DescSonora Community Hospital, dba Adventist Health Sonora (AHSR), provides quality medical healthcare regardless of race, creed, sex, national origin, handicap, age, or ability to pay.Although, reimbursement for services rendered is critical to the operation and stability of Sonora Community Hospital, the organization recognizes that not all individuals possess the ability to pay for essential medical services.Adventist Healths vision is to enhance the health of the communities where we live and serve by engaging our communities and our patients in a new definition of and partnership for personal community health. In keeping with this commitment to serve all members of the community, the following coverage will be considered when individuals who need health care cannot afford to pay: Free care and/or subsidized care Care to persons covered by governmental programs at/or below cost Health/wellness activities and community education programsNot only does Sonora Community Hospital provide low-cost care to individuals covered by government programs, and those unable to afford healthcare, it also helps patients find and access private and governmental resources for healthcare benefits.AHSR recognizes below-cost reimbursement as charity and uncompensated care in meeting its mission to the entire community. The unreimbursed cost of providing care to these patients in 2023 was $ 680,660.The following Inpatient services were provided to all our patients:483 babies delivered781 surgeries performed32,496 patient days The following Outpatient services were provided to all our patients:34,584 emergency department visits3,283 outpatient surgeries performed175,633 outpatient visits 200,460 clinic visitsAHSR recognizes it has an obligation to provide human services above and beyond its role as a healing facility. Schedule H provides more details on the tangible ways in which the organization is fulfilling its mission. The total unreimbursed cost of these community benefits in 2023 was $34,361,048.Adventist Healths mission statement of living Gods love by inspiring health, wholeness and hope is coupled with a vision to transform the health experience of our communities through collaborative programs, community investments and community outreach. We are inspired by the healing ministry, as represented by the life of Jesus Christ, and believe we are called to live out our mission intentionally in the communities we serve. In the small towns, suburbs, and inner cities we serve, we continue our journey to provide quality healthcare until every person made in Gods image has experienced the best health today, hope for tomorrow, and Gods love that endures forever. The following story illustrates how Adventist Health Sonora is able to provide quality care for our community in an extraordinary way:In September 2023, Adventist Health Sonora partnered with the Seventh-Day Adventist Church, the wellness initiative Blue Zones Project Tuolumne County, the county Public Health Department, and individual volunteers from across the community to offer free health services. More than 200 volunteers including dental, vision, medical, massage therapy, and haircare professionals provided about $221,000 in free services and labor for 259 patients at the Life Hope Clinic. One grateful patient thanked the hosts for his wifes free root canal. It was a great experience and I'm very grateful for getting my fillings done, Tim and Susan Burkett wrote when they left the clinic. Very grateful to all of the volunteers and the doctor, and my wife is very grateful for the root canal and now she will be able to eat and drink without pain.Church staff shared numbers from the event that showed dental volunteers helped 136 dental patients receive a total of 63 fillings, 73 cleanings, 54 tooth extractions, and two root canals. Vision volunteers did 128 eye exams and patients received 125 pairs of prescription glasses. Medical volunteers did 25 cardiology exams, 25 electrocardiograms, and provided 95 patients with foot care. Volunteer professionals also did 61 haircuts and 108 massages.Altogether, 248 volunteers provided $140,425 in services and $80,660 in donated labor, for a total cost of $221,085 in free health care and other services provided.Great staff in all aspects of treatment, Evelyn Medeiro wrote. God bless all of you. Thank you. Some people who came to the clinic for services wrote their thanks in Spanish: Muchas Gracias. Familia Castillo.People were urged to come early because services were offered first-come, first-served. No insurance or ID was required to register. All ages 10 years and older were welcome.Websites for our community benefit information: https://www.adventisthealth.org/about-us/community-benefit/Desc

Facility: - Part V, Section B, Line 5The CHNA Steering Committee identified vulnerable populations and worked with local organizations to coordinate key informant interviews to ensure that minority populations the voices of those with chronic disease, low incomes, and the underserved were heard. Civic government, community-based healthcare workers focusing on behavioral health, faith, law enforcement, low-income, medically underserved, minority populations, older adults, providers, and unhoused populations participated in fourteen key informant interviews. The 2022 Community Health Needs Assessment was developed using four separate sources of primary and secondary data. This mixed methods approach is considered a preferred practice for needs assessments because it allows for the greatest understanding of community needs from the broadest range of perspectives. Primary data refers to data collected and analyzed specifically for this project, while secondary data refers to data compiled and analyzed by external groups and utilized here. Qualitative primary data collection involved focus group interviews with local service providers and service recipients and individual key informant interviews with local leaders. These were conducted in-person and virtually. Direct quotes were taken from a transcription of key informant interviews and are intended to be 100% accurate but could not be verified in all situations. This information was collected by the Adventist Health Community Well-Being team and evaluation consultants from the Center for Behavioral Health Integration. Secondary data was amassed and analyzed across 45 different data sets by the University of Missouri Extension Center for Applied Research and Engagement Systems (CARES). Finally, survey data sets of registered voters in the community were collected and analyzed by UC Berkeley Institute of Governmental Studies (IGS).
