| Desc | IN THE MOST RECENT FISCAL YEAR (FY24), RML DELIVERED ON ITS MISSION TO PROVIDE QUALITY, COMPASSIONATE CARE TO PATIENTS FROM OUR REFERRING COMMUNITY WHO SUFFER FROM PROLONGED, SEVERE ILLNESS. OUR CARE PROGRAMS INCLUDED THE CARE AND WEANING OF PATIENTS WHO REQUIRE PROLONGED MECHANICAL VENTILATION, THE CARE AND TREATMENT OF PATIENTS WITH COMPLEX WOUNDS, AND THE CARE AND TREATMENT OF PATIENTS THAT REQUIRED PROLONGED ACUTE CARE HOSPITALIZATION. RML IS RECOGNIZED AS A LEADER IN LONG-TERM ACUTE CARE AND IS ONE OF THE LARGEST VENTILATOR WEANING HOSPITALS IN THE NATION. RML SERVED APPROXIMATELY 90 REFERRAL SOURCES THROUGHOUT ILLINOIS, INCLUDING OUR PARTNER ORGANIZATIONS, MAJOR TERTIARY CARE CENTERS, AND COMMUNITY HOSPITALS. NEARLY 100% OF RML'S INPATIENT ADMISSIONS ARE DIRECT TRANSFERS FROM THESE HOSPITAL REFERRAL SOURCES. RML OPERATES A FACILITY LOCATED IN A LOW-INCOME AREA OF CHICAGO. OVER THE YEAR, DESPITE DECREASED VOLUME, WE HAVE MANAGED TO SUSTAIN THE VIABILITY OF AN ENTITY THAT PROVIDES QUALITY HEALTH CARE AND WELL-PAYING JOBS FOR THE INNER-CITY COMMUNITY. LONG-TERM ACUTE CARE WAS PROVIDED TO MORE THAN 1,075 PATIENTS BETWEEN BOTH RML FACILITIES, INCLUDING MORE THAN 375 MOSTLY MINORITY PATIENTS AT THE CHICAGO FACILITY. IN ORDER TO SUPPORT THE SPECIALIZED NEEDS OF OUR PATIENTS AND COMMUNITY, RML UTILIZES A PHYSICIAN-LED INTERDISCIPLINARY TEAM TO PROVIDE TREATMENT TO PATIENTS WITH ACUTE OR CATASTROPHIC ILLNESSES AND INJURIES COMPLICATED BY COMPLEX OR MULTIPLE CO-MORBIDITIES. OUR COMMITMENT TO EXCELLENCE AND SUPERIOR CLINICAL OUTCOMES HAS ALLOWED US TO IMPROVE ON OUR ALREADY-LOW HOSPITAL-ACQUIRED INFECTION AND PRESSURE ULCER RATES, AS WELL AS MAINTAIN A LOW RATE OF ADVERSE OUTCOMES (DEFINED AS EXPIRATIONS AND TRANSFERS TO ACUTE CARE HOSPITALS) | Desc |

| Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote | RML is a not-for-profit limited partnership and has been recognized as a tax-exempt organization by the Internal Revenue Service pursuant to Section 501(c)(3) of the Code. Income earned in furtherance of RML's tax-exempt purpose is exempt from federal and state income taxes. The Code provides for taxation of unrelated business income; however, such status is subject to final determination upon examination of the related income tax returns by the appropriate taxing authority. U.S. GAAP prescribes recognition thresholds and measurement attributes for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Tax benefits will be recognized only if the tax position is more-likely-than-not to be sustained in a tax examination, with a tax examination being presumed to occur. The amount recognized will be the largest amount of tax benefit that is greater than 