Schedule H, Part V, Section B, Line 3ETHE SIGNIFICANT HEALTH NEEDS ARE A PRIORITIZED DESCRIPTION OF THE SIGNIFICANT HEALTH NEEDS OF THE COMMUNITY AND WERE IDENTIFIED THROUGH THE CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - GARNET HEALTH MEDICAL CENTER. Garnet Health worked with the Local Health Department Prevention Agenda Collaborative, established in 2017, that includes 7 local health departments and 16 other local hospitals to develop the second regional Community Health Assessment for the Mid-Hudson Region that contributed to the development of the Garnet Health Medical Center and Garnet Health Medical Center- Catskills Community Health Needs Assessments. The hospitals contributed funds which allowed the Collaborative to contract with Siena College Research Institute to conduct a random digit dial regional community health survey to supplement the Regional Community Health Assessment and to gauge the perception of residents surrounding health and resources in their communities. Responses from 5,699 residents of the Mid-Hudson Region were collected. Residents aged 18 and older were interviewed from the Mid-Hudson Region to ensure representative county-wide samples. The overall sample of 5,699 was weighted by age, gender, reported race/ethnicity, income and county using the 2015-2020 American Community Survey 5-year estimates to ensure statistical representativeness. A total of 906 surveys were completed for Orange County. To further supplement the data collected from the community, the Collaborative members held focus groups with service providers to further understand the needs of specific communities and populations, and the barriers they face to achieving optimal health. Additionally, focus groups were conducted by the Orange County Health Department with providers that serve underrepresented populations such as those who are low-income, veterans, seniors, people experiencing homelessness, LGBTQ members, and people with a mental health diagnosis. The Orange County Health Department conducted two focus groups. The first was with the Joint Membership of Health and Community Agencies. Their focus is on providing residents of Orange County with a welcoming, comprehensive, and seamless service delivery system for recovery, health, and wellness. The second was with the Changing the Orange County Addiction Treatment Ecosystem. Orange County Health department took the lead in pulling the Mid-Hudson Region assessment together with the support, feedback, review and input from the collaborative's participating health departments and hospitals throughout the entire process. Along with the primary data collected by the Collaborative through the survey and focus groups, secondary data was compiled to show health indicators for the area a narrative was written around each health indicator to contextualize the data and outline how each indicator is related. Input to the Community Health Needs Assessment and Implementation Plan included focus groups, interviews, literature review, and surveys. Organizations providing input included Departments of Health from seven counties, school districts, offices of aging, offices of veteran's affairs, Federally Qualified Healthcare Centers (FQHC's), food banks of the Hudson Valley, etc. Work Groups were formed including behavioral health, workplace wellness, social determinants of health, etc. These work groups were each led by a steering committee comprised of community stakeholders and assisted in determining health priorities. While Garnet Health participated in the collaborative process, data collected along the way was also used to develop the health systems Community Health Assessment and improvement plans and were developed with the assistance of Crowe LLP.

Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - GARNET HEALTH MEDICAL CENTER. The hospital is addressing the significant needs identified in its most recent CHNA in a variety of ways. First and foremost, the hospital played an active, collaborative role with the county health department to develop the Community Health Improvement Plan (CHIP) which clearly outlines the priority areas chosen and serves as the hospital's implementation strategy. By working closely with the health department and many other community-based organizations, we were able to create a comprehensive plan whereby all voted on and agreed to support specific evidence-based interventions from the NYS Prevention Agenda that will improve health outcomes in Orange and Sullivan County. The priority areas include - Prevent Chronic Disease & Improve Mental Health and Prevent Substance Use Throughout the plan, specific organizations, including our hospital are identified as providing support and/or leadership for individual strategies. Garnet Health specifically is targeting efforts towards the below strategies. CHIP goal Healthy Eating and Food Security Evidenced Based Strategies *Provide resources for local food pantries upon discharge *Develop and refer potential participants to the FreshRx Nutrition Incentive Program. CHIP goal Increase early detection of cardiovascular disease diabetes, prediabetes and obesity Evidenced Based Strategies *Remove Structural barriers for screenings by increasing primary care provider connections by developing a system to refer patients without primary care when presenting to the emergency department or urgent care setting *Provide Community based preventative program opportunities *relaunch the health heart program to reach local businesses and local Community events *provide the Diabetes prevention program through multiple media platforms and onsite location to improve accessibility *Remove structural barriers to cancer screening by working with employers to provide employees with paid leave or the option to use flex time for cancer screenings. CHIP goal increase cancer screening rates for breast, cervical and colorectal cancers Evidenced Based Strategies *Remove structural barriers to cancer screening by increasing primary care provider connections by developing a system to refer patients without primary care when presenting to the emergency department or urgent care setting *remove economic barrier to cancer screening by ensuring access to health insurance by developing a system to connect insurance patient navigators to patients waiting for care in the emergency department. CHIP goal Prevent opioid and other substance misuse and deaths Evidenced Based Strategies *increase the availability of/access and linages to medication-assisted treatment (MAT) including Buprenorphine by developing internal policies/procedures for the initiation of MAT administration in the emergency department *increase the availability of access to MAT including Buprenorphine by organizing and funding MAT implementation trainings for healthcare providers prescribing Buprenorphine *promote and support the expansion of the Peer RX application for peer referrals at the emergency department by organize and fund MAT implementation training for health care providers prescribing Buprenorphine. In addition, other identified health needs including Access to Medical Providers, Cancer, Food Insecurity/Lack of Nutritious Food, Health Literacy, Heart Disease, Obesity, Preventative Care, Women's and Children's Health have not been specifically addressed through the development of the Implementation Strategy However, many of these needs are covered through Garnet Health Medical Center's provision of comprehensive services or through the above-mentioned priority areas to be focused on over the next three years. Finally, identified health needs including Affordable Housing, Domestic Violence, Lack of Affordable/Reliable Transportation are Outside the scope of services provided by Garnet Health Medical Center, but opportunities may exist for Garnet to support organizations addressing these needs.
