SCHEDULE H, PART V, SECTION B, LINE 11St. Peter's Health is committed to making a positive impact on the health of the community it serves. One of the many strategies we use to do this is by providing for the unique health needs of area residents by utilizing the Community Health Needs Assessment and resulting Community Health Improvement Planning & Implementation Strategy process. Community Health Improvement Planning and Implementation Strategy Process The goals of these processes are: To improve residents' health status, increase their life spans, and elevate their overall quality of life. A healthy community is not only one where its residents suffer little from physical and mental illness, but also one where its residents enjoy a high quality of life. To reduce the health disparities among residents. By gathering demographic information along with health status and behavior data, it will be possible to identify population segments that are most at-risk for various diseases and injuries. Intervention plans aimed at targeting these individuals may then be developed to combat some of the socioeconomic factors which have historically had a negative impact on residents' health. To increase accessibility to preventive services for all community residents. More accessible preventive services will prove beneficial in accomplishing the first goal (improving health status, increasing life spans, and elevating the quality of life), as well as lowering the costs associated with caring for late-stage diseases resulting from a lack of preventive care. A Two-Part Process: - first, identify the health needs facing our community (conduct a Community Health Needs Assessment, or CHNA); every three years and partner with a third party vendor, Professional Research Consultants, Inc., of Omaha, Nebraska, to complete a Community Health Needs Assessment (CHNA). This CHNA included a random-sample telephone survey of community residents on their health conditions, behaviors, preferences; it also included an online "key informant" survey targeted to individuals with a broad interest in the health of the community. St. Peter's partnered with the Lewis & Clark Public Health to plan and develop the survey instruments. Second, the St. Peter's Health Population Health Team reviews the CHNA to understand the identified community needs to develop a measurable plan (an Implementation Strategy) that focuses on the areas of greatest needs, taking into account the total available resources to effectively address those issues. The Implementation Strategy creates a strategic plan to focus energy, resources, policies, projects, and programs that will be most effective in improving the health of our community. After a series of strategic planning meetings, the draft Implementation Strategy is presented to the St. Peter's Health (SPH) Executive Leadership Team (ELT), where the organization's leaders review the plan to address the identified needs within the St Peter's Health Service Area. Once the Implementation Strategy is reviewed through ELT, it is presented, discussed, and approved at the St. Peter's Health System Board and published on the SPH Website. In the 2021 CHNA, the following health needs were identified as priority areas and during Fiscal Year 2022 (FY22) the following actions steps were taken to address the needs: 1) Mental health a) Health Promotion: i) Suicide Prevention and Education: During September, National Suicide Prevention Month, St. Peter's Health, community organizations and people from across our community once again worked together to host free activities, training opportunities and more to support people affected by suicide and raise awareness. SPH contributed to the educational offerings and supportive activities of this month through their annual "We Speak Mental Health Week" outlined below. (1) TAKE ACTION with us: St. Peter's team circulated information on ways you can take action and assess the Mental Health of Yourself and those Around You 1. Check in with yourself 2. Check in with a coworker and a loved one 3. If needed, check in with your provider. (2) RAISE AWARENESS with us: St. Peter's Population Health Team partnered with a local coffee shop to share information about mental health and support services at SPH and in our larger community. (3) Ask the BHU: Our SPH Behavioral Health Unit team hosted a virtual presentation and question/answer session on Sept 24th giving an overview of the unit, sharing pictures of the space, and answering questions about their work. (4) LEARN with us: St. Peter's partnered with community organizations to co-host a number of FREE mental health trainings in the month of September (a) Question, Persuade, Refer (QPR) Suicide Prevention Training: September 216 (b) Youth Mental Health First Aid: September 22nd (5) WALK with us: St. Peter's sponsored a team and provided interested employees the opportunity to take part in the Walk to End Alzheimer's which raises funds surrounding Alzheimer's disease and other dementias. The National Alliance on Mental Illness is hosting its Annual NAMI Walk to raise awareness around mental illness the walk took place on Sunday, September 19. ii) Standardized Process QPR Education for New Employee Orientation: In late FY21 SPH began including QPR (Question, Persuade, Refer) Suicide Prevention as required education for all new employees. We have maintained and standardized this process throughout FY22 and have provided education to 499 new SPH staff members. iii) Chaplain Services and Therapy Dog Program: In FY22 SPH continued to include 2 chaplains and therapy dogs as support for patients and community members. In FY22 the dogs were able to serve at numerous mass vaccination clinics in the community offering support and warmth to children and adults alike. iv) Community Involvement/Giving: St. Peter's Health is an active member of several community coalitions and groups focused on the mental health and wellbeing of the community as well as those specifically focused on suicide prevention. St. Peter's Health leadership is represented in the Lewis and Clark County Suicide Prevention Coalition, the Lewis and Clark County Mental Health Local Advisory Council, Mental and Social Wellbeing Workgroup, the LOSS team (Local Outreach to Suicide Survivor) of Lewis and Clark County Area, Lewis and Clark County Behavioral Health System Leadership Team, Elevate MT Helena Affiliate, and the Maternal Mental Health Task Force. Quarterly classes were provided which provided education to 57 community members in FY22. b) Universal Screening/Services i) Screening for Depression: In FY22 St. Peter's Health continued to focus on increasing the completion rate for depression screening (PHQ-9) at least annually for all primary care patients ages 12+ including the use of evidence-based tools to screen perinatal patients using the Edinburgh Postnatal Depression Scale. During FY22 SPH completed 96,606 PHQ9 screenings, and 808 Edinburgh screenings. This represents a 163% increase from FY21. ii) Screening for Anxiety: In FY22 St. Peter's Health continued to focus on increasing the completion rate for anxiety screening (GAD-7) for patients when clinically indicated. During FY22 SPH completed 4,206 GAD-7 screenings. If depression, suicidal ideation or anxiety is identified during the screen, support and care provided by the primary care providers is also augmented by the Behavioral Health Professionals integrated into primary care for timely intervention and follow up (see Integrated Behavioral Health for more information). c) Enhanced Support Services: i) Integrated Behavioral Health within Primary Care: During FY22 St. Peter's Health has maintained an Integrated Behavioral Health (IBH) Team practicing within the Behaviorist Model and the SPH Team has begun to expand access to SPH patients through the implementation of the Collaborative Care Model. Patients are able to access the support for any Behavioral Health Professionals via virtual and telephonic visits in addition to face-to-face visits. (1) Behaviorist Model: The BHPs at St. Peter's Health Medical Group are employed licensure candidates as well as fully Licensed Clinical Social Workers (LCSWs) and Licensed Clinical Professional Counselors (LCPCs), who work with the identified patien