Facility: - Part V, Section B, Line 11Tax year 2023 was Year One of the Implementation Strategy for the 2022 CHNA. All programs and activities provided to the community were in response to the priority health needs identified in the 2022 CHNA. Our 2022 CHNA identified three areas of significant need: Financial Stability, Housing, and Mental Health. Priority Need One: Financial Stability: The goal is to increase financial stability by reducing the negative impacts caused by chronic health conditions that lead to lost work time and increased health care costs. A secondary goal is expanding the number of staff and providers able to see underinsured patients and increase the number of family practice practitioners in our community to reduce expensive travel and delayed treatment, which can lead to financial instability. Sonora Community Hospital provided screenings and other health services to educate, prevent chronic disease and increase access to healthcare and well-being services thereby relieving a financial burden. This was accomplished through the following events in 2023: 1) Hosted the Second Annual Turkey Trot to increase diabetes awareness. The event brought increased awareness of diabetes prevention and screening tools to members of the community. 2) Sonora Community Hospital hosted the Ladies Night Out Event to bring the community together to learn about the importance of early screening, education about breast health, and cancer treatment. Approximately 500 community members attended the event. 3) The Hospital hosted Heart Health Month events, which included community outreach events to provide blood pressure screening, cardio/pulmonary health information, and access to no-cost exercise to prevent chronic disease. Over 300 individuals attended in February 2023. 4) Sonora Community Hospital is serving as the co-chair for the 2024 Tuolumne County Health Fair planning event after a hiatus of three years. Planning started in 2023. The workgroup established protocols to provide health screenings and services such as lab draws. 5) The Hospital supported and co-hosted Tuolumne County Blue Zones Project cooking demonstrations to provide affordable, healthy meal options to community members. Five cooking demos were provided to over 50 participants. Three Sonora Community Hospital representatives served on the Tuolumne Thrives Committee to promote healthy, low-cost foods, WIC, and food assistance services to the community. 6) Promoted low-to-no-cost fitness classes offered by partners including Blue Zone Project (Tai Chi, Yoga, and swim classes) and the Living Well Fitness Center were promoted to community members. Continuing the emphasis on physical exercise, the Hospital also provided funding for the building of an accessible, free outdoor trail system in Groveland, an underserved area of Tuolumne County, which approximately 500 community members currently have access. This number will likely increase in the upcoming year. 7) Sonora Community Hospital used 2023 to plan for their May 2024 low/no cost physicals for high school students interested in participating in high school sports and created a tobacco cessation program for youth & adults to begin in 2024. The Hospital service area reports a 13.73% of adults aged 18 years and over are current smokers. 8) Provided volunteers and medical staff at the Life Hope Clinic screenings included blood pressure, cholesterol, blood sugar, vision, dental, and foot health. 259 individuals were served at the foot screening. 9) First Aid was provided at the July local county fair at no cost. Educational materials and services were provided to over 1500 attendees. 10) Increased access to dental screening, varnishes, and dental referrals through a collaborative program called Smilekeepers which served over 5000 youth in preschool through twelfth grade programs including juvenile hall. 11) Pilot program at Columbia Elementary School to provide basic hygiene kits for low-income students where approximately 100 students were provided hygiene kits. Infant dental education to new parents to approximately 25 families was also provided. 12) In 2023, Breastfeeding Basics classes were resumed and open to the public in the beginning with approximately 25 individuals served each quarter. 13) In June 2023, the COPE Scholars Medical Assistant program was implemented as a means to address financial stability in the community. The COPE Health Scholars Medical Assistant Program is an innovative joint effort of COPE Health Solutions and partnered health systems to provide an affordable and convenient way to enter the field. Over the course of several months, participants in the program will gain the skills and foundational knowledge needed to succeed as a medical assistant in a clinical environment through virtual self-study lectures as well as in-person training guided by experienced professionals in the field. Students can enter the workforce with on-the-job training and experience. Services were also provided for individuals in financial distress/experiencing homelessness and general community support through free transportation which was provided to 540 individuals who earn less than 250% of poverty level. Our local clothing closet provided clean, new clothing for individuals who are unhoused