DescShort term general and primary health care for Grundy County and the surrounding area. See also Schedule H, Part II, community building report. As part of a commitment to supporting our community's future healthcare providers, the Hospital devoted over 493 hours of staff time mentoring students who are studying to become nurses, paramedics, respiratory therapists, pharmacists, lab and radiology technologists through Joliet Junior College, College of DuPage, Illinois Valley Community College, Kankakee Community College, Lewis University, Midwestern University, University of St. Francis, Rasmussen University, Chamberlain University, Midwestern University, Illinois State University, St. Ambrose University, Benedictine College, Elmhurst University, Aurora University, Grundy Vocational Center and more. It is imperative that these students have a place to complete their critical training. By helping students train in preparation for entering the healthcare workforce, we are helping to improve access to healthcare services in the future.Morris Hospital continues to provide financial donations at the request of a number of community organizations whose mission closely mirrors the hospital's. Requests for donations were significantly down in 2021 because many events were cancelled. Here's what we did do: in 2021, donations were made to Bristol Kendall fire department for fire safety education materials, Joliet Junior College Foundation, Will Grundy Medical Clinic for free services for the underserved, living alternatives pregnancy resource, American Cancer Society, United Way of Grundy County, Dwight Township H.S. and Coal City H.S. for their post prom events, the greater Joliet area YMCA, and Grundy area pads. Morris Hospital collaborates with the Illinois Department of Aging by sponsoring local volunteers from their senior health insurance program. The program provides free health insurance counseling for Medicare beneficiaries and their caregivers, with the ship volunteers certified by Medicare to educate consumers and answer questions about Medicare, Medicare supplements, prescription drug plans, and advantage plans. As sponsor of the local ship volunteers, Morris Hospital provides space at the hospital for the volunteers to meet with community members seeking counseling.Desc

Morris HospitalPart V, Section B, Line 5: Interviews were used to gather information and opinions from persons who represent the interests of the community served by the Hospital. Community stakeholders identified by the hospital were contacted and asked to participate in the needs assessment. Interviewees included individuals who are leaders and/or representatives of medically underserved, low-income, and minority populations, local health or other departments or agencies that have current data or other information relevant to the health needs of the community. Input was obtained from the Grundy County health department and the Kendall County Health Department. Fourteen interviews were completed from July through August 2019.
Morris HospitalPart V, Section B, Line 6b: CHNA planned and developed in cooperation with the following organization: Grundy County Health Department.
Morris HospitalPart V, Section B, Line 11: Morris Hospital has created a three-year implementation strategy that focuses on the four priority health areas identified in the study: access to health care, chronic diseases, overweight and obesity, and health behaviors and prevention. Mental health was one need identified in the CHNA that was not selected as a priority health need and that Morris hospital does not intend to actively address. Morris Hospital has chosen not to focus on mental health as the hospital does not have the existing infrastructure, identified organizational competencies or established partnerships that focus on mental health. Morris Hospital will indirectly address mental health and substance abuse through increasing access to care. However, the current focus of the hospital's charitable resources will be placed on the four identified priority health needs.
Morris HospitalPart V, Section B, Line 13h: Morris Hospital also designates patients as presumptively eligible by utilizing an automated system called FastTag with our early out vendor, Meduit. They use a scoring system to rank patients based off eligibility to pay and if the account is scored as presumptive, we will adjust the account off without the patient having to submit a financial application.
Morris HospitalPart V, Section B, Line 16j: Morris Hospital has information on financial assistance posted in all registration areas and a copy of the plain language summary for review. Where possible, prior to the admission or rendering of service, a Financial Service Representative will conduct an interview with the patient, the guarantor, and/or his other legal representative. If unavailable to meet prior to or due to medical reasons, the patient may schedule an appointment in person or over the phone with the Financial Service Representative to discuss their options.

Part I, Line 3c:Not applicable as the organization does follow federal poverty guidelines to determine eligibility for providing charity and discounted care to low-income uninsured and underinsured individuals.
Part I, Line 6a:The Hospital's community benefit report is available upon request and is provided to the state agency that makes it available to the public.
Part I, Line 7g:The Hospital has not included any costs attributable to physician clinics as part of their subsidized health services.
