Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationSchedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. Their related income is exempt from federal income tax under Section 501(A). Some of the subsidiaries are taxable entities, and some of the income of the tax-exempt entities is subject to taxation as unrelated business taxable income. The Company and its subsidiaries file U.S. federal income tax returns, and they also file in various state and foreign jurisdictions. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist as of December 31, 2023. Accounting for uncertainty in income taxes, ASC Topic 740-10 prescribes a comprehensive model for how an organization should measure, recognize, present and disclose in its financial statements uncertain tax positions that an organization has taken or expects to take on a tax return. The Company is subject to routine audits by taxing jurisdictions. There are no current audits in progress. As of December 31, 2023, the Company has no uncertain tax positions. As of December 31, 2023 and 2022, the Company's taxable subsidiaries had net operating loss carryforwards for Federal income tax purposes of $182,444 and $169,279, respectively, which are available to offset future Federal taxable income subject to various limitations. The Company had net operating loss carryforwards for state income tax purposes of $119,717 and $113,528 as of December 31, 2023 and 2022, respectively, which are available to offset future state taxable income subject to various limitations. The Company accounts for income taxes under the asset and liability method. Under this method, deferred tax assets and liabilities are recognized for the estimated future tax consequences attributable to differences between the financial statement carrying amounts of existing assets and liabilities and their respective tax bases. Deferred tax assets and liabilities are measured using enacted tax rates expected to be in effect during the year in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. Interest and penalties related to income taxes are accounted for as income tax expense. As of December 31, 2023, the Company has placed a partial valuation allowance of $83,168 and $18,945 on the gross deferred Federal and state tax assets, respectively. As of December 31, 2022, the Company has placed a partial valuation allowance of $156,538 and $103,396 on the gross Federal and state deferred tax assets, respectively. When determining the valuation allowance for deferred tax assets, the Company annually considers whether it is more likely than not that these amounts will not be utilized in future periods. There was a decrease in the valuation allowance for gross deferred Federal and state tax assets of $73,370 and $84,451, respectively, as of December 31, 2023. There was an increase in the valuation allowance for gross deferred federal tax assets of $1,354 and a decrease in the valuation allowance for gross deferred state tax assets of $10,532 as of December 31, 2022.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - St. Francis Hospital, Inc.. Bon Secours St. Francis Health System, including Bon Secours St. Francis Downtown, Bon Secours St. Francis Eastside, Bon Secours St. Francis Millennium, and Bon Secours St. Francis Medical Campus at Simpsonville completed a joint CHNA report which reflects the collaborative efforts to conduct an assessment of the health needs of the community they serve. The CHNA was created utilizing quantitative data which was collected at national, state, regional, and local levels as well as qualitative data that was collected through community input. Key data sources included the United States Census Bureau; Healthy People 2030; County Health Rankings and Roadmaps; Centers for Disease Control; and South Carolina DHEC biostatistics, vital records, county health profiles and hospital discharge data. Dates of data collected ranged from 2000 - 2021. For each indicator, data was pulled for the most recent year available. To assess trends and progress, data points were compared to data pulls from the previous CHNA study when updated data points were available. Collection and analysis of qualitative data was completed through a series of town hall meetings and the implementation of a community survey. Qualitative data was collected by the Johnson Group, the consulting firm that compiled that data in the previous CHNA studies. Three Town Hall meetings were conducted in January 2022. Originally set to be completed in person, due to a community spike in COVID cases, all three Town Halls were completed by Zoom. The Town Halls included over 70 community residents, faith-based representative, and some health and human service representatives. These groups represented