Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - HUDSON VALLEY HOSPITAL CENTER. The 2022-2024 NewYork-Presbyterian Hudson Valley Hospital Community Health Needs Assessment (CHNA) was conducted to gain an updated understanding of the needs, assets, and priorities of the communities the hospital serves, and to inform a three-year Community Service Plan (CSP) consistent with the New York State Prevention Agenda 2019-2024. The CHNA is also critical to understanding disparities that must be addressed to achieve health justice. In 2019, NewYork-Presbyterian Hudson Valley Hospital undertook an extensive Community Health Needs Assessment process. This involved use of a zip code level Community Need Index and a separate analysis of NewYork-Presbyterian Hudson Valley Hospital patient data to identify communities with high use of hospital services. Based on findings from these two analyses (i.e., need and hospital use), NewYork-Presbyterian Hudson Valley Hospital focused its 2019-2022 Community Service Plan on the city of Peekskill in Westchester County. Given the commitment to Peekskill, and the progress made with respect to programs included in the 2019-2022 CSP, the process for the 2022-2024 CHNA focused on confirming continuing overall need in this community, confirming need in specific Prevention Agenda priority areas, and recommending approaches to addressing those needs. The 2022-2024 CHNA methods included 90 community member surveys in English and Spanish, five focus groups, five key stakeholder interviews, and a Think Tank session with clinical and community-facing NewYork-Presbyterian Hudson Valley Hospital and university affiliated staff, and publicly available data on 70 health indicators. Analyses from these multiple sources reaffirmed the commitment to Peekskill. The 2022 CHNA methods included 270 community member surveys in five languages, 8 focus groups in three languages (English, Spanish and Mandarin), 5 key stakeholder interviews, a Think Tank session with clinical and community- facing NewYork-Presbyterian Hudson Valley Hospital and university affiliated staff, and publicly available data on 70 health indicators. Analyses from these multiple sources reaffirmed the commitment to the neighborhood of Peekskill. The health status of Westchester County residents varies based on race, ethnicity, and neighborhood. Residents of neighborhoods with lower income and higher concentrations of Black and Latino/a residents have poorer health and higher rates of premature mortality, compared to higher-income neighborhoods and neighborhoods comprised of predominantly White residents. NewYork-Presbyterian Hudson Valley Hospital identified Peekskill as a community with health disparities. The 2022-2024 CHNA process confirmed continuing overall need in Peekskill, and the hospital will retain a specific focus on the community.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - HUDSON VALLEY HOSPITAL CENTER. NewYork-Presbyterian Hudson Valley Hospital conducted their triennial Community Health Needs Assessment (CHNA) for 2022-2024 to assess needs and health outcomes of the communities we serve. The CHNA process involved extensive data analysis of zip code level data and patient zip codes to identify communities with high need and instances of negative health outcomes. For this cycle, the city of Peekskill was identified as the priority area. Findings across each data source and from community feedback were largely consistent and show that many of the health issues and priorities identified in previous CHNAs remain the same: food security, challenges related to diet and nutrition, poor mental health, and substance use. In concert with clinicians, leadership of partnering Community-Based Organizations (CBOs), and the Hospital, the following New York State Department of Health (NYSDOH) Prevention Agenda items were selected for the 2022-2024 CSP period based on the analysis of data and community feedback: * Prevent Chronic Disease - Healthy Eating and Food Security * Promote Healthy Women, Infants, and Children - Cross Cutting Healthy Women, Infants, and Children * Promote Well-being and Prevent Mental and Substance Use Disorders - Well-being Due to the catchment areas and unique community feedback received from each regional facility, NYSDOH Prevention Agenda items may differ. Although substance use was identified as a need by the community, NewYork-Presbyterian is focused on the above Prevention Agenda items due to the prevalence of disease. Based on the above identified prevention agenda items, NewYork-Presbyterian Hudson Valley Hospital implemented the following programs as part of the CSP. * Choosing Healthy and Active Lifestyles for Kids (CHALK): CHALK will partner with emergency food providers in Peekskill to provide nutrition education, Snap enrollment, renewal and social services connection, and free health grocery boxes delivered to the home. NewYork-Presbyterian Hudson Valley Hospital patients will be screened for social determinants of health and enrolled if screened positive for food insecurity. * Healthy Steps: Health Steps engages uninsured and low-income pregnant parents during the prenatal period and continues to provide services through the child's 3rd birthday. In so doing, this program establishes a two-generation approach for improving maternal-child health in primary care and community settings. The program will also leverage telehealth to meet mothers in their home environment and connect them with community health workers to ensure that families can successfully navigate the medical and social service system. * Mental Health First Aid (MHFA): MHFA is an international training program that builds skills on how to identify, understand, and respond to signs of mental illness and substance use disorders. Hospital staff, CBOs, and Faith-Based Organizations (FBO) are offered the training to become certified Mental Health First Aiders, where they can support their communities and peers that are at an elevated risk for developing mental health challenges. Two trainings are offered: Mental Health First Aid and Mental Health First Aid Youth training.
