Schedule H, Part V, Section B, Line 5-IRA DAVENPORT MEMORIAL HOSPITALIn 2022, Arnot Health participated in a comprehensive assessment of community needs across county lines with partners such as the Chemung County Health Department, Steuben County Public Health, other hospitals and healthcare agencies in Chemung, Steuben and Schuyler Counties, along with a diverse, collaborative community of partners and residents.
Schedule H, Part V, Section B, Line 6a-IRA DAVENPORT MEMORIAL HOSPITALThe CHA is conducted with Arnot Health, Ira Davenport Memorial Hospital, Guthrie Corning Hospital, and St. James Hospital
Schedule H, Part V, Section B, Line 6b-IRA DAVENPORT MEMORIAL HOSPITALSteuben County Public Health, Common Ground Health, Smart Steuben, Steuben Rural Health Network, Finger Lakes Community Health, Pro Action of Steuben & Yates, Inc., Pivital Public Health Partnership
Schedule H, Part V, Section B, Line 11-IRA DAVENPORT MEMORIAL HOSPITALArnot Health works diligently to partner with the organizations throughout the service area in an effort to enhance healthcare services for the most vulnerable residents. Due to the rural nature of Chemung and Steuben Counties, the high percentage of low-income residents and the limited resources available in the community, Arnot Health understands the need to create meaningful partnerships to best serve the community at large. Arnot Health has formed collaborative relationships with organizations throughout Chemung and Steuben Counties to address health concerns across the continuum. Chemung and Steuben Counties maintain coalitions to collaboratively meet the healthcare needs of the community. The Chemung County Health Priority Partnership (HP2) has expanded to include insurance navigators. The Steuben County Health Priority Team (SHPT) worked to brand their coalition and has the new name, Smart Steuben. Both coalitions include organizations that are committed to improving the health of Chemung and Steuben County residents. The groups have conducted a Community Health Needs Assessment (CHNA) and developed a Community Health Improvement Plan (CHIP). The members of HP2 and Smart Steuben have agreed to continue expanding their partnerships and meet on a regular basis to ensure that the initiatives outlined in the Community Health Improvement and Community Service Plans are implemented, monitored, and evaluated. The CHIP can be found on our website www.arnothealth.org

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Schedule H, Part I, Line 7Ira Davenport Memorial Hospital uses a Cost to Charges ratio to calculate the Financial Assistance and Community Benefit Section. The Medical Center offers a variety of services to the community it serves as part of an integrated delivery system. It is necessary to financially subsidize some of these services as the revenue generated does not cover the cost of providing the services.
Schedule H, Part III, Section A, Line 4The Organization determines the transaction price based on standard charges for services provided, reduced by contractual adjustments provided to third-party payors, discounts provided to uninsured patients in accordance with its financial aid policies, and implicit price concessions. Estimates of contractual adjustments and discounts are determined based on contractual agreements, discount policies, and historical experience. For uninsured patients that do not qualify for charity care, the Organization recognizes revenue on the basis of its standard rates for services provided. Based on historical experience, a significant portion of the Organization's uninsured patients will likely be unable or unwilling to pay for services provided. Thus, the Organization records an implicit price concession to reduce accounts to their net realizable value at the time management believes it is probable a receivable will not be recovered. There are various factors that can impact collection trends, such as changes in the economy, unemployment rates, the number of uninsured and underinsured patients, the volume of patient through the emergency department, the increased burden of co-pays, co-insurance amounts and deductibles to be made by patients with insurance, and business practices related to collections efforts. These factors continuously change and can have an impact on collection trends and the estimation process. Subsequent changes to the estimate of the transaction price are generally recorded as adjustments to patient service revenue in the period of the change. Estimates of implicit price concessions are determined based on historical collection experience with these classes of patients using a portfolio approach as a practical expedient to account for patient contact as collective groups rather than as individual contracts. The financial statement effect of using this practical expedient is not materially different form an individual contract approach. In Line 2 & 3, the Organization's methodology to estimate bad debt expense is based on historical data of bad debt write-offs by Primary Financial Class. A percent of AR is calculated, and that percent is applied to Open Accounts Receivable and posted against the AR on the balance sheet.