Part I, Line 7, Column (f):The Bad Debt expense included on Form 990, Part IX, Line 25, Column (A), but subtracted for purposes of calculating the percentage in this column is $ 8,135,793.
Part II, Community Building Activities:Community Coalition and Collaborative EffortsThroughout the year, Morris Hospital furthers its mission of improving the health of area residents by collaborating with other organizations. Morris Hospital has been partnering with the Morris Community YMCA since 2015. In addition to covering half of the facility operating expenses in 2021 totaling over $61,000, Morris Hospital holds its senior exercise classes at the YMCA and encourages graduates of the Morris Hospital Phase II cardiac rehabilitation program to continue their exercise program at the Y so they can maintain optimal heart health. Through the YMCA's Prescription for Fitness program, Morris Hospital physicians are able to refer patients with chronic disease or certain health risks to the Y by prescribing a free, 30-day membership. In order to promote a healthy workforce, Morris Hospital employees can receive a free individual membership to the Morris Community YMCA, with the hospital reimbursing the Y for the cost. In 2021, Morris Hospital & the Morris Community YMCA launched a Pedaling for Parkinson's class that is taught by Morris Hospital Wellness and Physical Therapy staff.2021 resulted in unprecedented collaboration with the Grundy County Health Department as we worked together to administer and promote COVID vaccination to our employees and community. As a Resource Hospital within Illinois Region VII Emergency Medical Services (EMS), Morris Hospital is licensed to provide initial education and continuing training for area emergency medical technicians and paramedics, working side by side with local EMS providers to ensure the very best pre-hospital care for local communities. Morris Hospital also serves as the Resource Hospital for Braceville Fire Protection District, Braidwood Fire Department, Coal City Fire Protection District, Dwight EMS, Dwight Fire Department, Eco Lab, Grand Ridge Volunteer Fire Department, Grundy County 911 Center, Lisbon Seward Fire Protection, Lisbon-Plattville Fire Department, Lyondell Basell Fire Department, Mazon-Verona-Kinsman Rescue Squad, Minooka Fire Protection District, Morris Fire Protection & Ambulance District, Newark Fire Protection District, Reddick Community Fire Protection District, Seneca Ambulance, and Troy Fire Protection District, assuring that ambulances are appropriately stocked with supplies and equipment; that EMS personnel meet all of the specified licensure qualifications, and that EMS providers comply with standard policies and protocol. Morris Hospital staff also helps to coordinate area disaster drills working in collaboration with the Grundy County Emergency Management Agency. Morris Hospital is a significant partner of the Will Grundy Medical Clinic, which provides free medical and dental care to qualifying residents. We demonstrate our support by providing free use of office space each month, volunteer time from Morris Hospital employed physicians, and free hospital services to Will Grundy Medical Clinic clients. In addition, a $5,000 donation was made in 2021 to support WGMC fundraising initiatives. In response to requests from area schools, providers from Morris Hospital went onsite to provide sports physicals at Coal City Middle School, Lisbon Grade School, Mazon Verona Kinsman School, Minooka Jr High, Morris H.S., Newark Middle School, Seneca Grade School, Seneca H.S., and South Wilmington Grade School in 2021. The schools requested this onsite service because the convenience of the service helps the athletes better comply with the sports physical requirement in a timely banner. 340 sports physicals were performed in 2021 for an estimated community benefit of $73,170. While $20 was collected from each student who had a physical, Morris Hospital donated the money back to the athletic departments of the respective school. Through a coordinated campaign that took place over a period of two weeks in November 2021, Morris Hospital employees donated $20,969 to the United Way of Grundy County through gifts of cash and PTO hours. The United Way of Grundy County has nearly 30 community partner agencies, so the collective gift from Morris Hospital employees goes very far to support health, education, youth, crisis and intervention, and basic human needs that are vital to the community. Morris Hospital improves access to health services by providing community members free rides to medical appointments through its free Patient Transportation Service. Rides are provided regardless of whether the medical provider is associated with Morris Hospital. The service was started in 1998 in response to an identified need for increased transportation service in the county. The service is available to any community member needing a ride and is especially used by seniors and low-income community members. In coordinating the