Schedule D, Part V, Line 4 Intended uses of endowment fundsThe earnings are to be used for education, charity care, and other hospital programs. Swedish Covenant Health is the related beneficiary of the endowment funds held by Swedish Covenant Hospital Foundation.
Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteThe affiliates and subsidiaries of the System are primarily tax-exempt organizations under Internal Revenue Code Section 501(c)(3), and their related income is exempt from federal income tax. Accordingly, there is no material provision for income tax for these entities. Some of the income received by exempt entities is subject to taxation as unrelated business income. The System files federal income tax returns and returns for various states in the U.S. ASC 740-10, Income Taxes, requires that realization of an uncertain income tax position is more likely than not (i.e., greater than 50% likelihood of receiving a benefit) before it can be recognized in the consolidated financial statements. Furthermore, this interpretation prescribes the benefit to be recorded in the consolidated financial statements as the amount most likely to be realized assuming a review by tax authorities having all relevant information and applying current conventions. This interpretation also clarifies the financial statement classification of tax-related penalties and interest and sets forth new disclosures regarding unrecognized tax benefits. No amount was recorded for the year ended December 31, 2023. At year-end December 31, 2023, the System has a net operating loss carryforward of $371,890, which generated deferred tax assets of $107,621. These assets are offset by a valuation allowance of $101,354.

Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - Swedish Hospital. Swedish Hospital (Swedish) and members of the Alliance for Health Equity (AHE), a collaborative of over 30 hospitals partnering with health departments and community based organizations, worked together throughout 2021-2022 to build a comprehensive Community Health Needs Assessment (CHNA) in Chicago and Cook County. Swedish Hospital and the Alliance for Health Equity adapted the Mobilizing for Action through Planning and Partnerships (MAPP) model for the CHNA, emphasizing the importance of community engagement, partnership development, and both primary and secondary data. The Alliance for Health Equity chose this inclusive, community-driven process to leverage and align with health department assessments and to actively engage stakeholders, including community members, in identifying and addressing strategic priorities to advance health equity. Swedish Hospital worked with the Alliance for Health Equity and local community partners in the communities we serve to identify and prioritize the health needs within our community through 2024. As part of the hospital's community outreach efforts, Swedish established a Community Leader Engagement Program, to invite leaders from throughout the community to convene several times per year to discuss community health needs and ways to collaborate for the benefit of all. Together, the group has worked on the following health initiatives: head injury prevention/bike helmet safety, bystander CPR and community wellness (fitness, nutrition, mind/body, prevention). This Community Leader group consists of more than 100 social service organizations, health care providers, elected officials, schools and others. Members include: Apna Ghar, Between Friends, Budlong Elementary School, Centro Romero, Chicago Police - 20th District, Common Pantry, Erie Family Health, The Friendship Center, HANA Center, Hamdard Health, Hatzalah, Heartland Health Centers, ICNA Relief, Lutheran Social Services, Neighborhood Boys and Girls Club and Rohingya Culture Center. As part of the CHNA process, Swedish's Community Leader group met with hospital leaders and the Alliance for Health Equity staff to provide feedback on initial findings and priorities from the CHNA. Additionally, various organizations from the Community Leader group were active contributors to focus groups related to health care providers and social services, including immigrant and refugee-serving organizations. The Alliance for Health Equity's collaborative CHNA combined robust public health data, community input, existing research, existing plans, and existing assessments to document the health status of communities within Chicago and Suburban Cook County and to highlight systemic inequities that are negatively impacting health. The CHNA also provided insight into community-based assets and resources that would benefit from support and leverage during the implementation of health improvement strategies. Swedish Hospital partnered with the Alliance for Health Equity (AHE), other hospitals, the Chicago Department of Public Health, Cook County Department of Public Health, and community organizations to complete this collaborative CHNA between March 2021 and March 2022, during a time that communities across our county, country, and globe have been experiencing profound impacts from the COVID-19 pandemic. The health, economic, and social impacts of the pandemic are strongly present