DescThe Villages Regional Hospital ("UFHV"): UFHV is an acute care hospital located in The Villages, Florida. UFHV has been serving residents of The Villages as well as patients in Lake, Sumter, and Marion Counties since 2002. UFHV offers 24-hour emergent diagnostic and treatment services to patients presenting with acute ischemic stroke and other neurovascular disorders. UFHV earned The Joint Commission's Gold Seal of Approval and the American Heart Association/American Stroke Association's Heart-Check Mark for advanced certification for primary stroke centers. Joint Commission experts evaluate compliance with stroke-related standards and requirements, including program management, the delivery of clinical care and performance improvement. UFHV also provides inpatient rehabilitation services. Located on the hospital's fifth floor. the 30-bed acute inpatient rehabilitation facility features a physical therapy gym, communal dining hall, and an activities of daily living (ADL) suite. Patients who suffer from conditions including, but not limited to, stroke, spinal cord injury, amputation, brain injury, and neurological disorders receive around-the-clock care, advanced physical therapy, occupational therapy, speech pathology, social services, case management, neuropsychology, and more. The rehabilitation program is accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). This designation represents the highest level of accreditation that can be awarded to a rehabilitation facility. To earn a three-year accreditation, the facility underwent a rigorous peer review process and demonstrated to a team of surveyors during an on-site visit that its programs and services are accountable, measurable, and of the highest quality. In addition to being accredited by The Joint Commission, UFHV has been recognized for excellence in patient care and workplace health. The American College of Cardiology awarded UFHV its highest chest pain center accreditation and U.S. News and World Report has rated several of the hospital's clinical areas as "high performing." In 2019, Becker's Hospital Review named UFHV on its list of the 150 Top Places to Work in Healthcare.Desc

Schedule H, Part V, Section B, Line 3EThe significant health needs are a prioritized description of the significant health needs of the community and identified through the CHNA.
Schedule H, Part V, Section B, Line 5 Facility , 1Facility , 1 - THE VILLAGES TRI-COUNTY MEDICAL CENTER. IN GENERAL, INPUT WAS OBTAINED FROM THE FOLLOWING PERSONS REPRESENTING THE BROAD INTEREST OF THE COMMUNITY SERVED BY THE HOSPITALS: BUSINESS LEADERS, MEMBERS OF LOCAL CHURCHES, COMMUNITY NON-PROFIT ORGANIZATIONS, HEALTH DEPARTMENTS, AND REPRESENTATIVES FROM LOCAL NEIGHBORHOOD COMMUNITIES. SEE PAGES 2 AND 3 OF THE 2019 COMMUNITY HEALTH NEEDS ASSESSMENT FOR A COMPLETE LIST OF THE ORGANIZATIONS THAT PROVIDED INPUT AS A PART OF THE UFHCF STEERING COMMITTEE. THE UFHCF STEERING COMMITTEE AND WELLFLORIDA BASED THE 2019 CHNA EFFORT ON A NATIONALLY RECOGNIZED MODEL AND BEST PRACTICE FOR COMPLETING HEALTH ASSESSMENTS CALLED MOBILIZING FOR ACTION THROUGH PLANNING AND PARTNERSHIPS (MAPP). THERE ARE FOUR CORE MAPP ASSESSMENTS: COMMUNITY HEALTH STATUS ASSESSMENT, COMMUNITY THEMES AND STRENGTHS ASSESSMENT, FORCES OF CHANGE ASSESSMENT, AND LOCAL PUBLIC HEALTH SYSTEM ASSESSMENT. THE UFHCF STEERING COMMITTEE, IN PARTNERSHIP WITH WELLFLORIDA COUNCIL, COLLABORATED TO FORMULATE TWO SIMILAR SURVEYS TO QUERY INDIVIDUALS ABOUT COMMUNITY HEALTH ISSUES AND THEIR PERSPECTIVES ON HEALTHCARE. BOTH SURVEYS EMPLOYED A CONVENIENCE SAMPLING APPROACH (RESPONDENTS WERE SELECTED BASED ON THEIR CONVENIENT ACCESSIBILITY). THE UFHCF STEERING COMMITTEE ASSISTED WITH THE SURVEY DISSEMINATION THROUGH THEIR RESPECTIVE ORGANIZATIONS AND BY CONNECTING WELLFLORIDA TO KEY STAKEHOLDER AND COMMUNITY PARTNERS. THERE WERE A TOTAL OF 583 RESPONDENTS TO THE COMMUNITY SURVEY AND 106 TO THE PROFESSIONAL SURVEY. AFTER ELIMINATING THE INCOMPLETE AND INELIGIBLE SURVEYS, THERE WERE 392 COMMUNITY SURVEYS AND 23 HEALTHCARE PROFESSIONAL SURVEYS FOR ANALYSIS. ONE OF THE MAIN ELEMENTS OF THE NEEDS ASSESSMENT PROCESS INCLUDED A FORCES OF CHANGE ASSESSMENT (FCA). THE FORCES OF CHANGE ASSESSMENT FOR UFHCF WAS CONDUCTED FOR BOTH HOSPITALS WITHIN THE HOSPITAL SYSTEM - THE VILLAGES REGIONAL HOSPITAL AND LEESBURG REGIONAL MEDICAL CENTER. THIS ASSESSMENT WAS AIMED AT IDENTIFYING FORCES SUCH AS TRENDS, FACTORS, OR EVENTS THAT ARE OR WILL BE INFLUENCING THE HEALTH AND QUALITY OF LIFE OF THE COMMUNITY.

