Form And Line ReferenceSchedule D, Part X, Line 2 FIN 48 (ASC 740) footnoteFormAndLineReferenceDesc
ExplanationSchedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. Their related income is exempt from federal income tax under Section 501(A). Some of the subsidiaries are taxable entities, and some of the income of the tax-exempt entities is subject to taxation as unrelated business taxable income. The Company and its subsidiaries file U.S. federal income tax returns, and they also file in various state and foreign jurisdictions. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist as of December 31, 2022. Accounting for uncertainty in income taxes, ASC Topic 740-10 prescribes a comprehensive model for how an organization should measure, recognize, present and disclose in its financial statements uncertain tax positions that an organization has taken or expects to take on a tax return. The Company is subject to routine audits by taxing jurisdictions. There are no current audits in progress. As of December 31, 2022, the Company has no uncertain tax positions. The Company's taxable subsidiaries had $282,807 and $269,112 federal and state net operating loss carryforwards as of December 31, 2022 and 2021, respectively, which are available to offset future taxable income subject to various limitations. The Company accounts for income taxes under the asset and liability method. Under this method, deferred tax assets and liabilities are recognized for the estimated future tax consequences attributable to differences between the financial statement carrying amounts of existing assets and liabilities and their respective tax bases. Deferred tax assets and liabilities are measured using enacted tax rates expected to be in effect during the year in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. Interest and penalties related to income taxes are accounted for as income tax expense. The Company has placed a partial valuation allowance of $259,934 on the deferred tax assets as of December 31, 2022, and a full valuation allowance on the deferred tax assets as of December 31, 2021. When determining the valuation allowance for deferred tax assets, the Company annually considers whether it is more likely than not the deferred tax assets will or will not be utilized in future periods. There was a decrease in the valuation allowance of $9,178 and 59,497 as of December 31, 2022 and December 31, 2021, respectively.ExplanationTxt

Schedule H, Part V, Section B, Line 5 Facility A, 1Facility A, 1 - St. Francis Hospital, Inc.. Bon Secours St. Francis Health System, including Bon Secours St. Francis Downtown, Bon Secours St. Francis Eastside, Bon Secours St. Francis Millennium, and Bon Secours St. Francis Medical Campus at Simpsonville completed a joint CHNA report which reflects the collaborative efforts to conduct an assessment of the health needs of the community they serve. The CHNA was created utilizing quantitative data which was collected at national, state, regional, and local levels as well as qualitative data that was collected through community input. Key data sources included the United States Census Bureau; Healthy People 2030; County Health Rankings and Roadmaps; Centers for Disease Control; and South Carolina DHEC biostatistics, vital records, county health profiles and hospital discharge data. Dates of data collected ranged from 2000 - 2021. For each indicator, data was pulled for the most recent year available. To assess trends and progress, data points were compared to data pulls from the previous CHNA study when updated data points were available. Collection and analysis of qualitative data was completed through a series of town hall meetings and the implementation of a community survey. Qualitative data was collected by the Johnson Group, the consulting firm that compiled that data in the previous CHNA studies. Three Town Hall meetings were conducted in January 2022. Originally set to be completed in person, due to a community spike in COVID cases, all three Town Halls were completed by Zoom. The Town Halls included over 70 community residents, faith-based representative, and some health and human service representatives. These groups represented the interests of urban and suburban residents, mostly from less affluent communities that are home to lower-income families, minority families and the elderly. The community-wide survey reached 775 randomly selected Greenville County residents who completed a lengthy questionnaire about community health issues. Both the secondary data and the town hall results were used to inform survey question selection and the prioritization process of identified needs. A 21-member Advisory team of local public health professionals, health and human service agencies, and Bon Secours St. Francis Health System leaders assisted in guiding the assessment, providing additional insight and feedback into community issues and priorities. The advisory committee met three times between January - April 2022. Additional detail can be found on the Bon Secours website at https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment
Schedule H, Part V, Section B, Line 11 Facility A, 1Facility A, 1 - St. Francis Hospital, Inc.. St. Francis Hospital, Inc. St. Francis Hospital, Inc. is comprised of Bon Secours St. Francis Downtown, Bon Secours St. Francis Eastside, Bon Secours St. Francis Millennium, and Bon Secours St. Francis Medical Campus at Simpsonville. These hospitals are collectively referred to as "the hospital needs identified in its CHNA are addressed with regional strategies. Unless noted below, both St. Francis Hospital - Downtown and St. Francis Hospital - Eastside participated in the regional strategies. Affordable Housing Implementation Activities: Goal: Advocate for a comprehensive affordable housing strategy to address housing inequities in Greenville County and its municipalities; Partner with organizations to address the housing affordability gap in Greenville; Expand housing for women affected by domestic violence. 2022 Strategies and Outcomes: * Education and advocating for policies and practices that specifically address the preservation of affordable housing in Greenville was a focus for the hospital during the year as it continued their engagement with affordable housing partners, such as the Greenville Housing Fund, to increase funding to support affordable housing units. The Greenville Housing Fund has been at the helm of advocating for government funds released by the federal government for municipalities for rent payment to landlords to those impacted by loss of wages or those who are homeless. During 2022, the Greenville Housing fund raised $5 million for affordable housing projects in Greenville County. * During 2022, the Hospital continued its participation with the Homeless Alliance which includes a cross sector of organizations looking at the needs and services of the homeless, particularly in the wake of COVID-19. * The Hospital continued its support of the Sterling Land Trust and its efforts to lead affordable housing initiatives in Greenville. During 2022, the Hospital and the Sterling land trust worked with Habitat for Humanity to finish building their 7th home as a part of a 10-year commitment. * The hospital completed their agreement with Mercy Housing SE to build 46 affordable 1-, 2- and 3-bedroom apartments in Mauldin, SC. By the end of 2022, all units were completed and occupied, and the hospital is currently working to provide health education to residents. * In 2022, the hospital supports residents of Jasmine Road, an organization that provides services to women involved in domestic violence, by identifying and leveraging funding resources to support the costs of housing expansion. In 2022, Bon Secours successfully identified a funder that helped develop additional housing for these residents. Behavioral Health Implementation Activities: Goal: Actively address behavioral health issues in the Upstate through community and partnership-based collaborative approaches; Increase education, early detection and easy access to behavioral health interventions through Bon Secours St. Francis Health System (BSSFHS) access points; Expand community addiction service resources. 2022 Strategies and Outcomes: * Greenville County is classified as a Medically Underserved area and resource guides were previously unavailable. During 2022, the hospital made available the Behavioral Health Resource Guide, which was developed in 2021, as part of its partnership with Greenville County Behavioral Health Coalition in both a physical and digital format. The hospital also began working with the Hispanic Alliance and Clemson to provide translation of this document. * The hospital successfully continued utilizing the Columbia-Suicide Severity Rating Scales (C-SSRS), which was implemented in 2021, in combination with the already established Patient Health Questionnaire (PHQ-9) depression screening in ambulatory settings locally. The Patient Point Committee focus is to share the message with all patients within the practices through the electronic message boards within the sites about how important Depression Screening is to complete and to encourage support in reaching out to a Behavioral Health Provider for an appointment as needed. * The hospital continues to seek opportunities to expand pathways to behavioral health education and training for providers to increase the behavioral health workforce. Schedule H, Part I, Line 6a Community benefit report prepared by related organizationBon Secours Mercy Health, Inc.
Schedule H, Part I, Line 7g Subsidized Health Services$85,690 of costs attributable to physician clinics was included as subsidized health services in 2022. These clinics provide community benefit by improving access to physician services for the poor and underserved.
Schedule H, Part I, Line 7 Bad Debt Expense excluded from financial assistance calculation71889559
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceCost of financial assistance at cost was calculated with a cost to charge ratio using worksheet 2. The cost related to Medicaid patients was determined using Bon Secours Mercy Health's cost accounting system and included both inpatients and outpatients for traditional Medicaid and Medicaid managed care plans. For subsidized services Bon Secours Mercy Health's cost accounting system used to determine cost related to the specific service excluding traditional Medicaid and Medicaid managed care patients. Costs for charity and bad debt accounts are deducted using a ration of cost to charge specific to that subsidized service. Costs for other programs reflect the direct and indirect costs of providing those programs.
