ExplanationPART V, SECTION B, LINE 5: The most recent three year cycle for community health needs assessment cycle in 2018 identified three priority issues for Lenoir County: Obesity- Exercise and Nutrition, Substance Abuse, and Access to Care. The full 2018 Lenoir County CHNA and Implementation Strategies are found by going to the Healthy Communities tab under the Minges Wellness Center at unclenoir.org or through https://www.lenoirwellness.org/healthy-communities/ In 2018, UNC Lenoir and other partners in Lenoir County along with 33 other eastern NC counties initiated an eastern NC community health needs assessment process that shifts the cycle to a regionally cohesive three year cycle on a 2018 timeline. This provides similar data collection and reporting structures to improve assessment process planning in the East for collaboratively impact strategies for the future. Regional eastern NC priorities from the 2018 CHNA have been established to include: access to care and health equity, healthy lifestyles and health behaviors, substance abuse and/or use, and mental health. The work involves community leaders from the region and in partnership with the Foundation for Health Leadership and Innovation and was funded in part through The Duke Endowment. A regional report as well as individual county reports will be also available at HealthENC.org. LENOIR MEMORIAL HOSPITAL, INC.: PART V, SECTION B, LINE 6A: THE CHNA WAS MAILED OUT TO COMMUNITY LEADERS LENOIR MEMORIAL HOSPITAL, INC.: PART V, SECTION B, LINE 11: LENOIR MEMORIAL IS NOT CURRENTLY INVOLVED IN THE ILLEGAL DRUGS AND DRUG ABUSE/SUBSTANCE AND YOUTH VIOLENCE/GANGS ISSUE BUT A GROUP OF COMMUNITY ORGANIZATIONS LED BY KINSTON PUBLIC SAFETY HAS FORMED A TASK FORCE TO WORK ON THIS ISSUE. LENOIR MEMORIAL HOSTED MEETING SPACE AND A BREAKFAST FOR LEADERS IN THIS REGARD TO GET THE INITIATIVE STARTED. YOUTH ACCESS AND USE OF GUNS MAY ALSO BE CLOSELY RELATED TO THIS ISSUE AND SAFE KIDS ENC WORKS TO PROVIDE THE COMMUNITY WITH GUN LOCKS AND GUN SAFETY EDUCATION. JOB OPPORTUNITIES WERE IDENTIFIED AS A TOP ISSUE IN THE COMMUNITY AND EVEN THOUGH LENOIR MEMORIAL RECEIVED A WORKFORCE DEVELOPMENT GRANT FROM THE GOLDEN LEAF FOUNDATION IN 2010 AND TRAINED 20 CERTIFIED NURSING ASSISTANTS FOR THE LOCAL WORKFORCE, LENOIR MEMORIAL DOES NOT HAVE ANY CURRENT PLANS IN THIS AREA OTHER THAN SERVING AS A TRAINING SITE FOR STUDENTS WHO MAY JOIN THE WORKFORCE ONCE THEY COMPLETE THEIR PROGRAMS. LENOIR MEMORIAL IS A MAJOR EMPLOYER IN LENOIR COUNTY AND WILL HIRE AS VACANT POSITIONS NECESSITATE. HOSPITAL LEADERSHIP PARTICIPATES IN A VARIETY OF ECONOMIC DEVELOPMENT COMMITTEES AND PROJECTS THROUGHOUT THE YEAR. DRINKING AND DRIVING CONTINUES TO BE SEEN AS A PRIORITY ISSUE IN THE 2017 CHNA. LOCAL LAW ENFORCEMENT AND STATEWIDE PROGRAMS HAVE INITIATED PROGRAMS IN OUR COMMUNITY SUCH AS BOOZE IT AND LOSE IT AND OTHER ROAD STOP PROGRAMS IN AN EFFORT TO ADDRESS THE PROBLEM. LENOIR MEMORIAL OFFERS SMOKING CESSATION PROGRAMMING THROUGH THE MINGES WELLNESS CENTER AND DOES NOT LIMIT THE PROGRAMS TO ADULTS ONLY. THE ISSUE OF CIGARETTE SMOKING AMONG PEOPLE UNDER AGE 18 HAS BEEN A DISCUSSION AT THE LENOIR COUNTY SCHOOL HEALTH ADVISORY COMMITTEE ON WHICH LENOIR MEMORIAL SERVES. IT WILL NOT BE A PRIORITY FOR LENOIR MEMORIAL SINCE SERVICES FOR CESSATION EXIST AND THE HOSPITAL SERVES AS AN ORGANIZATIONAL ROLE MODEL AS A TOBACCO-FREE CAMPUS. LENOIR MEMORIAL HOSPITAL, INC.: PART V, SECTION B, LINE 13H: SLIDING SCALE BASED ON % OF FPG LENOIR MEMORIAL HOSPITAL, INC.: PART V, SECTION B, LINE 15E: LIST THE WEBSITE WHERE THE INFORMATION IS PROVIDED AND OFFER BROCHURES EXPLAINING FINANCIAL ASSISTANCE. LENOIR MEMORIAL HOSPITAL, INC.: PART V, SECTION B, LINE 16J: BROCHURES ARE PLACED IN WAITING AREAS AND REGISTRATION AREAS THROUGHOUT THE FACILITY AS WELL AS COMMUNITY HEALTH CENTERS AND THE HEALTH DEPARTMENT.ExplanationTxt

