Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - THE CHRIST HOSPITAL. The Christ Hospital conducted a Community Health Needs Assessment (CHNA) in Fiscal Year 2022 (July 1, 2021 - June 30, 2022) and sought input from a variety of community partners to gain valuable insight into the overall health and well-being of the community we serve. One of our primary partners in this endeavor was The Health Collaborative (THC), a nonprofit organization that works with its member hospitals on healthcare improvement projects and shares best practices. The Health Collaborative brought local hospitals together and led the effort to better understand the health needs of the community. Their community report shares data for the Southwest Ohio region (including Greater Dayton and Greater Cincinnati, which include Northern Kentucky and Southeastern Indiana). Service areas of hospitals vary, so this approach provides the most thorough picture of health needs locally and regionally. The Fiscal Year 2022 CHNA revealed the most prevalent health conditions in our community and conditions for which people most commonly did not receive treatment in past year, the social determinants of health (SDOH) that impact these poor health outcomes, and the systemic barriers that influence health disparities and inequities for our community members. Because physical, environmental, and behavioral factors greatly impact health conditions, this CHNA focused on the SDOH and the underlying structural barriers influencing the SDOH that impact the health of community members. The priorities were determined by the number of votes in the community meetings, the number of mentions on surveys, and areas where data demonstrated performance worse than state or national benchmarks or trending in the wrong direction. The prevalent health concerns identified in the Hamilton County area through this needs assessment include: * Access to Care and Services * Health Related Social Needs * Workforce Pipeline and Diversity Through a collaborative and meticulous process, The Christ Hospital and its community partners reviewed the most prevalent health concerns in Hamilton County and established a course of action as outlined below. The Implementation Plan focuses on programs, research, and education geared to target the prevalent health concerns facing the residents of Hamilton County. It is The Christ Hospital's goal to make a measurable impact on the health of our community. We recognize that community health needs are broad and comprehensive. While we bring our expertise and leadership to addressing these needs, we also understand that one entity's expertise cannot be broad enough to be the sole answer. Collaborations and partnerships are key to meeting those needs, and we partner with community organizations that complement our resources. The Implementation Plan was approved by The Christ Hospital Board of Directors on 10/19/2022. Access to Care and Services Access to Care and Services was highlighted as one of the top needs in Hamilton County. Improving access to care and services is an important step in to improve equitable outcomes for the region's top health needs. While our CHNA has outlined separate needs related to cardiovascular health, mental health services, substance abuse, and infant mortality, we believe each of these needs can be addressed through improved access to healthcare services. We envision a community where everyone in the region has access to healthcare when they need it, including services for behavioral health, heart disease and maternal health. Our implementation plan focuses on four main goals: (1) decreasing preventable emergency department visits for mental health and heart disease issues; (2) increasing access to behavioral health heart disease, and maternal health; (3) establishing a hospital-based Outpatient Behavioral Health Program, including substance abuse treatment; and (4) eliminating barriers to access and increase use of preventative services for behavioral health, heart disease, and maternal health. In order to achieve these goals, we will stand up a communicate cardiovascular clinic in partnership with St. Vincent DePaul to reduce the number of recurring heart related appointments and relatedly reduce the number of deaths from heart disease in the community. Additionally, in order to further address concerns surrounding infant mortality, we will become "Mama Certified," a program designed to address racial disparities in infant