Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - WINTER HAVEN HOSPITAL, INC.. In 2022 Winter Haven Hospital SOLICITed INPUT FROM KEY INFORMANTS, THOSE INDIVIDUALS WHO HAVE A BROAD INTEREST IN THE HEALTH OF THE COMMUNITY, via AN ONLINE KEY INFORMANT SURVey. A LIST OF RECOMMENDED PARTICIPANTS WAS PROVIDED BY WINTER HAVEN HOSPITAL; THIS LIST INCLUDED NAMES AND CONTACT INFORMATION FOR PHYSICIANS, PUBLIC HEALTH REPRESENTATIVES, OTHER HEALTH PROFESSIONALS, SOCIAL SERVICE PROVIDERS, AND A VARIETY OF OTHER COMMUNITY LEADERS. POTENTIAL PARTICIPANTS WERE CHOSEN BECAUSE OF THEIR ABILITY TO IDENTIFY PRIMARY CONCERNS OF THE POPULATIONS WITH WHOM THEY WORK, AS WELL AS OF THE COMMUNITY OVERALL. KEY INFORMANTS WERE CONTACTED BY EMAIL INTRODUCING THE PURPOSE OF THE SURVEY AND PROVIDING A LINK TO TAKE THE SURVEY ONLINE; REMINDER EMAILS WERE SENT AS NEEDED TO INCREASE PARTICIPATION. IN ALL, 100 COMMUNITY STAKEHOLDERS IN THE WINTER HAVEN HOSPITAL SERVICE AREA TOOK PART IN THE ONLINE KEY INFORMANT SURVEY, AS OUTLINED ON APPENDIX E OF THE CHNA. SEVERAL OF THE PARTICIPANTS RESPONDING TO THE SURVEY REPRESENT ORGANIZATIONS WHICH WORK WITH LOW-INCOME, MINORITY OR OTHER MEDICALLY UNDERSERVED POPULATIONS. Additionally, in an effort to ensure the voices of those who are historically underrepresented in our communities were heard, specific focus groups were conducted by engaging individuals from diverse backgrounds including children, older adult populations, Hispanic/LatinX, LGBTQIA+, and Black/African American communities.
Schedule H, Part V, Section B, Line 6a Facility , 1Facility , 1 - WINTER HAVEN HOSPITAL, INC.. The Community Health Needs Assessment was conducted with the following non-hospital facilities: Florida Department of Health in Hillsborough County Florida Department of Health in Pasco County Florida Department of Health in Pinellas County Florida Department of Health in Polk County
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - WINTER HAVEN HOSPITAL, INC.. Winter Haven Hospital concentrated or will concentrate efforts in the 2023-2025 time period to address the following significant health needs of our community as identified in the 2023-2025 CHNA: * Access to health and social services * Behavioral Health (Mental Health and Substance Use) * Exercise, Nutrition and Weight Winter Haven Hospital's 2023-2025 Implementation plan describes specific activities that were underway to address these significant health needs in the 2023-2025 time period. Although the majority of the overarching goals, objectives and efforts focus on these top three focus areas, Winter Haven Hospital remained committed to supporting positive advancements in addressing diabetes, heart disease and stroke, while addressing the focus area of exercise, nutrition and weight. There were five additional health focus areas identified through the CHNA. These were: * Cancer * Immunization and infectious disease * Maternal, fetal and infant health * Oral health * Respiratory disease These remaining health needs are significant, but they're not directly addressed in 2023-2025 health improvement plan strategies. However, they continued to be impacted through existing hospital strategies for clinical excellence and Winter Haven Hospital partnerships within the community, including the Florida Department of Health and community organizations who may be in a better position to address these health issues. SEE NARRATIVE FOR SCHEDULE H, PART VI, LINE 5 FOR ADDITIONAL DETAIL REGARDING HOW THE HOSPITAL ADDRESSES NEEDS IDENTIFIED IN THE CHNA.
Schedule H, Part V, Section B, Line 13 Facility , 1Facility , 1 - WINTER HAVEN HOSPITAL. PATIENTS MAY BE ELIGIBLE FOR FINANCIAL ASSISTANCE ON THE BALANCE OWED ON THEIR ACCOUNT IF THE HOSPITAL BILL EXCEEDS 25% OF THEIR ANNUAL INCOME.