Schedule H, Part V, Section B, Line 15 Facility , 1Facility , 1 - HUDSON VALLEY HOSPITAL CENTER. HUDSON VALLEY HOSPITAL HAS A FINANCIAL ADVOCACY PROGRAM STAFFED BY REPRESENTATIVES WHO REACH OUT TO PATIENTS TO PROVIDE INFORMATION REGARDING MEDICAID, EXCHANGE PLANS, FINANCIAL AID AND TO ASSIST THOSE PATIENTS WHO NEED HELP TO APPLY TO SUCH PROGRAMS.
Schedule H, Part V, Section B, Line 16 Facility , 1Facility , 1 - HUDSON VALLEY HOSPITAL CENTER. THE HOSPITAL FOLLOWS TWO BASIC APPROACHES TO PUBLICIZING THE AVAILABILITY OF FINANCIAL AID. FIRST, IT MAKES THE FINANCIAL AID POLICY ITSELF, A PLAIN LANGUAGE SUMMARY, AND THE FINANCIAL AID APPLICATION AVAILABLE AT VARIOUS HOSPITAL PATIENT ACCESS POINTS, POSTS SIGNS CONSPICUOUSLY IN PUBLIC AREAS OF THE HOSPITAL, INCLUDES INFORMATION ON BILLING STATEMENTS, POSTS INFORMATION (INCLUDING HOW TO OBTAIN THE POLICY, SUMMARY AND APPLICATION) ON THE WEBSITE, AND RESPONDS TO INQUIRIES FROM PATIENTS AND MEMBERS OF THE COMMUNITY ON FINANCIAL AID. SECONDLY, THE HOSPITAL PROVIDES UPDATES AND INFORMATION (INCLUDING THE POLICY, THE SUMMARY AND/OR THE APPLICATION) ON A REGULAR BASIS TO LEADERS OF LOCAL COMMUNITY BASE ORGANIZATIONS, ELECTED OFFICIALS, THE CITY HEALTH DEPARTMENT AND WESTCHESTER COUNTY DEPARTMENT OF HEALTH.

Schedule H, Part VI, Line 2 Needs assessmentDescription: As part of the 2019-2022 CHNA process, which included an analysis of hospital patien datta and needs across various indicators, NewYork-Presbyterian Hudson Valley Hospital identified Peekskill as a community with significant health disparities. The 2022-2024 CHNA process confirmed continuing overall need in Peekskill, and the hospital will retain a specific focus on the community. However, NewYork-Presbyterian Hudson Valley Hospital is committed to serving all communities within its reach. Socioeconomic status and selected health indicators of Peekskill residents are described immediately below. The health indicators presented are primarily summary indicators (e.g., avoidable hospitalizations) and indicators consistent with the priorities and programs of NewYork-Presbyterian Hudson Valley Hospital, including those demonstrating disparities as well as those highlighted by CHNA focus group participants and interviewees. Neighborhood history and demographics Incorporated as a city in 1940, Peekskill is located in the northern part of Westchester County, inhabiting more than four square miles. In the 19th century, Peekskill hosted several northern sites through which enslaved people exited the Underground Railroad to freedom. In 2021, Peekskill had an estimated population of 25,740: 44% were Latino/a, 39% White, 23% Black, and 2% Asian/Pacific Islander. The Latino/a population is almost 20 percentage points higher than Westchester overall and reflects a significant population shift in Peekskill over the last two decades. 27% of Peekskill residents were born outside the U.S., 42% speak a language other than English at home and 20% of residents over age five speak English less than "very well." More than 11% of residents do not have health insurance, and 13% live below the poverty line. The median household income is Peekskill is approximately $66,000, compared to $99,000 for Westchester County, overall. According to individuals engaged in the CHNA process, the COVID-19 epidemic exacerbated financial issues faced by the local population. They described high housing costs and an inadequate supply of affordable housing in Peekskill and the surrounding area, resulting in crowding, the need to work multiple jobs, and insufficient resources to meet other basic needs, including healthcare. Approximately 25% of Peekskill households are considered severely cost burdened. Waiting lists for the Section 8 Housing Choice Voucher Program for Peekskill average 42 months; the waiting list for Peekskill Public Housing averages 26 months. Community health and well-being Nearly 60% of births in Peekskill are to parents enrolled in Medicaid, and 28% of pregnant parents had no or late prenatal care. Forty percent of school-age children in Peekskill are considered overweight or obese. Peekskill residents experience wide disparities in average age of death: 81 years for White residents, 75 years for Latino/a residents, and 70 years for Black residents. Disparities are more stark for Black women and Latino men whose average age of death is 