and maternal mortality in the community. Health-Related Social Needs Health-Related Social Needs addresses access to and use of resources for food and housing, with a focus on the development and strengthening of partnerships between providers and Community-Based Organizations. We envision a community where everyone in the region has access to food and stable housing. Our implementation plan focuses on four main goals: (1) increase coordination between healthcare systems and social service agencies to screen, refer, and follow up on patients' health-related social needs; (2) decrease hospital readmissions for health-related social needs; (3) improve coordination between healthcare systems and social service agencies by establishing a shared mechanism to screen, refer, and follow-up on patients' health-related social needs (i.e., housing, food insecurity); and (4) increase patient screenings in primary care offices to understand a patients Social Determinants of Health (SDOH). In order to achieve these goals, we will stand up a formal SDOH screening program in partnership with Aunt Bertha to increase capture of SDOH for primary care patients. Additionally, we will expand access to and referrals to Community-Based Organizations to help stabilize a patient's socially determined circumstances. In our efforts, we will prioritize the people and places experiencing significant health disparities and inequities (minority community members (Black, Asian American, Indian Native), military (active and veteran), LGBTQ+ individuals, under-insured and uninsured). Workforce Pipeline and Diversity Workforce Pipeline and Diversity addresses the healthcare education pipeline and encourages a workforce that is strong, reflects the diversity of our region, and delivers equitable care to everyone. We envision a community where everyone in the region has the possibility, potential and access to stable health care jobs. Our implementation plan focuses on four main goals: (1) increase the size of the healthcare education pipeline, (2) increase the number of racially and ethnically diverse students in the healthcare education pipeline, (3) reduce vacancy rates for key healthcare positions (i.e., physicians, nurses, clinical staff, management), (4) increase healthcare workforce diversity for key positions. In order to achieve these goals, we will stand up formal training program and pipeline with an established Community Based Organization to reduce vacancy rates for key entry level roles (i.e., Medical Assistants and Phlebotomists); establish a formal interviewing process that includes a diverse candidate pool and panel interview for management and above roles; provide incumbent worker training program opportunities, apprenticeships, and scholarships to assist employees in advancing education and careers in healthcare; and career exploration and work-based learning can include training experiences like an internship required for a credential or entry into an occupation, a clinical experience, or other paid or non-paid work experience.

Schedule H, Part V, Section B, Line 1 PART V, SECTION A, LINE 1THE STATE OF OHIO DOES NOT LICENSE HOSPITALS, BUT THE OHIO DEPARTMENT OF HEALTH DOES REGISTER HOSPITALS. AS PART OF THIS REGISTRATION PROCESS, HOSPITALS IN THE STATE OF OHIO ARE REQUIRED TO COMPLETE AND SUBMIT THE ANNUAL HOSPITAL REGISTRATION AND PLANNING REPORT (AHR) EACH CALENDAR YEAR.
Schedule H, Part V, Section B, Line 22 PART V, SECTION B, LINE 22DINDIVIDUALS ELIGIBLE FOR FINANCIAL ASSISTANCE - HOSPITAL DETERMINATION OF THE MAXIMUM AMOUNTS THAT CAN BE CHARGED TO FAP-ELIGIBLE INDIVIDUALS FOR EMERGENCY OR MEDICALLY NECESSARY CARE. TCH MAINTAINS A FINANCIAL ASSISTANCE PROGRAM POLICY THAT OUTLINES THE MAXIMUM AMOUNT THAT CAN BE CHARGED TO FINANCIAL ASSISTANCE PROGRAM ELIGIBLE INDIVIDUALS FOR EMERGENCY OR OTHER MEDICALLY NECESSARY CARE. THE AMOUNT CHARGED TO FINANCIAL ASSISTANCE PROGRAM ELIGIBLE INDIVIDUALS FOR EMERGENCY OR MEDICALLY NECESSARY CARE IS REPRESENTATIVE OF AN ANNUAL COMPUTATION OF AMOUNTS GENERALLY BILLED (AGB) TO INDIVIDUALS WHO HAVE INSURANCE COVERING SUCH CARE. TCH PROVIDES FURTHER ADJUSTMENTS TO AMOUNTS CHARGED BASED ON A VARIETY OF FACTORS, INCLUDING PATIENT INCOME INFORMATION AND OTHER SOURCES WHEN INADEQUATE INFORMATION IS PROVIDED BY THE PATIENT TO QUALIFY UNDER TCH'S CHARITY PROGRAMS. TCH WORKS WITH ALL PATIENTS BASED ON THEIR INDIVIDUAL SITUATION TO ENSURE OUR MISSION IS BEING CARRIED OUT WITHIN THE COMMUNITY IN WHICH WE SERVE.
