| Explanation | Part I Line 2: We are a charitable organization ministering to those in need. All monetary grants and in-kind distributions are made to individuals who indicate financial need primarily in compliance with attestations of income levels below those required by federal and state food bank programs. Clients must also provide evidence of residence for ongoing help as per our by-laws we primarily serve those in our parish area. Monetary grants crisis financial assistance are typically provided only to avoid utility shut-off rent eviction and similar situations. Any such cash payments are made directly to the utility provider landlord or transport or medical provider after verification of need with that provider. We do not give cash or checks to clients thereby safeguarding that funds are used as intended. Records of our financial and food assistance are maintained in an on-line system developed for those purposes. These records are updated at each visit by or to a client. Our practices track and significantly limit the amount of assistance any one client can receive as well as limiting the times they can re-apply for further assistance. Therefore no individual client received aid in excess of $2000 total within the year with one exception this year when we spent $20,000 for a van equipped with a lift for a handicapped individual. Part III col b: Line 1 food distribution: number of recipients came directly from our Pantrytrak system numbers given by clients at each distribution. Line 2 financial aide number of recipients taken from our Servware system at time help was given with a multiplier for typical family size. | Part VI, Line 7a | Our organization has around 43 members variation depending on activity level all volunteers all of whom are invited to attend our regular monthly meetings. The members nominate and elect by vote the president who then can appoint or re-appoint the other officers. None of the officers may be family members of the president. |

| Explanation | Our original return was mailed by USPS on 1 14 22. It was returned by IRS with LTR 2694C 6 9 22 Ref 0422826892 informing us to resubmit electronically. I am a volunteer acting as Treasurer for the organization it's the first time I have filed a 990 and I missed the instruction that required electronic submission. I am re-submitting as timely as possible I needed to seek out a software vendor and respectfully request that no penalties be assessed.