the interests of urban and suburban residents, mostly from less affluent communities that are home to lower-income families, minority families and the elderly. The community-wide survey reached 775 randomly selected Greenville County residents who completed a lengthy questionnaire about community health issues. Both the secondary data and the town hall results were used to inform survey question selection and the prioritization process of identified needs. A 21-member Advisory team of local public health professionals, health and human service agencies, and Bon Secours St. Francis Health System leaders assisted in guiding the assessment, providing additional insight and feedback into community issues and priorities. The advisory committee met three times between January - April 2022. Additional detail can be found on the Bon Secours website at https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - St. Francis Hospital, Inc.. St. Francis Hospital, Inc. St. Francis Hospital, Inc. is comprised of ST. FRANCIS HOSPITAL - DOWNTOWN and ST. FRANCIS HOSPITAL - EASTSIDE. These hospitals are collectively referred to as "the hospital needs identified in its CHNA are addressed with regional strategies. Unless noted below, both St. Francis Hospital - Downtown and St. Francis Hospital - Eastside participated in the regional strategies. Housing and Homelessness Implementation Activities: Goal 1: Continue to work with Habitat for Humanity to complete our build commitment in the Sterling Community. Goal 2: Work with organizations that focus on home ownership and home preservation/protection (Rebuild, Habitat, Greenville Human Relations Commission, Sterling Land Trust, etc.) to promote and protect home ownership in target neighborhoods among target populations. 2023 Strategies and Outcomes: * The Hospital continued its efforts to complete the original commitment with Habitat for Humanity to partner on 10 builds. During 2023, the Hospital had a goal to complete one new build to add to the seven that were previously completed. The Hospital surpassed this by providing enough resources to assist four families instead of just one. * The Hospital partnered with organizations that focus on home ownership and home preservation in target neighborhoods among target populations, primarily seniors, to increase their ability to stay in their own homes and improve their housing knowledge. During 2023, the partnership with Rebuild Upstate identified homeowners who needed help with repairs, and the hospital contributed $50,000 which enabled Rebuild Upstate to help 5 senior homeowners complete needed repairs, including providing a new roof for one homeowner who had been waiting for help for over two years. * During 2023, the Hospital partnered with LifeWise to raise awareness of home repair and other programs that allow seniors to remain in their own homes. This partnership led to the Hospital and LifeWise hosting an Aging in Place event that drew over 100 attendees and focused on home repair and meal delivery programs that help seniors to remain in their own homes. * In order to help facilitate housing-related educational programs, the hospital developed partnerships with four other community organizations. These partners are Habitat for Humanity, Rebuild Upstate, Mercy Housing SE, and Area Agency on Aging. Access to Care (with a health equity and cultural competency lens) Implementation Activities: Goal 1: Improve access to care in target neighborhoods by helping local churches and community centers create and/or equip health ministries. Goal 2: Increase awareness of and access to available health and Social Determinants of Health (SDoH) related resources for senior adults. Goal 3: Improve continuity of care by increasing the number of at-risk individuals with obesity and chronic conditions who have a care plan. 2023 Strategies and Outcomes: * The hospital worked to improve access to care by helping local churches and community centers create and/or equip health ministries. During 2023 the Hospital accomplished the following towards this strategy: * Continued to have strong relations with many churches and regularly work with their pastors and ministers of health to provide information classes. Additionally, the Hospital's Faith Community Coordinator holds quarterly meetings with the LiveWell's At Worship (LAW) group, which will allow the hospital to begin empowering local congregations to take charge of their own health. * Developed three community partnerships that have been formed for mental health trainings, including the Hold Out the Lifeline for Mental Health First Aid trainings, the Hispanic Alliance, the Piedmont Health Foundation for MHFA as well as other trainings. * Developed a portfolio of evidence-based