In 2022, BayCare worked alongside other local not-for-profit hospitals and county departments of health as the All4HealthFL Collaborative. The purpose of the All4HealthFL Collaborative is to improve health by leading regional, outcome-driven health initiatives that have been prioritized through community health assessments. The Collaborative came together to conduct the triennial Community Health Needs Assessment (CHNA) to drive the next cycle of CHNA programming. To provide as complete an overview as possible of the health behaviors and perceptions of Polk County residents, a 59-question community survey was developed. The primary purpose of utilizing a community survey was to ensure that the voices of Polk County residents were heard, engaging those most impacted by health issues where they live, work, play and raise families. The survey addressed health and lifestyle behaviors, quality of life and access to care. The community survey used validated and reliable questions obtained from state and national needs assessments, passed a rigorous literacy review and was available in English, Spanish, and Creole. In addition, the All4HealthFL Collaborative engaged a health equity expert to validate the verbiage of survey questions and responses to ensure equity for all respondents. Intentional and strategic outreach was integral in getting a robust and representative response rate. Due to the ongoing COVID-19 pandemic, surveys were primarily deployed electronically, using either a direct link or QR code for easy access. The collaborative was prepared for this and leveraged our partners across the county to ensure that the voices of our residents were heard. Even during the challenges posed by the pandemic, the survey was disseminated in libraries, churches, food pantries and community centers. This strategy allowed for engagement of low-income, minority and medically underserved populations. Through the collaborative's efforts, nearly every zip code in Polk County was represented in the survey collection. A county-wide prioritization meeting was held in the spring of 2022 with community leaders to rank the needs identified in the collected data. Top priority areas identified included Behavioral Health (Mental Health and Substance Use), Access to Health and Social Services, and Exercise, Nutrition and Weight. Winter Haven Hospital and Winter Haven Women's Hospital are committed to providing financial and other support to not-for-profit organizations whose missions are to improve the health and well-being of our community. In 2023, Winter Haven Hospital and Winter Haven Women's Hospital provided funding to community-based organizations including: * The Neighborhood Service Center received $97,000 from Winter Haven Hospital to provide an array of senior day programming, which includes health and wellness classes, and a community food pantry. * The Mission of Winter Haven, which serves individuals and families who are experiencing crisis or homelessness through a variety of wraparound services, was awarded $84,000 from Winter Haven Hospital to support the soup kitchen and food pantry, personal hygiene kits for unhoused community members, and trainings for staff and volunteers on topics including food safety and mental health coaching. * Gospel Inc. provides stable and permanent housing, as well as wraparound social and mental health services, to those who have been chronically unhoused. Winter Haven Hospital awarded Gospel Inc. $31,500 to expand their Wellness Center and food pantry services to Gospel Village residents. Winter Haven Hospital hosts a Cancer Resource Room, which offers support to navigate cancer diagnosis and treatment as well as needs associated with life after remission such as spiritual, emotional, healthy eating and follow-up guidelines. In addition, WHH provided cancer support groups throughout 2023. Winter Haven Hospital supports the Haley Center. The mission of the Haley Center is to provide healthcare to any person or family who reside in Polk County whose income is at or below 200% of the Federal Poverty Guidelines and are not otherwise insured by the government or a private health insurance. The center then seeks to provide a complete medical home to all who meet this requirement. Winter Haven Hospital provides the space for the Haley Center to operate as well as free labs and x-rays. The Center for Behavioral Health (CBH) of Winter Haven Hospital provides professional outpatient behavioral health services to people of all ages. Staff of highly credentialed licensed counselors and psychiatrists and certified mental health professionals provide counseling and treatment services that may include psychiatric diagnosis and evaluation, psychotropic medication management, individual, group and family therapy as well as case management, crisis support and peer support services. The Center for Psychiatry (CFP) of the Winter Haven Hospital provides inpatient psychiatric hospitalization and treatment. Board certified Psychiatrists lead an interdisciplinary treatment team in the provision of comprehensive care. Treatment is recovery focused to engage the person to achieve treatment goals and improve functioning at home and in the community upon discharge. The treatment team addresses psychiatric as well as co-occurring medical and substance use issues that may be affecting the person and ensures that follow up care is arranged prior to discharge. In 2023, the Winter Haven Hospital's Care Coordination Team provided approximately $1.1 million in dedicated time and community benefit services. This program includes pharmacy, transportation, skilled nursing facilities, assisted living facilities and other discharge needs for patients. Many patients are either uninsured, unable to appropriately care for themselves or do not have a caregiver. In 2023 across BayCare, our Financial Assistance team members provided support in overcoming barriers to accessing health care by screening approximately 68,000 patients, completing approximately 21,000 applications for reimbursable programs like Medicaid and another 46,000 charity related applications. Through BayCare's partnership with Uber Technologies, a non-emergency transportation solution is provided for low-income patients across BayCare's four-county service area. In 2023, the program provided 30,002 rides to patients. (Continued)