in what we heard from community members and healthcare and public health workers over the course of the assessment. Primary and secondary data from a diverse range of sources were utilized for robust data analysis and to identify community health needs in Chicago and Suburban Cook County. Primary data for the CHNA was collected through three methods: Community input surveys, community resident focus groups, and social service provider focus groups. Epidemiologists from the Cook County Department of Public Health (CCDPH) and Chicago Department of Public Health (CDPH) and Metopio are invaluable partners in identifying, compiling, and analyzing secondary data for the CHNA. IPHI and the Alliance for Health Equity steering committee worked with CDPH and CCDPH to refine a common set of indicators based on an adapted version of the County Health Rankings and Roadmaps Model. Secondary data used in the CHNA were compiled from a range of sources. In alignment with the purpose, vision, and values, the Alliance for Health Equity prioritizes engagement of community members and community-based organizations as a critical component of assessing and addressing community health needs. Community partners have been involved in the assessment and ongoing implementation process in several ways both in providing community input and in decision making processes. The community based organizations engaged in the Alliance for Health Equity represent a broad range of sectors such as workforce development, housing and homeless services, food access and food justice, community safety, planning and community development, immigrant rights, youth development, community organizing, faith communities, mental health services, substance use services, policy and advocacy, transportation, older adult services, health care services, higher education, and many more. All community partners work with or represent communities that are disproportionately affected by health inequities such as communities of color, immigrants, youth, older adults and caregivers, LGBTQ+, individuals experiencing homelessness or housing instability, individuals living with mental illness or substance use disorders, individuals with disabilities, veterans, and unemployed youth and adults. Community input is the most important data input into the Alliance for Health Equity Community Health Needs Assessment. Particularly in the context of the current COVID-19 pandemic, first-hand information from communities most impacted by inequities is the most up-to-date data we have available about priority community health needs. Swedish Hospital (Swedish) worked closely with the AHE and community-based organizations that are members of Swedish's Community Leader Program to collect in-depth community input data through a community input survey and focus groups. We collected 1108 surveys and conducted 8 focus groups with residents from Swedish's service area. The community input survey was a qualitative tool designed to understand community health needs and assets from community members, with a focus on hearing from community members most impacted by health inequities. The community input surveys, along with focus group data, informed the priority areas and strategies for community health improvement in Chicago and suburban Cook County. Between August 2021 and February 2022, the Illinois Public Health Institute (IPHI) worked with Alliance for Health Equity partners to hold a total of 43 focus groups with priority populations such as veterans, individuals living with mental illness, communities of color, older adults, caregivers, teens and young adults, LGBTQ+ community members, adults and teens experiencing homelessness, families with children, faith communities, adults with disabilities, and children and adults living with chronic conditions such as diabetes and asthma. In total, eight focus groups were held with community members and service providers in Swedish's service area. Swedish Hospital values the community health needs assessment process as an opportunity to engage with community leaders and organizations and with our colleagues from other healthcare institutions across the County through the Alliance for Health Equity. In partnership with communities, the Chicago Department of Public Health, the Illinois Public Health Institute, and the Alliance for Health Equity, we have taken an in-depth look at the needs and assets in the communities we serve, and we are committed to addressing those needs through implementation strategies in partnership with communities most impacted by health inequities. We undertake this collaborative, collective impact approach to community health needs assessment and implementation in order to address the underlying root causes of health disparities and to support greater community health and well-being in the communities we serve. Swedish Hospital makes the Community Health Needs Assessment available at SwedishCovenant.org/community-benefit. It is also shared broadly with internal and external stakeholders, including employees, volunteers, physicians, elected officials and members of our community, including the Community Leader Engagement Program.

Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - Swedish Hospital. The implementation strategy was developed after the comprehensive Community Health Needs Assessment (CHNA) was completed. Swedish Hospital intends to undertake the following strategies to meet the identified community health needs. It is important to note that our health equity and social determinants of health (SDOH) work is fundamental and integrated throughout our priority needs' strategies. Access to Health and Social Services - Expand efforts to identify and respond to social determinants of health (SDOH). - Expand efforts to support benefits enrollment for under resourced community members. - Increase access to specialty care and diabetes education via Healthcare Transformation Program. - Strengthen Swedish Hospital's capacity to respond to survivors of sexual and intimate partner violence via Pathway Programs. - Develop Community Health strategy addressing health and racial disparities through community partnership and program development. Mental and Behavioral Health - Deepen partnerships with community organizations addressing mental health - Continue and enhance behavioral mental health and substance abuse services. - Explore opportunities to educate the community about mental health via programs and partnerships. Chronic Health Conditions and Wellness - Address high blood pressure, diabetes and preventative cancer screenings through targeted interventions and outreach. - Enhance partnerships with local community organizations to better address chronic health conditions. - Expand education and outreach to community and patients to promote nutrition, health lifestyle choices and wellbeing. - Enhance Galter LifeCenter offerings to support health and wellbeing. Below is a list of the other significant health needs identified through the CHNA and the reasons why Swedish Hospital did not identify as a priority need in the implementation strategy plan. Schedule H, Part I, Line 7dOn Schedule H, Line 7b, Column (c), the $107,063,423 in the community benefit expense includes the provider assessment tax of $20,249,998. Had the provider tax been excluded, the total community benefit expense would have been $86,813,425. On Schedule H, Line 7b, Column (d), the direct offsetting revenue of $133,707,118 includes the provider tax revenue of $65,742,766. Had the public aid provider revenue been excluded from Schedule H, the direct offsetting revenue would have been $67,964,352. Using an adjusted total community benefit expense of $86,813,425 noted above, minus an adjusted direct offsetting revenue amount of $67,964,352, also noted above, the resulting community benefit dollar amount on Schedule H, Line 7b, Column (e) would have been $18,849,073. On Schedule H, Line 7b, Column (f) percentage of total expense would have increased from 0% to 5.5%. The provider assessment tax benefit is not reimbursement for direct patient care and therefore we believe it should be excluded from the Schedule H calculations.
Schedule H, Part I, Line 3c Other Factors Used to Determine Eligibility for Financial AssistanceIn addition to the Federal Poverty Guidelines, Endeavor Health Clinical Operations uses the following factors to determine eligibility for financial assistance: asset level, medical indigency, insurance status, underinsurance status, and residency.
Schedule H, Part I, Line 7g Subsidized Health ServicesNo physician clinic costs have been included on Line 7g.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceSWEDISH HOSPITAL CALCULATED THE COST OF CHARITY CARE USING THE COST-TO-CHARGE RATIO. AMOUNTS IN THE OTHER BENEFITS SECTION OF LINE 7 WERE CALCULATED USING COSTS CHARGED DIRECTLY TO THE INDIVIDUAL PROGRAMS FROM THE FINANCIAL ACCOUNTING SYSTEM.
