Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - BAYCARE ALLIANT HOSPITAL, INC.. In 2022 BayCare Alliant Hospital SOLICITed INPUT FROM KEY INFORMANTS, THOSE INDIVIDUALS WHO HAVE A BROAD INTEREST IN THE HEALTH OF THE COMMUNITY, via AN ONLINE KEY INFORMANT SURVey. A LIST OF RECOMMENDED PARTICIPANTS WAS PROVIDED BY BayCare Alliant HOSPITAL; THIS LIST INCLUDED NAMES AND CONTACT INFORMATION FOR PHYSICIANS, PUBLIC HEALTH REPRESENTATIVES, OTHER HEALTH PROFESSIONALS, SOCIAL SERVICE PROVIDERS, AND A VARIETY OF OTHER COMMUNITY LEADERS. POTENTIAL PARTICIPANTS WERE CHOSEN BECAUSE OF THEIR ABILITY TO IDENTIFY PRIMARY CONCERNS OF THE POPULATIONS WITH WHOM THEY WORK, AS WELL AS OF THE COMMUNITY OVERALL. KEY INFORMANTS WERE CONTACTED BY EMAIL INTRODUCING THE PURPOSE OF THE SURVEY AND PROVIDING A LINK TO TAKE THE SURVEY ONLINE; REMINDER EMAILS WERE SENT AS NEEDED TO INCREASE PARTICIPATION. IN ALL, 119 COMMUNITY STAKEHOLDERS IN THE BayCare Alliant HOSPITAL SERVICE AREA TOOK PART IN THE ONLINE KEY INFORMANT SURVEY, AS OUTLINED ON APPENDIX E OF THE CHNA. SEVERAL OF THE PARTICIPANTS RESPONDING TO THE SURVEY REPRESENT ORGANIZATIONS WHICH WORK WITH LOW-INCOME, MINORITY OR OTHER MEDICALLY UNDERSERVED POPULATIONS. Additionally, in an effort to ensure the voices of those who are historically underrepresented in our communities were heard, specific focus groups were conducted by engaging individuals from diverse backgrounds including children, older adult populations, Hispanic/LatinX, LGBTQIA+, and Black/African American communities.

Although the majority of the overarching goals, objectives and efforts focus on these top three focus areas, BayCare Alliant Hospital remained committed to supporting positive advancements in addressing diabetes, heart disease and stroke, while addressing the focus area of exercise, nutrition and weight. There were five additional health focus areas identified through the CHNA. Schedule H, Part VI, Line 5 Promotion of community healthBayCare Alliant Hospital (Alliant) opened its doors on March 8, 2008. This Specialty Acute Care Hospital was created after BayCare Health System identified a need in the community for patients requiring hospital-level care for an extended period. This new hospital in north Pinellas County would provide care for patients on ventilators, with complex illnesses, cardiac needs, or complex wounds by transitioning them out of intensive care units into a facility with critical care specialists. Alliant would serve the community by providing focused care for chronic medical conditions requiring weeks of treatment and by helping the acute care hospitals meet the demand for critical care patient beds. Physically located within Mease Dunedin Hospital, with a separate entrance on Main Street, Alliant is a hospital-within-a-hospital (HIH). As an HIH, Alliant has 24-hour access to essential diagnostic equipment and surgical services. Medical emergencies can be managed onsite without the need for ambulance transport or external transfer. Alliant's state-of-the-art Intensive Care Unit is equipped with eCare, which means physicians can monitor patients round-the-clock. The ICU allows Alliant to accept critically ill patients earlier in their hospitalization and provide 2-to-1 nursing ratios in the unit; the eCare team uses advanced technology to electronically monitor each patient's medical condition 24 hours a day, seven days a week and provides a specialty-trained critical care physician from 7 pm to 7 am. Alliant's mission has remained the same: to provide excellence in care for patients with prolonged illnesses, recognizing that optimum healing requires a team approach to address mind, body, and spirit. Its Board of Trustees is comprised of persons who reside in the organization's community who are neither employees nor independent contractors of the organization, nor family members thereof. The organization extends medical staff privileges to all qualified physicians in its community. The organization uses surplus funds for improvements in facilities and equipment, patient care, medical training, education, and research. Alliant continues its commitment to enhancing residents' quality of life by actively participating in community health events and providing education to patients, physicians, and allied health professionals as important resources within the community. For several years, Alliant has partnered with Dunedin Cares Community Food Pantry holding food drives, donating foodstuffs, participating in health screenings, and offering nutrition education. Since 2021, as part of its CHNA initiative, Alliant has awarded Dunedin Cares a monetary contribution to support procurement of fresh produce and dairy products to help eradicate food insecurity. This support became especially crucial during the pandemic and has remained essential due to the increased prices of food and lodging in the community. BayCare Alliant has also established an ongoing partnership with the Homeless Empowerment Program (HEP) assisting homeless and low-income individuals and families, including veterans, with housing, food, clothing, and support. When COVID forced Alliant to stop its educational inservices at HEP (Wellness Wednesdays), Alliant started holiday and back-to-school drives to assist families in need; a practice that continues. Since 2021, as part of its CHNA initiative, Alliant has provided contributions to fund both HEP's Vision Program and Behavioral Health Program. HEP has provided testimonials to speak to the positive impact Alliant's sponsorship has had on its clients. As activities in the community reopen post-pandemic, Alliant actively participates and builds alliances with local business leaders and residents through BayCare-sponsored activities such as Pitch for Pink and Holiday Lights. These events provide enjoyable learning opportunities to educate residents about health resources in their community. In addition, Alliant clinical nurse liaisons establish relationships with care coordinators and physicians in Pinellas County and beyond and partner with these healthcare professionals to help establish the correct level of care for patients in the community. The liaisons meet with patients and families as an important resource for information and to help manage the anxiety created by critical illness. When needed, Alliant also assists family members with transportation to visit their loved ones at the hospital; Alliant also collaborates with sister hospitals on complex and hardship cases to ensure that those in the community can receive the care they need.