Schedule H, Part III, Section B, Line 8Ira Davenport Memorial Hospital uses a Cost to Charges ratio to calculate the Financial Assistance and Community Benefit Section. The Medical Center offers a variety of services to the community it serves as part of an integrated delivery system. It is necessary to financially subsidize some of these services as the revenue generated does not cover the cost of providing the services.
Schedule H, Part III, Section C, Line 9bSelf-pay collection staff reviews any prior accounts to check for commercial insurance or Medicaid that may need to be applied to current self-pay accounts. These accounts are reviewed using the self-pay work list. If insurance is located, self-pay collectors must verify eligibility for the date of service. The self-pay staff will change the health plan to the correct one for all covered dates of service and Arnot Health Billing will submit claim or claims. If no insurance is located, it is confirmed if a patient may have qualified for Medicaid. If Medicaid coverage is approved, self-pay staff will change the health plan to Medicaid for all covered dates of service and follow the correct billing process. Self-Pay Collections Staff attempts to collect full balance due by check or credit card. If they are unable to make the full payment, Self-pay staff will attempt to make payment arrangements and notify patients about the Arnot Health Community Care Program. If the patient wants to apply for the community care program, the patient will be sent an application for the community care program and will be informed of documentation they will need to submit with the application. Self-pay collectors document all activity in Soarian, and enter any necessary hold to support resolution. Self-pay collections staff enters the payment plan and documents comments in Soarian. A payment plan statement is demanded and sent to the patient. If patient does not enter into a payment plan agreement, or fails to pay as agreed, account will go through the standard dunning and collection agency activity. See Guarantor Billing and Collections.
Schedule H, Part VI, Line 2In 2022, Ira Davenport Memorial Hospital, with Arnot Health, participated in a comprehensive assessment of community needs in collaboration with Steuben County Public Health and Common Ground Health, our local planning organization for improving population health across the Finger Lakes region. Also instrumental in the completion of the assessment were other hospitals including St. James Hospital and Guthrie Corning Hospital and healthcare agencies in Steuben County including Smart Steuben, Steuben Rural Health Network, Finger Lakes Community Health, Pro Action of Steuben & Yates, Inc., Oak Orchard Health, Pivital Public Health (formerly known as S2AY Rural Health Network), along with a diverse, collaborative community of partners and community members. Partners' roles in the assessment were to help inform and select the 2022-2024 priority areas by sharing any pertinent data or concerns and actively participating in planning meetings The three county lines of Chemung, Steuben and Schuyler Counties comprise the primary service area. Arnot Health, in partnership with this dual-county collaborative network, collects and analyze data from a variety of secondary sources including, but not limited to, County Health Rankings, New York State Department of Health Vital Statistics, Cornell University Program on Applied Demographics. Valuable input was gained from data retrieved and analyzed by the Finger Lakes Health System Agency from the New York State Prevention Agenda Dashboard, multi-payer claims data, EBRFSS, SPARCS, US Census Bureau American Community Survey, Behavioral Risk Factor Surveillance System, Common Ground Health's My Health Story 2018 survey, and additional sources. Also, input was gained from diverse community focus groups made of both Chemung and Steuben County residents. The residents were engaged in discussions concerning their current health status and opinions on specific county-wide health challenges. Areas of disparity were assessed within each county. Findings from the Community Health Assessment (CHA) data and community focus groups identified some overlapping areas of public health concerns and disparities in Chemung and Steuben Counties. The top priority areas identified are promoting well-being, preventing mental and substance use disorders and preventing chronic diseases. From there, a Community Health Improvement Plan (CHIP) is developed based on those priority health care needs.
Schedule H, Part VI, Line 3The Arnot Health credit services department helps you find programs and apply for them. We want to relieve the burden of unpaid bills on our patients and continue to provide top-quality healthcare to our community. Arnot Health communicates our Community Care Program in the following ways: Signage and brochures in the waiting rooms of inpatient, outpatient, and emergency departments, brochures enclosed in Admission Packets, notations on billing notices, staff education to share availability of program and contact information with patients. Communication and education are provided to the financial assistance team for patient insurance enrollment, so they are informed of the program details and contact information to help further educate our patients. Communication is also done with collection agencies and collection attorneys so they can also share the availability of the program and contact information with prospective enrollees. The information is also posted on our website at https://www.arnothealth.org/financial-assistance. The website provides instructions on how to apply for our assistance program as well as contact information. Aside from our Community Care Program, we also educate patients on other financial assistance programs such as free services provided through Chemung County Health Department, Community Health Advocates, Crime Victims Financial Assistance, and American Cancer Society Financial Assistance.
