DescNorton Community Hospital (NCH) is located in Norton, Virginia. Our 129-bed acute-care facility has served Southwest Virginia and Southeastern Kentucky since 1949. As the largest healthcare facility in the coalfield region, NCH provides a wide array of services through highly trained physicians and support staff. NCH is a Virginia nonprofit corporation.During FY20, MSHA purchased CHCFs 49% membership resulting in MSHA being the sole member. Additional hospitals located in Wise Co. are Mountain View Regional Hospital (MVRH) and Lonesome Pine Hospital (LPH) which are reported on Wellmont Health System (WHS)s 990. In February 2021, NCH merged with these two hospitals in order to optimize healthcare in Wise County. This will be the final separate return for NCH. Norton Community Physician Services will have a member substitution from MSHA to WHS as its parent.Prior to the February merger, NCH was the sole owner of Dickenson Community Hospital (DCH), a critical access hospital located in Clintwood, Virginia. NCH and DCH are part of the Ballad Health network, created when Mountain States Health Alliance and Wellmont Health System merged February 2018. DCH will also have a member substitution from NCH to WHS as its parent. NCH is recognized for its outstanding inpatient rehabilitation services, an osteopathic internal medicine residency program, black lung treatment services, high-quality imaging technology, and many other service lines. Improving the Communitys Health Status:NCHs black lung clinic offers the regions only program approved and federally certified by the U.S. Department of Labor for the Federal Black Lung program. We have eleven approved providers for our black lung program. Our black lung clinic offers disease screening/diagnostic testing and pulmonary rehabilitation services. Black lung disease, or coal workers pneumoconiosis, is a lung disease that results from breathing in dust from coal graphite, or man-made carbon over a long period of time. We provided just under 1,900 black lung procedures this year. NCH offers a free support group, The Better Breathers Club, for people with COPD, asthma, chronic bronchitis, lung cancer and other pulmonary diseases. The club meets monthly where members can receive valuable information and develop skills to help better manage breathing difficulties and improve their quality of life. Improving Access to Healthcare Services:Norton Community Hospital continues to subsidize a rural physician clinic to provide some relief of the physician shortage in our area. Our FY21 subsidy was just under $520,000. The primary care physician ratio in our county (number of residents to 1 primary care physician) is 46% higher than the overall state ratio. Recruiting physicians to a rural county such as ours is often challenging due to a myriad of factors, such as geography, economics, culture and education. Geographically, rural communities are often far removed from suburban and urban centers that provide access to educational, cultural and economic opportunities. These limitations influence the relocation decision of the physician candidate and his/her spouse/children to locate to a rural area. The American Medical Colleges (AAMC) issued a press release dated June 11, 2021 stating that the United States could see a shortage of up to 124,000 physicians by 2034 including shortfalls in both primary and specialty care. The U.S. population is estimated to grow by nearly 11%, with those over age 65 increasing 42.4% by 2034. Much of the increased demand comes from a growing, aging population. The aging population will also affect physician supply since more than 2 out of every 5 of all currently active doctors will be older than 65 in the next decade and likely to retire. AAMCs president and CEO David J. Skorton, MD stated: The COVID-19 pandemic has highlighted many of the deepest disparities in health and access to health care services and exposed vulnerabilities in the health care system. The pandemic also has underscored the vital role that physicians and other health care providers play in our nations health care infrastructure and the need to ensure we have enough physicians to meet Americas needs. AAMC points out that data conducted in 2019 indicated that an additional 31,758 physicians would have been needed if underserved populations utilized the same health care services as insured individuals. The data looked at people in non-metropolitan areas and people without insurance compared to people living in metropolitan areas. Furthermore, the study indicated nearly half of the shortage was in the South. NCH spent almost $11,000 during FY21 to recruit and retain physicians. NCH recruitment efforts are based on documented community need. NCH is located within a MUA (medically underserved area), as designated by the U.S. Health Resources & Services Administration (HRSA). MUA designation indicates an area as: having too few primary care providers, having a high infant mortality, a high poverty rate or a high elderly population.Desc

DescBenefits of an Integrated Health System:Since 2010, more than 130 rural hospitals have closed in the United States, according to the North Carolina Rural Health Research Program. Studies have shown rural communities that lose their hospitals struggle with reductions in access to physicians, healthcare services and in overall health status. Through Ballad Health, the scale and ability for larger and more financially solvent hospitals to subsidize losses in smaller rural communities is a clear and convincing foundational public benefit. It is clear the steps Ballad Health has taken to reduce overhead cost, realign services, invest capital and prioritize high quality have led to the sustainability of services - expanding, rather than reducing access, as has been seen in rural regions throughout the nation. Ballad Health invested more than $200 million to adopt a common information technology platform for all their merged hospitals and care sites. With Epic as the single technology platform for Ballad Health, patient data will be controlled by patients and their providers, and it will be more secure than ever before. New services will be available to health information, transparency in pricing and, most importantly, better coordination between healthcare providers, regardless of where the providers are or what data systems they use. Mountain States Health Alliance (Mountain States) is one of only 29 organizations in the country participating in the Centers for Medicare & Medicaid Services (CMS) new initiative, Accountable