50% likely of being realized on examination. For tax positions not meeting the more-likely-than-not test, no tax benefit will be recorded. Management is not aware of any tax benefits or liabilities to be recognized May 31, 2024, and 2023. The Partnership does not expect the total amount of unrecognized tax benefits to significantly change in the next 12 months. Due to its tax-exempt status, the Partnership is not subject to U.S. federal income tax or state income tax. The Partnership recognizes interest and/or penalties related to income tax matters in income tax expense. The Partnership did not have any amounts accrued for interest and penalties at May 31, 2024 or 2023. Tax returns filed by the Partnership are no longer subject to examination for the years ended May 31, 2020 and prior. |

| Schedule H, Part V, Section B, Line 3E | THE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND IDENTIFIED THROUGH THE CHNA. |
| Schedule H, Part V, Section B, Line 5 Facility A, 1 | Facility A, 1 - FACILITY REPORTING GROUP A. THE COMMUNITY INPUT DATA FOR THE MOST RECENT RML CHNA WAS COLLECTED BETWEEN MARCH AND APRIL 2022. CROWE, LLP (CROWE) WORKED WITH RML TO CONDUCT KEY INFORMANT INTERVIEWS AND ANALYZE DATA. PARTICIPANTS INCLUDED FORMER PATIENTS, PROVIDERS IN OTHER POST ACUTE SETTINGS, AND MEMBERS OF COMMUNITY ORGANIZATIONS THAT TREAT AND PROVIDE ASSISTANCE TO CHRONICALLY, CRITICALLY-ILL PATIENTS IN RML'S COMMUNITY. AS THE ELDERLY AND LOW-INCOME ARE LEAST LIKELY TO HAVE THE RESOURCES TO RECEIVE TREATMENT AS A CHRONICALLY, CRITICALLY-ILL PATIENT, RML FOCUSED ON THESE POPULATIONS. THE CHNA WAS ALSO INFORMED BY SECONDARY DATA ON THE COMMUNITY FROM SOURCES LIKE U.S. CENSUS DATA, THE DARTMOUTH ATLAS OF HEALTHCARE, AND THE CENTERS FOR MEDICARE AND MEDICAID SERVICES. |
| Schedule H, Part V, Section B, Line 6a Facility A, 1 | Facility A, 1 - FACILITY REPORTING GROUP A. RML SPECIALTY HOSPITAL (HINSDALE) CONDUCTED THEIR CHNA WITH RML HOSPITAL (CHICAGO). RML SPECIALTY HOSPITAL (CHICAGO) CONDUCTED THEIR CHNA WITH RML HOSPITAL (HINSDALE). |
| Schedule H, Part V, Section B, Line 11 Facility A, 1 | Facility A, 1 - FACILITY REPORTING GROUP A. IN THE SPRING OF 2022, RML CONDUCTED ITS FOURTH COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA). RML RECOGNIZES THAT AS THE FIRST STOP IN THE POST-ACUTE CARE CONTINUUM, IT IS WELL POSITIONED TO ADDRESS THE NEEDS OF CHRONICALLY, CRITICALLY ILL PATIENTS AFTER THEY LEAVE THE HOSPITAL AND RETURN TO THE COMMUNITY. FOCUSING ON THESE NEEDS PRESENTS UNIQUE OPPORTUNITIES FOR RML TO ADVANCE OUR MISSION BY ENHANCING THE CARE WE PROVIDE TO THE UNIQUE PATIENT POPULATION WE SERVE. DURING THE CHNA PROCESS, RML'S CHNA TEAM, COMPRISED OF EIGHT RML LEADERS