trainings that includes Diabetes Prevention Program, HeartMath, National Association for Mental Illness courses (two courses both in English and Spanish), Mental Health First Aid, and Matter of Balance. * Provided 16 individual trainings to staff members to increase the breadth of evidence-based trainings we can offer in the community. * To increase awareness of and access to available health and SDoH-related resources for seniors, the hospital in partnership with LifeWise created a virtual resource guide during 2023, with revisions to be made on an ongoing basis. * The hospital hosted an Aging in Place event in May 2023 and had over 100 attendees, with presentations on housing, meals, senior support programs, and reducing isolation. * The hospital continues to improve continuity of care by increasing the number of at-risk individuals with obesity and chronic conditions who have a care plan. The Hospital's strategy was to work with internal stakeholders to improve continuity of care. This was accomplished by the hospital performing the following actions during 2023: * Collaborated with Bon Secours case management, Legacy Clinic, and Bon Secours Mobile Clinic to develop and implement a plan to actively identify patients who are at high risk for readmission, returning to the ED, or initial unnecessary ED visits and inpatient stays who are also eligible for Wellness Outreach services. Wellness Outreach designated staff members also started reviewing inpatient census information to proactively identify patients that are appropriate candidates for services and communicate directly with the Inpatient Case Management Team to facilitate services upon discharge. * This collaboration also worked on implementing Wellness Outreach notes into EPIC. Steps were taken to formulate a process within EPIC, including multiple meetings with attendees ranging from market presidents all the way to staff. Future activities are being considered pending Compliance review. Behavioral and Mental Health Implementation Activities: Goal 1: Expand mental/behavioral health educational opportunities and awareness of resources for the senior population. Goal 2: Expand mental/behavioral health resources available in under-resourced communities. 2023 Strategies and Outcomes: * During 2023, the Hospital worked to expand mental and behavioral health educational opportunities and awareness of resources for the senior population by developing a partnership with the Regional Practice Administrator over behavioral health. This partnership helped the hospital in identifying three providers/therapists who would be willing to partner with the LifeWise program to present mental and behavioral health topics of critical interest to senior adults. The three identified providers/therapists in collaboration with the Hospital also held a Walk With A Doc event and a provider presentation. * Continued efforts were made to expand mental and behavioral health resources available in under resourced communities through churches, community centers, and other partner organizations. During 2023, the hospital identified three mental health trainings that would allow staff to provide critical trainings in the community. Additionally, seven staff members completed one or more evidence-based classes in mental/behavioral health. All prioritized health needs identified in the hospital's most recent CHNA have been addressed.

Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation118491082
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCost of financial assistance at cost was calculated with a cost to charge ratio using worksheet 2. The cost related to Medicaid patients was determined using Bon Secours Mercy Health's cost accounting system and included both inpatients and outpatients for traditional Medicaid and Medicaid managed care plans. For subsidized services Bon Secours Mercy Health's cost accounting system used to determine cost related to the specific service excluding traditional Medicaid and Medicaid managed care patients. Costs for charity and bad debt accounts are deducted using a ration of cost to charge specific to that subsidized service. Costs for other programs reflect the direct and indirect costs of providing those programs.