Health Communities Model (referred to as AHC Program), aimed at improving the health of eligible Medicare and Medicaid beneficiaries. The Southwest Virginia Accountable Health Communities Program (AHC) is a collaboration between community groups and Ballad Health that seeks to improve the overall health of Medicare and Medicaid beneficiaries by helping address patients health-related social needs. As part of the program, Medicare and Medicaid patients in Southwest Virginia are screened for health-related social needs when they visit a Ballad Health facility. Once those needs are identified, Ballad Health tries to link those patients with resources and programs within their communities to resolve those needs. On March 10, 2020, Ballad Health executed its disaster plan in response to the coronavirus disease 2019 (COVID-19) global pandemic. This included the activation of its Corporate Emergency Operations Center to coordinate efforts across the system, including NCH, to rapidly plan for, and execute, ongoing response to the issues resulting from the pandemic. In May 2020, Ballad Health partnered with Premier, Inc. on a new program to ensure our team members and patients are always protected through the availability of personal protective equipment, such as gloves, gowns and masks. This program focused on investing in domestic manufacturers to enhance supply chains for essential medical products and PPE. This program led to Ballad Health acquiring a minority stake in Prestige Ameritech, the largest domestic manufacturer of face masks, including N95 respirators and surgical masks. PPE products critical for the daily operations of health systems are overwhelmingly sourced overseas. The risks associated with this overreliance on overseas providers came into focus twice in 2020, as the supply chain was disrupted when an overseas provider failed to meet FDA requirements for sterile gowns, and again as the COVID-19 pandemic swept across the globe. Prestige Ameritech operates primarily in the United States, where it obtains its raw materials. Ballad Health provides numerous services to improve the health of our communities. The following services are provided by the Ballad Health entity but funded by the various hospitals and other divisions within our health system. The funding levels by NCH for these services are reflected below:The Ballad Health parish nurse program is designed to provide holistic ministry and special promotion services within the faith community. The parish nurse program assists individuals in gaining optimal mental, physical and spiritual health by complementing the ministry provided by pastors and other lay ministers. Some of the services and programs provided include screenings for blood pressure, cholesterol, diabetes, glaucoma and overall fitness, classes for all age groups offered on health related issues (nutrition, exercise, parenting, care for elderly parents, stress management, CPR), hospital, nursing home and home visits for assessment of health care needs, referrals to appropriate community resources and the creation of health tips in congregational newsletters, bulletin boards and other forms of communication. Currently, there are dozens of churches in Northeast Tennessee and Southwest Virginia that are serviced by the Ballad Health parish nurse program. NCH funded $2,005 for this program in FY21. Ballad Healths Nurse Connect is a toll-free line that connects community members with experienced nurses around the clock. The nurses provide expert medical advice anytime, day or night, make referrals to a new primary care provider with a location and hours convenient to the community member and referrals to a physician specialist when they need advanced local medical care. They also provide health information and resources, including health screenings and immunizations, and make referrals to urgent care clinics to see a doctor near the community member the same day. NCH funded $15,211 toward this popular service during FY21.During FY21, Ballad Health unveiled a new name for its contracted emergency medical air service: The Highlands Emergency Air Rescue & Transport service, or HEART. Ballad Healths air transport service is conducted in partnership with Med-Trans Corporation. Ballad Health maintains four bases that cover the 21-county region served. Ballad Health also provide staffing, physicians, and medical supplies to the Commonwealth of Virginia for the Virginia State Polices (VSP) Med Flight air ambulance service. VSP Med Flight maintains its base in Abingdon, VA. NCHs contribution this year for the air ambulance services that are especially critical in rural areas was $12,811.Ballad Health is focused on investing in various initiatives to improve population health-the overall health and well-being of the residents living in the Appalachian Highlands. In FY19 Ballad Health established a new department, Population Health, to focus on these initiatives. NCH invested $55,854 in FY21 toward this initiative that supplements the many other forms of community benefit provided by our health systems hospitals.Desc

Form And Line ReferenceFormAndLineReferenceDescExplanationExplanationTxt
Facility: - Part V, Section B, Line 3jTo understand each community's individual needs, Ballad Health conducted a Community Health Needs Assessment (CHNA) for each Ballad hospital to profile the health of the residents within its service area. Throughout the CHNA process, high priority was given to determining the health disparities and available resources within each community. Community members from each county met with Ballad representatives to discuss current health priorities and identify potential solutions. During FY21, NCH was operating under the three-year Community Health Needs Assessment (CHNA) plans approved by its board of directors at the end of FY18. Priorities established for their CHNA were determined by the most significant health needs of the community. Ballad Health hospitals, including NCH, conducted their fifth CHNA this tax period with board approval and publication occurring at the end of FY21. For FY21 CHNAs, Ballad Health utilized a mixed-methods approach for primary and secondary data collection to gather regional information to inform the 2021 community health needs assessments. The secondary data collection entailed the compilation of secondary data pertaining to agreed upon metrics and indicators from an array of verified sources. The primary data collection component involved both a key stakeholder survey and key stakeholder focus groups. Findings from both research methods were used to prioritize the needs of the community served by each Ballad Health facility and determine priority focus areas for future improvement efforts. Analysis of secondary data for Wise County, findings from the key stakeholder survey, and the perspectives of diverse key stakeholders led to the prioritization of community issues for Norton Community Hospital. For Norton Community Hospital, the three priority areas for future improvement efforts that were selected by key stakeholders in Wise County are: -Substance Abuse-Chronic Disease-Access to Care
