DescTHESE SERVICES DESCRIBED BELOW WERE CAPITAL REGION MEDICAL CENTER PRACTICES WHEN THE ORGANIZATION STILL HAD CLINICAL OPERATIONS. AS A PART OF THE REORGANIZATIONAL AGREEMENT, ALL OPERATIONS WERE TRANSFERRED IN 2023. CAPITAL REGION MEDICAL CENTER WAS AN ACUTE CARE MEDICAL CENTER LOCATED IN JEFFERSON CITY, MISSOURI. THE MEDICAL CENTER OPERATED AS A TWO-CAMPUS MEDICAL CENTER SYSTEM, WHICH CONSISTED OF THE SOUTHWEST CAMPUS AND MADISON CAMPUS COMPLEMENTED BY COMMUNITY MEDICAL CLINICS. THE MEDICAL CENTER PRIMARILY EARNED REVENUES BY PROVIDING INPATIENT, OUTPATIENT, AND EMERGENCY CARE SERVICES TO PATIENTS IN THE JEFFERSON CITY, MISSOURI, AREA. IT ALSO OPERATED MEDICAL CLINICS IN THE SURROUNDING COMMUNITIES. CAPITAL REGION MEDICAL CENTER SERVED THE COMMUNITY BY PROVIDING MEDICAL CARE TO ALL PATIENTS REGARDLESS OF THEIR ABILITY TO PAY. PATIENTS WHO QUALIFY FOR CHARITY CARE WERE IDENTIFIED AS SOON AS POSSIBLE, EITHER BEFORE THE SERVICES WERE PROVIDED OR AFTER AN INDIVIDUAL HAD RECEIVED SERVICES TO STABILIZE THEIR MEDICAL CONDITION. IF IT IS DIFFICULT TO DETERMINE A PATIENT'S ELIGIBILITY FOR CHARITY CARE PRIOR TO PROVIDING THE SERVICE, THE DETERMINATION WAS MADE AS SOON AS POSSIBLE AFTER THE PROVISION OF THE SERVICE. UPON REGISTRATION, PATIENTS WITHOUT MEDICARE/MEDICAID, ADEQUATE HEALTH INSURANCE OR OTHER LOCAL HEALTH CARE FINANCIAL ASSISTANCE WOULD RECEIVE EITHER A PACKET OF INFORMATION THAT ADDRESSES THE FINANCIAL ASSISTANCE POLICY AND PROCEDURES OR IMMEDIATE FINANCIAL COUNSELING ASSISTANCE FROM STAFF WITHIN ESTABLISHED BUSINESS HOURS. THE PACKET OF INFORMATION WILL CLEARLY INDICATE THAT CRMC PROVIDES CARE WITHOUT REGARD TO ABILITY TO PAY, TO INDIVIDUALS WITH LIMITED FINANCIAL RESOURCES AND EXPLAIN HOW PATIENTS APPLY FOR FINANCIAL ASSISTANCE. CRMC PROVIDED A VARIETY OF SERVICES RANGING FROM EMERGENCY SERVICES TO INPATIENT AND OUTPATIENT ELECTIVE SURGERY, DIAGNOSTIC TESTING AND EDUCATION PROGRAMS. CHARITY CARE POLICIES AND PROCEDURES WERE IN PLACE TO ADDRESS ANY INSTANCES IN WHICH PATIENTS MAY ACCESS SERVICES. SUPPORT OF MEDICAL EDUCATION IN 1952 CRMC ENTERED INTO THE TEACHING AND TRAINING OF PHYSICIANS WITH PROGRAMS TO TEACH MEDICAL STUDENTS AND INTERNS. TODAY CRMC SERVED AS THE PRIMARY CLINICAL TRAINING SITE FOR ONE OF THE TWO FAMILY MEDICINE RESIDENCY PROGRAMS SPONSORED BY THE UNIVERSITY OF MISSOURI COLUMBIA SCHOOL OF MEDICINE. CRMC ALSO SERVED AS A CLINICAL TRAINING SITE FOR OTHER SPECIALTY RESIDENCY PROGRAMS SPONSORED BY THE UNIVERSITY OF MISSOURI COLUMBIA SCHOOL OF MEDICINE. IN ADDITION MEDICAL STUDENTS FROM THE KIRKSVILLE COLLEGE OF OSTEOPATHIC MEDICINE AND UNIVERSITY OF HEALTH SCIENCES LOCATED IN KANSAS CITY SCHOOLS OF MEDICINE COMPLETED CLINICAL ROTATIONS AT CRMC. CAPITAL REGION MEDICAL CENTER SUPPORTED LINCOLN UNIVERSITY SCHOOL OF NURSING PROGRAM, COLUMBIA COLLEGE NURSING PROGRAM, MISSOURI STATE UNIVERSITY AND DRURY UNIVERSITY RN, BSN AND MSN FROM CENTRAL MISSOURI STATE, CENTRAL METHODIST, LPN AND RN AT STATE TECH SOCIAL WORK AND CASE MANAGEMENT STUDENTS FROM MU, MISSOURI STATE UNIVERSITY, AND LINCOLN MSN STUDENTS FROM ST. LOUIS UNIVERSITY, UMKC, MARYVILLE, CHAMBERLAIN COLLEGE, FRONTIER NURSING GRACELAND, HERZING, VANDERBILT, UNIVERSITY OF SOUTHERN ALABAMA, NEW YORK INTERNATIONAL UNIVERSITY. AND WALDEN UNIVERSITY IN FAMILY NURSE PRACTITIONER CLINICALS FOR WHICH WE PROVIDE THE PRECEPTORS/CLINICAL SUPERVISORS AS WELL AS THE FACILITIES. CRMC ASSISTED STEPHENS COLLEGE, BELLBROWN COLLEGE, AND STATE FAIR COLLEGE WITH REGISTERED MEDICAL ASSISTANTS STEPHENS COLLEGE AND THE UNIVERSITY OF NORTH DAKOTA PHYSICIAN'S ASSISTANT PROGRAMS AND LOCAL SCHOOLS FOR PARAMEDIC AND EMT TRAINING. CRMC ALSO ASSISTED STATE TECH FOR RADIOLOGY. DIETARY STUDENTS FROM HERZING UNIVERSITY, KEISER UNIVERSITY, UMC AND UMSL, AS WELL AS ASSISTANCE TO THE UNIVERSITY OF MISSOURI DIETETICS PROGRAM. IN ADDITION TO THE DIRECT FINANCIAL SUPPORT THAT CAPITAL REGION PROVIDED, WE ALSO PROVIDED SUPPORT TO COLUMBIA COLLEGE MSN STUDENTS AND LINCOLN UNIVERSITY SCHOOL OF NURSING BSN PROGRAM IN LEADERSHIP TRAINING. WE ASSISTED STUDENTS FROM EAST CENTRAL COLLEGE IN THE SURGICAL TECH PROGRAM. CRMC PROVIDED TRAINING TO PHLEBOTOMIST/TECH STUDENTS FROM MOBERLY COMMUNITY COLLEGE