FACILITIATED BY A CONSULTANT, CROWE, LLP (CROWE), IDENTIFIED FOUR HIGH-PRIORITY NEEDS: TRAINING AND EDUCATION FOR CAREGIVERS, POST-DISCHARGE CARE COORDINATION, POST-DISCHARGE SOCIAL SERVICES, AND POST-DISCHARGE ANSWERS TO MEDICAL QUESTIONS. HIGH PRIORITY IMPLEMENTATION: TRANING AND EDUCATION FOR CAREGIVERS: - CONDUCTED A COMPREHENSIVE REVIEW AND STANDARDIZATION OF THE DISCHARGE EDUCATION PROCESS. - ENHANCED DISCHARGE DOCUMENTATION, ENSURING IT IS NEED-BASED WITH ADDITIONAL TIME AVAILABLE AS REQUESTED BY CARE GIVERS. - EXPANDED THE USE OF MATERIALS IN CAREGIVERS' NATIVE LANGUAGES AND INCREASED ACCESS TO LANGUAGE SERVICES TO IMPROVE COMMUNICATION DURING DISCHARGE TRAINING. POST-DISCHARGE CARE-COORDINATION: - IMPLEMENTED UPDATES TO THE SOCIAL ASSESSMENT TO INCLUDE QUESTIONS ON SOCIAL DETERMINANTS OF HEALTH (SDOH), SUPPORTED BY A SCORING TOOL. THIS TOOL PROMPTS CARE COORDINATORS TO PROVIDE REFERRALS WHEN ADDITIONAL ASSISTANCE IS NEEDED. POST-DISCHARGE ANSWERS TO MEDICAL QUESTIONS: - UPDATED OUR DISCHARGE FOLLOW-UP TOOL WITHIN MEDITECH EXPANSE TO INCLUDE A DISCHARGE EDUCATION RATING SYSTEM (1-4 SCALE, WITH 4 BEING THE HIGHEST). FOLLOW-UP CALLS WERE MADE TO PATIENTS DISCHARGED TO HOME, YIELDING AN AVERAGE SCORE OF 3.75 OUT OF 4 AT HINSDALE AND 3.9 OUT OF 4 AT CHICAGO, WITH 42 OUT OF 44 DISCHARGED PATIENTS PARTICIPATING. TRAINING FOR CARE COORDINATION TEAMS: - CARE COORDINATION TEAMS UNDERWENT MULTIPLE TRAINING SESSIONS FOCUSING ON DIVERSITY, EQUITY, AND INCLUSION (DEI) AND THE USE OF NEW SDOH TOOLS. - PROVIDED TEAMS WITH AN EXTENSIVE DATABASE OF RESOURCES TO FACILITATE REFERRALS AS NEEDED. ADDITIONAL PROGRESS SUMMARY: - DOCUMENTATION STANDARDIZATION: IMPROVED CAREGIVER SATISFACTION AND COMMUNICATION THROUGH TAILORED AND LANGUAGE-ACCESSIBLE DISCHARGE TRAINING. - PATIENT ASSESSMENTS: ENHANCED SOCIAL ASSESSMENTS HAVE PROVEN EFFECTIVE, TRIGGERING APPROPRIATE INTERVENTIONS AND ALIGNING WITH NATIONAL PATIENT SAFETY GOALS. - DISCHARGE FOLLOW-UP AND FEEDBACK: HIGH AVERAGE SATISFACTION RATINGS DEMONSTRATE EFFECTIVE EDUCATION DELIVERY AND PATIENT READINESS POST-DISCHARGE. |

| Schedule H, Part I FINANCIAL ASSISTANCE & COMMUNITY BENEFIT SUPPORTING STATEMENT | RML IS LICENSED BY THE STATE OF ILLINOIS AS AN ACUTE CARE HOSPITAL. MEDICARE HAS DESIGNATED RML AS A LONG-TERM ACUTE CARE HOSPITAL (LTCH) BECAUSE IT HAS A LENGTH OF STAY GREATER THAN 25 DAYS. LTCHS ARE DESIGNED FOR PATIENTS WHO NEED INTENSE, EXTENDED CARE FOLLOWING TREATMENT AT A TRADITIONAL ACUTE CARE FACILITY. THESE PATIENTS USUALLY HAVE MEDICALLY COMPLEX OR MULTIPLE MEDICAL CONDITIONS AND GENERALLY REQUIRE EXTENDED HOSPITALIZATION. OVER 90% OF RML'S PATIENTS HAVE SPENT AT LEAST THREE DAYS IN THE INTENSIVE CARE UNITS