PART I, LINE 3C:IN ADDITION TO DETERMINING ELIGIBILITY FOR FREE OR REDUCED-PRICE CARE USING INCOME LEVELS AND FEDERAL POVERTY GUIDELINES, PATIENTS MAY ALSO QUALIFY FOR FINANCIAL ASSISTANCE BASED UPON THEIR ELIGIBILITY FOR SUBSIDIZED HOUSING UNDER HUD GUIDELINES. ALTHOUGH THE HOSPITAL PREFERS TO BE ABLE TO DISCUSS OPTIONS FOR FINANCIAL ASSISTANCE WITH HEALTHCARE COSTS - INCLUDING POSSIBLE ELIGIBILITY FOR MEDICAID OR OTHER PROGRAMS - WITH PATIENTS, IF EFFORTS TO DO SO PROVE UNSUCCESSFUL, ELIGIBILITY FOR LMH'S FINANCIAL ASSISTANCE PROGRAM MAY BE DETERMINED BASED UPON PATIENT ADDRESS IN A SUBSIDIZED HOUSING COMMUNITY. COMMUNITIES MEETING THIS DEFINITION, AS WELL AS INCOME AND ASSET CRITERIA FOR RESIDENTS, ARE REVIEWED PERIODICALLY WITH THE LOCAL HOUSING AUTHORITY. ADDITIONALLY, PATIENTS FACING EXTENSIVE MEDICAL COSTS WHO DO NOT MEET STANDARD FINANCIAL INDIGENCE CRITERIA BASED UPON INCOME LEVEL MAY QUALIFY FOR DISCOUNTED CARE IF THEY ARE DETERMINED TO BE "MEDICALLY INDIGENT THAT IS, HAVING INCURRED COSTS OF CARE IN EXCESS OF 20 OF FAMILY GROSS INCOME, AND FOR WHICH PAYMENT WOULD REQUIRE LIQUIDATION OF THOSE ASSETS CRITICAL TO LIVING OR EARNING A LIVING. THEREFORE, FINANCIAL AND OTHER ASSETS MUST BE FULLY DISCLOSED BY PATIENTS SEEKING THIS DESIGNATION IN ORDER FOR A MEDICAL INDIGENCE DETERMINATION TO BE MADE. ADDITIONALLY, DISCOUNTS FOR PROMPT PAYMENT OF BALANCES ARE AVAILABLE FOR ALL UNINSURED PATIENTS, AND ARE TIERED BASED UPON THE AMOUNT OWED (LARGER DISCOUNTS ARE APPLIED TO HIGHER BALANCES). PATIENT ACCOUNT LIAISONS (PALS) AND FINANCIAL COUNSELORS SCREEN PATIENTS FOR FINANCIAL ASSISTANCE EITHER AT THE POINT OF SERVICE OR AFTER SERVICE HAS BEEN RENDERED. FINANCIAL COUNSELORS CONTACT PATIENTS AFTER THEY RECEIVE A STATEMENT FOR SERVICES TO DISCUSS PAYMENT OPTIONS INCLUDING FINANCIAL ASSISTANCE. PART I, LINE 7G: THE SUBSIDIZED HEALTH SERVICES PROGRAMS INCLUDED DIABETES, ENDOCRINOLOGY, HEMATOLOGY/ONCOLOGY, AND NEONATOLOGY. PART II, COMMUNITY BUILDING ACTIVITIES: LENOIR MEMORIAL PARTNERS WITH AREA ORGANIZATIONS TO PROVIDE COMMUNITY HEALTH PROGRAMMING, HEALTH COALITION WORK, AND IN-KIND LEADERSHIP TO AREA BOARDS, AND NON-PROFITS. LENOIR ALSO PARTICIPATES IN A MENTORING PROGRAM WITH SOUTH EAST ELEMENTARY SCHOOL. LENOIR PROVIDES MEETING SPACE FOR VARIOUS LOCAL COMMITTEES. PART III, LINE 3: The Organization's bad debt does not include any amounts attributable to those patients who applied for and received financial assistance under the Organization's Financial Assistance Policy. PART III, LINE 4: BAD DEBT EXPENSE FOR FY21 IS PRESENTED AS CHARGED NOT COST. PART III, LINE 8: THE MEDICARE COST REPORT WAS USED TO DETERMINE MEDICARE ALLOWABLE COSTS OF CARE. PART III, LINE 9B: THE HOSPITAL SENDS PATIENTS A STATEMENT EACH MONTH NOTING THE BALANCE DUE. INFORMATION IS INCLUDED ON THE STATEMENT REGARDING OPTIONS FOR PAYMENT INCLUDING FINANCIAL ASSISTANCE AND CONTACT NUMBERS. SELF PAY PATIENTS ARE OFFERED A PROMPT PAY DISCOUNT. NOTIFICATION OF THE DISCOUNT IS INCLUDED ON THE PATIENT'S STATEMENTS. FINANCIAL COUNSELORS AND PATIENT ACCOUNT LIAISONS ARE TRAINED TO INFORM PATIENTS OF FINANCIAL ASSISTANCE AS AN OPTION SHOULD THEY HAVE DIFFICULTY PAYING THEIR ACCOUNTS. THE PALS UTILIZE A SOFTWARE SYSTEM (RAMP) THAT SCREENS PATIENTS FOR POTENTIAL MEDICAID ELIGIBILITY AND FOR FINANCIAL ASSISTANCE. THE SCREENING IS BASED ON SOME PRELIMINARY QUESTIONS, AND THEN PATIENTS ARE SCHEDULED FOR AN INTERVIEW TO COMPLETE THE APPLICATION AND SUBMIT THE NECESSARY DOCUMENTS. PATIENTS RECEIVING LESS THAN 100 FINANCIAL ASSISTANCE WILL BE REQUESTED TO MAKE PAYMENT ARRANGEMENTS FOR THE REMAINING BALANCE. TELEPHONE CONTACTS TO DISCUSS ACCOUNT BALANCES WITH PATIENTS AND OFFER OPTIONS FOR RESOLUTION ARE ATTEMPTED ON MOST ACCOUNTS. THE HOSPITAL WILL