Schedule H, Part II Community Building ActivitiesMembers of Swedish Hospital staff work with other organizations and participate in many activities to advocate for health improvement in our community. These include: involvement with the association for community safety net hospitals and the IHA advocacy outreach team, participation in the Medicaid rate reform tag group, community meetings, chambers of commerce, education, and seeking of resources to enhance health care services in the community. During the fiscal year, Swedish Hospital continued its active involvement in a collaborative of area hospitals entitled the Alliance for Health Equity (AHE). AHE is a partnership between the Illinois Public Health Institute, hospitals, health departments, and community organizations across Chicago and Cook County. This initiative is one of the largest collaborative hospital-community partnerships in the country with the current involvement of over 30 hospitals, 7 health departments, and 100 community partners throughout Chicago and Cook County serving on various action teams. AHE is a collaboration of Chicago and Cook County -based hospitals that strives to promote a collective impact on health outcomes in the city of Chicago through an environment that fosters learning, sharing of resources, data and best practices. They are also the driving force to facilitate and execute the community health needs assessment (CHNA) for their hospital members. Swedish Hospital has staff who participate in a variety of sub-groups via AHE including the following: AHE steering committee, social and structural determinants of health committee, housing and health workgroup, food security/food access workgroup, mental health and substance use disorders committee, CHNA committee, policy committee and trauma informed hospitals collaborative. In addition to the above mentioned activities, Swedish Hospital regularly engages in conversation to advocate for community health improvement and support, including attending meetings with the local police districts on safety/community policing and meeting with organizations who serve disadvantaged populations within our community to develop relationships/partnerships to provide preventive health screenings (including mammograms), violence prevention resources, and other free health education offerings throughout the community.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountBad debt expense was reported at cost using a cost-to-charge methodology. The Schedule H, Worksheet 2, Ratio of Patient Care Cost-to-Charges calculated to report charity care on Part I, Line 7a was multiplied by the bad debt expense to determine the bad debt expense at cost. Discounts and payments on patient accounts are excluded from bad debt expense in order to reflect the expected amount to be paid.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThere are no bad debt amounts included in community benefits.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteSee pages 9 and 14-18 of the audited financial statements for footnotes describing accounts receivable and bad debt expense.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsThe Medicare allowable costs were determined from the amounts in the Medicare Cost Report excluding costs already reported on Schedule H, Part I, Line 7f as health professions education.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistanceSwedish Hospital is included in the billing and collections policy for Endeavor Health. The policy applies to all patients. Patients are screened for eligibility for financial assistance before collection procedures begin. Financial counseling will be provided to help patients identify available federal or state healthcare coverage programs which may be available to them, as well as to determine eligibility under the financial assistance policy. Collection activity will be placed on hold pending the outcome of these determinations. After a patient meets the qualifications for financial assistance, the account balance is either partially or entirely written off. Any remaining balance would be collected under the existing billing and collections policy.

Schedule H, Part VI, Line 2 Needs assessmentSwedish Hospital (Swedish) and members of the Alliance for Health Equity (AHE), a collaborative of over 30 hospitals partnering with health departments and community based organizations, worked together throughout 2021-2022 to build a comprehensive Community Health Needs Assessment (CHNA) in Chicago and Cook County. The CHNA was reviewed and approved by the hospital board of directors during August 2022. Swedish Hospital worked with the Alliance for Health Equity and local community partners in the communities we serve to identify and prioritize health needs within our community through 2024. Swedish Hospital serves the culturally-diverse residents of Chicago's north and northwest side communities, with a full service hospital campus located in Lincoln Square at the intersection of Foster and California Avenues. Swedish Hospital provides a full range of comprehensive health and wellness services including an acute care hospital, primary care and specialists in the medical group, strong community outreach programs and Chicago's only certified medical fitness center, Galter LifeCenter. Swedish offers more than 50 medical specialties with practices conveniently placed throughout hospital's campus and on multiple sites throughout Chicago's north side communities (including several Immediate Care Centers) to make care accessible to our patients, wherever they are. Swedish maintains a department dedicated to addressing its outreach objectives of serving the entire community, not only those who come through its doors. Building