OF APPROXIMATELY 90 ACUTE CARE FACILITIES LOCATED THROUGHOUT THE STATE OF ILLINOIS. RML SPECIALTY HOSPITAL PRIMARILY FOCUSES ON TREATING AND WEANING PATIENTS DEPENDENT ON MECHANICAL VENTILATORS, AND TODAY IS ONE OF THE LARGEST VENTILATOR WEANING HOSPITALS IN THE NATION. WITH LOCATIONS IN HINSDALE AND CHICAGO, RML SPECIALTY HOSPITAL HAS RECEIVED MULTIPLE AWARDS AND RECOGNITION FROM THE NATIONAL ASSOCIATION OF LONG-TERM CARE HOSPITALS (NALTH) FOR ITS INNOVATIVE METHODOLOGIES IN TREATING PATIENTS WITH COMPLEX MEDICAL CONDITIONS. RML IS IN COMPLIANCE WITH THE EMERGENCY MEDICAL TREATMENT AND ACTIVE LABOR ACT (EMTALA), HOWEVER PROVIDING EMERGENCY MEDICAL TREATMENT IS NOT THEIR PRIMARY PURPOSE. OVER 80% OF RML'S REVENUE IS GENERATED BY SERVING MEDICARE/MEDICAID PATIENTS. THE AVERAGE AGE OF AN RML PATIENT IS 61 AND THE AVERAGE LENGTH OF STAY IS 46 DAYS. RML OFFERS FINANCIAL ASSISTANCE TO PATIENTS AND HELPS THEM QUALIFY FOR GOVERNMENT ASSISTANCE PROGRAMS LIKE MEDICAID. DURING THE TAX YEAR, RML WAS 100% SUCCESSFUL IN SECURING ASSISTANCE FOR ALL OF ITS UNINSURED PATIENTS. |
| Schedule H, Part I, Line 7g Subsidized Health Services | THE HOSPITAL DOES NOT OPERATE ANY PHYSICIAN CLINICS. THEREFORE, NO COSTS REPORTED ON THIS LINE ARE ATTRIBUTABLE TO A PHYSICIAN CLINIC. |
| Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistance | MEDICAID COSTS ARE TAKEN DIRECTLY FROM RML'S FY24 FILED MEDICAID COST REPORT. |
| Schedule H, Part II Community Building Activities | RML HAS ESTABLISHED A NUMBER OF PARTNERSHIPS TO HELP SUPPORT THE COMMUNITY. RML HAS WORKED CLOSELY WITH THE EMERGENCY RESPONSE NETWORKS IN CHICAGO AND THE WESTERN SUBURBS TO IMPROVE THESE COMMUNITIES' DISASTER PREPAREDNESS. |
| Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount | BAD DEBT REPORTED IN PART III, LINE 2, IS REPORTED AT NET REALIZABLE VALUE; WHICH IS THE SAME METHODOLOGY USED TO REPORT BAD DEBT IN THE AUDITED FINANCIAL STATEMENTS. |
| Schedule H, Part III, Line 3 Bad Debt Expense Methodology | THE HOSPITAL IS UNABLE TO ESTIMATE ACCURATELY THE AMOUNT OF BAD DEBT EXPENSE ATTRIBUTABLE TO PATIENTS ELIGIBLE FOR FREE SERVICES UNDER THE FINANCIAL ASSISTANCE POLICY. ALTHOUGH A PORTION OF BAD DEBT EXPENSES MAY RELATE TO PATIENTS WHO WOULD QUALIFY FOR CHARITY CARE, A REPORTABLE FIGURE CANNOT BE REASONABLY ESTIMATED. |
| Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote | RML'S AUDITED FINANCIAL STATEMENTS CONTAIN AN "ACCOUNTS RECEIVABLE" FOOTNOTE. THE FOOTNOTE IS LOCATED ON PAGE 10 OF THE ATTACHED FINANCIAL STATEMENTS. |
| Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs | THE ENTIRE SHORTFALL SHOULD BE TREATED AS COMMUNITY BENEFIT. CARING FOR MEDICARE PATIENTS FULFILLS A COMMUNITY NEED AND RELIEVES A GOVERNMENT BURDEN. THE SOURCE OF THIS INFORMATION IS RML'S FILED FY24 MEDICARE COST REPORT. |
| Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance | RML WILL NOT PURSUE LEGAL ACTION FOR NON-PAYMENT OF A HOSPITAL BILL AGAINST UNINSURED PATIENTS WHO HAVE CLEARLY DEMONSTRATED THAT THEY HAVE NEITHER SUFFICIENT INCOME NOR ASSETS TO MEET THEIR FINANCIAL OBLIGATIONS, PROVIDED THE PATIENT: 1. ACTS REASONABLY AND COOPERATES IN GOOD FAITH WITH RML. 2. PROVIDES RML WITH ALL REASONABLY REQUESTED FINANCIAL AND OTHER DOCUMENTATION NEEDED TO DETERMINE PATIENT ELIGIBILITY FOR FINANCIAL ASSISTANCE AND REASONABLE PAYMENT PLAN OPTIONS. 3. PROVIDES INFORMATION WITHIN 30 DAYS OF THE HOSPITAL'S REQUEST. 4. COMMUNICATES TO THE HOSPITAL ANY MATERIAL CHANGES IN FINANCIAL SITUATION THAT MAY AFFECT THE PATIENT'S ABILITY TO COMPLY WITH PAYMENT PLANS OR QUALIFY FOR HOSPITAL FINANCIAL ASSISTANCE WITHIN 30 DAYS OF SUCH CHANGE. |

| Schedule H, Part VI, Line 2 Needs assessment | RML WAS ESTABLISHED IN 1996 AS A PARTNERSHIP BETWEEN TAX EXEMPT ACUTE CARE HOSPITALS. PRESENTLY, ADVOCATE HEALTH CARE AND LOYOLA UNIVERSITY MEDICAL CENTER ARE THE LIMITED PARTNERS AND RMLHP CORPORATION THE GENERAL PARTNER, ALL 501(C)3 ORGANIZATIONS. WITH QUALITY AS ITS PRIMARY FOCUS, RML WAS ESTABLISHED TO PROVIDE INTENSE, EXTENDED CARE TO THOSE PATIENTS WITH MEDICALLY COMPLEX OR MULTIPLE MEDICAL CONDITIONS. THE AVERAGE AGE OF AN RML PATIENT IS 61 YEARS AND THE AVERAGE LENGTH OF STAY IS 46 DAYS. RML PATIENTS ARE DIRECT TRANSFERS FROM THE INTENSIVE CARE UNITS OF APPROXIMATELY 90 ACUTE CARE FACILITIES THROUGHOUT ILLINOIS. NEARLY ONE-THIRD OF THE TRANSFERS ARE FROM RML'S PARTNER HOSPITALS -- ADVOCATE AND LOYOLA. TO ENSURE THE FURTHERANCE OF THE PARTNERS' EXEMPT PURPOSE, THE GOVERNING BODY IS APPOINTED BY THE EXEMPT PARTNERS. RML IS A LONG-TERM ACUTE CARE HOSPITAL (LTCH). THE DEFINITION OF "COMMUNITY" TO AN LTCH IS DIFFERENT THAN THAT OF A TRADITIONAL ACUTE CARE HOSPITAL. RML'S PURPOSE IS TO SERVE MEDICALLY COMPLEX PATIENTS REQUIRING EXTENDED CARE. RML'S "COMMUNITY" IS DEFINED IN OUR COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) AS PEOPLE IN CHICAGO, SUBURBAN COOK COUNTY, DUPAGE COUNTY AND WILL COUNTY WHO HAVE SUFFERED A SEVERE, LIFE-CHANGING, DEBILITATING ILLNESS REQUIRING EXTENSIVE PSYCHO-SOCIAL AND HEALTH SUPPORT SERVICES WHEN THEY RETURN HOME. AS THE ELDERLY AND LOW INCOME ARE LEAST LIKELY TO HAVE THE RESOURCES TO ADAPT WELL TO THESE CIRCUMSTANCES, WE PAY SPECIAL ATTENTION TO THESE POPULATIONS. RML ASSESSES THE NEEDS OF ITS COMMUNITY THROUGH CONSTANT DISCUSSIONS AND COLLABORATIONS WITH ITS REFERRAL HOSPITALS AND ITS DISCHARGE LOCATIONS. RML SUPPORTS A STAFF OF SKILLED HEALTHCARE PRACTITIONERS