTOMER SERVICE PHONE CALLS, TO SUBMIT FINANCIAL INFORMATION TO THE FINANCIAL ASSISTANCE DEPARTMENT TO DETERMINE ELIGIBILITY FOR PROGRAMS, SUCH AS COUNTY, MEDICAID, DISABILITY, VICTIMS OF CRIME, CHARITY, ETC. FOR THOSE PATIENTS WHO PROVIDE ALL THE NECESSARY DOCUMENTATION AND QUALIFY FOR CHARITY ACCORDING TO THE FINANCIAL ASSISTANCE POLICY, (DEFINED IN PART I, LINE 3C), the PATIENT's ACCOUNT BALANCE WOULD BE WRITTEN OFF COMPLETELY TO CHARITY AND NOT BILLED TO THE PATIENT. Since the charity care policy is 100% for patients who are eligible, there is no remaining patient portion; therefore, there would never be a situation where a patient is known to qualify for financial assistance but is experiencing collection actions.

Schedule H, Part VI, Line 5 Promotion of community healthBayCare Alliant Hospital opened its doors on March 8, 2008. Recognizing a gap in the continuum of care and services for the medically complex needs of patients in our community, this Specialty Long Term Acute Care Hospital was created for patients who were identified as needing hospital-level care for an extended period. Located in Pinellas County, BayCare Alliant provides care for patients who need cardiopulmonary care, particularly ventilator-dependent patients; treatment of patients with multiple illnesses or multiple system failure; treatment of wounds caused by disease or accident and treatment of patients requiring interdisciplinary comprehensive medical rehabilitation. By providing placement for these patients, we can transition them out of acute care settings and into a facility with critical care specialists. BayCare Alliant continues to serve the community by providing specialized care for chronic medical conditions requiring extended care and treatment, while helping the acute care hospitals meet the demand for critical care patient beds. BayCare Alliant is identified as a hospital within-a-hospital (HIH). Physically located within our host hospital, Mease Dunedin Hospital, BayCare Alliant has its own entrance. This provides distinction between hospital setting as well as our team members, providers and visitors direct access to Alliant Hospital. As a HIH, BayCare Alliant has 24-hour access to essential diagnostic equipment and surgical services. Medical emergencies can be managed onsite, preventing unnecessary external transfers. BayCare Alliant's Intensive Care Unit is equipped with eCare, allowing our physicians to monitor patients round-the-clock. Our ICU allows us to accept critically ill patients earlier in their hospitalization and provide 2-to-1 nursing ratios for patient care. The eCare team uses advanced technology to electronically monitor each patient's medical condition 24 hours a day, seven days a week and provides a specialty-trained critical care physician from 7 pm to 7 am. BayCare Alliant's mission: We will provide excellence in care for patients with prolonged illnesses recognizing that optimum healing requires a team approach to both medical and human values. Our Board of Trustees is comprised of people who reside in the community we serve. These board members are neither employees nor independent contractors of the organization. A majority of the organization's governing body is comprised of persons who reside in the organization's primary service area who are neither employees nor independent contractors of the organization, nor family members thereof. The organization extends medical staff privileges to all qualified physicians in its community for some or all of its departments or specialties. The organization uses surplus funds for improvements in facilities and equipment, patient care, medical training, education, and research. BayCare Alliant participates in the collaboration of the community health needs assessment (CHNA) for Pinellas County to better understand our community and identify areas of need. The CHNA process ensures the voices of Pinellas County are heard and that we engage those most impacted by health issues and access to care. For several years, BayCare Alliant has partnered with Dunedin Cares Community Food Pantry, holding food drives, participating in health screenings, and offering nutrition education. Since 2021, as part of our CHNA initiative, BayCare Alliant awards Dunedin Cares a monetary contribution to support procurement of fresh produce and dairy products to help eradicate food insecurity. BayCare Alliant has also established an ongoing partnership with the Homeless Empowerment Program (HEP), assisting homeless and low-income individuals, families, and veterans with housing, food, and clothing. In addition, Throughout the year, BayCare Alliant facilitates holiday toy drives and back to-school drives for families in need. As an additional CHNA initiative, BayCare Alliant has provided contributions to fund both HEP's Vision Program and Behavioral Health Program. BayCare Alliant actively participates and builds strong relationships within our community. We work to provide visibility and collaboration with our local business leaders. BayCare Alliant remains active in BayCare-sponsored initiatives such as Pitch for Pink and Holiday Lights. These events provide enjoyable learning opportunities to educate our community members about health resources and services in their local communities. In addition, BayCare Alliant's dedicated leadership team maintains strong relationships with our community care coordinators and physicians. These healthcare professionals work together to ensure the correct level of care for patients is identified and that patients and their families feel included and engaged in decisions being made about their loved one.