to fifty individuals. Contracted services by the Hospital provided community members with Medicaid eligibility enrollment and patient advocacy to uninsured underinsured status. Work on financial stability saw the expansion of job opportunities available to working families and individuals in the community through certified nursing assistant and registered nurse recruitment that began in 2023 and will continue into 2024. Over 20 certified nursing assistants graduated in 2023. A new cohort of medical assistant recruits started training in 2023 and will complete in 2024. A registered nurse pathway was established in 2023 and recruits will start the program in 2024. Rural Medicine Residency Program was started with four residents beginning the program with Year 1 completion anticipated in June 2024. Hospital operations executive/leaders, education partners, colleges, and workforce partners collaboratively developed healthcare career pathways track as part of an $18 million grant that was awarded in Fall 2023. Lastly, the addition of an addiction medicine physician, one primary care, two urologists and one pain physician to Hospital medical staff occurred in 2023. Data shows that 98.11% of the population in the Hospital primary and secondary service area reside in a health professional shortage area and 22.43% of that population are considered underserved. Tuolumne Blue Zone Project (BZP): Adventist Health and Blue Zones continued their partnership in 2023 through the desire to improve community well-being and make sustainable changes to our environment by presenting the final Linoberg report, concepts and recommendations to the City of Sonora City Council who unanimously approved the concept report and granted permission for BZP to move forward with the planning of two demonstration projects which create walking spaces. Began collaborative meetings with Chicken Ranch Rancheria of Me-Wuk Indians, County of Tuolumne, Tuolumne County Transportation Council, and the City of Sonora to implement a community outreach and engagement process funded through Smart Growth Americas Transformative Climate Communities (TCC) Planning grant (also known as the Jamestown Happiness Project -JHP). The BZP Policy Advocate attended the Safe Routes to School Summit (SRTS) a three-day series of virtual workshops designed to give communities information and resources on how to make walking & biking to school safe for all. Workshops included: Finding Safe Routes Funding; Engaging Youth Voices for Walking, Biking & Transit Use; and Bike Education & Traffic Gardens. Outer Aisle Food Hub, a year-round supplier of fresh fruits and vegetables, provided bi-weekly delivery of produce boxes to Tuolumne County. BZP assisted Outer Aisle to apply to accept EBT transactions. On 12/29/23, they received notification that they are an approved vendor to accept EBT making produce box subscription accessible to CalFresh beneficiary card holders. BZP also continues the Year-round Gardening workshops in Tuolumne County. The leader of the Gardening workshops, Chris Montesano, says, "107 200 sq. ft. of organic biointensive year-round garden space can remove one ton of CO 2. I want this to be a reality in Tuolumne County. While the garden is able to provide healthy, organic foods, Chris also values the community connection and engagement made through the local workshops. Due to overwhelming interest, a gardening club was initiated as well.Priority Need Two: Housing: The goal is for residents to have access to safe, affordable, and stable housing and resources that provide the conditions necessary for health and well-being through the development and maintenance partnerships to address unhoused individuals/fami
Facility: - Part V, Section B, Line 13hPatients who do not meet the income criteria, may be eligible for financial assistance based on essential living expenses and resources. The following two (2) qualifications must both apply:1. Essential living expenses: Exceed fifty percent (50%) of the household income; and2. Resources: The patient's excess medical expenses (the amount that allowable medical expenses are greater than 50% of annual household income) must be greater than available qualifying assets.

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Part I, Line 7 - Explanation of Costing MethodologyThe costs were determined by using a cost-to-charge ratio. The cost-to-charge computation is based on hospital specific data included in the system-wide combined audited financial statements. The formula used for computation equals financial statement data as follows:(Total expenses - (Provision for bad debts + Other revenue + Interest income)) / Gross patient charges
Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseUncollected patient accounts are analyzed using written patient financial services policies that apply standard procedures for all patient accounts. The result of the analysis is what is recognized as bad debt expense. For example, all self-pay patients receive a discount. If the discounted account is unpaid after collection efforts, the unpaid balance is classified as bad debt. The cost-to-charge ratio is multiplied times the hospital's bad debt expense. The resulting figure has been reported as bad debts at cost on Part III, Line 2.