Schedule H, Part II Community Building ActivitiesBon Secours Mercy Health (BSMH) addresses various community concerns including health improvement, poverty, workforce development, and access to health care. BSMH hospitals conduct community health education and support groups, health fairs and screenings for the communities served. BSMH hospitals work with state and local leadership to address community needs and provide healthcare services to the poor and underserved. BSMH is committed to addressing the social determinants of health (SDOH) and social needs for patients and communities throughout our footprint. With a deep understanding of both areas, the team is working systematically across the ministry and in our local communities to ensure we respond holistically and impactfully. BSMH addresses SDOH through its Community Health Needs Assessment (CHNA), Community, investment strategy, advocacy/public policy partnerships, and cross sector solutions. BSMH addresses social needs through SDOH patient assessment, health education/promotion programs, capacity building of local community resources, and closed loop referral system. BSMH hospitals provide programs to improve the physical surroundings and housing in the communities served. Inadequate housing has a negative impact on the health of residents in the area by leading to violence in the neighborhoods. A robust economy positively impacts residents covered by health insurance and improves the capacity of the community to support health services. Additional detail regarding BSMH's community building activities and the promotion of health of its communities can be found in its 2022 Community Health Annual Report available at https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment and https://www.mercy.com/about-us/mission/giving-back/community-health-needs-assessment.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. NET PATIENT ACCOUNTS ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL RECEIVABLES BASED UPON BON SECOURS MERCY HEALTH'S (BSMH) HISTORICAL COLLECTION EXPERIENCE ADJUSTED FOR CURRENT ENVIRONMENTAL RISKS AND TRENDS FOR EACH MAJOR PAYOR SOURCE. SIGNIFICANT PROVISION IS MADE FOR SELF-PAY PATIENT ACCOUNTS IN THE PERIOD OF SERVICE BASED ON PAST COLLECTION EXPERIENCE. BSMH'S CONCENTRATION OF CREDIT RISK RELATED TO NET PATIENT ACCOUNTS IS LIMITED DUE TO THE DIVERSITY OF PATIENTS AND PAYORS. NET PATIENT ACCOUNTS CONSIST OF AMOUNTS DUE FROM GOVERNMENTAL PROGRAMS (PRIMARILY MEDICARE AND MEDICAID), PRIVATE INSURANCE COMPANIES, MANAGED CARE PROGRAMS AND PATIENTS THEMSELVES. NET PATIENT SERVICE REVENUE FOR SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY PAYOR COVERAGE IS RECOGNIZED BASED ON CONTRACTUAL RATES FOR SERVICES RENDERED. BSMH RECOGNIZES A SIGNIFICANT AMOUNT OF PATIENT SERVICE REVENUE AT THE TIME SERVICES ARE RENDERED EVEN THOUGH IT DOES NOT ASSESS THE PATIENT'S ABILITY TO PAY. AS A RESULT, THE PROVISION FOR BAD DEBTS IS PRESENTED AS A DEDUCTION FROM PATIENT SERVICE REVENUE (NET OF CONTRACTUAL PROVISIONS AND DISCOUNTS). AMOUNTS RECOGNIZED ARE SUBJECT TO ADJUSTMENT UPON REVIEW BY THIRD-PARTY PAYORS. FOR UNINSURED PATIENTS THAT DO NOT QUALIFY FOR CHARITY CARE, BSMH RECOGNIZES REVENUE WHEN SERVICES ARE PROVIDED. BASED ON HISTORICAL EXPERIENCE, A SIGNIFICANT PORTION OF BSMH'S UNINSURED PATIENTS WILL BE UNABLE OR UNWILLING TO PAY FOR SERVICES PROVIDED. THUS, BSMH RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS RELATED TO UNINSURED PATIENTS IN THE PERIOD THE SERVICES ARE PROVIDED. ANY DISCOUNTS APPLIED TO SELF-PAY PATIENTS WOULD BE DEEMED EITHER CHARITY OR A CONTRACTUAL ADJUSTMENT. BAD DEBT WOULD BE BASED ON THE BALANCE AFTER THE CHARITY OR CONTRACTUAL ADJUSTMENT THAT IS DEEMED UNCOLLECTABLE FOLLOWING A REASONABLE COLLECTION EFFORT.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBON SECOURS MERCY HEALTH'S (BSMH) FINANCIAL ASSISTANCE POLICY DOES NOT PERMIT THE COST OF PATIENTS WHO ARE UNCOOPERATIVE OR UNABLE TO BE LOCATED TO BE RECLASSIFIED FROM FINANCIAL ASSISTANCE TO BAD DEBT. BSMH'S FINANCIAL ASSISTANCE POLICY REQUIRES AN APPLICATION AND SUPPORTING DOCUMENTATION. THEREFORE, ZERO DOLLARS ARE BEING REPORTED ON PART III, LINE 3 AS AMOUNTS INCLUDED IN BAD DEBT THAT COULD BE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER BSMH'S FINANCIAL ASSISTANCE POLICY. THE HOSPITAL FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE BAD DEBT EXPENSE AS COMMUNITY BENEFIT.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteBON SECOURS MERCY HEALTH'S (BSMH) AUDITED FINANCIAL STATEMENTS DO NOT CONTAIN A FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE. BSMH ELECTED TO EARLY ADOPT ASU 2011-07. ACCORDINGLY, BAD DEBT EXPENSE IS REFLECTED AS A DEDUCTION FROM REVENUE RATHER THAN AS AN OPERATING EXPENSE. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS, 2. SIGNIFICANT ACCOUNTING POLICIES, (d) NET PATIENT ACCOUNTS AND NET PATIENT SERVICE REVENUE (PAGES 10 & 11) STATES Patient receivables are recorded at net realizable value based on certain assumptions determined by payor class. For third party payors including Medicare, Medicaid, and commercial insurance, the net realizable value is based on the estimated contractual reimbursement percentage, which is based on current contract prices or historical paid claims data by payor. For self-pay receivables, which includes patients who are uninsured and the patient responsibility portion for patients with insurance, the net realizable value is determined using estimates of historical collection experience. These estimates are adjusted for estimated conversions of patient responsibility portions, expected recoveries and any anticipated changes in trends.