WHO VISIT THE REFERRAL HOSPITALS TO EVALUATE THE COMPLEX MEDICAL CONDITIONS PRIOR TO TRANSFER TO RML. IN ADDITION, RML STAFF DISCUSS THE NEEDS AND OUTCOMES OF DISCHARGED PATIENTS WITH PATIENTS AND CAREGIVERS AT DISCHARGE LOCATIONS. IN THIS WAY, RML PRACTITIONERS ARE ASSESSING THE NEEDS OF ITS COMMUNITY AND RML'S ROLE IN A PATIENT'S CONTINUUM OF CARE. THESE SAME HEALTHCARE PRACTITIONERS COLLABORATE WITH THE REFERRAL HOSPITALS AND DISCHARGE LOCATIONS IN IDENTIFYING AND ANTICIPATING THE NEEDS OF OTHER MEDICAL CONDITIONS. IF THE PRACTITIONERS IDENTIFY A NEED IN THE COMMUNITY, THIS NEED IS SHARED WITH RML SENIOR LEADERSHIP TO DETERMINE RML'S CONTINUING ROLE. RML ALSO COLLECTS DATA FROM OUR PATIENTS AND CAREGIVERS VIA OUR SOCIAL AND DISCHARGE FOLLOW UP ASSESSMENTS. DATA COLLECTED FROM THOSE ASSESSMENTS ARE USED TO REFER PATIENTS IN NEED TO DEDICATED RESOURCES FOR ASSISTANCE WHEN NEEDED. |
| Schedule H, Part VI, Line 3 Patient education of eligibility for assistance | THE AVAILABILITY OF THE HOSPITAL FINANCIAL ASSISTANCE AS DEFINED UNDER RML'S POLICY IS COMMUNICATED TO PATIENTS THROUGH THE POSTING OF SIGNS IN AREAS OF THE HOSPITAL COMMONLY UTILIZED FOR ADMISSION AND REGISTRATION OF PATIENTS WITH THE FOLLOWING NOTICE: "YOU MAY BE ELIGIBLE FOR FINANCIAL ASSISTANCE UNDER THE TERMS AND CONDITIONS THE HOSPITAL OFFERS TO QUALIFIED PATIENTS. FOR MORE INFORMATION, PLEASE CONTACT EITHER THE DIRECTOR OF PATIENT FINANCIAL SERVICES OR THE ADMITTING MANAGER." THE SIGNAGE CONTAINS A PHONE NUMBER AND RML'S WEBSITE. THE SAME INFORMATION IS ALSO INCLUDED ON BILLING STATEMENTS AND ON RML'S WEBSITE, WITH THE WEBSITE ALSO CONTAINING A LINK TO THE FINANCIAL ASSISTANCE POLICY AND THE FINANCIAL ASSISTANCE APPLICATION. BECAUSE 100% OF RML'S PATIENTS COME DIRECTLY FROM OTHER HOSPITALS AND UNDERGO A ROBUST ADVANCE INTAKE PROCESS BEFORE THEY CAN BE TRANSFERRED TO RML, RML ALSO PUBLICIZES THE AVAILABILITY OF FINANCIAL ASSISTANCE DURING THE ADVANCE EVALUATION AND INTAKE PROCESS (AND DIRECTLY WITH THE PATIENT/FAMILY IF THE NEED SUBSEQUENTLY ARISES AFTER THEIR ADMISSION TO RML). UPON PATIENT REFERRAL, IF A PATIENT HAS NO INSURANCE, THE SOCIAL WORKER FROM THE REFERRING ACUTE CARE HOSPITAL WILL REQUEST ON BEHALF OF THE PATIENT FINANCIAL ASSISTANCE FROM RML WHICH IN TURN RML'S INTAKE PERSONNEL PROVIDE THE CHARITY APPLICATION TO THE SOCIAL WORKER WHO HAS THE FAMILY COMPLETE IT ON BEHALF OF THE PATIENT AND FORWARD IT BACK TO RML. |