OFFER PAYMENT PLANS TO PATIENTS THAT ARE ABLE TO PAY WITH A MINIMUM MONTHLY PAYMENT OF $50.00. PATIENTS ARE ALSO OFFERED ASSISTANCE WITH MEDICAID ELIGIBILITY APPLICATIONS. IF THE PATIENT IS NONRESPONSIVE AFTER RECEIVING THREE STATEMENTS (120 DAYS FROM DATE OF SERVICE OR LAST PAYMENT DATE), THE ACCOUNT MAY BE FORWARDED TO A COLLECTION AGENCY FOR FURTHER COLLECTION PROCEDURES. THE HOSPITAL DOES NOT UTILIZE PRACTICES SUCH AS GARNISHMENTS, LIENS, AND PERSONAL ATTACHMENTS AS PART OF THE COLLECTION PROCESS. PART VI, LINE 2: LENOIR MEMORIAL WORKS CLOSELY WITH THE LENOIR COUNTY HEALTH DEPARTMENT TO CONDUCT THE COMMUNITY HEALTH NEEDS ASSESSMENT FOR LENOIR COUNTY, OUR LARGEST POPULATION SERVED. OUR COMMUNITIES ARE SURVEYED WITH BOTH ENGLISH AND SPANISH SURVEY INSTRUMENTS. FORM 990, PART III, LINE 4A, PROGRAM SERVICE ACCOMPLISHMENTSCommunity Health Needs Assessment and Health ENC In 2019/2021, the latest cycle for completing community health needs assessment was initiated in Lenoir County by Lenoir County Health Department and UNC Lenoir Health Care. Lenoir County works within the Health ENC partnership in unifying regional process and timeline. Community input was obtained through survey process and input of community health leaders using the Lenoir County Alliance for a Healthy Community to collaborate and partner in establishing community priorities for the next three years. Community priorities were established as: + Wellness & Lifestyle + Immunizations & Infectious Diseases + Substance Abuse The full 2019/21 Lenoir County CHNA and Implementation Strategies as well as past assessments and strategies will be found by going to the Healthy Communities tab under the Minges Wellness Center at unclenoir.org or through https://www.lenoirwellness.org/healthy-communities/ Healthy Communities Community Portal Using the Healthy Communities Institute, a comprehensive health status database at www.lenoirwellness.org provides the latest Lenoir County demographics and leading health indicators. It allows the user to search and report on more than 200 health, economic, social, and well-being indicators, demographics, and comparisons with NC and national benchmarks. The database provides promising practices for community health interventions and allows the community to add local reports for a one stop inspection for local community health assessment. Community leaders have been informed of the free resource. The resource is provided to the community through a joint effort of UNC Lenoir and the Lenoir Hospital Foundation. Community Coalition Activities UNC Lenoir provides leadership and administrative support for community health coalitions. The Lenoir County Alliance for a Healthy Community met virtually and monthly for community networking and information sharing. Almost 20 different community organizations participate in the meetings. Heart Failure Program The Duke Endowment funded a three year project starting in 2019 supporting multiple key strategies to reduce avoidable heart failure readmissions and increase heart failure self-management in a high risk patient population. At the end of CY 2021 heart failure readmissions had been reduced to 11.6 % for persons age 65 and older. The work of the program this year included a partnership with Lenoir County EMS for an active community paramedic program with home visits. 