67. Disparities relative to the Westchester County more generally are also evident. For example: * In Peekskill, 25% of deaths were considered premature (before age 65), compared to 19% in Westchester County; * The age-adjusted potentially preventable hospitalization rate per 10,000 adults was 157 in Peekskill, compared to 92 per 100,000 for Westchester County; * Asthma emergency department visits in Peekskill were 101 per 10,000 population, compared to 58 in Westchester County. Focus group participants and key stakeholder interviewees described an unhealthy food environment with an abundance of fast food and poor access to grocery stores. A number of indicators are similar or better for Peekskill than Westchester County overall. The age-adjusted homicide rate from 2016-2018 was 0 per 100,000 in Peekskill, compared to 2.3 per 100,000 in Westchester County. The rate of preterm birth was slightly lower for Peekskill than Westchester County (8% compared to 9%) and the percentage of infants exclusively breastfed in the hospital was higher (79% compared to 53%). The age-adjusted suicide rate for Peekskill was similar to the County rate: 7 per 100,000 in Peekskill compared to 8 per 100,000 for Westchester County.

Schedule H, Part VI, Line 5 Promotion of community healthDescription: A variety of resources are available in Peekskill to promote good health and to assist individuals and families facing challenges that affect their health and well-being. These include community- and faith-based organizations (CBOs and FBOs), government programs and services, school-based resources, and informal networks and support systems. Community and faith-based organizations often serve as trusted sources of support, information, and camaraderie. Their services are often relatively easy to access, because they are local and commonly offered free of charge. Many CBOs employ staff that speak the same language and have a similar background or ethnicity to the people served. They generally offer or refer to an array of services that address the social determinants of health, helping people address their needs in a coordinated fashion. CBOs sometimes employ community health workers (CHWs) or caseworkers to assist clients or members to navigate and access governmental programs, including SNAP and public insurance, to provide health education and support disease management, and to serve as liaisons to healthcare systems. CHWs, who typically share characteristics with the community they work in (e.g., language, ethnicity, neighborhood of residence), can serve as more accessible sources of information to community members less comfortable in larger institutions, facilitating improved health outcomes. Many CBOs that engaged in the CHNA process represent important assets for the community. These include but are not limited to: * Caring for the Hungry and Homeless of Peekskill (CHHOP) * El Centro Hispano * Feeding Westchester * Field Library * Sun River Health * The SUNY Westchester Community College, Peekskill Extension Center * Support Connection * Transportation-Resources-Access (TRA) * Westchester Jewish Community Services NewYork-Presbyterian Hudson Valley Hospital also offers resources to support community health and well-being. A few key programs and services include: * Choosing Healthy & Active Lifestyles for Kids (CHALK) aims to lower the prevalence of obesity among children and teens by reducing food insecurity and creating environments where all families adopt healthy lifestyles. As part of CHALK, patients at selected healthcare sites who are facing food insecurity receive referrals to the Food FARMacia, which offers healthy groceries from Feeding Westchester, at no cost to recipients. * Healthy Steps, a collaboration between NewYork-Presbyterian Hudson Valley Hospital and Sun River Health, offers integrated mental health services in primary-care and community settings for low-income and uninsured pregnant women and their newborn children. * The Dalio Center for Health Justice was founded to address racism and discrimination and to advance health justice. The work of the Center focuses on improving race and ethnicity documentation in healthcare, funding community programs, vaccine equity, and other activities documentation in healthcare, funding community programs, vaccine equity, and other activities that improve health outcomes for all.