Part III, Line 3 - Methodology of Estimated Amount & Rationale for Including in Community BenefitThe portion of the bad debts attributed to charity care as reported on Part III, Line 3 was calculated by an independent third-party consulting firm. A statistically valid sampling of patient accounts written-off was evaluated. The evaluation used various factors to determine which patients would have been eligible for charity care. Had the hospital obtained sufficient information from all patients who qualified for financial assistance, these additional accounts would have been recorded as charity care instead of bad debt.
Part III, Line 4 - Bad Debt ExpenseThe system-wide combined audited financial statements do not contain a footnote describing bad debt expense.
Part III, Line 8 - Explanation Of Shortfall As Community BenefitThe Medicare cost report apportions the hospitals costs on the basis of inpatient days and ancillary and outpatient charges to establish the costing methodology.Healthcare delivery by hospitals is a complex, highly regulated business in the United States. Healthcare unit cost inflation is driven by compliance with ever-expanding regulatory requirements, shortages of highly skilled labor and evolving medical and information technology. The health care market basket is unrelated to that of the average individual consumer. Medicare annual payment updates have fallen behind actual healthcare cost inflation to the point that Medicare payments too many U.S. hospitals are well below the cost of providing care. These unreimbursed costs are a community benefit for seniors and others in the community as these individuals are continuing to receive care without which many would become dependent on other governmental resources such as Medicaid. The benefit to the community for healthier Medicare recipients is no different than those benefits the community realizes for uninsured and underinsured patients who are eligible for partial and full charity care. Medicare is a safety net for seniors and others. Without Medicare coverage, many individuals would undoubtedly qualify for charity care.In addition to the mismatch between Medicare payment increases and healthcare cost inflation, the highly complex Medicare payment systems and formulas produce disparate payment levels from one hospital to another for the same service. These disparate payment levels create disparate results within groups of hospitals. For further information please refer to schedule H part III section B.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsWhen a patient has requested screening for charity care, the hospital must immediately cease collection activity and place the account in a charity pending status. If 100% charity is approved, the entire account balance is written off to charity care. If the patient has a sliding scale liability based on the federal poverty guidelines, they are billed only for that liability. If the patient fails to pay their after-charity liability, they are assigned to a collection agency with an identifier that indicates to the agency that the patient is "low income and the following criteria must be followed by the agency:1. They may not report the patient to a credit bureau2. They may not file a lawsuit to recover the outstanding liability3. They may not charge interest
Part VI, Line 2 - Needs AssessmentSonora Community Hospital promotes access to their 2022 Community Health Needs Assessment (CHNA), the 2023 Community Health Implementation Strategy (CHIS) approved in fiscal year 2023, and the Year One Update, FY 2023 for the Community Health Implementation Strategy by posting their reports on the Hospital's website at:https://www.adventisthealth.org/sonora/about-us/community-benefit/ The two most recent Community Health Needs Assessments, the two most recent Implementation Strategy reports and the 2021, 2021 and 2023 Community Health Plan Updates are also available on the Adventist Health Corporate website at https://www.adventisthealth.org/about-us/community-benefit/The Community Health Needs Assessment (CHNA) includes both the activity and product of identifying and prioritizing a community's health needs, accomplished through the collection and development of a community health plan. The second component of the CHNA, the community health plan, includes strategies and plans to address prioritized needs, with the goal of contributing to improvements in the community's health. Qualitative and quantitative data sources were used in conducting the CHNA. To accomplish the many important systemic goals that are underway in our community, Adventist Health supports local partners to augment our own efforts, and to promote a healthier community. Partnership is not used as a legal term, but a description of the relationships of connectivity that are necessary to collectively improve the health of our region. One of our objectives is to partner with other nonprofit and faith-based organizations that share our values and priorities to improve the health status and quality of life of the community we serve. This is an intentional effort to avoid duplication and leverage the successful work already in existence in the community. In addition to the CHNA, our hospital engages in a process of continuous quality improvement, focusing on questions such as: are our interventions making a difference in improving health outcomes; are we providing the appropriate resources in the appropriate locations; and how are we using technology to track our health improvements and provide relevant feedback at the local level. Building a healthy community requires multiple stakeholders working together with a common purpose. With that in mind, our hospital leadership serves on a variety of community boards and committees focused on addressing community-specific needs, allowing for ongoing responsiveness to the health care needs of the community and collaboration with local agencies and organizations, enabling maximum effectiveness through collective impact.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceThe plain language summary of the Financial Assistance Policy (FAP) is posted along with the complete FAP policy and FAP Application on the hospitals website at: https://www.adventisthealth.org/patient-resources/bill-pay/financial-assistance/ The documents are available in multiple languages. The link to the hospitals Self-Pay Billing and Collection Policy is posted at: https://www.adventisthealth.org/documents/system/13863-self-pay-billing-and-collection-policy.pdfAt the time of registration, patients who are uninsured and underinsured are provided information about government healthcare programs. Patients are also orally informed of their right to request charity assistance. Signs are displayed in the patient business office, patient registration areas and the emergency room in multiple languages informing patients of this right as well. The Hospital also provides a brochure during the registration process that explains the hospital billing and collection procedures, and how to request financial assistance. In addition, every billing statement sent to patients contains information on how to request financial assistance.