Schedule H, Part VI, Line 6 Affiliated health care systemBayCare Health System (BayCare), headquartered in Clearwater, Florida, is the leading, not-for-profit health care system that connects individuals and families to a wide range of services at 16 hospitals and hundreds of other convenient locations throughout the Tampa Bay and West Central Florida regions. Inpatient and outpatient services include acute care, primary care, imaging, laboratory, surgery centers, wellness centers, behavioral health, home care and urgent care. BayCare's 2022 Report to the Community can be viewed at BayCare.org/Annual-Report. BayCare's 16 hospitals are Bartow Regional Medical Center, BayCare Alliant, BayCare Hospital Wesley Chapel, Mease Countryside, Mease Dunedin, Morton Plant, Morton Plant North Bay, St. Anthony's, St. Joseph's, St. Joseph's Children's, St. Joseph's Women's, St. Joseph's-North, St. Joseph's-South, South Florida Baptist, Winter Haven and Winter Haven Women's. (In certain cases, hospital locations with the same tax identification and state license number are listed as one facility on Form 990, Schedule H, consistent with IRS reporting guidelines.) During 2022, BayCare continued construction of two new hospitals to provide acute care services in parts of the region experiencing rapid population growth: BayCare Hospital Wesley Chapel opened in March 2023, and construction progressed on a replacement South Florida Baptist Hospital near its current location, with opening expected in late 2023. BayCare celebrated its 25th anniversary in 2022. It was founded in 1997 after leaders of several of the area's independent, not-for-profit hospitals began discussing ways to ensure that high-quality, not-for-profit health care would remain a viable option for the Tampa Bay community for decades to come. The founding hospitals agreed to sacrifice some of their autonomy to be operated by a new entity, BayCare. With $5.1 billion in operating revenue in 2022, BayCare is now a fully integrated health system dedicated to providing high-quality, compassionate care to all we serve, regardless of their ability to pay. BayCare has 3,946 hospital beds, 21 urgent care centers, 4 surgery centers, 15 outpatient imaging facilities, 193 physician practice locations and 36 walk-in care stations in Publix supermarkets. BayCare annually generates $8.5 billion in economic impact in the region and state. BayCare is one of the largest employers in the Tampa Bay area, with 29,402 team members. During 2022, BayCare provided $699.7 million in benefits for its employees, including $218.8 million in retirement, $192 million in medical insurance, $174 million in paid time off and $53 million in education and development. In 2022, BayCare was named to these prestigious national lists: Fortune Best Workplaces in Health Care, PEOPLE Companies that Care, and BOLD Programs of Distinction from the National Center for Healthcare Leadership. BayCare also made the 2022 Top Workplaces list of the Tampa Bay Times newspaper. In 2022, BayCare provided $492 million in total Community Benefit, including $332 million in Medicaid and other income-based programs, $97 million in traditional charity care and $63 million in unbilled community services, all measured in unreimbursed costs. After a Community Health Needs Assessment conducted in 2019 identified food insecurity as one of the top issues in the Tampa Bay and West Central Florida regions - an issue only exacerbated by the pandemic - BayCare stepped up to help not just our patients, but individuals and families throughout the area we serve. BayCare partnered with not-for-profit Feeding Tampa Bay and in 2022 helped to fund food pantries in 42 public schools in lower-income neighborhoods across four counties. In 2022, BayCare distributed more than 9,200 "Healing Bags" of food to our patients who identified food insecurity as a problem. BayCare also adjusted its patient record system to prompt case managers to ask every vulnerable, high-risk patient about food insecurity and record the answer so that hunger can be diagnosed and treated like other medical problems. During 2022, 103,000 patients were asked those questions and their answers were recorded in their electronic medical records. BayCare's financial strength, the efficiencies gained from its operating model, its visionary leadership, the hard work of its team members, and its determination to provide exceptional care to its patients helped BayCare in 2022 retain its ranking in the top 20% of large U.S. health systems clinical excellence by Fortune/Merative. Twenty-five years after it was created, BayCare is achieving the goal of its founders to preserve not-for-profit health care for the communities of Tampa Bay and West Central Florida, and it is fulfilling its Mission as an organization to "improve the health of all we serve through community-owned services that set the standard for high-quality, compassionate care."