Schedule H, Part V, Section A List of hospital facilitiesThe Villages Tri-County Medical Center and TVRH Rehabilitation Hospital are being reported as one hospital facility because they operate under one hospital license (license No. 4464), they are physically located in the same building, and they would each provide the same answer to each question posed on Schedule H, Part V, Section B of the Form 990.
Schedule H, Part I, Line 7 Costing Methodology used to calculate financial assistanceThe estimated cost of financial assistance provided was determined by applying UFHV's overall cost to charge ratio to total charges foregone. Cost of benefits for the broader community represents actual expenses incurred.
Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amountFOR FINANCIAL STATEMENT PURPOSES, THE ORGANIZATION DIRECTLY REDUCES THE AMOUNT OF PATIENT SERVICE REVENUE REPORTED ON THE ORGANIZATION'S AUDITED FINANCIAL STATEMENTS BY THE AMOUT OF BAD DEBT EXPENSE. THEREFORE, BAD DEBTS ARE INCLUDED IN NET PATIENT REVENUE SERVICE AND BAD DEBT EXPENSE IS NOT SEPARATELY REPORTED AS AN EXPENSE.
Schedule H, Part III, Line 3 Bad Debt Expense MethodologyThe organization reports $0 bad debt expense because bad debts are included in net patient revenue service. Therefore, the amount of bad debts attributable to patients eligible under the organization's financial assistance policy is also $0.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnoteThe provision for bad debts is based on management's assessment of historical and expected net collections, considering business and economic conditions, trends in federal and state governmental health care coverage, and other collection indicators. Throughout the year, management assesses the adequacy of the allowance for uncollectible accounts based upon these trends. The results of this review are then used to make any modification to the provision for bad debts to establish an appropriate allowance for uncollectible accounts. Schedule H, Part VI, Line 2 Needs assessmentTHE UFHCF STEERING COMMITTEE AND WELLFLORIDA BASED THE 2019-2022 CHNA EFFORT ON A NATIONALLY RECOGNIZED MODEL AND BEST PRACTICE FOR COMPLETING HEALTH ASSESSMENTS AND IMPROVEMENT PLANS CALLED MOBILIZING FOR ACTION THROUGH PLANNING AND PARTNERSHIPS (MAPP). THE MAPP TOOL WAS DEVELOPED BY THE NATIONAL ASSOCIATION OF CITY AND COUNTY HEALTH OFFICIALS (NACCHO) IN COOPERATION WITH THE PUBLIC HEALTH PRACTICE PROGRAM OFFICE, CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC). NACCHO AND CDC'S VISION FOR IMPLEMENTING MAPP IS: "COMMUNITIES ACHIEVING IMPROVED HEALTH AND QUALITY OF LIFE BY MOBILIZING PARTNERSHIPS AND TAKING STRATEGIC ACTION." AT THE HEART OF THE MAPP PROCESS ARE THE FOUR CORE MAPP ASSESSMENTS. THESE ARE: - COMMUNITY HEALTH STATUS ASSESSMENT (CHSA) - COMMUNITY THEMES AND STRENGTHS ASSESSMENT (CTSA) - FORCES OF CHANGE ASSESSMENT (FCA) - LOCAL PUBLIC HEALTH SYSTEM ASSESSMENT (LPHSA) In response to the COVID-19 pandemic, UFHV halted elective procedures in order to protect patients and providers. UFHV made early efforts to proactively address the large population of high-risk senior citizens that reside within the hospital's service area and provided some of the earliest COVID-19 testing in Florida. Additionally, the hospital significantly expanded its telehealth services as a result of the pandemic. At the onset of the pandemic, medical leaders at UFHV focused on the need to deliver round-the-clock critical care at the hospital. In just days, a solution was in place: a telehealth-based system that connects critical-care physicians at UF Health in Gainesville with patients in the UFHV and Leesburg Regional Medical Center. This telehealth system, commonly known as an eICU, typically