Facility: - Part V, Section B, Line 5The community health needs assessment process was guided by the Mobilizing for Action Through Planning and Partnerships (MAPP) model, with an understanding that aspects of the model may have to be adapted due to the purpose of the assessment for Ballad Health and constraints related to the COVID-19 pandemic. In coordination with the MAPP model, concepts from both Community-Based Participatory Research (CBPR) and the Arkansas Center for Health Improvement (ACHI) Community Health Assessment Community Health Assessment Toolkit were also utilized for the assessments. Following guidance from the MAPP model, a Key Stakeholder Survey was designed with the primary aim of identifying the most-pressing community issues. In aligning with principles of CBPR, the key stakeholder survey was designed to allow key stakeholders to frame community issues in their own words through the use of open-ended questions. In addition to the identification of community issues, the key stakeholder survey was also designed to discern why survey respondents believed the community issues they selected had the greatest effect on the overall health and wellbeing of their community. Questions related to ideas and suggestions for improvement efforts, gauging the success of efforts after the previous community health needs assessments, and community struggles related to the COVID-19 pandemic were also included in the survey. Independent focus groups were conducted for each Ballad Health facility in order to provide specific and unique information for each community being served. The MAPP model and questions from the key stakeholder survey were used to guide the development and construction of the focus groups. Because the key stakeholder survey primarily dealt with the identification of community issues, the focus groups were primarily designed to prioritize community issues identified through the key stakeholder survey and discuss actionable items around how to best address these community issues. Questions related to root causes of community issues, the current state of resources to address community issues, needed resources to initiate improvement efforts and be successful, and community struggles related to the COVID-19 pandemic were also included in the focus group facilitation guide. The key stakeholder focus groups were conducted virtually via WebEx and were one hour and thirty minutes in length. For Norton Community Hospital, there were fifteen (n=15) focus group participants. Similar to the key stakeholder survey representation, focus group participants represented an array of different sectors in Wise County, which included: the school system, businesses, government, the health care system, health departments, faith based organizations, and a diverse group of community based organizations. For stakeholders who were not able to attend the focus group in real-time, blank facilitation guide templates with questions concerning the three priority areas identified by the focus group participants were sent to them immediately after the conclusion of the focus group. This allowed key stakeholders who were not able to attend the focus group in real-time to still provide input and the hospital to ensure the involvement of diverse stakeholders. Activities associated with the June 2021 assessment took place from summer of 2020 through the spring of 2021. The assessment activity including focus groups & surveys were used to develop the hospital implementation plan that was completed fall 2021.
Facility: - Part V, Section B, Line 6aEach hospital within Ballad Health completed a CHNA. NCH'S CHNA was conducted with all Ballad Health hospitals to include: Bristol Regional Medical Center, Hancock County Hospital, Hawkins County Memorial Hospital, Greeneville Community Hospital, Holston Valley Medical Center, Johnson City Medical Center (includes Niswonger Children's Hospital and Woodridge Hospital), Franklin Woods Community Hospital, Indian Path Community Hospital, Lonesome Pine Hospital (including Mountain View campus), Johnson County Community Hospital, Johnston Memorial Hospital, Norton Community Hospital, Dickenson Community Hospital, Russell County Hospital, Smyth County Community Hospital, Sycamore Shoals Hospital, and Unicoi County Hospital.
Facility: - Part V, Section B, Line 11During the year, NCH began to focus on its CHNA priorities as identified in its FY21 CHNA report. NCHs primary areas of focus includes: chronic disease, access to care and substance abuse. Many additional community needs exist in our region. It is fiscally impossible for a hospital to address every health need in a community, which is why the CHNA process is used to identify and prioritize areas of focus. A thoughtful CHNA evaluates overall community health needs to determine which ones the hospital can best influence in a positive way. Consideration is given to other organizations in the hospitals geographic area that already offer services addressing specific health needs. In some cases, it is best to simply support an identified health need through a financial donation to another nonprofit organization skilled in certain areas: teen pregnancy, dental health, fighting homelessness, etc. Hospitals also lend support to other nonprofit organizations by serving on their boards, committees, and assisting with fundraising efforts. Ballad Health made financial contributions to other nonprofit organizations providing community services that support the hospitals CHNA. For example, Ballad Health contributed to Mountain Empire Older Citizens, United Way of Southwest VA., Feeding America Southwest VA,Communities in Schools Southwest VA and others. The hospital continued low-dose lung screenings this year to enable earlier detection of lung cancer. We provide the low-dose CT scans free of charge to community members. The hospital continued its practice of providing medications and supplies to area rescue squads this year.