rides, Morris Hospital works in cooperation with a number of other area agencies and healthcare providers when providing this service, including Illinois Valley Industries, Elliott Manor, Saratoga Towers, the Grundy County Health Department, Morris Senior Center, Morris Library, We Care, Hospice, Catholic Charities, Will/Grundy Medical Clinic, Grundy County Housing Authority, Narvick House, Veterans Assistance, We Care of Grundy County, Illinois Valley Industries, Morris Community Dialysis, DaVita Dialysis, Grundy Transit, Gardner American Legion, and area home health agencies and hospice. Many of these riders have indicated that they have no other way to get to their appointment. By helping students train in preparation for entering the healthcare workforce, we are helping to improve access to healthcare services in the futureMorris Hospital continues to provide financial donations at the request of a number of community organizations whose mission closely mirrors the Hospital's. In 2021, donations were granted to: Joliet Junior College Foundation, Grundy County Emergency Planning Committee, United Way of Grundy County, United Way of LaSalle County, Aden Lamps Foundation for safe sleep packets for babies, Lightways Hospice, Grundy County Health Department, Bristol Kendall Fire Department for fire safety materials, and Channahon-Minooka Rotary for their school backpack program. Morris Hospital permits the Illinois Valley Epilepsy Advocacy Network to their free monthly Epilepsy Support Group at the hospital free of charge.
Part III, Line 2:Uncollectable patient accounts are written off as bad debt in the period they are determined to be uncollectible. The accounting for this is to post bad debts against the Allowance for Doubtful Accounts, which is a contra Accounts Receivable reserve to reduce the expected amount to collect. At each month end close, the Allowance for Doubtful Accounts is replenished based upon the new estimates for the Accounts Receivable that evaluates the amount expected to be collected. This estimate is based upon historic collection rates. The replenishment amount is recorded as bad debt expense on the financial statements.
Part III, Line 3:Review of prior year bad debt amounts that subsequently converted to charity pursuant to the time allotted in our financial assistance policy. This method compares accounts that are actually written off to bad debt that are subsequently converted to charity.
Part III, Line 4:The following information can be found in the attached audited financial statements on page 7, significant accounting policies for accounts receivable:Accounts receivable for patients, insurance companies, and governmental agencies are based on gross charges, reduced by explicit price concessions provided to third-party payors, discounts provided to qualifyingindividuals as part of our financial assistance policy, and implicit price concessions provided primarily to self-pay patients. Estimates for explicit price concessions are based on provider contracts, payment terms for relevant prospective payment systems, and historical experience adjusted for economic conditions and other trends affecting the hospital's ability to collect outstanding amounts. For receivables form self-pay patients, which include both patients without insurance and patients with deductible and copayment balances due for which third-party coverage exists for part of the bill, the hospital records significant implicit price concessions in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible.
Part III, Line 8:The Hospital continually strives to provide excellent patient care in the most cost-effective fashion. Nonetheless, the Medicare program, in many cases, does not provide payment that covers the full cost of the care provided. Since it is the mission of the hospital to respond to community need, hospital management continually advocates for improved Medicare payment so that the cost of quality care to those patients that are not able to afford it is not compromised and is fairly subsidized by all payers. While this shortfall in Medicare payments is not classified as community benefit by the IRS, we nonetheless believe it is an important contribution made by the hospital to the health and wellbeing of the community. Our mission calls us to serve all patients with the highest possible quality and efficiency, even if we are not paid fully for doing so.The Hospital uses the information obtained from its most recent Medicare Cost Report to calculate the program costs; the methodology utilizes direct costs for each department plus the overhead cost assigned/allocated using simple step-down accounting methods. A series of activity drivers are assigned to each clinical area and allocated to each department as a percent of total overhead costs for each driver. Examples of drivers to allocate overhead (support departments) includes square footage, patient days, dietary meals prepared and other unique units to allocate support costs to each clinical area.