Schedule H, Part I, Line 7 COLUMN FBAD DEBTS ARE NETTED AGAINST REVENUES IN PART VIII, AND AS SUCH ARE NOT INCLUDED IN THE TOTAL EXPENSES IN ORDER TO CALCULATE THE PERCENTAGE REPORTED IN COLUMN F ON SCHEDULE H, PART I AND PART II.
Schedule H, Part III SECTION BIN ADDITION TO HOSPITAL-BASED REVENUE AND ALLOWABLE COST GENERATED FROM MEDICARE (INCLUDING DSH AND IME) ELIGIBLE PARTICIPANTS REPORTED IN PART III SECTION B, THE CHRIST HOSPITAL PROVIDES HOSPITAL-BASED SERVICES TO OTHER MEDICARE ELIGIBLE PARTICIPANTS, THROUGH MEDICARE PLANS SPONSORED BY MANAGED CARE PROVIDERS (MEDICARE MANAGED CARE PLANS). MEDICARE MANAGED CARE PLANS ARE HEALTH MAINTENANCE ORGANIZATIONS (HMOS) OR PREFERRED PROVIDER ORGANIZATIONS (PPOS) THAT PROVIDE BASIC MEDICARE COVERAGE PLUS OTHER COVERAGES TO FILL GAPS IN MEDICARE COVERAGE. THESE PLANS HAVE ARRANGEMENTS WITH CERTAIN PHYSICIANS, HOSPITALS AND HEALTH CARE PROVIDERS TO SERVE PATIENTS WHO ARE PLAN MEMBERS AT A CONTRACTED RATE. BELOW IS THE SHORTFALL RELATED TO HOSPITAL-BASED REVENUE AND ALLOWABLE COSTS UNDER MEDICARE MANAGED CARE PLANS: REVENUE RECEIVED FROM MEDICARE MANAGED CARE PLANS $158,149,377 ALLOWABLE COST OF CARE RELATED TO PAYMENTS FROM ABOVE $(209,403,177) SHORTFALL FROM MEDICARE MANAGED CARE PLANS $(51,253,800) ALSO, THE CHRIST HOSPITAL HAS CERTAIN MEDICARE PART B (PHYSICIAN-BASED) REVENUE AND ALLOWABLE COST RELATED TO SERVICES RENDERED ON BEHALF OF MEDICARE ELIGIBLE PARTICIPANTS BY EMPLOYED PHYSICIANS OF THE CHRIST HOSPITAL. THIS WOULD INCLUDE SERVICES PROVIDED UNDER TRADITIONAL MEDICARE, AS WELL AS MEDICARE MANAGED CARE PLANS. BELOW IS THE SHORTFALL RELATED TO THE DISREGARDED ENTITIES OF THE HOSPITAL: REVENUE RECEIVED FROM MEDICARE PART B PLANS (PHYSICIAN-BASED) $107,207,507 ALLOWABLE COST OF CARE RELATED TO PAYMENTS FROM ABOVE $(155,983,855) SHORTFALL RELATED TO THE MEDICARE PART B PLANS (PHYSICIAN-BASED) $(48,776,348)

Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceTHE CHRIST HOSPITAL (TCH) APPLIES A COST-TO-CHARGE RATIO FROM WORKSHEET 2 OF THE FORM 990, SCHEDULE H INSTRUCTIONS TO DETERMINE FINANCIAL ASSISTANCE PROVIDED AT COST.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. PERIODICALLY THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. THE RESULTS OF THIS REVIEW ARE THEN USED TO MAKE MODIFICATIONS TO THE PROVISION FOR BAD DEBTS TO ESTABLISH AN APPROPRIATE ALLOWANCE FOR UNCOLLECTIBLE ACCOUNTS.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteIN MAY 2014, THE FINANCIAL ACCOUNTING STANDARDS BOARD (FASB) AND INTERNATIONAL ACCOUNTING STANDARDS BOARD ISSUED ACCOUNTING STANDARDS UPDATE (ASU) 2014-09, REVENUE FROM CONTRACTS WITH CUSTOMERS (TOPIC 606). TCH ADOPTED ASU 2014-09 ON JULY 1, 2018 USING A FULL RETROSPECTIVE BASIS. UPON ADOPTION, THE MAJORITY OF WHAT WAS PREVIOUSLY CLASSIFIED AS PROVISION FOR UNCOLLECTIBLE ACCOUNTS AND PRESENTED AS A REDUCTION TO PATIENT SERVICE REVENUE ON THE CONSOLIDATED STATEMENT OF OPERATIONS AND CHANGES IN NET ASSETS IS TREATED AS AN IMPLICIT PRICE CONCESSION THAT REDUCES THE TRANSACTION PRICE, WHICH IS REPORTED AS PATIENT SERVICE REVENUE. AS SUCH, BAD DEBT EXPENSE IS NOT REFERENCED IN TCH'S AUDITED FINANCIAL STATEMENTS. THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsMEDICARE SURPLUS (OR SHORTFALL) OCCURS WHEN THE FEDERAL GOVERNMENT REIMBURSES HOSPITALS IN EXCESS, OR BELOW, THEIR COSTS FOR TREATING MEDICARE PATIENTS. THE CHRIST HOSPITAL, LIKE MOST HOSPITALS AND HEALTH SYSTEMS, INCURRED A MEDICARE SHORTFALL FOR THE YEAR ENDED JUNE 30, 2022. THE SHORTFALL AMOUNT DISCLOSED IN PART III, SECTION B IS BASED