Part I, Line 3c - Charity Care Eligibility Criteria (FPG Is Not Used)Financial assistance approval can apply to an assortment of patients such as those who have exhausted their Medicaid/TennCare benefits, those who qualified for Medicaid/TennCare after the date of service, deceased patients with no estate or assets, uninsured patients, and underinsured patients. While Ballad Healths qualifications for financial assistance is based on federal poverty guidelines, asset values may also be used to determine financial assistance eligibility. Unique circumstances may be assessed on case-by-case basis. Charity approval covers all dates of service for the patient when they are approved and there is no limitation or cap on the amount of charity that they can receive. Ballad Health hospitals do not stop approving financial assistance for patient accounts if a hospitals charity write-offs exceed the hospitals charity budget. All Ballad Health hospitals provide an uninsured discount; the current uninsured discount is 85%. In addition to the uninsured discount, many patients will further qualify for additional financial assistance. All patients seeking financial assistance must submit an application for financial assistance and submit documents in support of the information on the application, unless specifically excluded according to our policy guidelines. Medicaid eligible patients will qualify for 100% financial assistance and not be required to complete the required documentation when: a) Medicaid eligibility requirements are met after the service is provided, b) non- covered charges occur on a Medicaid eligible encounter, or c) benefits have been exhausted. Deceased patients with no estate also qualify for 100% financial assistance.Financial assistance determinations may be retroactive for all outstanding balances. In addition, we have a number of programs with special discounts such as lactation consultation services; oncology treatment regimens; enrollment in various community programs, and prescription drugs filled post-discharge.
Part I, Line 7 - Explanation of Costing MethodologyThe cost to charge ratio (worksheet 2 "ratio of patient care cost to charges") was used to calculate line 7a financial assistance (charity care) cost. Our cost accounting system was used to determine losses from Medicaid/TennCare reported on line 7b, with the exception of home health and our physician clinic. We used cost to charge ratios for home health and our physician clinic because their data is not available in our cost accounting software. Line 7e community health improvement includes costs that are taken directly from departmental operating reports or expenses specific to a community health event, with no additional overhead included in the cost. Line 7f health professions education is comprised of internships (primarily medical residents, nursing, pharmacy, EMT, respiratory therapy, public health, physical/occupational therapy, and students from other allied health programs) with local schools and universities, allowing their health profession students to receive hands-on training in a hospital setting. NCHs filed Medicare cost report is the source for cost and reimbursement reported on line 7f for our Medicare-approved family medical and internal medicine resident program. For line 7g subsidized health care services, we used our cost accounting system because we established, standard costing reports for these services. There is one exception where we do not use our cost accounting system. We have a physician clinic in a rural, low income area that is subsidized by the hospital. NCH owns and operates the clinic and subsidizes the clinics losses ($519,782). We are careful to ensure no double counting of cost although we have many service lines within our hospital that lose money, we do not report services that hospitals are required by state licensure to provide, routine services or ancillary services. Line 7h research represents NCHs expense allocation from Ballad Health for research. Line 7i cash and in-kind contributions include cash disbursements and in-kind donations of medications to local nonprofit rescue squads and fire departments. In-kind donations of medications are based on our actual cost for these items.
Part III, Line 2 - Methodology Used To Estimate Bad Debt ExpenseSelf-pay balances include accounts after payments and contractual adjustments (discounts) have been applied from all third-party payers such as Medicare, Medicaid, commercial insurers, and others - generally leaving the patient responsible for any remaining deductible and/or co-payment. Other self-pay accounts are from patients with no insurance or other third-party coverage. Under Ballad Healths system-wide self-pay policy, any patient who has no insurance and is ineligible for any government assistance program received an 85% discount. Many self-pay patients will further qualify for financial assistance (sometimes referred to as charity care) if they provide the financial information, we need to deem them eligible. After the normal collection process has indicated an account is uncollectible, NCH writes the account off to bad debt. The hospital's overall self-pay accounts receivable balance is evaluated on an ongoing basis to evaluate the age of accounts receivable, historical write-offs and recoveries and any unusual instances (such as local, regional or national economic conditions) which affect the collectability of receivables.
Part III, Line 3 - Methodology of Estimated Amount & Rationale for Including in Community BenefitOur primary external collection agency historically estimated that approximately 37% of NCHs bad debt would have qualified for financial assistance if patients had provided a financial assistance application and required documentation. Pursuant to the merger, NCH has begun to score accounts using a presumptive eligibility tool. This tool utilizes various data points for a proprietary algorithm operated by an outside vendor to provide an individuals score. We have many instances of patients with large account balances and no health insurance coverage that we believe would qualify for financial assistance. Although patients are encouraged to apply for assistance, many will not do so. NCH would prefer for patients to submit completed financial assistance applications given that historical data clearly indicates that most uninsured patients and many underinsured will qualify for financial assistance under our program. Without a completed application, these are recorded as bad debt instead of charity care.
Part III, Line 4 - Bad Debt ExpenseBallad Healths audited financial statements included a footnote on page 17 that describes bad debt. NCH is included in the June 30, 2021 audited financial statements of Ballad Health (attached).