Part VI, Line 4 - Community InformationLocated in the heart of Californias gold country, Tuolumne County is nestled in the Sierra Nevada Foothills and is the gateway to the world-famous Yosemite National Park. Founded during the California Gold Rush, Sonora, has a rich heritage, small town feel, and historic charm. These are all communities represented in the Sonora CHNA service area where the largest age group is over age 55 (45.13%). The community is vibrant and cultured with art, galleries, festivals, events, and small businesses. Of the population, 12.57% are Hispanic and 74.74% own their home. The median household income is $58,959 of which 58.6% is spent on housing and transportation. Among this population, 15.2% of children live in poverty and 5.45% of students are unhoused, compared to the state average of 4.25% and national average of 2.77%. The following zip codes represent Sonora Community Hospitals primary service area (PSA), accounting for 75% of hospital discharges. Additionally, we took a collaborative approach and expanded our PSA by inviting Steering Committee members to include the zip codes of those they serve. The Sonora Community Hospital CHNA market has a total population of 84,499 (based on the 2020 Decennial Census). The largest city in the service area is Sonora, with a population of 4,904. The service area is comprised of the following zip codes: 95311, 95310, 95248, 95251, 95224, 95318, 95223, 95370, 95383, 95321, 95364, 95247, 95329, 95222,, 95372, 95305, 95346, 95379, 95375, 95233, 95327, 95335, 95228, 95249, 95246. 74.74% of the population own their home and 25.26% of the population rents their home. Total Population by Age Groups: Age 0-4: 4.4%, Age 5-17: 11.9%, Age 18-24: 6.3%, Age 25-34: 11.5%, Age 35-44: 9.8.
Part VI, Line 4 - Community Building ActivitiesThe hospital is involved in numerous community building activities which promote the health of the community. Overwhelmingly, we see diseases of despair including suicide, substance abuse, mental health and chronic illnesses plaguing the communities in which we have a significant presence in. These community concerns are addressed through health improvement, education, poverty, workforce development and access to care. This is why we have focused our work around addressing behavior and the systems keeping the most vulnerable people in cycles of poverty and high utilization. In an effort to heal these communities, we have strategically invested in our communities by partnering with national leaders in community well-being. We believe the power of community transformation lies in the hands of the community. Our solution for transformation is to create a sustainable model of well-being that measurably impacts the well-being of people, well-being of place and equity. In 2020, Adventist Health acquired Blue Zones as the first step toward reaching our solution. By partnering with Blue Zones, we will be able to gain ground in shifting the balance from healthcare treating people once they are ill to transformative well-being changing the way communities live, work and play. In 2021, Adventist Health committed to launching six Blue Zone Projects within our community footprint in 2022, these projects were active and growing. In 2023, we saw tremendous growth in several of our markets promoting walking moais, worksite wellness, Blue Zones Project school pledges to create an environment that instills healthy lifestyle choices for our children and Blue Zones Project restaurant pledges to provide healthy options. Blue Zones widens our impact from only reaching our hospitals communities in four states to a global mission practice. We also encourage our employees to serve on community collaboration boards, health advocacy programs, and physical improvement projects to promote the health of the communities we serve. In addition, we work with neighborhood programs, including schools, work sites and safety net providers to promote health and wellness and prevent disease.