Part III, Line 9b:Morris Hospital & Healthcare Centers will not engage in any extraordinary collection actions (ECAs), either directly or by any debt collection agency or other representative, before reasonable efforts are made to determine whether the patient is eligible for assistance under MHHC's Financial Assistance Policy. Patients are given 120 days from their first statement to work with us on a payment plan or determine if they're eligible for financial assistance and before placing with a bad debt agency. Prior to placing with a collection agency, we use a presumptive scoring system to scrub those accounts for financial assistance. For any account already placed with an agency, they have 240 days from the first statement date to still apply for financial assistance. If a Financial Assistance Application (whether complete or incomplete) is submitted by a patient at any time during the Application Period, MHHC will suspend any ECAs underway for so long as the patient's Financial Assistance is pending.
Part VI, Line 2:In compliance with provisions of the patient protection and affordable care act and internal revenue service regulations for tax-exempt hospitals, Morris Hospital conducted a community health needs assessment in 2019 through Biel Consulting, Inc., and through the oversight of the Grundy County Public Health Administrator, Morris Hospital Planning Manager, and Morris Hospital Education Instructor.Primary data collection was gathered through targeted interviews from persons who represent the broad interest of the community served by the hospital. This included leaders and representative of medically underserved, low-income and minority populations, including representatives from Coal City School District, Grundy County and Kendall County Health Departments, Channahon Park District, and area healthcare providers. Secondary data was collected from a variety of local, county and state sources including the US census, American Community Survey, Illinois Department of Public Health, Illinois Department of Employment Security, National Cancer Registry, County Health Rankings, Illinois Youth Survey, Illinois State Police and other.Like previous years, heart disease, cancer, stroke, and lung disease continue to surface as the leading causes of death in the communities served by Morris Hospital. Whereas the last community health needs assessment showed that over one-third of adults in the counties served by Morris Hospital (Grundy, Kendall, Lasalle, Livingston and Will) were overweight and close to one-fourth were obese, new data shows over one-quarter of adults are overweight and close to one-third of those 20 years of age or more, are obese. These percentages equate to over 60% of the adult population being overweight or obese. In Grundy County, 36.6% of the adult population is overweight and 31% are obese. Here are some of the other key findings:- In Grundy County, 10% of adults had frequent mental distress. 21.4% of Grundy County adults rated their mental health as not good' for 1 to 7 days in the past month. Mental health ratings of not good' for 8 to 30 days in the past month were reported by 13.4% of Grundy County adults. Mental or physical health issues limited the activities of 21% of Grundy County adults for 1 to 7 days out of the past 30 days. Activities were limited for 23.2% of Grundy County adults for 8 to 30 days of the past 30 days.- Service area counties have a higher overall cancer incidence rate than the state rate. Among service area counties, Grundy County has the highest incidence rate for all cancers (528.1 per 100,000 persons), the highest incidence rate of female breast cancers (150.1 per 100,000 persons), colorectal cancer (47.8 per 100,000 persons) and esophageal cancer (8.2 per 100,000 persons). Lasalle county has the highest rate of prostate cancer (152.7 per 100,000 persons) among service area counties. Livingston county has the highest rate of lung and bronchial cancers (86.9 per 100,000 persons) and leukemia (16.5 cases per 100,000 persons).- 33.8% of adults in Livingston County have been diagnosed with arthritis, which is higher than the state rate of 25.1%. Among adults, 13.9% in Grundy County have been diagnosed with asthma, which is higher than the state rate of 9.1%. 10% of Lasalle County adults have been diagnosed with diabetes, which is higher than the state rate of 9%. Livingston county (4.9%) and Will County (4.5%) have higher rates of coronary heart disease than the state (3.6%). - The rate of binge drinking (five or more drinks per occasion for males and four or more drinks per occasion for females) in two area counties exceed the state rate of 21% and is equal to the state rate in the other three counties.- Smoking continues to be a leading cause of preventable death in the United States. Smoking rates in the service area range from 14% in Kendall County to 17% in Lasalle County. The healthy people 2020 objective for smoking is 12%. The assessment also includes the county health rankings which are based on adult smoking, obesity, physical inactivity, excessive drinking, sexually transmitted infections, and others. Illinois counties are ranked from 1 (most healthy) to 102 (least healthy). Of the five counties served by Morris Hospital, Kendall County is ranked 7th healthiest in the state, Will County is ranked 19th, Grundy County is ranked 33rd, Lasalle County is ranked 44th and Livingston County is ranked 62nd. As a result of the review of needs and application of the above criteria, Morris Hospital will address: access to health care; chronic diseases; overweight and obesity; health behaviors and prevention through a commitment of community benefit programs and charitable resources. An entire copy of the 2019 community health needs assessment is available for public viewing on the hospital website, www.morrishospital.org/about-us/community-health-needs-assessment. Paper copies are available by calling 815-705-7009 or by filling out the form under contact us on the hospital's website and typing "community health needs assessment" in the questions/comments field.