Part III, Line 8 - Explanation Of Shortfall As Community BenefitExcluding Medicare losses reported in Part I subsidized health, the Part III Medicare allowable costs are reported using NCH's filed Medicare cost report (C/R). The C/R uses a cost to charge ratio based on a step-down allocation methodology. We believe Medicare losses should be allowed as a reportable community benefit, similar to governmental programs such as Medicaid. As a participating provider in the Medicare program, hospitals are required to provide the full regimen of care for the Medicare population. There are a number of care regimens that are compensated by the Medicare program at levels below cost. Therefore, it is only logical to allow hospitals to report these uncompensated services as a community benefit. By making this change, nonprofit providers will be encouraged to continue important care delivery models for our aging population in spite of the fact it may be economically injurious.
Part III, Line 9b - Provisions On Collection Practices For Qualified PatientsRequests for financial assistance are evaluated using established guidelines, while allowing for unique financial circumstances - for example, medically indigent patients with catastrophic medical costs that would threaten the patients household financial viability. When a patient requests financial assistance or when an application has been received, the patient's account is placed in a hold status to prevent further collection activities until financial assistance eligibility is determined. All Ballad Health hospitals comply with IRS 501(r) regulatory guidelines. Our collection policy clearly states that all patients are treated equally - with dignity and respect. We ensure that outside collection agencies used by our hospitals adhere to our billing and collection guidelines. Our hospitals' collection program includes communicating expected financial responsibility prior to service. Our hospitals provide assistance to help underinsured and uninsured patients determine sources of payment for medical bills and to help patients determine eligibility for programs such as Medicaid or TennCare. After insurance benefit verification, the hospital bills insurance carriers. If the insurance carrier denies payment of the service/procedure as non-covered or the patient has exceeded their maximum benefits, the service/procedure will qualify for the uninsured discount.Financial counselors are available to discuss financial assistance with patients and their families. Our hospital provides a number of payment options:-a pre-service discount may be offered-a discount in excess of established discounting rates may be granted for catastrophic high dollar accounts-our hospital accepts all non-contracted and out-of-network payers and will make attempts to work with these payers regarding appropriate reimbursement and billing to their members-payment arrangements are available so long as the account is not with a collection agencyReasonable efforts are made to determine if a patient is eligible for financial assistance - see Schedule H, Part VI, line 3 for information on how patients are informed about our financial assistance policy.
Part VI, Line 2 - Needs AssessmentFocusing on population health improvement and associated priority metrics allows Ballad Health to further engage the efforts of its hospitals in partnership with communities in our service areas. It has helped Ballad to better identify health disparities that appear across the individual communities and has helped Ballad to prioritize issues that are most important in each hospitals community. Engaging local community organizations expands partnerships so that organizations work together more to address community health needs. Ballad Health uses a comprehensive process to gather input for and continues to evolve its population health plan. Because our hospitals are located in a region with many chronic disease challenges and high levels of health-related social risks, Ballad Healths goal is to target population health issues to make lasting improvements. Ballad conducts ongoing interviews, focus groups and meetings with external groups, including the regional health departments, United Way agencies, chambers of commerce, schools and community organizations, the regional accountable care community leadership council, as well as internal groups such as our population health and community benefit committee of the Ballad Health board of directors, the Ballad Health population health clinical committee, social needs council, grant advisory committees and our hospital community boards in the creation and on-going implementation of its population health plan.
Part VI, Line 3 - Patient Education of Eligibility for AssistanceConsistent with the Ballad Health financial assistance policy, NCH communicates with and provides education to our patients through various avenues regarding governmental assistance programs and hospital financial assistance. Various educational and application documents related to obtaining financial assistance are widely available at NCH and all documents are available on the Ballad Health Website. Printed financial assistance educational materials are part of each registration packet and posters are displayed in highly visible areas of the hospital. Our financial assistance policy and documents are available in our Emergency Department and admitting areas. We are also happy to mail all documents to patients. We offer a plain language summary and all of our documents are available in English and Spanish. Financial assistance information is available during pre-registration, registration and/or during financial counseling. We offer governmental program eligibility representatives to assist patients in securing eligibility for Medicaid or TennCare, federal disability and other governmental assistance programs. Additionally, if a patient or community resident expresses an interest in the ACA-healthcare exchange, our representatives have the qualifications and experience to assist them through the entire process. Our financial counselors offer financial assistance applications to patients who do not qualify for governmental assistance programs and are unable to pay for some or all of their healthcare. All patient billing statements have verbiage discussing financial assistance along with contact information. Our last letter to the patient displays the plain language summary. In all oral correspondences with the patient, if it is identified they cannot meet payment requirements on their account, financial assistance is discussed as an option.Applicants are notified of financial assistance determination in writing.