Part VI, Line 5 - Promotion of Community HealthOur hospitals mission is, Living Gods love by inspiring health, wholeness and hope. Our community benefit work is rooted deep within our mission and merely an extension of our mission and service. We have also incorporated our community benefit work to be an integral component of improving the Triple Aim. The Triple Aim concept broadly known and accepted within health care includes:1) Improve the experience of care for our residents.2) Improve the health of populations.3) Reduce the per capita costs of health care. Our strategic investments in our community are focused on a more planned, proactive approach to community health. The basic issue of good stewardship is making optimal use of limited charitable funds. Defaulting to charity care in our emergency rooms for the most vulnerable is not consistent with our mission. An upstream and more proactive and strategic allocation of resources enables us to help low income populations avoid preventable pain and suffering; in turn allowing the reallocation of funds to serve an increasing number of people experiencing health disparities.Hospitals and health systems are facing continuous challenges during this historic shift in our health system. Given todays state of health, where cost and heartache is soaring, now more than ever, we believe we can do something to change this. These challenges include a paradigm shift in how hospitals and health systems are positioning themselves and their strategies for success in a new payment environment. This will impact everyone in a community and will require shared responsibility among all stakeholders. As hospitals move toward population health management, community health interventions are a key element in achieving the overall goals of reducing the overall cost of health care, improving the health of the population, and improving access to affordable health services for the community both in outpatient and community settings. The key factor in improving quality and efficiency of the care hospitals provide is to include the larger community they serve as a part of their overall strategy. Population health is not just the overall health of a population, but also includes the distribution of health. Overall health could be quite high if the majority of the population is relatively healthyeven though a minority of the population is much less healthy. Ideally such differences would be eliminated or at least substantially reduced.Community health can serve as a strategic platform to improve the health outcomes of a defined group of people, concentrating on three correlated stages:1) The distribution of specific health statuses and outcomes within a population; 2) Factors that cause the present outcomes distribution; and 3) Interventions that may modify the factors to improve health outcomes.Improving population health requires effective initiatives to: 1) Increase the prevalence of evidence-based preventive health services and preventive health behaviors, 2) Improve care quality and patient safety, and 3) Advance care coordination across the health care continuum. We will work together with our community to ensure the community health improvements are identified and then targeted for programs to influence behaviors to obtain improved health within the whole community.
Part VI, Line 6 - Affilated Health Care SystemThe hospital is a member of Adventist Health, a health care system which provides healthcare services in diverse markets within the Western United States. A member hospital may share some services with other member hospitals in its geographic area, such as clinical, management and support services. Using today's technology, hospitals outside the geographic area are able to provide support through remote services such as telepharmacy and robotics surgery. The Corporate Office provides important shared administrative support for member hospitals' rural health clinics and home care agencies, quality of care, other clinical needs, financing and risk management, and shared clinical and financial information technology. As many experienced and new physicians search for alternatives to independent practice, there is also corporate administrative support for hospital affiliated medical groups that engage physicians through employment or other contracts. This provides stability and growth of qualified physicians across many specialties, which is very important to make healthcare services available and to maintain and improve health within the communities served by all member hospitals.