Part VI, Line 3:As stated in the provision of financial assistance policy, Morris Hospital & Healthcare Centers is a not-for-profit, tax-exempt entity with a charitable mission of providing emergency and medically necessary health care services to residents of the city of Morris and the Hospital's defined primary and secondary service area, regardless of their financial status and ability to pay. The purpose of the policy is to ensure that processes and procedures exist for identifying and assisting patients whose care may be provided without charge or at a discount commensurate with their financial resources and ability to pay. When possible, a financial service representative conducts an interview with the patient, he guarantor, and/or his or her legal representative, prior to the admission or rendering of service. If an interview is not possible prior to the admission or rendering of service, the interview is conducted upon admission or as soon as possible thereafter. In the case of an emergency admission, the evaluation of payment alternatives may not take place until the required medical care has been provided. At the time of the initial patient interview, the financial representative will gather routine demographic information and information regarding all existing third-party coverage to determine if the patient qualifies for medical assistance from other existing financial resources such as Medicare, Medicaid, kid care, family care, Will-Grundy medical clinic, or other state and federal programs. In 2018, the hospital instituted a point of service collection process in which patients are asked to make a modest deposit toward their out-of-pocket expense at the time of service. Having the financial conversation in advance of providing service enables the registrationstaff to better identify patients who may qualify for financial assistance so the paperwork can get started in advance, rather than waiting until after the patient is billed. The Morris Hospital website prominently features a link to financial assistance from the home page. From there, the financial assistance policy and application is available in both English and Spanish. A graph was also added to the website that shows eligibility for financial assistance based on household income and size.Information on financial assistance is also included in the patient guide that is given to all inpatient and observation patients. In addition, patient account correspondence sent to patients includes a statement regarding the availability of the financial assistance program and a contact number.
Part VI, Line 4:Morris Hospital & Healthcare Centers serves a 19-zip code area encompassing 5 counties: Grundy, Lasalle, Kendall, Will and Livingston. According to the U.S. Census Bureau, American Community Survey, 2013-2017, the combined population of the 19 zip codes served by Morris Hospital is 130,755. Communities served are primarily rural with a few that are more suburban. The majority population (84.6%) in the Morris Hospital service area is white/Caucasian, and 9.8% is Hispanic/Latino, with the remaining racial/ethnic groups making up less than 6% of the population. The percentage of Caucasians is significantly higher than that of the State of Illinois. English is the most frequently spoken language (92.2%). 5.2% of the population speaks Spanish. Children and youth ages 0-17 make up 26.2% of the population, and 13% of the population are seniors age 65 and older. The percentage of seniors in the service area is less than found in the state. Among children, Livingston County has the highest rate of uninsured at 4.7%. Lasalle county has the highest rate of uninsured adults 18-64 at 9.4%. The rate of uninsured seniors is very small in all counties served by Morris Hospital.