172 patients completed the 31 day program. Patients completing the community paramedic program were able to reduce ED visits, reduce EMS calls and positively impact unnecessary readmissions. MedActionPlan was implemented in the second year of the project and in CY 2021, 184 participants were enrolled in the medication education plan portion with those having 100% improved medication knowledge. A community wide health promotion campaign, "Failure is Not an Option" continues with multimedia education. See ManageHeartFailure.org Stroke Prevention and Care The Stroke Program at UNC Lenoir Health Care is accredited by the Joint Commission Survey for Certification as a Primary Stroke Center. We were recertified for the third time in September of 2021. Stroke patients are followed using the COMPASS (Comprehensive Post-Acute Stroke Services) program for 30 days after discharge to their home from the hospital. The Stroke Program Coordinator follows patients post discharge and is with the patient at the follow up Neurology appointment. Health coaching, risk factor management, and assistance in accessing community resources for the patient and their caregiver is provided. The patient is also offered a community paramedic home visit free of charge. The program is a Wake Forest Baptist Telestroke site. We were awarded The 2021 American Heart Association Get with the Guidelines Gold plus Award, Target Stroke Honor Roll Elite, and Target Type 2 Diabetes Honor Roll for excellence in meeting stroke care quality metrics. Economic Development Cash donations included support and sponsorships for Community Council for the Arts, LaGrange Arts Council, NC East Alliance, Downeast Wood Ducks, BBQ Festival on the Neuse, Junior Lenoir Leadership/Lenoir County Education Foundation, Neuse Regional Library, ECU College of Nursing 5K. Community Donations Cash donations included support for Friends of the Homeless Shelter renovations, Caswell Center Operation Santa Claus, Shady Grove UMC Cystic Fibrosis Walk, and Lenoir County American Cancer Society Relay for Life. Kinston/Lenoir County Farmers Market UNC Lenoir assisted underserved residents of the community having diabetes with access to local fresh fruits and vegetables through 50 incentive Rx coupons within program in partnership with Lenoir County Cooperative Extension. Additionally, UNC Lenoir provided several onsite cooking demonstrations by a dietitian.
Maternal/Child Health Initiatives- "Baby Friendly" practices such as "rooming in"skin to skin" for mom and baby are Standard of Care. Skin to skin education is given prior to birth and discussed with moms. Skin to skin provides quicker thermoregulation of the newborn, stabilization of blood glucose levels, decreases newborn crying and maternal anxiety, and promotes a positive breastfeeding experience. - Breastfeeding support is provided for those moms who choose to breastfeed. Breastfeeding is initiated within the first hour and encouragement given for breastfeeding on demand. Proper bottle feeding techniques, hunger ques and feeding patterns are taught to those moms that choose to formula feed. Feeding logs are encouraged for all new parents, to help transition parents into their new roles and responsibilities. - Delayed cord clamping is performed at the discrepancy of the infant's transition phase. Delayed cord clamping leads to multiple positive long-term effects on the neonate, including; increased hemoglobin, decreased anemia, increased ferritin levels, increased urine output, and decreased occurrence of heart murmurs. - As advised by the American Academy of Pediatrics and the American Congress of Obstetrics and Gynecologists (AAP & ACOG) all infants receive Vitamin K within one hour with parental consent. Infants also receive eye prophylaxis to prevent gonococcal opthalmia neonatorum as recommended by the CDC within one hour of birth in conjunction with parental consent. - Participating in current collaborative with the Perinatal Quality Collaborative of North Carolina to decrease opioid use disorder impact on mothers and babies. This will focus on screening women with a validated tool prenatally and upon hospital admission for substance abuse disorder and assisting them to receive needed resources which includes a safe plan of