requires significant amounts of planning and specialized equipment; however, a core group of UF Health leaders, physicians, and information technology experts were able to provide a solution within 48 hours of the initial planning meeting. This solution consisted of a repurposed patient tray table outfitted with a Microsoft Surface laptop computer that allows UF Health critical-care specialists to remotely access patients' electronic health records, view X-rays and other diagnostic images, monitor vital signs, consult with the on-site medical staff, and provide expert medical guidance in real time. Physicians are not required to be in a hospital to access this system and can log in from anywhere with a computer and an internet connection for a virtual bedside visit. A real-time camera allows the physician to examine and communicate with the patient.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistanceIN ADDITION TO SIGNAGE THROUGHOUT VARIOUS PARTS OF THE HOSPITAL AND WRITTEN INFORMATION PROVIDED TO PATIENTS DURING PRE-REGISTRATION, THE HOSPITAL PROVIDES THE FOLLOWING PATIENT EDUCATION: - FINANCIAL COUNSELING - WRITTEN ESTIMATES FOR HOSPITAL SERVICES - ITEMIZED BILLINGS (UPON REQUEST) - INFORMATION ABOUT EXTRAORDINARY COLLECTION ACTIONS THAT MAY BE USED AFTER REASONABLE COLLECTION EFFORTS HAVE TAKEN PLACE - A LIST OF SERVICE PROVIDERS THAT PROVIDE SERVICES UNDER THE FAP

Form 990, Part VI, Line 15a PROCESS TO ESTABLISH COMPENSATION OF TOP MANAGEMENT OFFICIALThe organization's CEO is compensated by a related organization. The filing organization does not compensate the top management official of the organization; therefore, Line 15a has been answered "no" in accordance with the Form 990 instructions. The related organization's process for establishing CEO compensation is as follows. Mercer, an independent firm engaged in the development of compensation surveys on executive pay levels across different industries, provides competitive data and guidance on determining appropriate and reasonable ranges and salaries. THE ORGANIZATION'S EXECUTIVE COMPENSATION COMMITTEE OF THE BOARD OF DIRECTORS UTILIZES THE INFORMATION PROVIDED BY MERCER TO ESTABLISH THE COMPENSATION OF THE CEO. Mercer's methodology consists of the following standard steps: *Analysis by position based on job content *Reference of multiple national survey sources similarly situated for not-for-profit healthcare executive compensation *Existing base salary range midpoints were updated to more closely align with the market 50th percentile value for each role. Mercer followed a similar process to evaluate the competitiveness of total cash compensation (base salary, annual incentive bonus, and benefits) levels, and provides recommended incentive compensation ranges to ENSURE total cash compensation is also reasonable. Mercer periodically reviews all other aspects of executive compensation-including all elements of supplemental benefits-to ENSURE that they compare with competitive and reasonable total remuneration levels. THIS PROCESS WAS LAST UNDERTAKEN IN SEPTEMBER 2021 AND WAS DOCUMENTED IN THE MINUTES OF THE EXECUTIVE COMPENSATION COMMITTEE MEETING. THIS PROCESS IS TYPICALLY UNDERTAKEN ANNUALLY. HOWEVER, IN 2020, MANAGEMENT FROZE ALL EMPLOYEE SALARIES DUE TO THE COVID-19 PANDEMIC AND THIS PROCESS WAS NOT CONDUCTED DURING THE FISCAL YEAR ENDED JUNE 30, 2021; THEREFORE, THIS QUESTION HAS BEEN ANSWERED "NO" IN ACCORDANCE WITH THE INSTRUCTIONS TO IRS FORM 990.
Form 990, Part VI, Line 15b PROCESS TO ESTABLISH COMPENSATION OF OTHER OFFICERS OR KEY EMPLOYEESAll executive, officer, and key employee positions are reviewed each year by Mercer to ensure salary scales are in line with comparable positions in similar institutions.