Schedule H, Part II Community Building ActivitiesBon Secours Mercy Health (BSMH) addresses various community concerns including health improvement, poverty, workforce development, and access to health care. BSMH hospitals conduct community health education and support groups, health fairs and screenings for the communities served. BSMH hospitals work with state and local leadership to address community needs and provide healthcare services to the poor and underserved. BSMH is committed to addressing the social determinants of health (SDOH) and social needs for patients and communities throughout our footprint. With a deep understanding of both areas, the team is working systematically across the ministry and in our local communities to ensure we respond holistically and impactfully. BSMH addresses SDOH through its Community Health Needs Assessment (CHNA), Community, investment strategy, advocacy/public policy partnerships, and cross sector solutions. BSMH addresses social needs through SDOH patient assessment, health education/promotion programs, capacity building of local community resources, and closed loop referral system. BSMH hospitals provide programs to improve the physical surroundings and housing in the communities served. Inadequate housing has a negative impact on the health of residents in the area by leading to violence in the neighborhoods. A robust economy positively impacts residents covered by health insurance and improves the capacity of the community to support health services. Additional detail regarding BSMH's community building activities and the promotion of health of its communities can be found in its 2022 Community Health Annual Report available at https://www.bonsecours.com/about-us/community-commitment/community-health-needs-assessment and https://www.mercy.com/about-us/mission/giving-back/community-health-needs-assessment.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountTHE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. NET PATIENT ACCOUNTS ARE REDUCED BY AN ALLOWANCE FOR DOUBTFUL RECEIVABLES BASED UPON BON SECOURS MERCY HEALTH'S (BSMH) HISTORICAL COLLECTION EXPERIENCE ADJUSTED FOR CURRENT ENVIRONMENTAL RISKS AND TRENDS FOR EACH MAJOR PAYOR SOURCE. SIGNIFICANT PROVISION IS MADE FOR SELF-PAY PATIENT ACCOUNTS IN THE PERIOD OF SERVICE BASED ON PAST COLLECTION EXPERIENCE. BSMH'S CONCENTRATION OF CREDIT RISK RELATED TO NET PATIENT ACCOUNTS IS LIMITED DUE TO THE DIVERSITY OF PATIENTS AND PAYORS. NET PATIENT ACCOUNTS CONSIST OF AMOUNTS DUE FROM GOVERNMENTAL PROGRAMS (PRIMARILY MEDICARE AND MEDICAID), PRIVATE INSURANCE COMPANIES, MANAGED CARE PROGRAMS AND PATIENTS THEMSELVES. NET PATIENT SERVICE REVENUE FOR SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY PAYOR COVERAGE IS RECOGNIZED BASED ON CONTRACTUAL RATES FOR SERVICES RENDERED. BSMH RECOGNIZES A SIGNIFICANT AMOUNT OF PATIENT SERVICE REVENUE AT THE TIME SERVICES ARE RENDERED EVEN THOUGH IT DOES NOT ASSESS THE PATIENT'S ABILITY TO PAY. AS A RESULT, THE PROVISION FOR BAD DEBTS IS PRESENTED AS A DEDUCTION FROM PATIENT SERVICE REVENUE (NET OF CONTRACTUAL PROVISIONS AND DISCOUNTS). AMOUNTS RECOGNIZED ARE SUBJECT TO ADJUSTMENT UPON REVIEW BY THIRD-PARTY PAYORS. FOR UNINSURED PATIENTS THAT DO NOT QUALIFY FOR CHARITY CARE, BSMH RECOGNIZES REVENUE WHEN SERVICES ARE PROVIDED. BASED ON HISTORICAL EXPERIENCE, A SIGNIFICANT PORTION OF BSMH'S UNINSURED PATIENTS WILL BE UNABLE OR UNWILLING TO PAY FOR SERVICES PROVIDED. THUS, BSMH RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS RELATED TO UNINSURED PATIENTS IN THE PERIOD THE SERVICES ARE PROVIDED. ANY DISCOUNTS APPLIED TO SELF-PAY PATIENTS WOULD BE DEEMED EITHER CHARITY OR A CONTRACTUAL ADJUSTMENT. BAD DEBT WOULD BE BASED ON THE BALANCE AFTER THE CHARITY OR CONTRACTUAL ADJUSTMENT THAT IS DEEMED UNCOLLECTABLE FOLLOWING A REASONABLE COLLECTION EFFORT.