Part VI, Line 4 - Community InformationNCH is located in Norton, Wise County, which is in rural Southwest Virginia. Southwest Virginia (SWVA) is located within the Appalachian Mountains, specifically the Blue Ridge Mountain Region. SWVA is considered rural and much different than the rest of the state. SWVA has been and remains a major coal mining region and has a high rate of uninsured adults. Based on research from our Community Health Assessment, Wise County's population is 44,158. The population projection projections for Wise County over the next five years show that the county will likely experience little to no population growth overall. However, the age 65+ population moves to 21.9% in 2026. The aging population of the county presents opportunities for earlier identification and better management of health conditions that oftentimes affect elderly populations in rural areas. The County Health Rankings estimates Wise County's median household income at $38,000 compared to the State of Virginias median income of $72,600. 28.7% of Wise County's children live in poverty compared to 13.3% statewide. Wise County is a rural community. This is a significant contributing factor in influencing health outcomes in a population. Many rural residents must travel a greater distance to access different points of the health care delivery system;however, due to geographic distance, sometimes extreme weather conditions, lack of public transportation and challenging roads, rural residents may be limited, and in some instances, even prohibited from accessing health care services. There is also an additional risk of injury-related deaths in rural communities. Wise County's premature (age adjusted) mortality is 81% higher than the state rate. Recruiting physicians to rural areas is often challenging due to a myriad of factors, such as geography, economics, culture and education. Geographically, rural communities are often far removed from suburban and urban centers that provide access to education, cultural and economic opportunities. These limitations influence the relocation decision of the physician candidate and his/her spouse/children to locate to a rural area. A number of factors contribute to a unique and challenging environment that influence overall health standing in our county. According to the County Health Rankings, 35% of the adults in NCHs home county are obese. Obesity increases the risk for many health conditions such as coronary heart disease, type 2 diabetes, hypertension, stroke, cancer, sleep apnea and respiratory problems, and osteoarthritis. Evidence indicates physical activity, independent of its effect on weight, has substantial benefits for health. Our county's inactivity rate is 34% compared to the statewide rate of 23%. Wise County's adult smoking rate is 19% higher than that of Virginias average. Other demographic factors influencing health status in the county also include education levels. A large portion of Wise County residents do not have an education past high school compared to the State of Virginia. Additionally, a large portion of Wise County residents (14%) do not have health insurance. According to the 2021 County Health Rankings, Wise County is ranked 125th in Virginia for health outcomes and 127th for health factors our of 133 counties/cities. Wise County also ranked 123rd in health behaviors. When compared to other Virginia counties, Wise County has higher rates of adult obesity and physical inactivity, adult smoking, and alcohol-impaired driving deaths. As for social and economic factors, Wise County ranks 125th due high rates of children in poverty and higher unemployment rates.
Part VI, Line 4 - Community Building ActivitiesNCH leaders support and encourage all team members to volunteer time, money and skills to community service projects and charitable organizations. Senior leaders and board members set a positive example for NCH team members, serving voluntarily on committees and boards of local service and nonprofit organizations. Some also serve as members and consultants on professional committees and task forces that affect regional development in healthcare and education. We do not capture costs associated with team members that serve on other nonprofit boards or provide services to other nonprofits.NCH, in collaboration with area health agencies and providers, may offer assistance with coordination, advocacy, or contribute supplies to support groups for their program activities that serve to assist special populations within our area. Most of these organizations work to improve the lives of community members that have limited, or no, financial resources.
Part VI, Line 5 - Promotion of Community HealthNCH is dedicated to operating efficiently so that waste is minimized. NCH'S leadership remains mindful of managing limited resources so that adequate facilities and equipment are available for the care of our patients. Surplus funds are invested into improving treatment options for our patients through new technologies, recruiting physicians and trained staff in shortage areas, and improving our facilities. Various checks and balances are established to ensure that expenditures for operating expenses and capital costs are reasonable and necessary.NCH has a Medicare-approved internal medicine residency education program. In addition, our hospital serves as a training site for many other types of health professions such as nursing, pharmacy, respiratory therapy, and other allied health programs. We provide for media coverage to educate our community on health issues, provide free clinical care to low income individuals in collaboration with other nonprofit organizations, and provide free drugs and supplies to local nonprofit emergency response organizations. NCH owns Dickenson County Hospital, a critical access hospital located in the Appalachian Mountains. Dickenson is 100% rural according to the 2021 County Health Rankings report. Dickenson County's median household income is just over 48% of the median statewide income and the county's health outcomes rank 122 out of 133 in the state. DCH operates at a substantial financial loss and relies on NCH to subsidize its operations. Physicians that request privileges who are qualified and credentialed are extended privileges by NCH.
Part VI, Line 6 - Affilated Health Care SystemNCH is a 129-bed, acute-care facility and one of the hospitals within the Ballad Health system. Ballad Health operates a family of 21 hospitals, including three tertiary care facilities, a dedicated children's hospital, community hospitals, three critical access hospitals, a behavioral health hospital, an addiction treatment facility, long-term care facilities, home care and hospice services, retail pharmacies, outpatient services and a comprehensive medical management corporation. Patients benefit from NCH's affiliation with a healthcare system where they can be efficiently moved along an integrated, comprehensive continuum of care as their health status dictates. If needed, patients can be moved to one of Ballad Healths tertiary care facilities, providing advanced treatment options.Hospitals in the Ballad Health system work closely with one another to share expertise and resources.