ON HOSPITAL-BASED REVENUE AND ALLOWABLE COST (INCLUDING DSH AND IME) FROM TRADITIONAL MEDICARE PROGRAMS (I.E., THOSE NOT SPONSORED BY A MANAGED CARE PLAN). NON-NEGOTIABLE MEDICARE RATES FOR THE YEAR ENDED JUNE 30, 2022 WERE OUT-OF-LINE WITH THE TRUE COSTS OF CARING FOR THESE PATIENTS, DRIVING THIS SHORTFALL. BY CONTINUING TO TREAT PATIENTS ELIGIBLE FOR MEDICARE, THE CHRIST HOSPITAL IS ALLEVIATING THE FEDERAL GOVERNMENT'S BURDEN FOR DIRECTLY PROVIDING MEDICAL SERVICES. Schedule H, Part VI, Line 2 Needs assessmentOur first step in conducting our 2022 Community Health Needs Assessment (CHNA) was to define the community we serve. To do so we considered a number of relevant facts and circumstances, including the geographic area we serve, the target populations we serve, and our principal functions as a hospital, which include our service lines. In defining the community we serve, we specifically included the medically underserved, low income and the minority populations who live in the geographic area from which we draw patients. In addition, we included all patients without regard to whether (or how much) they or their insurers pay for care received or whether they are eligible for assistance under our financial assistance policy. To assess health needs and conduct a comprehensive and collaborative CHNA, we engaged The Health Collaborative (THC). THC is a nonprofit organization serving the Greater Cincinnati area and working with its member hospitals on health care improvement projects and shares best practices. This collaboration was facilitated by THC in partnership with the Greater Dayton Area Hospital Association (GDAHA) and includes partnership with 36 hospitals, 22 health departments, across 26 counties in Greater Cincinnati and the Greater Dayton Area, southeast Indiana, and northern Kentucky. The success of this CHNA is a result of the collaboration from local community champions, and strategic partners throughout the region who helped with community engagement and data collection efforts. Along with the data produced by collaboration with THC, we analyzed a variety of data sources including public health data, health risk factor surveys, socioeconomic needs assessments, environmental standards and existing programs developed for residents in the Greater Cincinnati region. In addition, we also sought to review any written comments received regarding community health needs in the previous assessment. The CHNA provides a comprehensive and detailed description of (1) how the Collaborative CHNA Team solicited and took into account input received from persons who represent the broad interests of the community, including those with special knowledge of, or expertise in, public health, (2) how and over what period of time such input was provided, (3) the names of organizations providing input and the nature and extent of the organizations input, (4) descriptions of the medically underserved, low income or minority populations being represented by organizations or individuals that provided input. Collectively, the entire process was overseen by an Advisory Committee of 41 members of the community, representing hospitals, public health departments, federally qualified health centers, community-based organizations, public health professional associations, funders, and hospital associations. Over 8,320 online community survey respondents, 859 health and social service provider respondents, in concert with over 50 targeted focused groups, provided a comprehensive, inclusive, and balanced data results.