The Hospital operations executive serves on the committee.Priority Need Three: Mental Health; To prevent substance and tobacco use and improve health outcomes and recovery by screening for substance use disorders (SUD) and co-occurring disorders in the community and to link individuals assessed as needing additional services to treatment and other resources. Substance use reduction programs and activities included the following in 2023: 1) A Substance Use Navigator (SUN) through the CalBridge program was added to Hospital staff in June 2023. Out of 230 referrals, 164 accepted services and follow-up consultations. The SUN was invited to join the Tuolumne County Opioid Coalition to create more opportunities to identify services. The Well-Being Director is also a part of the Opioid Coalition. 2) The Tuolumne County Tobacco Coalition was formed in 2023 to address the impacts of tobacco use and to develop tobacco use cessation programs for youth and adults and a Sonora Community Hospital staff member serves on that coalition. 3) On 10/3/2023, Tuolumne Blue Zones Project (BZP) and the Sonora Chamber of Commerce issued a joint press release announcing the Chambers resolution to make ALL their event's smoke and vape-free. 4). On 11/7/2023, Tuolumne BZP presented recommendations to the City of Sonora, City Council to allow BZP to move forward with plans to design smoke-free/vape-free signage to post on Linoberg St. The business owners on the designated block were extremely supportive to host the smoke-free area. Signs were made and posted along the block. These efforts were replicated with the next three events. The Sonora Chamber agreed to make a resolution for all of their events to be smoke/vape free. The Sonora Chamber of Commerce became the designated entity to plan and execute of all city events and all public events in the City of Sonora are now officially smoke/vape free. "The Tuolumne County Blue Zones Project is impacting current and future generations of residents by advocating for, and getting a commitment from, key community leaders to create tobacco free events. I see significant renewed energy towards individual and community wellbeing as a result of the Blue Zones Project. - Bob White, Prevention Programs/YES Partnership Director, Amador Tuolumne Community Action Agency (ATCAA) and Tuolumne County Tobacco Coalition Partner. 5) Smoking cessation and pain management classes were developed in 2023 and will be implemented in 2024 and are open to the community. 6) The Hospital Emergency Department medical/clinical staff participated in the countywide Drugstore Project and Summerville High Schools Every 15 minutes programs to highlight the impacts of drug and alcohol use by youth. Mental Health Support programs and activities in 2023 included the following: 1) Sonora Community Hospital co-sponsored and provided speakers at a regionwide youth mental health education event, Not My Kid where 400 people attended the two events. 2) The Hospital co-sponsored Adult and Youth Mental Health Training along with the YES Partnership where 103 individuals were trained at eight sessions from May 2023-December 2023. Three trainers were certified. 3) The cancer patient support group meets weekly with about ten to fifteen attendees. 4) The neurological group meets monthly with ten to fifteen attendees. 5) Mental health support for veterans was provided by sponsoring the Horses of Warriors which provides disabled veterans the opportunity to socialize an improve mental health outcomes. Over 150 veterans and law enforcement officers have been served. 6) The Tuolumne County Behavioral Health Advisory Board has Hospital representation at monthly meetings. 7) Behavioral health staffing was increased with the addition of one counselor and increased access to licensed providers who joined the county wide Mental Health Coalition. 8) The YES Partnership and EPIC Coalition have Hospital representation at monthly meetings and at the Executive Board. 9) The Blue Zone Project (BZP) public policy advocate is on the executive committee for the EPIC Youth Coalition. BZP principles have been incorporated into the program and address how social connectivity, purpose, healthy eating, physical movement, volunteerism, and mindfulness impact health and well-being. 10) The Calaveras and Tuolumne County Youth Mentoring programs were supported through direct donations and one hundred youth were served in 2023. Sonora Community Hospital applied for an Adverse Childhood Experience (ACEs) grant and was awarded in 2023. Three meetings with Unite Us/Help Me Grow have identified a pathway for easier referrals through the Help Me Grow program. Fifteen referrals were provided in 2023. In 2023, the Hospital applied for a Department of Health Care Services (DHCS) Evidence-Based and Community Defined Evidence Practices Grant and was awarded funds to support the program over 18 months for $400,000. This grant program seeks to identify and scale successful practices that improve access to critical youth behavioral health interventions - including prevention, early intervention, and resiliency/recovery with a specific focus on underserved children and youth. Purpose Workshops in 2023 saw the Tuolumne Blue Zone Project host 22 workshops with 214 youth/adults served. One ten-week purpose moai for nine people was completed. Purpose workshops help community members pinpoint their purpose and find their why. Its known that when an individual finds clarity of purpose, they can enjoy a more centered and vibrant life and even live longer. At the end of the workshop, attendees walk away encouraged and equipped with a purpose statement and tools to live a happier and more purposeful life.Identified Needs from CHNA, Not Addressed and Why - No hospital can address all the health needs identified in its community. Sonora Community Hospital is committed to serving the community by adhering to its mission, and using its skills, expertise, and resources to provide a range of community benefit programs. Significant health needs that will not be directly addressed but fulfilled by other community organizations include access to care, health risk behaviors, health conditions, food security, environment & infrastructure, inclusion & equity, COVID, education, community vitality and community safety. While these significant health needs are not address directly, they are likely to benefit from collective efforts of our community partnerships. Other Items:On April 9, 2022, Sonora Community Hospital proudly launched Blue Zones Project Tuolumne County (BZPTC). The BZPTC team wakes up each morning focused on partnering and collaborating with community leaders and organizations active in the sectors of built environment, education, economic and workforce development, mental and physical well-being,