Part VI, Line 5:One way Morris Hospital promotes the health of the community is by offering a free patient transportation service that provides rides to doctors and dentists' offices, physical therapy, kidney dialysis, laboratory tests, and diagnostic imaging appointments, regardless of whether the service is provided by Morris Hospital. Many of the individuals who use this service have no other way of getting to their health-related appointments. In total, 6,567 individuals received rides free of charge in 2021. This was a $522,118 investment on behalf of Morris Hospital for the cost of gas, maintenance of the buses, and salaries for paid staff who coordinate the services. Morris Hospital continues to retain its status as a level II trauma center in order to best serve the health needs of the community, an investment of over $310,250 for the hospital which includes the cost for the on-call trauma surgeon. As a resource hospital for emergency medical services, Morris Hospital also provides education and training for emergency medical technicians and paramedics to assure the highest quality pre-hospital care, a $237,310 investment in 2021.Additionally, Morris Hospital serves as an agent for Philips's lifeline and at home medication dispensers, two services that allow people to remain living independently in their homes. By going through Morris Hospital, community members are able to subscribe to these services at more affordable rate than they would be able to otherwise. In 2021, Morris Hospital invested $47,000 toward these vital community services. Our in-person health education and support group offerings continued to remain minimal in 2021 due to covid restrictions. Basic life support and heart saver first aid classes were held for healthcare providers and community members, in addition to a babysitter's training class for children 11 years and older. Now in its 8th year, Morris Hospital continues a strong partnership with the Morris community YMCA. This includes offering 8 sessions of free senior exercise classes during the week, which includes a combination of cardio-respiratory, muscle, endurance, and flexibility training. In 2021, a new pedaling for Parkinson's class was launched in partnership with the y, with classes meeting 3 days a week, free of charge to help improve quality of life for individuals with Parkinson's. The hospital often uses its weekly 5-minute paid radio program to promote community health. Some of the community health topics in 2021 included warning signs of heart attack, importance of primary care, safe driving, covid information and vaccine promotion, carb counting, stroke awareness, obesity management, summer safety tips, cancer early detection, blood shortage, and wellness tips for the new year. Morris Hospital social workers provided over 500 hours of time assisting patients and community members with advance directives, caregiver options, substance abuse and counseling options, and assisting with referrals to community resources. There is no charge for the counseling and guidance they provide. Morris Hospital also promotes the health of the community by making monetary donations to other nonprofit organizations that share a similar mission of improving the health of area residents. Some examples from 2021 included $1,000 to Joliet Junior College Foundation for the advancement of healthcare careers, $10,000 to Will Grundy medical to support free medical services for qualifying residents, $1,000 to the united way of Grundy County to support area agencies, $500 to the Aden Lamps Foundation for safe sleep packets for babies, $450 to Lightways, a non-profit hospice program serving our community, $500 to the Grundy County Health Department, and $3,500 to the greater Joliet area YMCA. Amounts less than $500 were donated to other organizations to help promote fire safety, pregnancy resources, and post prom and post-graduation events. In collaboration with the Grundy County Health Department, we continued to serve as a leader providing covid education and promote vaccination to our community through news articles, radio interviews, videos and Facebook posts. At the request of the Grundy County Health department, we even took the lead by hosting free, pediatric covid vaccine clinics for 5-11-year-olds as the health department felt we were better suited to vaccinate the pediatric population. The Morris Hospital & Healthcare Centers' board of directors is comprised of 11 members, all who reside in the hospital's service area, none of whom are employees. Some of the board members are associated with businesses that occasionally do business with Morris Hospital & Healthcare Centers. However, board members must disclose any conflict of interest, and they are required by policy to refrain from discussing or voting on any matters that could be considered a conflict of interest.Morris Hospital maintains an open medical staff, with privileges granted to all qualified physicians.

There are so many other ways we carry out our mission daily beyondtraditional hospital services, including our free patient transportation service, free exercise classes for seniors, financial assistance program, education and training for pre-hospital providers, student mentoring experiences, student career exploration, free health screenings, support groups and education programs. There's also the behind-the-scenes support that we provide for community social service agencies, including preparing daily meals for clients of mobile meals, providing clean linens and meals for the local pads, meals for St. Vincent table, and monetary support for the Will-Grundy medical clinic and we care of Grundy County.

E. Violations Of The Conflicts Or Dualities Of Interest Policy. 1. If the board or committee has reasonable cause to believe that an interested person has failed to disclose actual or possible conflicts of interest, it shall inform the person of the basis for such belief and afford him or her an opportunity to explain the alleged failure to disclose. 2. If, after hearing the response of the person and making such further investigation as may be warranted in the circumstances, the board or committee determines that the person has in fact failed to disclose an actual or possible conflict of interest, it shall take appropriate disciplinary and corrective action. F. Records And Proceedings. The minutes of the board and of all board committees shall contain: 1.