Schedule H, Part VI, Line 5 Promotion of Community Health (continued)For more than 30 years, BayCare's Children's Wellness and Safety Center has been committed to keeping kids and families healthy, safe, and informed through a multifaceted outreach approach focusing on community education, unintentional injury prevention, children's health and wellness, and legislative advocacy. In 2023, the BayCare Wellness and Safety Center educated more than 94,076 children and their families through community programs and events across BayCare's footprint. Recognizing that behavioral health needs among hospital patients are varied and that hospitalization itself is stressful, BayCare hired Behavioral Health Liaisons in many hospitals across our system. The liaison team is comprised of 10 master's level clinicians who provide therapeutic care within acute medical settings. These clinicians provide bedside support to referred patients struggling or in crisis during hospitalization. They facilitate connections between the patient and community-based treatment services prior to discharge and will often coordinate with social workers, care coordinators, nursing, and other providers to address specific patient's psychiatric needs. The liaisons also provide care for our team members. BayCare's Community Health team develops community partnerships with area agencies, providing wellness education and disease prevention screenings directly into area neighborhoods. Community Health participated in 307 events and programs in 2023 and was able to promote better health to more than 4,486 people. BayCare's Healthy Living Coach program provides 6 free clinics/federally qualified health centers located in high-risk neighborhoods within the region. In 2023 BayCare continues to provide funding support for 6 trained Healthy Living Coaches that are responsible for providing in-person, telehealth, and telephonic coaching for those living with diabetes or pre-diabetes. Coaches provide support in decision making, motivation, and resource navigation in managing their diagnosis. Our Faith Community Nurses and Health Ministers work to improve the health of their faith community by encouraging preventive care and referring members to the appropriate health resources. In 2023 across BayCare, our Faith Community Nursing team of 280 nurses and 74 community health promoters had direct contact with 69,900 faith community members and provided 6,100 referrals to local resources. BayCare, as a regional anchor institution, was positioned and committed to developing innovative strategies to address food insecurity. * Partnering with Feeding Tampa Bay, BayCare works to engage patients who are food insecure and connect them with a Healing Bag of 2-3 days of non-perishable food upon discharge. The Healing Bags also contain simple to read resources to assist patients in finding help within their community. In 2023, the project provided over 13,000 Healing Bags to patients. This successful project is the first of its kind regionally and was expanded to our Nursing teams in March 2023 to increase our efforts in screening nearly 100% of our patient population. * In collaboration with Feeding Tampa Bay, BayCare partnered to support 46 food pantries in Hillsborough, Pinellas, Pasco and Polk Counties. A total of 1,435,643 lbs. of food was provided to students and their families through the Feeding Minds School Pantry program supplying over 1 million meals to those in need. BayCare's Clinical Research Operations Teams provide specialized staffing support to Principal Investigators across BayCare Health System in the conduct of clinical research involving human subjects. The primary purpose is to increase access opportunities for participation in clinical research by both clinicians and patients in an effort to provide potentially life-changing care for patients today and to advance the science of medicine for future generations. Cutting-edge inpatient and outpatient studies focus on pharmaceutical and device trials, infectious diseases, and pediatric and adult trials including investigational research in hematology, oncology, cardiology, and tissue banking studies. Volunteer Activities: In 2023, Winter Haven Hospital's 34 volunteers contributed 10,371 hours of volunteer service. Beginning in 1938, the Winter Haven Hospital Auxiliary has found many ways to contribute directly to the welfare of the hospital, its patients, and the community at large. Striving to maintain the highest standards of conduct, the Auxiliary and volunteers provide a healthy, safe, caring environment for staff, visitors, and patients. Our volunteers offer assistance as lobby ambassadors, gift shop clerks, piano players, as well as some providing light clerical support. Team members contributed to the health of the community by: * Raising over $9,000 in financial support and 2,556 lbs. of food in donations to FTB. Combined, this support provided over 71,000 meals to community members in need. *Holding the annual Team Member Giving Campaign. Across the system, team members donated over $580,000 to BayCare hospital foundations and other local charities.