Schedule H, Part VI, Line 4Ira Davenport Memorial Hospital, located in Bath, NY of Steuben County, is a not-for-profit, 35-bed acute care hospital with an adjoining 120-bed skilled nursing facility serving its community since 1959. Steuben County is located in the southernmost portion of the Finger Lakes Region, situated between Allegany and Chemung Counties and bordering the Pennsylvania state line. With the rural nature of Steuben County, there is a high percentage of low-income residents and an aging population with limited resources available in the community. There are 93,584 total residents spread throughout the county with Bath, NY being one of the densest populated areas. 35% of Steuben County's population is living in a Health Professional Shortage Area, which causes concern for those who lack transportation to access services. When comparing across the Finger Lakes Region, 211 calls within the past year regarding transportation assistance were highest within Steuben County with 947 calls (followed by Monroe County with 741). Approximately 88% of those transportation request calls were for medical transportation, and another 8% were for public transportation. The majority of transportation requests originated from Bath (about 31%). The Steuben County population is primarily white non-Hispanic (94%), with Black non-Hispanic, Hispanic and other individuals each representing 2% of the remainder of the population. Women of childbearing age comprise approximately 21% of the population, about 11% are veterans, and 15% of individuals are living with some form of disability. The majority of those living with a disability in Steuben County are 65 years of age or older (about 68%). The three types of disabilities most prevalent to this age group are independent living difficulty (about 11%), hearing difficulty (about 14%), and ambulatory difficulty (about 20%). Additionally, about 28% of the population aged 65 years or older are living alone. 14% of the population in Steuben County are living in poverty.
Schedule H, Part VI, Line 5Ira Davenport Memorial Hospital (IDMH) has approximately 200 active employees. The hospital has formed great partnerships within the community including local law enforcement, local churches, and Bath's local primary care physicians, who are independent and provides all on-call coverage for IDMH. The IDMH Board of Directors is comprised of community leaders in the business and healthcare industries in the Steuben County area. The chairperson, Patty Baroody, is also an active member of the Arnot Health Board. IDMH is the brick-and-mortar site for Steuben County Child Advocacy Center, as well as the location for their mobile crisis unit. IDMH supports and hosts the Project Search Students Program. The BOCES program offers a unique school-to-work program for high school seniors with disabilities to learn basic entry level job skills. The students work with our dietary, environmental services, and medical records departments. In 2023, the organization is working towards the Rural Emergency Hospital designation, which is designed to maintain access to critical outpatient hospital services in communities that may not be able to support or sustain a small rural hospital. If granted, this designation demonstrates the instrumental role IDMH has in the community it serves and will allow the organization to continue providing high-quality care.
Schedule H, Part VI, Line 6Arnot Health, the parent organization, is comprised of three hospitals: Arnot Ogden Medical Center and St. Joseph's Hospital in Elmira; and Ira Davenport Memorial Hospital in Bath, New York. Arnot Health is one of the largest employers in both Chemung and Steuben County and an integral part of those communities. Arnot Health provides diagnostic, ambulatory, secondary and tertiary acute care, as well as substance abuse, psychiatric, rehabilitative, and wellness services to meet the needs of residents in Chemung, Steuben, and Schuyler Counties in the Southern Tier of New York, and Bradford and Tioga Counties in the Northern Tier of Pennsylvania. Arnot Health System has more than 300 physicians from over 50 specialties. The independent, not-for-profit regional healthcare system currently has a total of 567 beds: 362 acute care beds and 205 skilled nursing/long-term beds. Arnot Ogden Medical Center and St. Joseph's Hospital has a total of 327 acute care beds and 85 skilled nursing/long-term beds. Ira Davenport Memorial Hospital has a total of 35 acute care beds and 120 skilled nursing/long-term beds.