on a long tradition of service, the Community Relations Department utilizes hospital strengths alongside those of other well-established community partners. This strategy allows Swedish to better understand and reach the most vulnerable sectors of the community, while meeting pressing healthcare needs. The goal is to improve the community's health status by empowering citizens to make healthy life choices. Based on assessment and analysis of the primary and secondary data compiled for the Swedish Hospital Service Area, Swedish plans to focus on the following needs, with health equity and social determinants of health woven throughout: Access to Health and Social Services, Mental and Behavioral Health, and Chronic Health Conditions and Wellness. Based on relationships with community partners, clinical expertise, strategic priorities and an ongoing commitment to community engagement, Swedish believes it is best equipped to make an impact in the above priority needs, including focused attention within communities of greatest need. The corresponding Community Health Implementation Plan describes programs Swedish is undertaking over the coming years to address the prioritized health needs within our community.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceCharity or discounted care is available for medically necessary services as defined by Medicare, to patients who meet the financial and documentation criteria defined below. Each situation is reviewed on an individual case-by-case basis. While not absolutely essential, the need for potential charity or discounted care should be established in advance of admission or rendering of service, or shortly thereafter. Notices in English, Arabic, Assyrian, German, Japanese, Korean, Mongolian, Polish, Russian, Spanish, and Vietnamese about the availability of Charity or Discounted Care are posted in the Emergency Room registration area, and the inpatient and outpatient registration areas. The information is also contained in the Patient Handbook, given to all patients upon inpatient admission. Patients and families are encouraged to speak to a financial counselor for more information and assistance, The Guidelines, along with the financial questionnaire, are posted in English, Arabic, Assyrian, German, Japanese, Korean, Mongolian, Polish, Russian, Spanish, and Vietnamese on the hospital's website. In order to be eligible for charity or discounted care, the patient must be willing to provide verification of income by filling out the Patient Financial Statement. The patient is responsible to voluntarily submit any and all documentation in order to be eligible to receive this discount. Swedish Hospital has 5 full-time financial counselors representing our hospitals, clinics, and physician offices. These counselors are responsible for helping patients understand their bills and coordinating financial assistance and insurance eligibility when appropriate. Any uninsured patient admitted to our hospitals will have their case reviewed by a financial counselor. Swedish has also earned Certified Application Counselor Organization (CACO) status with the Centers for Medicare and Medicaid Services (CMS). Our counselors are certified and capable of helping patients navigate Medicaid and Insurance Exchange options. During the registration and information gathering process, the financial counselors determine if the patient qualifies for medical assistance from other existing financial resources such as Medicare, Medicaid, Kid Care, Family Care or other state or federal programs, If the patient refuses to apply for existing financial resources or to provide information necessary to the application process, charity or discounted care cannot be granted. If the application for existing financial resources is denied, or has been previously denied, consideration for charity or discounted care will then be given. Once the Financial Assistance Application form is received, the Financial Service Center determines the eligibility for charity or discount care. SCH suspends the collection process while the Financial Assistance Application is being reviewed. The only criteria considered for Financial Assistance is income and family size. Income is evaluated against the matrix of Federal Poverty Guidelines to determine whether full or partial discount can be approved, Documentation of income is submitted in the form of paycheck stubs, income tax returns, Social Security checks, and/or other documents that are indicative of income. If the information submitted is not perceived to be accurate or reliable, SCH reserves the right to request additional documentation to substantiate income or family size. Patients who are approved for financial assistance and earn a family income less than 200% of the federal poverty guidelines will receive care at no cost for their patient balance after $150 per inpatient stay or outpatient services. The insured patient with a large balance due to deductibles and/or co-payments may be eligible for charity or discounted care. In order to qualify, the patient must complete the Patient Financial Statement and return it to the financial counselors for evaluation and recommendation. No legal action is taken against uninsured patients for the first one hundred twenty (120) days after discharge. Patients who have a family income that is no more than 600% of the Federal Poverty Guidelines (as determined each year), and who do not have any health insurance (or coverage under workers' compensation, accident liability insurance, or other third party liability) as documented through SCH's insurance verification procedures, receive a discount in accordance with the Hospital Uninsured Patient Discount Act (210 ILCS 89/) (the Act).