Schedule H, Part VI, Line 5 Promotion of community healthOur mission at Winter Haven Hospital as part of BayCare Health System is to improve the health of all we serve through community-owned services that set the standard for high-quality compassionate care. By focusing on education and prevention, we strive to create a healthier community and lower health care expenditures. Winter Haven Hospital, Inc., located in Winter Haven, Florida, includes an acute care hospital and an obstetric/gynecological specialty hospital, respectively referred to as Winter Haven Hospital and Winter Haven Women's Hospital. Winter Haven Hospital (WHH) was established in 1926, and today concentrates in the treatment of heart disease, behavioral health, cancer, stroke, and orthopedic conditions. The hospital also provides emergency care, outpatient diabetes education; endoscopy and surgical services; cardiac, neurological, orthopedic, and pulmonary rehabilitation; bariatrics and imaging services. Winter Haven Women's Hospital (WHWH), built in 1987 and located approximately 1.5 miles south of WHH, includes a level II neonatal intensive care unit and an outpatient breast and imaging center. Services include on-site continuous obstetric coverage, maternal fetal medicine appointments, urgent care and lactation consultations. A majority of the organization's governing body is comprised of persons who reside in the organization's primary service area who are neither employees nor independent contractors of the organization, nor family members thereof. The organization extends medical staff privileges to all qualified physicians in its community for some or all of its departments or specialties. The organization uses surplus funds for improvements in facilities and equipment, patient care, medical training, education, and research. Community Involvement: WHH involves community members as an essential component of its Board of Trustees. The trustees donate their time, talents and support; assist with policy formulation, decision making and oversight. WHH is a member of the Central Florida Economic Development Council, Main Street Winter Haven, the Winter Haven Chamber of Commerce and the Northeast Polk Chamber of Commerce. Conduit for Using Donated Funds: BayCare has received funding from Allegany Franciscan Ministries to provide capacity and support to our Faith Community Nursing program to reduce unnecessary hospital utilization and improve health outcomes for participants. Funding of $90,000 awarded in 2023 supports this work across BayCare Health System. BayCare has also received funding from Allegany Franciscan Ministries to provide assistance to those who face financial barriers in getting their needed prescription medications. Medication assistance coordinators work with patients to determine if there are vouchers available or work with the patient's doctor to see if a more cost effective, alternative medication is available. Annual funding of $50,000 provides support for this program across BayCare Health System. BayCare has received funding from Allegany Franciscan Ministries to support Hope Health. Hope Health is co-located on the grounds of Pinellas Hope, an outdoor homeless community managed by the Catholic Charities Diocese of St. Petersburg. Part of Hope Health is the Medical Respite Program (MRP), a 10-unit, safe, secure place for continued recuperation after an acute hospital encounter for patients with no permanent or transitional housing available at discharge. MRP enables homeless patients an opportunity to heal and recover while providing continued wound care, IV antibiotics, assistance with medication management, and scheduling and attending medical follow-up appointments. Funding of $60,000 awarded in 2023 supports this work. The Florida Department of Health provides funding support for the Gettin' REAL program that employs a comprehensive, collaborative community-based approach to address disparities in the health outcomes of minority populations related to diabetes and cardiovascular disease across Pinellas, Hillsborough, Pasco, and Polk Counties. In 2023, through funding support, BayCare provided screenings to 2,108 individuals, and made 2,071 referrals to lifestyle-change programs (Get into Fitness Today, National Diabetes Prevention Program, Diabetes Education Empowerment Program, and the FL Tobacco Quitline), food insecurity services, and instructor-led physical activity to achieve the best possible results for the most vulnerable. Special services for adults and their families throughout Polk, Highlands and Hardee counties are provided through contracts with the state of Florida's Department of Children and Families and the Polk County Indigent HealthCare Division. These services are designed to provide assistance and stability to families. These special programs include Helping Families Succeed (HFS) and Specialized Community Care Team (SCCT). The SCCT is for adults with mental health difficulties that affect their daily functioning. Services provided are at-home therapy, medication management, and resource referrals to help the patient attain wellness, resiliency and self-sufficiency. Children with emotional issues in Polk County public schools have access to on site mental health consultation and counseling services from our professional staff. These services are made available through a contract with the Polk County School Board. Included are children from pre-school age through high school. Additionally, through specialty contracts our trained pre-school specialists provide on-site counseling and consultation to assist in the developmental and emotional needs for young children in pre-school head start centers and migrant head start centers. Economic Impact: There are 2,220 extraordinary WHH team members and 672 physicians, who together bring the WHH mission to life. By inviting the next generation of health care workers to learn from the skill and expertise of our current team members, Winter Haven Hospital provides valuable benefit to the community. Student clinical