Part VI, Line 7 - States Filing of Community Benefit ReportVA TN
Part VI - Additional InformationBallad Health is required to report community benefit estimates on a quarterly basis with the states of Virginia and Tennessee. The reporting includes all of Ballads hospital organizations and is reported using IRS Form 990, Schedule H instructions for reporting community benefit. Ballad operates under a Cooperative Agreement (CA) in Virginia and a Certificate of Public Advantage (COPA) in Tennessee as obligated by agreements between Ballad and the two states to allow Mountain States Health Alliance and Wellmont Health System to merge.

Form 990, Part III, Line 4d: Other Program Services DescriptionOTHER PROGRAM SERVICES 4: Health Professions Education:NCH serves as a clinical training facility for health professional education students. We have dedicated staff to work with regional colleges and universities to coordinate the placement of healthcare professional students as part of their educational curriculum. In addition to clinical training, the health care students entering our system are required to have orientation and computer training. Participants receiving clinical experience at NCH were 75 nursing students from various colleges, universities and programs. This nursing clinical experience required extensive NCH nursing staff involvement. The clinical setting and hands-on instruction cost NCH $115,252. In addition to nursing students, NCH provided a clinical setting for another 59 students training in health-related programs such as pharmacy, social work, OT/PT, laboratory, EMT/paramedic and other allied-health disciplines. The hospitals costs related to this training was $86,801.Charity and Other Unreimbursed Costs: Charity Care: While reimbursement for healthcare services rendered is critical to the operation and sustainability of the organization, NCH recognizes its obligation to provide care to individuals who cannot afford essential medical services, including emergency care. NCH accepts all patients regardless of their ability to pay. A patient is classified as a charity patient when they meet the established policies of Ballad Health and guidelines outlined by the federal government. However, financial assistance decisions are not solely based on income. Unique financial circumstances are weighed with verified patient assets which can determine financial assistance eligibility. It is not until after verification of income and assets that a decision regarding the amount of financial assistance can be made. In Fiscal Year 2021, NCH incurred a loss of $546,809 attributable to the provision of charity care. This amount does not include the costs associated with bad debt accounts.Medicaid/TennCare: NCH provides care to persons covered by governmental programs, such as Medicaid and TennCare (Tennessee residents). NCH incurred a loss of a little over $1,000,000 providing care to this population of patients during the year. Uninsured Discount: Uninsured patients received an 85% discount. Certain elective procedures are not eligible for the uninsured discount. This uninsured discount is calculated each year in accordance with Tennessee regulations and without regard to a patients income or assets. Although the uninsured discount is not required by the Commonwealth of Virginia, Ballad Health applies the same discount to our Virginia hospitals, including NCH. The approximate cost of this discount in FY21, using a cost to charge ratio, was $598,187 for NCH. Community Donations:For many years, NCH has provided assistance to local non-profit lifesaving organizations such as ambulance services, fire departments and rescue squads. NCH donates free medications and pharmaceutical supplies to these organizations. The cost of donated medications to rescue organizations during FY21 was $5,068. NCH also made cash donations of $16,661 to University of Virginia College at Wise Foundation for their athletic programs. OTHER PROGRAM SERVICES 5: Awards and Recognitions:Annually, U.S. News & World Report releases a list of hospital rankings to help guide patients to the right hospital when they need excellent care. Hospitals receive various scores. A hospitals score is based on multiple data categories, including patient survival, discharging patients to home, nurse staffing and more. Over 6,000 hospitals were evaluated, and eligible hospitals received one of three ratings (high performing, average, or below average). Hospitals that earned a high performing rating were significantly better than the national average. In their 2021-2022 report, NCH received High Performing 5/5 in 2 adult procedures: 1.Chronic Obstructive Pulmonary Disease (COPD) 2.Pneumonia CareHealthgrades recognized NCH for six years running with the Pulmonary Care Excellence Award, which recognizes hospitals for superior outcomes in treating chronic obstructive pulmonary disease (COPD) and pneumonia. Patients who receive COPD or pneumonia treatment at hospitals receiving this clinical quality award have a lower risk of dying. Healthgrades evaluates hospital quality for conditions and procedures based solely on clinical outcomes. Their analysis is based on more than 45 million Medicare medical claims records for the most recent three-year time period available from nearly 4,500 hospitals nationwide. Norton Community Hospitals inpatient Rehabilitation Unit was recognized for thirteen consecutive years of top-decile (top 10%) performance-out of more than 800 facilities for functional patient outcomes. The evaluation was based on the delivery of quality care that is effective, efficient, timely and patient-centered. To determine the rankings, Uniform Data System for Medical Rehabilitation (UDSMR) used a system that measures the efficiency and effectiveness of a hospitals rehabilitation programs by evaluating and tracking patient progress through the rehabilitation process. Patients functional levels refer to their ability to return to their daily lives and activities without impairment. The unit first opened in 1998, and since that time it has served over 4,000 patients recovering from a variety of injuries, illnesses and accidents.Norton Community Hospitals internal medicine residency program is accredited by the Accreditation Council for Graduate Medical Education (ACGME). ACGME sets standards for U.S. Medical Education (residency and fellowship) programs and the institutions that sponsor them. Accreditation decisions are based on evaluation and review of published accreditation quality standards. NCH takes great pride in training young physicians for our rural, predominantly low-income and medically underserved area. Internal medicine residents rotate through all areas of medicine at NCH, from health maintenance to critical care. The residents assist our physicians in providing quality hands-on patient care through observation, education and by participating in grand rounds. In addition to services within the hospital setting, medical residents are exposed to treating patients in a physician office setting at our community clinic (resident continuity clinic) located in Wise, Virginia. Our unreimbursed cost for this residency program this year was $357,979.