TO ASSIST THEM WITH FULFILLING THEIR CLINICAL REQUIREMENTS. CRMC PROVIDED OVERSIGHT AND MENTORING TO THE ST. LOUIS COLLEGE OF PHARMACY, UNIVERSITY OF MISSOURI COLUMBIA AND KANSAS CITY PHARMACY PROGRAMS. WE PROVIDED SUPPORT TO ST. LOUIS UNIVERSITY AND UNIVERSITY OF MISSOURI COLUMBIA PT AND KANSAS CITY OZARK TECHNICAL COLLEGE AND STATE TECH PTA, OTA AND SPEECH THERAPY PROGRAMS. OUR SUPPORT FOR THESE PROGRAMS IN NOT ONLY THROUGH THE PROVISION OF TRAINING FACILITIES BUT ALSO ADJUNCT FACULTY AND RESOURCES. COMMUNITY HEALTH RESIDENT CLINIC: CRMC PROVIDED RESIDENTS AND PHYSICIAN SUPERVISORS TO STAFF THE COMMUNITY HEALTH CENTER CLINIC. IN ADDITION TO PROVIDING A VALUABLE TRAINING TOOL FOR THE MEDICAL STUDENTS, THE RESIDENT CLINIC SERVED A UNIQUE COMMUNITY BENEFIT BY PROVIDING SERVICE TO PATIENTS FROM THE EIGHT COUNTY SERVICE AREA. THE CHARGE FOR SERVICE WAS BASED ON THE INDIVIDUAL'S ABILITY TO PAY, WHICH IN MANY CASES IS FREE.Desc

Schedule H, Part V, Section B, Line 3EThe Areas of Opportunity were determined after consideration of various criteria, including: standing in comparison with benchmark data (particularly national data); the preponderance of significant findings within topic areas; the magnitude of the issue in terms of the number of persons affected; and the potential health impact of a given issue. These also take into account those issues of greatest concern to the community stakeholders (key informants) giving input to this process. ACCESS TO HEALTH CARE SERVICES * Barriers to Access * Appointment Avail * Primary Care Physician Ratio * Eye Exams * Ratings of Local Health Care Cancer * Leading Cause of Death * Lung Cancer Deaths Diabetes * Diabetes deaths Heart Disease and Stroke * Leading Cause of Death * High Blood Pressure Prevalence * Overall Cardiovascular Risk Injury and Violence * Motor Vehicle Crash Deaths * Firearm-Related Deaths Kidney Disease * Kidney disease deaths * Hypertensive Renal Disease Death Mental Health * Suicide Deaths * Key informants: Mental health ranked as a top concern Nutrition, physical activity, and weight * Fruit and vegetable consumption * Overweight and Obesity (adults) Potentially disabling conditions * High-impact chronic pain Respiratory disease * Lung disease deaths Substance Abuse * Use of prescription opioids * Sought help for alcohol/drug issues * Key informants: substance abuse ranked as a top concern.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - CAPITAL REGIONA MEDICAL CENTER. To solicit input from key informants, those individuals who have a broad interest in the health of the community, an Online Key Informant Survey also was implemented as part of this process. A list of recommended participants was provided by Capital Region Medical Center; this list included names and contact information for physicians, public health representatives, other health professionals, social service providers, and a variety of other community leaders. Potential participants were chosen because of their ability to identify primary concerns of the populations with whom they work, as well as of the community overall. Key informants were contacted by email, introducing the purpose of the survey and providing a link to take the survey online; reminder emails were sent as needed to increase participation. In all, 26 community stakeholders took part in the Online Key Informant Survey. Through this process, input was gathered from several individuals whose organizations work with low-income, minority, or other medically underserved populations. In the online survey, key informants were asked to rate the degree to which various health issues are a problem in their own community. Follow-up questions asked them to describe why they identify problem areas as such and how these might better be addressed. Results of their ratings, as well as their verbatim comments, are included throughout this report as they relate to the various other data presented.