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyBON SECOURS MERCY HEALTH'S (BSMH) FINANCIAL ASSISTANCE POLICY DOES NOT PERMIT THE COST OF PATIENTS WHO ARE UNCOOPERATIVE OR UNABLE TO BE LOCATED TO BE RECLASSIFIED FROM FINANCIAL ASSISTANCE TO BAD DEBT. BSMH'S FINANCIAL ASSISTANCE POLICY REQUIRES AN APPLICATION AND SUPPORTING DOCUMENTATION. THEREFORE, ZERO DOLLARS ARE BEING REPORTED ON PART III, LINE 3 AS AMOUNTS INCLUDED IN BAD DEBT THAT COULD BE ATTRIBUTABLE TO PATIENTS ELIGIBLE UNDER BSMH'S FINANCIAL ASSISTANCE POLICY. THE HOSPITAL FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE BAD DEBT EXPENSE AS COMMUNITY BENEFIT.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteBON SECOURS MERCY HEALTH'S (BSMH) AUDITED FINANCIAL STATEMENTS DO NOT CONTAIN A FOOTNOTE THAT DESCRIBES BAD DEBT EXPENSE. BSMH ELECTED TO EARLY ADOPT ASU 2011-07. ACCORDINGLY, BAD DEBT EXPENSE IS REFLECTED AS A DEDUCTION FROM REVENUE RATHER THAN AS AN OPERATING EXPENSE. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS, 2. SIGNIFICANT ACCOUNTING POLICIES, (d) NET PATIENT ACCOUNTS AND NET PATIENT SERVICE REVENUE (PAGE 11) STATES Patient receivables are recorded at net realizable value based on certain assumptions determined by payor class. For third party payors including Medicare, Medicaid, and commercial insurance, the net realizable value is based on the estimated contractual reimbursement percentage, which is based on current contract prices or historical paid claims data by payor. For self-pay receivables, which includes patients who are uninsured and the patient responsibility portion for patients with insurance, the net realizable value is determined using estimates of historical collection experience. These estimates are adjusted for estimated conversions of patient responsibility portions, expected recoveries and any anticipated changes in trends.
Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costsBON SECOURS MERCY HEALTH (BSMH) FOLLOWS THE CATHOLIC HEALTH ASSOCIATION OF THE UNITED STATES POLICY DOCUMENT, COMMUNITY BENEFIT PROGRAM, A REVISED RESOURCE FOR SOCIAL ACCOUNTABILITY ("CHA GUIDELINES") FOR DETERMINING COMMUNITY BENEFIT. THE CHA GUIDELINES RECOMMEND THAT HOSPITALS NOT INCLUDE MEDICARE LOSSES AS COMMUNITY BENEFIT. BSMH'S COST ACCOUNTING SYSTEM WAS USED TO DETERMINE THE MEDICARE AMOUNTS IN PART III.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistancePATIENTS KNOWN TO QUALIFY FOR CHARITY CARE OR FINANCIAL ASSISTANCE ARE NOT SENT TO A COLLECTION AGENCY. THE ORGANIZATION REPEATEDLY OFFERS PATIENTS ACCESS TO FINANCIAL HELP DURING THEIR HOSPITAL STAYS AND AFTER, AS WELL AS WITH EACH BILLING NOTICE. BILLS ARE SENT TO A COLLECTION AGENCY AS A LAST RESORT AND ONLY: WHEN PATIENTS HAVE THE ABILITY TO PAY SOME PORTION OF THEIR HEALTHCARE EXPENSES BUT REFUSE TO DO SO; WHEN PATIENTS REFUSE TO WORK WITH THE ORGANIZATION TO DETERMINE IF THEY QUALIFY FOR FREE OR DISCOUNTED CARE VIA FEDERAL, STATE, LOCAL OR HOSPITAL ASSISTANCE PROGRAMS; WHEN THE ORGANIZATION IS UNABLE TO LOCATE THE PATIENT OR PERSON RESPONSIBLE FOR THE BILL. Patients that are presumed to be eligible for financial assistance based on individual life circumstances will be provided 100% financial assistance. Patients determined to have presumptive financial assistance eligibility will not be required to meet income criteria, asset eligibility criteria, or fill out a financial assistance application. BSMH utilizes available resources (e.g. technology solutions, service organizations, etc.) to obtain information such as credit scores to assist in determining a patient's presumed eligibility.