Form 990, Part VI, Line 4: Description of Significant Changes to Organizational DocumentsIn February of 2021, an Articles of Merger was completed between NCH and Wellmont Health System (WHS). On February 28, 2021 NCH merged into WHS (the surviving entity), which is owned by Ballad Health.
Form 990, Part VI, Line 6: Explanation of Classes of Members or ShareholderNorton Community Hospital is a Virginia non-stock, nonprofit corporation. After Mountain States Health Alliance (MSHA) purchase of Community Health Care Foundations 49% membership (FY20), it resulted in MSHA being the sole member. Additional hospitals located in Wise Co. are Mountain View Regional Hospital (MVRH) and Lonesome Pine Hospital (LPH) which are reported on Wellmont Health System (WHS)s 990. As of February 2021, NCH merged with these hospitals in order to optimize healthcare in Wise County. This will be the final separate return for NCH. Norton Community Physician Services will have a member substitution from NCH to WHS as its parent.
Form 990, Part VI, Line 7a: How Members or Shareholders Elect Governing BodyAfter purchasing Community Health Care Foundations 49% membership during FY20,MSHA board of directors also became NCHs board of directors.
Form 990, Part VI, Line 7b: Describe Decisions of Governing Body Approval by Members or ShareholdersCertain decisions of the board are, pursuant to charter and Virginia stature, subject to approval of the members. These decisions include: dissolution of the corporation; merger of the corporation; non-ordinary course of business sale of assets, etc. No ordinary day-to-day decisions are subject to member approval.
Form 990, Part VI, Line 11b: Form 990 Review ProcessThe Ballad Health Tax Department prepares and reviews the Form 990. During preparation other functional areas within the organization provide information and support to complete an accurate return. The return is reviewed by the organization's CFO and is provided in electronic form to all members of the Board of Directors prior to being filed with the IRS.
Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of ConflictsBallad Health has a conflict-of-interest policy for all members of the Board of Directors, the Executive Chair/President, Executive Vice Presidents, Senior Vice Presidents, and Vice Presidents, and applies to all Ballad Health organizations, including Norton Community Hospital. All persons covered by this policy are required to complete a conflict of interest disclosure form on an annual basis. Should a conflict arise, it is the responsibility of the conflicted individual to update his or her disclosure immediately. All meetings of the board or board committees have a standing agenda item first on the agenda titled Conflicts of Interest. If a member of the board or board committee has a conflict of interest involving any issue on the board agenda, he or she must declare the conflict of interest during the period allotted for disclosure. If any issue arises during a meeting in which the board member has a conflict of interest, he or she must immediately declare the conflict. While each member of the board or board committee is responsible for disclosing conflicts of interest, it is also the responsibility of any board member aware of a conflict which has not been disclosed to ensure the board is made aware. Part VI, Line 15b - Compensation Process for OfficersSimilar to NCH/DCH's CEO, NCH's CFO receives compensation that complies with Ballad Healths salary policy. The pay is set at a market percentile specific to his position as CFO of NW Region which includes NCH.The executive committee reviewed and approved compensation for all Ballad Health executives at the vice-president level and above during FY21 using the same methodology used to determine the CEO's compensation. In addition, Ballad Health offers an incentive plan to executives based on targeted achievement metrics approved by the Ballad Health Board of Directors which include Quality of Care, Access to Care, Cost Management, etc.Key employees, Robert Kennedy, Wise Co. COO, and Bonnie Jensen, Wise Co. CNO, are not Vice-Presidents or in a senior leadership position, therefore their compensation and benefits are consistent with other NCH non-executive positions.
Part VII - Related OrganizationsDirector Compensation: NCH's Board of Directors, Alan Levine, Marvin Eichorn, Eric Deaton and Tim Belisle receive compensation for services provided to related organizations and do not receive compensation for services as a NCH board member.
Part Vl, Line 15a-Compensation Process for Top OfficialThe executive compensation committee serves as the compensation oversight committee of Ballad Healths Board of Directors. The compensation plan for Alan Levine, Ballad Healths President and CEO, was reviewed and approved by the executive compensation committee and then by the Ballad Health Board of Directors. An outside and independent compensation consultant was used to determine his compensation and benefits. Studies and surveys were used to ensure his pay is comparable to like positions at similarly situated organizations. Mark Leonard, VP served as Wise Co. CEO until mid-October 2020. Also, in October 2020, Dale Clark, VP began serving as Wise Co. CEO.