Schedule H, Part VI, Line 6 Affiliated health care systemBayCare Health System, headquartered in Clearwater, Florida, is the leading, not-for-profit health care system that connects individuals and families to a wide range of services at 16 hospitals and hundreds of other convenient locations throughout West Central Florida, including Tampa Bay. Inpatient and outpatient services include acute care, primary care, imaging, laboratory, surgery centers, wellness centers, behavioral health, home care and urgent care. BayCare's 2023 Report to the Community can be viewed at BayCare.org/Annual-Report. BayCare's 16 hospitals are Bartow Regional Medical Center, BayCare Alliant, BayCare Hospital Wesley Chapel, Mease Countryside, Mease Dunedin, Morton Plant, Morton Plant North Bay, St. Anthony's, St. Joseph's, St. Joseph's Children's, St. Joseph's Women's, St. Joseph's-North, St. Joseph's-South, South Florida Baptist, Winter Haven and Winter Haven Women's. (In certain cases, hospital locations with the same tax identification and state license number are listed as one facility on Form 990, Schedule H, consistent with IRS reporting guidelines.) In March 2023, BayCare opened its 16th hospital, BayCare Hospital Wesley Chapel, to serve a rapidly growing area of Pasco County on Florida's gulf coast. BayCare also continued construction of a replacement hospital for South Florida Baptist Hospital in Plant City, with an expected opening in 2024, and announced plans to build a new hospital for St. Joseph's Children's Hospital in Tampa. In April 2023, BayCare made its first move into Manatee County with the opening of a primary care medical office. BayCare is working on plans to build Manatee's first not-for-profit hospital, with a projected opening in 2027. BayCare was founded in 1997 after leaders of several of the area's independent, not-for-profit hospitals began discussing ways to ensure that high-quality, not-for-profit health care would remain a viable option for the Tampa Bay community for decades to come. The founding hospitals agreed to sacrifice some of their autonomy to be operated by a new entity, BayCare Health System. With $5.5 billion in operating revenue in 2023, BayCare is now a fully integrated health system dedicated to providing high-quality, compassionate care to all we serve, regardless of their ability to pay. In 2023, BayCare had 4,004 hospital beds, 18 urgent care centers, 4 surgery centers, 15 outpatient imaging facilities, 185 physician practice locations and 20 behavioral health practice locations. BayCare annually generates $10 billion in economic impact in the region and state. BayCare is one of the largest employers in the Tampa Bay area, with 31,993 team members. During 2023, BayCare provided $766 million in benefits for its employees, including $238 million in retirement savings, $208 million in medical insurance, $193 million in paid time off and $70 million in education assistance. In 2023, BayCare was ranked in the top quartile of large U.S. health systems by Fortune/PINC AI based on clinical outcomes, operational efficiency and patient experience. In addition, all 11 of BayCare's eligible acute care hospitals received "A" grades on patient safety from The Leapfrog Group in fall 2023; BayCare hospitals earned numerous five-star ratings on clinical outcomes from Healthgrades; and BayCare earned 12 Press Ganey awards for extraordinary patient experience based on patient surveys. In the area of workplace honors, BayCare was named to two prestigious national lists: Fortune Best Workplaces in Health Care and PEOPLE Companies that Care. BayCare also made the 2023 Top Workplaces list of the Tampa Bay Times newspaper, an achievement annually since 2015. In 2023, BayCare provided $557 million in total Community Benefit, including $399 million in Medicaid and other income-based programs, $81 million in traditional charity care and $77 million in unbilled community services, all measured in unreimbursed costs. Addressing hunger has been a vital mission for BayCare since 2019, when a Community Health Needs Assessment identified food insecurity as one of the top issues in the Tampa Bay and West Central Florida regions. After a second needs survey in 2022 established that food insecurity was still a pressing issue, BayCare expanded its efforts. During 2023, BayCare contributed major financial support to 42 school-based food pantries operated by BayCare partner Feeding Tampa Bay, providing free food to students, their families and others who live near each school. The pantries served 30,000 households during 2023. BayCare also asks all patients admitted to its 16 hospitals if they are food insecure and offers a "Healing Bag" upon discharge to anyone who answers "yes." The bags contain two to three days of nonperishable food, plus information about food resources near their homes. More than 13,000 Healing Bags were distributed during 2023. Also in 2023, BayCare stepped up on another issue of great concern across Florida and the nation: children's mental health. BayCare brought to the Tampa Bay region "On Our Sleeves," a national movement to address the mental health needs of children. BayCare is making a $250,000 annual investment so parents and caregivers can utilize the program's tools and resources. Twenty-six years after BayCare was created, it is achieving the goal of its founders to preserve access to not-for-profit health care for the communities of Tampa Bay and West Central Florida, and it is fulfilling its Mission as an organization to "improve the health of all we serve through community-owned services that set the standard for high-quality, compassionate care."