| Schedule H, Part VI, Line 4 Community information | AT RML WE SERVE A UNIQUE COMMUNITY COMPOSED OF APPROXIMATELY 90 REFERRAL SOURCES THROUGHOUT ILLINOIS, OUR PARTNER ORGANIZATIONS, AND THE COMMUNITIES IMMEDIATELY SURROUNDING OUR TWO FACILITIES. RML PLAYS A UNIQUE AND INTEGRAL ROLE IN THE CONTINUUM OF CARE. RML PROVIDES TREATMENT TO THOSE PATIENTS WITH CATASTROPHIC, ACUTE ILLNESSES; INJURIES COMPLICATED BY MULTIPLE COMORBIDITIES; AND THOSE PATIENTS THAT REQUIRE AGGRESSIVE, SPECIALIZED INTERDISCIPLINARY CARE. OUR PATIENT POPULATION IS LARGELY GERIATRIC WITH HIGH ACUITY AND REQUIRES PROLONGED LENGTHS OF STAY IN AN ACUTE SETTING. MEDICAID IS THE PRIMARY INSURER FOR OVER 30% OF RML'S POPULATION. WE RECOGNIZE THAT THIS COMPLICATES DISCHARGE PLANNING AND WE HAVE ATTEMPTED TO ADDRESS THIS THROUGH SEVERAL OF THE INITIATIVES IDENTIFIED IN OUR CHNA IMPLEMENTATION PLAN. |
| Schedule H, Part VI, Line 5 Promotion of community health | A MAJORITY OF RML'S GOVERNING BODY IS COMPRISED OF PERSONS WHO RESIDE IN THE HOSPITAL'S PRIMARY SERVICE AREA WHO ARE NEITHER EMPLOYEE NOR INDEPENDENT CONTRACTORS OF RML, NOR FAMILY MEMBERS THEREOF. RML HAS AN OPEN MEDICAL STAFF, MEANING ANY QUALIFIED PHYSICIAN CAN APPLY FOR PRIVILEGES TO PRACTICE AT RML, AS LONG AS THEY MEET RML'S CREDENTIALING REQUIREMENTS AND SPECIFIC EXPERTISE NEEDED FOR THE TREATMENT OF CHRONICALLY CRITICALLY ILL PATIENTS. RML APPLIES SURPLUS FUNDS TO IMPROVEMENTS IN FACILITIES AND EQUIPMENT, PATIENT CARE, MEDICAL TRAINING, EDUCATION, AND RESEARCH. RML IS COMMITTED TO THE ADVANCEMENT OF MEDICAL CARE FOR LONG-TERM ACUTE CARE PATIENTS AND THE ACHIEVEMENT OF SUPERIOR CLINICAL OUTCOMES. WE BELIEVE IT IS IMPERATIVE TO CONDUCT CLINICAL RESEARCH TO REALIZE OUR VISION OF BECOMING A NATIONAL CENTER FOR EXCELLENCE IN LONG-TERM ACUTE CARE, RECOGNIZED FOR SUPERIOR CLINICAL OUTCOMES AND PATIENT SATISFACTION, AND FOR VALUED CONTRIBUTIONS TO THE ADVANCEMENT OF MEDICAL CARE. RML OVERSEES AND PARTICIPATES IN NUMEROUS CLINICAL RESEARCH STUDIES FOCUSED ON THE TREATMENT AND CARE OF VENTILATED PATIENTS, PATIENTS WITH INFECTIOUS DISEASES AND MEDICALLY COMPLEX CONDITIONS. RML SUPPORTS CLINICAL RESEARCH STUDIES BY ALLOWING THE USE OF ITS FACILITIES FOR PATIENT TRIALS AND PROVIDING ADMINISTRATIVE ASSISTANCE TO RESEARCHERS. RESEARCH EFFORTS DURING FY24 INCLUDED 12 PROJECTS AT RML INCLUDING FIVE SPONSORED PROJECTS, WHICH CONSIST OF MULTI-CENTER AND DEVICE TRIALS AS WELL AS PILOT STUDIES, THREE COLLABORATIVE STUDIES INVOLVING SURVEILLANCE OF MULTI-DRUG-RESISTANT ORGANISMS AND 2 INTERNAL INITIATIVES ASSOCIATED WITH MIST THERAPY AND WEANING. ACROSS THE COUNTRY AND IN THE STATE OF ILLINOIS, THE HEALTH CARE INDUSTRY IS FACING STARTLING SHORTAGES OF QUALIFIED HEALTH PROFESSIONALS. TO ADDRESS THIS NEED, RML IS COMMITTED TO PROVIDING OPPORTUNITIES FOR STUDENTS AND HEALTH PROFESSIONALS TO GAIN HANDS-ON CLINICAL EXPERIENCE. EACH YEAR, RML PROUDLY WELCOMES STUDENTS AND HEALTH PROFESSIONALS IN THE FIELDS OF MEDICINE, NURSING, RESPIRATORY