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsBON SECOURS MERCY HEALTH (BSMH) FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE MEDICARE LOSSES AS COMMUNITY BENEFIT. BSMH'S COST ACCOUNTING SYSTEM WAS USED TO DETERMINE THE MEDICARE AMOUNTS IN PART III.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS KNOWN TO QUALIFY FOR CHARITY CARE OR FINANCIAL ASSISTANCE ARE NOT SENT TO A COLLECTION AGENCY. THE ORGANIZATION REPEATEDLY OFFERS PATIENTS ACCESS TO FINANCIAL HELP DURING THEIR HOSPITAL STAYS AND AFTER, AS WELL AS WITH EACH BILLING NOTICE. BILLS ARE SENT TO A COLLECTION AGENCY AS A LAST RESORT AND ONLY: WHEN PATIENTS HAVE THE ABILITY TO PAY SOME PORTION OF THEIR HEALTHCARE EXPENSES BUT REFUSE TO DO SO; WHEN PATIENTS REFUSE TO WORK WITH THE ORGANIZATION TO DETERMINE IF THEY QUALIFY FOR FREE OR DISCOUNTED CARE VIA FEDERAL, STATE, LOCAL OR HOSPITAL ASSISTANCE PROGRAMS; WHEN THE ORGANIZATION IS UNABLE TO LOCATE THE PATIENT OR PERSON RESPONSIBLE FOR THE BILL. Patients that are presumed to be eligible for financial assistance based on individual life circumstances will be provided 100% financial assistance. Patients determined to have presumptive financial assistance eligibility will not be required to meet income criteria, asset eligibility criteria, or fill out a financial assistance application. BSMH utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain information such as credit scores to assist in determining a patient's presumed eligibility.

Schedule I, Part II SCHEDULE I, PART IIDescription of the Bon Secours Mercy Health Mission Fund: Bon Secours Mercy Health performs its philanthropic work through its mission department. This initiative, called the Bon Secours Health System Mission Fund ("Mission Fund"), was developed to promote the Catholic Health Ministry and the Bon Secours Mercy Health, Inc. Mission. This purpose is realized through the funding of initiatives that improve the health and well-being of communities, particularly for disenfranchised and marginalized people, served by Bon Secours Mercy Health Local Systems ("Local Systems"), Cosponsors and the Congregation of the Sisters of Bon Secours. The scope of its purpose and use of funds would be to: -promote healthy community coalition initiatives in conjunction with local system efforts, -develop local system and community excellence for a specific health condition and preventive need, and -improve access for uninsured populations and reduce health disparities among populations in the community. The Strategic Quality Plan of the health system calls for focused efforts to Build Healthier Communities. The health system understands "health" to include social and communal dimensions and has adopted the following articulation of a healthy community. The conditions of communities and individuals served by Bon Secours Mercy Health reflect the interaction of significant factors and complex behaviors at the individual, communal, and societal level. It is not likely that interventions by any one organization will result in substantial improvement or benefit to the community. Rather, increased participation by stakeholders and greater cooperation among entities with appropriate skills and resources is necessary for systemic change and improved outcomes. Mission Fund grants place emphasis on increased collaboration among community based members (Healthy Community Initiative), public health officials and other providers of services. The Mission Fund anticipates that most endeavors that seek to bring meaningful improvement require time and commitment. Consequently, local system grant recipients may expect continuity of support (several years) to establish and track outcomes. At the same time, grant applicants need to cultivate and achieve a wide array of financial resources necessary to sustain promising projects and service programs.
Schedule I, Part I, Line 2 Procedures for monitoring use of grant funds.THE ORGANIZATION COMPLIES WITH BON SECOURS MERCY HEALTH, INC.'S SYSTEM-WIDE FINANCIAL AND ACCOUNTING POLICIES. CONTRIBUTIONS ARE GENERALLY MADE AS REIMBURSEMENTS FOR FUNDS SPENT. IN SUCH CASES, THE DONEE/GRANTEE ORGANIZATION MUST PROVIDE DOCUMENTATION TO THE FILING ORGANIZATION BEFORE FUNDS ARE APPROVED FOR DISBURSEMENT. IN OTHER CASES, GRANTEES SUBMIT PROGRESS REPORTS ON THE ANNIVERSARY DATE ON WHICH THE GRANT WAS RECEIVED.