Schedule H, Part V, Section B, Line 11 Facility , 1Facility , 1 - CAPITAL REGION MEDICAL CENTER. Capital Region Medical Center used the information from this Community Health Needs Assessment to develop an Implementation Strategy to address the significant health needs in the community. While the hospital will likely not implement strategies for all of the health issues listed, the results of this prioritization exercise will be used to inform the development of the hospital's action plan to guide community health improvement efforts in the coming years.

Schedule H, Part VI, Line 2 Needs assessmentCRMC conducted a comprehensive community health assessment using both secondary and primary analysis, which allowed us to measure perception, health risks, health factors, health outcomes, and characteristics specific to the communities in these five central counties. This is the third comprehensive regional health assessment completed in this collaborative fashion by the CMCHAP. The objectives were to identify factors influencing health status, determine and prioritize issues of greater concern, identity, unmet health needs, and project a source of reliable information that will support the development of solutions. The needs assessment was conducted using three methods: secondary data analysis, discussions with community groups, and providing clients and surveys completed by community members, community leaders, and local physicians. Existing data previously collected for other purposes, called secondary data was compiled from various credible local, state, and federal sources to provide a context for analysis and interpretation. In addition to the data collection for 2021, a survey was included to assess the perception of health care and health status across the five county region. In the analysis, these survey were available in physician offices, online, and in community health departments. Upon reviewing the findings from all the sources of information, the steering team identified the most serious health issues and then ranked them using a priority matrix. The results were: 1. Mental health 2. Substance abuse 3. Diabetes, disability, and chronic pain 4. Heart disease and stroke 5. Nutrition, physical activity, and weight.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceAt every location, registration staff have financial assistance packets available to hand out. All self pay patients are referred to one of the three financial counselors in the area. Patient financial counselors visit all self pay inhouse patients. Also Human Arc help patients apply for Medicaid. Signs are posted in rooms and at registration sites. For every statement sent, there is a number printed to call for assistance. All first letters indicate who to call for assistance. All clinics are aware of the policy, so is the medical staff. It is posted on our website and on our online bill pay.
Schedule H, Part VI, Line 4 Community informationThe five county regions consisting of Callaway, Cole, Miller, Moniteau, and Osage, included urban and rural areas. The local area economy is heavily dependent on the state government. The total population for the region, based on the latest census was 176,474. Between the 2000 and 2010 censuses, the population of the total service area increased by 12,829 or 7.9% Cole County, which has the largest population in the region had the highest percentage increase in the population at 9.3% It is important to understand the population's age distributions, as different age groups have unique health needs that should be considered separately from other along the age spectrum. In the total service area 22.6% of the population are children aged 0-17, 61% are aged 18-24, and 16.4% are aged 65 and older.
Schedule H, Part VI, Line 5 Promotion of community healthCapital Region Medical Center conducted its last CHNA in 2021 and reviewed the health priorities identified through that assessment. Taking into account the top-identified needs, as well as hospital resources and overall alignment with the hospital's mission, goals and strategic priorities - it was determined at that time that CRMC would focus on developing and/or supporting strategies and initiatives to improve: - Access to Health Care and Support Services - Mental Health Disorders and Substance Abuse - Chronic Disease & Health Risks Prevention - Improved Health Literacy - Address Social Determinants of Health