THERAPY, RADIOLOGY TECHNOLOGY, REHABILITATION THERAPY, AND OTHER DISCIPLINES TO OUR FACILITIES TO COLLABORATE WITH OUR STAFF AND ADVANCE THEIR SKILLS THROUGH PRACTICE AND MENTORSHIP. DURING FY24, RML WELCOMED OVER 200 STUDENTS TO STUDY IN ITS UNIQUE LONG-TERM ACUTE CARE ENVIRONMENT. RML ROUTINELY MAKES CASH AND IN-KIND DONATIONS TO OTHER NON-PROFIT ORGANIZATIONS ACTIVELY INVOLVED IN IMPROVING THE HEALTH OF THE COMMUNITY. RML'S LARGEST RECIPIENT DURING THE PAST YEAR WAS THE AMERICAN LUNG ASSOCIATION (ALA). IN ADDITION TO A DIRECT GIFT, RML STAFF ALSO PARITICIPATED IN FUND-RAISING FOR THE ALA THROUGH THEIR ANNUAL CLIMB-THE-TOWER EVENT. RML IS CONTINUOUSLY WORKING TO IDENTIFY MORE EFFICIENT USES OF RESOURCES, PROVIDE GREATER CONTINUITY AND COORDINATION OF CARE, AND IMPROVE HEALTH OUTCOMES FOR THE MOST CRITICALLY ILL AND INJURED PATIENTS. |

| Schedule J, Part I, Line 1a Tax indemnification and gross-up payments | Per the CEO's employment contract, he is to receive 8% of the compensation as deferred compensation. The only deferred compensation vehicle available is the 457(b) Plan. Eight percent of compensation exceeds the statutory contribution limit into the 457(b) Plan. Therefore, the Board annually takes action to provide the excess as cash to the CEO, grossed up to simulate receiving it as deferred compensation. |
| Schedule J, Part I, Line 4b Supplemental nonqualified retirement plan | The RML Health Providers, Limited Partnership ("RML") maintains a Supplemental Executive Retirement Plan that is a nonqualified deferred compensation plan subject to the provisions of Section 409A and 457(f) of the Internal Revenue Code of 1986, as amended. The plan is designed to provide benefits which, when combined with other retirement benefits, are reasonable and competitive for covered Executives. Eligibility for participation in the Plan is limited to a select group of management or highly compensated employees as determined by the Governing Body. Current participants, as designated by the Governing Body, and respective amounts are: James Prister, President/CEO - $44,382. Thomas Pater, Vice President Finance/CFO - $13,562. The distributed amounts are as follows: James Prister, President/CEO - $50,528. Thomas Pater, Vice President Finance/CFO - $13,518. Kenneth Pawola, Vice President/Chief Operating Officer-$11,631 |
| Schedule J, Part I, Line 7 Non-fixed payments | The executive Incentive Plan provides variable compensation based on the achievement of organization-wide goals. These goals represent key indications of success in the areas of finance, clinical quality, patient satisfaction and employee safety. They are approved by the Board for each fiscal year. |