experiences allow health care students opportunities to learn skills and knowledge necessary to function in acute care facilities and gain competence in clinical techniques. In total, nearly $4 million in staff time for nurses, dietitians, lab technologists and other clinical providers was used to educate these future health care providers about many aspects of patient care, in addition to resident and intern stipends. In addition, WHH provides an ASHP-accredited pharmacy residency program. The 12-month training allows four residents per year to collaborate with medical staff, work with patients and partner with nursing. WHH clinical pharmacists have helped prepare graduates for practice in diverse clinical settings, become board-certified, and to continue training in a second-year program. Winter Haven Hospital formally contracted with Florida State University in 2017 to begin offering Graduate Medical Education for family health physicians. The program provides additional health access in our community and helps recruit family health physicians to practice in the area. This was in direct response to health disparities in the WHH service area. Good Neighbors: In 2023, Winter Haven Hospital provided over $20,000 in financial support to community not-for-profit organizations serving those disadvantaged across the community through sponsorship funds. (In certain cases, hospital locations with the same tax identification and state license number are listed as one facility on Form 990, Schedule H, consistent with IRS reporting guidelines.) In March 2023, BayCare opened its 16th hospital, BayCare Hospital Wesley Chapel, to serve a rapidly growing area of Pasco County on Florida's gulf coast. BayCare also continued construction of a replacement hospital for South Florida Baptist Hospital in Plant City, with an expected opening in 2024, and announced plans to build a new hospital for St. Joseph's Children's Hospital in Tampa. In April 2023, BayCare made its first move into Manatee County with the opening of a primary care medical office. BayCare is working on plans to build Manatee's first not-for-profit hospital, with a projected opening in 2027. BayCare was founded in 1997 after leaders of several of the area's independent, not-for-profit hospitals began discussing ways to ensure that high-quality, not-for-profit health care would remain a viable option for the Tampa Bay community for decades to come. The founding hospitals agreed to sacrifice some of their autonomy to be operated by a new entity, BayCare Health System. With $5.5 billion in operating revenue in 2023, BayCare is now a fully integrated health system dedicated to providing high-quality, compassionate care to all we serve, regardless of their ability to pay. In 2023, BayCare had 4,004 hospital beds, 18 urgent care centers, 4 surgery centers, 15 outpatient imaging facilities, 185 physician practice locations and 20 behavioral health practice locations. BayCare annually generates $10 billion in economic impact in the region and state. BayCare is one of the largest employers in the Tampa Bay area, with 31,993 team members. During 2023, BayCare provided $766 million in benefits for its employees, including $238 million in retirement savings, $208 million in medical insurance, $193 million in paid time off and $70 million in education assistance. In 2023, BayCare was ranked in the top quartile of large U.S. health systems by Fortune/PINC AI based on clinical outcomes, operational efficiency and patient experience. In addition, all 11 of BayCare's eligible acute care hospitals received "A" grades on patient safety from The Leapfrog Group in fall 2023; BayCare hospitals earned numerous five-star ratings on clinical outcomes from Healthgrades; and BayCare earned 12 Press Ganey awards for extraordinary patient experience based on patient surveys. In the area of workplace honors, BayCare was named to two prestigious national lists: Fortune Best Workplaces in Health Care and PEOPLE Companies that Care. BayCare also made the 2023 Top Workplaces list of the Tampa Bay Times newspaper, an achievement annually since 2015. In 2023, BayCare provided $557 million in total Community Benefit, including $399 million in Medicaid and other income-based programs, $81 million in traditional charity care and $77 million in unbilled community services, all measured in unreimbursed costs. Addressing hunger has been a vital mission for BayCare since 2019, when a Community Health Needs Assessment identified food insecurity as one of the top issues in the Tampa Bay and West Central Florida regions. After a second needs survey in 2022 established that food insecurity was still a pressing issue, BayCare expanded its efforts. During 2023, BayCare contributed major financial support to 42 school-based food pantries operated by BayCare partner Feeding Tampa Bay, providing free food to students, their families and others who live near each school. The pantries served 30,000 households during 2023. BayCare also asks all patients admitted to its 16 hospitals if they are food insecure and offers a "Healing Bag" upon discharge to anyone who answers "yes." The bags contain two to three days of nonperishable food, plus information about food resources near their homes. More than 13,000 Healing Bags were distributed during 2023. Also in 2023, BayCare stepped up on another issue of great concern across Florida and the nation: children's mental health. BayCare brought to the Tampa Bay region "On Our Sleeves," a national movement to address the mental health needs of children. BayCare is making a $250,000 annual investment so parents and caregivers can utilize the program's tools and resources. Twenty-six years after BayCare was created, it is achieving the goal of its founders to preserve access to not-for-profit health care for the communities of Tampa Bay and West Central Florida, and it is fulfilling its Mission as an organization to "improve the health of all we serve through community-owned services that set the standard for high-quality, compassionate care."