Schedule I, Part III, Column (b) Estimated Number Of RecipientsHOP Program Patient Assistance : Estimated 600 individuals based on the number of active enrollees in the program.
Schedule I, Part II SCHEDULE I, PART IIDescription of the Bon Secours Mercy Health Mission Fund: Bon Secours Mercy Health performs its philanthropic work through its mission department. This initiative, called the Bon Secours Health System Mission Fund ("Mission Fund"), was developed to promote the Catholic Health Ministry and the Bon Secours Mercy Health, Inc. Mission. This purpose is realized through the funding of initiatives that improve the health and well-being of communities, particularly for disenfranchised and marginalized people, served by Bon Secours Mercy Health Local Systems ("Local Systems"), Cosponsors and the Congregation of the Sisters of Bon Secours. The scope of its purpose and use of funds would be to: -promote healthy community coalition initiatives in conjunction with local system efforts, -develop local system and community excellence for a specific health condition and preventive need, and -improve access for uninsured populations and reduce health disparities among populations in the community. The Strategic Quality Plan of the health system calls for focused efforts to Build Healthier Communities. The health system understands "health" to include social and communal dimensions and has adopted the following articulation of a healthy community. The conditions of communities and individuals served by Bon Secours Mercy Health reflect the interaction of significant factors and complex behaviors at the individual, communal, and societal level. It is not likely that interventions by any one organization will result in substantial improvement or benefit to the community. Rather, increased participation by stakeholders and greater cooperation among entities with appropriate skills and resources is necessary for systemic change and improved outcomes. Mission Fund grants place emphasis on increased collaboration among community based members (Healthy Community Initiative), public health officials and other providers of services. The Mission Fund anticipates that most endeavors that seek to bring meaningful improvement require time and commitment. Consequently, local system grant recipients may expect continuity of support (several years) to establish and track outcomes. At the same time, grant applicants need to cultivate and achieve a wide array of financial resources necessary to sustain promising projects and service programs.
Schedule I, Part I, Line 2 Procedures for monitoring use of grant funds.THE ORGANIZATION COMPLIES WITH BON SECOURS MERCY HEALTH, INC.'S SYSTEM-WIDE FINANCIAL AND ACCOUNTING POLICIES. CONTRIBUTIONS ARE GENERALLY MADE AS REIMBURSEMENTS FOR FUNDS SPENT. IN SUCH CASES, THE DONEE/GRANTEE ORGANIZATION MUST PROVIDE DOCUMENTATION TO THE FILING ORGANIZATION BEFORE FUNDS ARE APPROVED FOR DISBURSEMENT. IN OTHER CASES, GRANTEES SUBMIT PROGRESS REPORTS ON THE ANNIVERSARY DATE ON WHICH THE GRANT WAS RECEIVED. THE EVALUATION REPORT INCLUDES: 1) PROGRESS TOWARD THE DEPLOYMENT OF THE STATED GOALS AND OBJECTIVES, 2) PROGRESS TOWARDS THE ACHIEVEMENT OF DESIRED OUTCOME AS DEMONSTRATED BY PROJECT WORK PLAN, 3) AN ACCURATE ACCOUNTING OF THE REVENUE AND EXPENSES AND THE AMOUNT OF THE MISSION FUND AWARD EXPENSED, AND 4) A SUMMARY PAST, CURRENT AND FUTURE FUNDING SOURCES AND EFFORTS TO SECURE SUSTAINING SOURCES OF FUNDING.