PARTS VIII, IX AND X - IMPACT OF NOVEL CORONAVIRUS DISEASE 2019 PANDEMICIn March 2020, the World Health Organization designated the novel coronavirus disease 2019 (COVID-19) outbreak as a global pandemic. The extent of the COVID-19 pandemic's impact on TCH's operating results and financial condition have been and will continue to be driven by many factors, including, but not limited to, any future government-mandated or recommended suspensions of elective or other procedures, reduced patient volumes, increases in the number of uninsured and underinsured patients as a result of higher sustained rates of unemployment, and incremental expenses required for personnel, supplies and personal protective equipment. Because of these and other uncertainties, the duration and extent of the COVID-19 pandemic-related implications remain unknown. In response to the COVID-19 pandemic, federal and state governments passed legislation and took other administrative actions intended to assist health care providers in providing care to COVID-19 and other patients during the public health emergency. The Coronavirus Aid, Relief, and Economic Security (CARES) Act, which was enacted on March 27, 2020, authorized funding earmarked for hospitals and other health care providers. The funds are intended to compensate health care providers for lost revenues and incremental expenses incurred in response to the COVID-19 pandemic and are not required to be repaid provided the recipients attest to and comply with certain terms and conditions, including limitations on balance billing and not using the funds to reimburse expenses or losses that other sources are obligated to reimburse. Based on TCH's assessment of the underlying terms and conditions surrounding distributions received and recognized in prior periods, TCH is reasonably assured that the terms and conditions have been met and does not expect that any stimulus funds will be required to be repaid. For the years ended June 30, 2022 and 2021, TCH recognized $2.7 million and $26.8 million, respectively, in stimulus funds as part of the CARES Act, which are recorded as other revenue in the consolidated statement of operations and changes in net assets. In addition, the CARES Act provides for an expansion of the Medicare Accelerated and Advance Payment Program, whereby inpatient acute care hospitals and other eligible providers may request accelerated payment of up to 100% of their Medicare payment amount for a six-month period to be repaid through the withholding of future Medicare fee-for-service payments. Beginning one year after receipt, the Centers for Medicare & Medicaid Services (CMS) began recoupment of the accelerated and advance payments by withholding 25% of a participating provider's Medicare fee-for-service payments, which continued over 11 months. After this initial recoupment period, CMS began withholding 50% of the Medicare fee-for-service payments, which will continue for another 6 months. Once the recoupment periods are complete, any amounts that have not been offset by Medicare claims will be required to be repaid. TCH received $91.7 million in Medicare accelerated and advance payments from CMS in the year ended June 30, 2020, and recoupments began on the Medicare accelerated and advance payments in April 2021. TCH has recorded deferred revenue for Medicare services not yet rendered in the consolidated balance sheets in contract liabilities, if expected to be recouped within one year of the consolidated balance sheet date, and in long-term contract liabilities, if expected to be recouped in more than a year from the consolidated balance sheet date. As of June 30, 2022 and 2021, total contract liabilities for the Medicare accelerated and advance payments not yet recouped were $29.5 million and $82.8 million, respectively. The CARES Act also provides for deferred payment of the employer portion of social security taxes between March 27, 2020 and December 31, 2020, with 50% of the deferred amount due December 31, 2021 and the remaining 50% due December 31, 2022. TCH has deferred $7.5 million and $15.0 million in social security taxes at June 30, 2022 and 2021, respectively, which are recorded in the consolidated balance sheets in compensation and benefits, if due to be repaid within one year of the consolidated balance sheet date, and in long-term benefits and other liabilities, if due to be repaid in more than a year from the consolidated balance sheet date.