care on discharge. Updated management for identification and treatment for the newborns has been implemented. - A series of newborn screenings are performed when appropriate and include; Critical Congenital Heart Defects (CCHD), jaundice, hearing, and a metabolic screening. Approximately 18 out of every 10,000 babies are born with a critical congenital heart defect (CCHD). CCHD is life threatening and requires intervention in infancy. CCHD is not always detected prenatally or upon exam in the nursery, as a result, some infants with CCHD are discharged from the nursery to home, where they quickly decompensate. To improve the early detection of CCHD, the Secretary of Health and Human Services (HHS) recommended that CCHD screening be added to the uniform newborn screening panel. Jaundice is an increase in bilirubin levels and treated with phototherapy. Hearing screenings are routine practice to help catch hearing loss early on and avoid any speech or developmental delays. Metabolic screenings are obtained and sent to the state lab in order to quickly identify any possible metabolic disorders of the newborn. - OBH (obstetric hemorrhage) is a major cause of maternal death and morbidity in North Carolina. The FBC is working with the perinatal quality improvement teams to identify and promptly manage OBH. All obstetric patients seeking care at UNC Lenoir Health Care are assessed for OBH (obstetric hemorrhage) risks on admission, at complete dilatation and at delivery. Intervention guidelines for the optimal response of the multidisciplinary team in the event of obstetrical hemorrhage (OBH) have been implemented. - Oral and written postpartum education on the topics of postpartum hemorrhage, preeclampsia, and postpartum depression is emphasized so that patients are aware of the signs/symptoms to look for and know when to seek immediate medical care. - Focused educational rounding is performed to provide open discussion learning opportunities for parents and educational videos are encouraged throughout the admission stay with topic focus on newborn care and safety. Signs and symptoms are reviewed of when to seek immediate medical care or call 911 for infant emergencies. - Community high risk OB providers group meets regularly at hospital. - A Parenting Education station with dozens of free educational resources is maintained in the FBC waiting room to support families in best practices for parenting. Support Groups Covid-19 impacted in-person support group meetings on location this period. The Parkinson's Support Group and Weight Management Support Group met off-site with 308 persons attending. Emergency Preparedness UNC Lenoir participates in the Local Emergency Planning Committee (LEPC); the Eastern Healthcare Preparedness Coalition and the Mid Carolina Regional Healthcare Coalition for planning, coordinating, and executing disaster readiness and drills. UNC Lenoir participates in the North Carolina Emergency Management Domestic Preparedness Region 2 (DPR2) emergency preparedness committee for 10 counties which service to develop and expand regional prevention, preparedness, response, and recovery capabilities for all hazards, both man-made and natural. UNC Lenoir also participates in the NC Hospital Emergency Management Council which is comprised of healthcare emergency managers from across the state and provides guidance for the N.C. Healthcare Association and N.C. Emergency Management. UNC Lenoir participates in the International Association of Healthcare Security and Safety. A number of free physical activity oriented educational sessions are offered throughout the year to the community by exercise specialists. Screening Events Provided early detection and screening events of diseases/ disorders/risks including colorectal cancer (1), mammogram support (18), lung cancer (10) and blood pressures (1,477). Cancer Center Volunteer Lay Navigation Once a diagnosis of cancer is received, patients often feel overwhelmed. Volunteer lay navigators, supervised by the social worker in the Cancer Center, help patients identify and remove barriers to care while linking them to available cancer support resources. The Lay Navigator sees each new patient in consult and makes supportive phone calls to patients recently finished with treatments. Lay Navigator made over 130 supportive phone calls in FY 2021. Over 149 in person volunteer hours were donated by the Lay Navigator in FY 2021.
Cancer Center Program OutreachThe Cancer Center provides an on-site free food pantry for patients and their caregivers. Patients that are unable to eat certain foods or go grocery shopping due to fatigue, nausea, and other symptoms related to their oncology treatments are welcome to any items in the pantry. With the input of the oncology dietitian, many items can be better managed by the patient undergoing cancer treatment. Toiletry items are available also. The community provides numerous donations of items, as well as monetary donations to be used for needed purchases, throughout the year. The Cancer Center food pantry was included as a recipient of UNC Lenoir Health Care's 2020 Festival of Food Drive and every patient on treatment for radiation therapy was provided with a bag of food. Despite ongoing challenges as a result of COVID, staff were able to resume the Ladies Cancer Support Group meetings in 2021. This group meets onsite at the UNC Lenoir Health Care Cancer Center on the second Thursday of every other month. All females in any stage of their cancer journey are invited to attend these group meetings. Meetings were held in 2021 during the following months: March (7 participants) and May (4 participants). The UNC Lenoir Health Care Cancer Center continued the work of increasing community education and awareness about HPV and the HPV vaccine through a grant with the American Cancer Society. A healthcare provider education event was held in April 2021 at Kinston Community Health Center to provide an update on the new age requirement guidelines for vaccination. 360 reminder cards were mailed to age-appropriate Kinston Community Health Center patients in the fall of 2020. In 2021, many community events and activities were still on hold or cancelled due to the ongoing COVID pandemic. In February 2021, Joy Shortell of Art 105 and staff from the Cancer Center filmed a virtual flow art class. The Cancer Center provided patients with art supplies in a kit they can use while watching the instructional video. Donna Potter, RN, OCN, nurse navigator, gave educational talks for the City of Kinston employees on colorectal cancer awareness and screening in June 2021. In 2020, the UNC Lenoir Health Care Cancer Center was included in LIFT- Lessening the Impact of Financial Toxicity, which is a study and grant through the National Cancer Institute and UNC Lineberger Cancer Center in Chapel Hill, focusing on "financial toxicity" in oncology care. Financial toxicity is "a term used to describe the financial burden that healthcare places on patients and their families" (Wheeler, Rosenstein). Staff, will be serving as financial navigators throughout this study. Financial navigation services will be available to all patients receiving cancer care at UNC Lenoir Health Care. Patients experiencing financial burdens related to their cancer treatment will be able to make an appointment with a financial navigator at the UNC Lenoir Health Care Cancer Center, who will be able to help patients find financial resources and services. Donna Potter, RN, OCN and Meagan Evans, MSW, LCSW, OSW-C are serving as financial navigators at UNC Lenoir Health Care for this study.