Schedule H, Part VI, Line 4 Community informationSwedish's community, as defined for the purposes of the Community Health Needs Assessment, includes each of the residential ZIP Codes that comprise the hospital's Primary Service Area (PSA) and Secondary Service Area (SSA): 60613, 60618, 60625, 60626, 60630, 60640, 60641, 60645, 60646, 60659, 60660 and 60712, as shown in the map in Figure 1. These zip codes encompass fourteen community areas in Chicago-Albany Park, Avondale, Edgewater, Forest Glen, Irving Park, Jefferson Park, Lake View, Lincoln Square, North Center, North Park, Portage Park, Rogers Park, Uptown, West Ridge-and the village of Lincolnwood. This community definition was determined because most Swedish's patients originate from these areas. The total population in Swedish's service area is 686,000. In the service area, 25% of the population identifies as Hispanic/Latinx and 75% Non-Hispanic. Fifty-three percent of the population identifies as white, 10% Asian, 8% Black/African American, 3% identifies as two or more races, and less than 1% as Native American. There is substantial variation in median income within Swedish's service area. The median income in Rogers Park is nearly $100,000 less than the median income in North Center. In addition, poverty rates are highest in Rogers Park and Uptown. Swedish Hospital serves a remarkably diverse population, and many of the community areas are home to large immigrant and refugee populations. Five of the 14 community areas within Swedish's service area have a limited English speaking population of 10% or greater. The Spanish language represents the majority of interpretive assistance requested at the hospital. To meet this need on an inpatient and outpatient basis, the hospital employs a full time Spanish medical interpreter. The hospital additionally uses telephonic and video interpretive services to meet the needs in other languages, as well as for when the employed interpreter is not present. During 2023, approximately 27% of Swedish's patients were uninsured or were Medicaid recipients. Within Swedish's service area, there are three federally-designated medically underserved areas. Swedish's service area also includes 3 other hospitals.
Schedule H, Part VI, Line 5 Promotion of community healthIn keeping with the hospital's mission to provide a continuum of excellent healthcare services, any surplus funds are invested in capital improvements related to patient care or in community benefit programs. The hospital extends medical staff privileges to all qualified physicians in the communities it serves, in accordance with our medical staff development process. The hospital has an active Board of Directors who meet every two months. In addition to reviewing the hospital's financial results, they discuss the hospital's plans and programs in support of our efforts to enhance the health care environment for the entire community we service. There is a dedicated committee of the board entitled the Community & Mission Committee, whose focus is to oversee and evaluate community outreach programs and services, as well as pastoral care and chaplaincy offerings. The hospital helps community members gain access to health care services by assisting low income families in applying for the Illinois Department of Public Aid's KidCare and FamilyCare programs. Information about these services is distributed to numerous community organizations, churches, health fairs and school events. It is also publicized throughout the hospital campus and on the hospital web site. Medicaid applications continue to be processed for all inpatients who qualify.
Schedule H, Part VI, Line 6 Affiliated health care systemSwedish Hospital is part of Endeavor Health. Endeavor Health is a fully integrated healthcare delivery system committed to providing access to quality, vibrant, community-connected care, serving an area of more than 4.2 million residents across six northeast Illinois counties. Our more than 25,000 team members and more than 6,000 physicians aim to deliver transformative patient experiences and expert care close to home across more than 300 ambulatory locations and eight acute care hospitals - Edward (Naperville), Elmhurst, Evanston, Glenbrook (Glenview), Highland Park, Northwest Community (Arlington Heights), Skokie, and Swedish (Chicago) - all recognized as Magnet hospitals for nursing excellence. Located in Naperville, Linden Oaks Behavioral Health provides for the mental health needs of area residents. Endeavor Health desires to continue providing clinical programs and services to meet community health needs, while also pursuing continuous improvement in existing and future programs to improve the overall health of individuals in the communities it serves. As such, hospitals within the Endeavor Health system conduct Community Health Needs Assessments (CHNA's) every three years, using primary and secondary data, to ensure community benefit programs and resources are focused on significant health needs as perceived by the community at large, as well as alignment with Endeavor Health's mission, services, and strategic priorities.

Schedule J, Part I, Line 3 Arrangement used to establish the top management official's compensationA detailed compensation review of the top executives, including the Chief Executive Officer, is conducted annually. Market data is collected and assessed by an external independent compensation consultant who specializes in compensation consulting within the healthcare industry. The work product from this study is reviewed separately with Endeavor Health legal counsel. Market data for base and variable compensation is assessed annually for integrated delivery systems and academic medical centers that are similar in size and complexity.
